효과적인 병원감염관리 프로그램의 구성 요소에 대한 조사연구 (A Survey on Elements of an Effective Hospital Infection Control Program for Korea)

  • The main Purpose of this survey was to identify for elements of an effective hospital infection control program for Korea. Nurses and doctors who had participated in an education program for infection control or were re s ponsible for hospital infection control were selected as data informers. The data were collected from 51 subjects by employing a Delphi technique in a series of three rounds from september 1, 1987, to March 31, 1988. IN each round the responses to questionaires were analyzed and the results were communicated back to the individuals. Finally 32 elements of effective hospital infection control program were reduced to 10 elements. 10 elements are as follows ; 1) Hospital administrator's knowledge of importance and necessity for HIC(hospital infection control) 2) Supporting HIC administratively 3) Constituting infection control committee and role of ICC 4) Developing an eductional program and providing a work manual for the hospital infection control staff 5) Educating and informing medical staff about hospital infection 6) Surveillance for hospital infection 7) Developing patient care technique 8) Controlling the hospital environment 9) Executing regular health examination of all medical staff 10) Recruiting the medical staff sufficiently Three rankings of response rate about 32 elements are as follows ; 1) Hiring a full tim staff member for the HIC(66%) 2) Establishing a hospital policy and standards for the HIC(66%) 3) Activating the infection control committee and taking administrative action to support the ICC(63%) In addition the rankings of importance score by Likert 5 scale are as follows ; 1) Washing hands scrupulously(4.88) 2) Nurses participation as key members of the ICC(4.75) 3) Reviewing and evaluating all ongoing aseptic techniques (4.69) In conclusion, first of all, administrative support must be given to hiring a full time staff member and to organization of infection control committee for the HIC in Korea.

한방의료기관평가에 대한 한방의료기관 근무자의 인지도 (A Study on Cognition of Oriental Medical Hospital Employees on the Oriental Medical Institution Assessment)

  • Objectives: To determine the factors affecting the assessment of Oriental medical institutions under the Oriental Medical Institution Assessment System and thereupon, provide for some basic data and alternative measures for assessment of Oriental medical institutions. Methods: The researcher sampled 320 people employed by 26 Oriental medical institutions designated as model Oriental medical institutions subject to assessment in 2008 and 2009 and thereupon, used a structured and open-ended survey table for them to collect the data. The size of the final sample was n=302. Results: The Oriental medical institution staff were highly aware of the Oriental medical institution assessment (OMIA), After adjusting the other factors by multiple regression, the factors affecting such recognition were different significantly depending on age (those in their 30's), types of job (nurses and treatment assistants) and locations of hospital (GyeongSang-do). The staff expected the OMIA could helpful for improving facilities and system of hospitals, thus promoting satisfaction of patients. To do so, they felt it necessary to develop an assessment scale reflecting the special conditions besetting the Oriental medical institutions as well as the indices for improvement of Oriental medical service quality. Conclusion: It is hoped that this study will be followed up by future studies which will comparatively analyze Oriental medical institution staff's perception of the assessment system before and after its operation and thereby, suggest some ideal policy alternatives for assessment of the Oriental medical institutions. Furthermore, future studies are requested to research into Oriental medical institution staff's needs and consumers' needs as well in consideration of the characteristics of the Oriental medical institutions and thereupon, suggest some alternatives for continued education, development of the assessment tools, methods and policies.

한방의료서비스 이용과 만족도의 영향요인에 대한 계통적 고찰 (Systematic Review on the Customers' Use of and Satisfaction with Oriental Medical Services)

  • Objectives: This study aimed to find determinants of customers' use of and satisfaction with oriental medical services in Korea. Methods: A total of 33 articles which consist of 9 articles published in academic journals, 20 master's theses, and 4 doctoral dissertations were included in the systematic review. Among the articles used in the study, 29 used primary survey data, while 4 used secondary data produced by the Korean National Health and Nutrition Evaluation Survey. Results: First, it was found that people who prefer to use oriental medical services usually have one or more of the following diseases: musculoskeletal disorders, paralysis, or diseases or injuries without clear diagnosis. Other variables of gender, age, expectation of treatment outcome, and kindness of doctor and other staff were found to be significant determinants of using the oriental medical services. Second, the determinants of customers' satisfaction with oriental medical services were found to be age, gender, level of education, chronic disease with long length of stay, kindness of staff, medical cost, clinical environment, doctor's reputation, and public image of the institutions. Conclusion: The results of this study can be used to develop marketing strategies for oriental medical institutions in Korea.

