MAINTENANCE COST ANALYSIS FOR LARGE HOSPITAL BUILDINGS

  • Large hospitals such as medical centers provide not only medical services, but also carry the responsibilities for emergency refuges, medical researches and education. The function of large hospitals is as important as other infrastructure systems such as highways, bridges, and utilities. When disasters occur, the hospitals have to provide emergency medical services for victims and support the patient's needs of health. In order to keep a smooth operation of hospitals, the maintenance management of hospital buildings should be carefully investigated. However, there are few researches focused on maintenance management issues of hospital buildings. This paper investigated the National Taiwan University Hospital (NTUH) and established a maintenance cost database. The NTUH is the best-known and most high-renowned medical center in Taiwan in which more than 4,000 employees serving approximately 2,000 in-patients and 7,000 out-patients daily. The data were collected from the NTUH which consisted of 16,228 maintenance records in the past ten years. This paper analyzed these data to obtain various characteristics of maintenance records, and revealed the key items of maintenance cost for large hospital buildings, which can provide the facility manager of hospital buildings to execute a proper maintenance policy for hospital buildings.

Implementing Rule-based Healthcare Edits

  • Automated medical claims processing and billing is a popular application domain of information technology. Managing medical related data is a tedious job for healthcare professionals, which distracts them from their main job of healthcare. The technology used in data management has a sound impact on the quality of healthcare data. Most of Information Technology (IT) organizations use conventional software development technology for the implementation of healthcare systems. The objective of this experimental study is to devise a mechanism for use of rule-based expert systems in medical related edits and compare it with the conventional software development technology. A sample of 100 medical edits is selected as a dataset to be tested for implementation using both technologies. Besides empirical analysis, paired t-test is also used to validate the statistical significance of the difference between the two techniques. The conventional software development technology took 254.5 working hours, while rule-based technology took 81 hours to process these edits. Rule-based technology outperformed the conventional systems by increasing the confidence value to 95% and reliability measure to 0.462 (which is < 0.5) which is three times more efficient than conventional software development technology.

Reversible and High-Capacity Data Hiding in High Quality Medical Images

  • Via the Internet, the information infrastructure of modern health care has already established medical information systems to share electronic health records among patients and health care providers. Data hiding plays an important role to protect medical images. Because modern medical devices have improved, high resolutions of medical images are provided to detect early diseases. The high quality medical images are used to recognize complicated anatomical structures such as soft tissues, muscles, and internal organs to support diagnosis of diseases. For instance, 16-bit depth medical images will provide 65,536 discrete levels to show more details of anatomical structures. In general, the feature of low utilization rate of intensity in 16-bit depth will be utilized to handle overflow/underflow problem. Nowadays, most of