감염환자 이송 로봇에 대한 의료종사자의 인식: SERVQUAL과 AHP를 활용하여 (Medical Staff's Awareness of Infected Patient Transfer Robots: Using SERVQUAL and AHP)

  • Purpose: The purpose of this study was to understand the perception of medical staff to propose an infected patient transport robot as a means of responding to infectious diseases. Methods: The data collected through the survey was analyzed through AHP analysis. The measurement tools used in this study were derived through the SERVQUAL model and Focus Group Interview(FGI), and consisted of four detailed questions for each of five classes: tangible, reliability, responsiveness, assurance, and empathy. Results: As a result of the study, there are concerns about risk factors that may occur in areas where medical staff intervention is minimized. Above all, we confirmed the consensus that safety should be the top priority during the process of robots to transport patients. In particular, highlighted were the resolution of device errors that may occur during the process for transporting patients and easy provision of the first aid. Additionally, the ability to monitor patients and suppress infection factors turned out to be important, which was directly related to the simplification of the role of medical staff and work efficiency. Conclusion: As one of the means of effectively controlling infectious diseases in a pandemic situation, a robot to transport the infected patient was considered. However, in order to commercialize this, specific verification of the safety of medical staff and patients is needed, and empirical data on providing the first aid, patient monitoring, and infection factor suppression should be presented.

조사자에 따른 환자만족도 조사결과의 차이분석 (The Difference in Patient Satisfaction Scores According to a Questionnaire Delivered by Hospital Staff and by Independent Surveyors)

  • Background : To study any interference of demonstrated in different patient satisfaction scores for the same questionnaires handled by hospital staff and by independent surveyors, respectively. Methods : This study included 728 subjects who were admitted to a university hospital from June 22 to July 1, 1999. The contents of the questionnaire were composed of six dimensions: hospital structure and process, staffs technical competence and humaneness, information & education and communication. Measurements were performed on a 5-score Likert scale. T-test and logistic regression analysis were also performed. Results : In an outpatient survey, satisfaction scores from a questionnaire delivered by hospital staff were significantly higher than independent surveyors for the dimension of communication, but no differences were shown among other dimensions. In an inpatient survey, satisfaction scores by hospital staff were higher for the process and communication dimensions(p<0.05). In particular, in both the inpatient and outpatient surveys, the difference of satisfaction scores for personnel items were significant between groups. After adjustment for age and sex those differences were significant between groups. Conclusion : To minimize the bias on questionnaire survey, the effects of personnel, who deliver and gather the questionnaire should be carefully considered in the evaluation of health service satisfaction.

의료이용 전.후 기대와 만족수준 비교 (Consumer expectation and consumer satisfaction before and after health care service)

  • The purpose of this study is to analyze the consumer's expectation before the health care service and the consumer's satisfaction after it. The participants of the study are inpatients in a general hospital located in Seoul. The resources were collected from the self-administration questionnaire survey run parallel with face to face interview. In order to measure the degree of the consumer's expectation, 349 samples were collected from the first questionnaire survey on the date of admission to the hospital. The second questionnaire survey was carried out on the date of discharge to the hospital with the participants responding to the first questionnaire survey. There are 154 samples collected from this survey. The results from the analysis of these resources are as follow. First, the survey shows that one of the highest consumers' expectations was about the generosity, kindliness and sincerity from the staff at the hospital, specially from doctors. Second, according to the analysis of the factors affecting the expectations of the consumers, with regard to path of admission to a hospital relating to patient's features, outpatient who gets into a hospital expected good medical care much more than the other patients. In regard of doctor's features, patients usually and highly expect good medical care from doctors who have good carrier and much experience. Third, according to the second questionnaire survey, what patients are satisfied most with is about the generosity and sincerity from staff at a hospital, especially from doctors and their gem attitudes. The results from survey show that the differences among the degree of consumers' satisfaction are very variable, depending on surrounding environments and facilities. The only fact that expectation didn't meet with satisfaction appeared to the case about technology and skill of medical care and the case about updated medical skills and equipments. Fourth, comparing the degree of expectation with the degree of satisfaction of consumers, correlative analysis was concerned significantly and specifically about the part of overall cleanliness relating to facilities and surrounding environments, the items about medical examination and test plan procedure relating to skill of medical care, professional specialties and convenience for procedure, and the items about satisfying explanations and concern about patients from doctors relating to staff's generosity and sincerity. Fifth, the analysis of the factors affecting the degree of how much patients are satisfied with shows that relating to sociodemographical features, patients are not satisfied with the case when the time and process of medical treatment are getting longer. It is surveyed that consumer were satisfied with the motivation to visit a hospital and the insurance type in patient's feature and so were the medical department and the factor of the degree of the expectation in disease's feature. Sixth, according to analysis based on the survey, patients would join again a hospital when they get satisfaction from the medical care and also they want to come again regarding to doctor's capability. For example, when doctors are old, have a good carrier and much experience, patients would come again. As seen from the above, consumers are usually satisfied with the medical treatment more than that they expected before. They would intend to use again when they get satisfaction from the medical care provided at a hospital. Patients and consumers highly expect good attitude as well as capacity from medical doctors and they are also generally satisfied with those things. Therefore, in order to increase the degree of consumer's satisfaction and their intention to come again, the hospital staff would have to commit themselves to achieve high quality service continuously and would have to make an effort to offer the finest quality service.

Safety Attitudes among Vietnamese Medical Staff in a Vietnam Disadvantaged Area: Latent Class Analysis

  • Purpose: We conducted this study with the aim of characterizing safety attitudes (SA) among medical staff in a disadvantaged area of Vietnam and examining associated factors with SA. Methods: A cross-sectional survey was conducted on 442 health staff members at four hospitals in Son La Province from June until August 2021. We used the Vietnamese shortened edition of the Safety Attitudes Questionnaire to measure the SA of study participations. We chose latent class analysis (LCA) to identifying the number of latent classes of SA among the study subjects. Multinomial logistic regression was used to examine factors associated with the identified SA classes. Results: The results of our LCA showed that there were three latent classes, namely high SA group (n=150, 33.9%), moderate SA group (n=236, 53.4%), and low SA group (n=56, 12.7%). The multinomial logistic regression analysis found that medical staff who had university education and above, who were nurses, and who served in non-clinical areas were more likely to be in the moderate SA group and in the high SA group than in the low SA group. Conclusion: Based on these results, several recommendations could be made to improve the SA of healthcare workers in disadvantaged areas. Further research with larger sample sizes and more diverse populations is needed to confirm these findings and to develop effective interventions to improve the SA of healthcare workers in disadvantaged areas.

의학교육 전문부서 교원의 탈진현상 (Burnout among Medical Education Specialists in Korean Medical Colleges)

  • Medical education departments or offices are established in response to public expectations relating to health care, societal trends towards increased accountability, educational developments, increased interest in what to teach and how to educate doctors. However, heavy workloads and mixed feelings towards medical education departments or offices by the other members of a medical school can threaten job satisfaction and increase burnout. The authors investigated the prevalence of burnout among medical education specialists and related issues. Individual in-depth interviews with four medical education specialists were conducted to develop a questionnaire. After content analysis of the interview, the authors generated a survey form with 28 items including 6 categories: motivation to choose medical education as a career, job satisfaction, intention to leave their current position in medical education, the frequency and causes of burnout, and demographics. In September 2013, an email survey was administered to 43 faculty including non-tenure staff who were working in the department/office of medical education in 41 medical colleges in Korea. Of 43 medical education specialists, 25 (60%) returned surveys. Forty three-point-three percent of them felt encouraged when their endeavors generated a visible educational improvement in the medical school. A majority (87%) reported feeling burned out. Fifty percent of them experienced the feeling once or twice a year. The extent of burnout tended to be greater in women, those in their forties, those with non-medical doctor degrees, and in non-tenured staff. To reduce and prevent burnout among medical education specialists, the participants suggested that leadership of medical schools and a systematic approach to medical education should be established. A majority of the medical education specialists reported experiencing burnout, although they were satisfied with their jobs. To reduce their burnout and allow them to focus on their own work in medical education, the following factors are needed: perceptual changes of other members of the college about medical education; more systematic institutional strategies; networking among medical education specialists; and personal efforts for professional development.

간호사의 의료인 간 의사소통에 대한 조사연구 (Nurses' Communications with Health Professionals)

  • Purpose: To present necessary data for improvement in communications between health professionals in as characterized by nurses' communications. Methods: This study was a descriptive survey research design with a survey of 1,510 registered nurses working in general hospitals (of at least 1,900 beds) in Seoul. A questionnaire on communication in the ICU, nurse-physician and nurse-nurse, was used. Data were collected from January 9 to 20, 2012, and the response rate was 85.0%. Results: Cronbach ${\alpha}$ values ranged from .75 to .89, except for .59 for accuracy (nurse-physician), with .89 overall. The highest mean score was for perception for timeliness [$3.83{\pm}.57$], followed by shift communication (nurse-nurse) [$3.64{\pm}.66$], openness (nurse-nurse) [$3.64{\pm}.65$], accuracy (nurse-nurse) [$3.14{\pm}.61$], openness (nurse-physician) [$2.90{\pm}.75$], understanding (nurse-physician) [$2.82{\pm}.65$], and accuracy (nurse-physician) [$2.70{\pm}.59$]. Subscales of openness, understanding, and shift communication were strongly associated with communication satisfaction. The general characteristics of nurses with different perceptions of communications included age, clinical experience, work pattern, and department. Conclusion: Proactive activities to improve accuracy, openness and mutual understanding between physicians and nurses are required for patient safety. Further studies are also needed to reassess communications and evaluate the relationship between patient outcomes and nurses' job satisfaction after application of strategies to improve communications.

대학병원 의사의 진료비심사기준 준수행동 분석 (An Analysis of the Medical Fee Review Standards Observance Behavior of a Tertiary Care Hospital Medical Staffs)

  • The medical fee reimbursement denied by HIRA(Health Insurance Review Agency) amounted to about 1.2% of the total medical fee claim to HIRA for reimbursement. Most of the denials stem from the inappropriate prescriptions of medical staff violating the medical fee review standards issued by HIRA. Considering the significant impacts of the standards observance behavior on the hospitals' financial viability, we attempted to analyze the predisposition factors of medical staffs' review standards observance behavior. The TPB(Theory of Planned Behavior) was adopted as the theoretical framework of the analysis. Data were collected by administrating a survey on the concepts included in TPB model to the 187 medical staff of a tertiary care hospital. Of the 187 questionaries distributed, 150 were responded resulting 80.2% of response rate. The mean differences among the groups classified by age group, years of experience, medical specialty and gender were analysis using ANOVA. The relationships among the TPB concepts were analysed by applying the Structural Equations Modeling method. The TPB model consists of three exogenous concepts (attitude toward the behavior, subjective norm, and perceived behavioral control) and two endogenous concepts (intention and the behavior). The results of ANOVA indicated significant mean differences among the groups classified by the medical staff's age, years of experience, and medical specialty. The older and the more experienced had the higher mean of observance behavior score. The results of Structural Equations analysis showed that the subjective norm and perceived behavioral control had statistically significant influences on intention, but the influence of attitude to intention was not statistically significant. The influences of perceived behavioral control and intention on behavior were significant. Based on these results the theoretical and practical implications were discussed.
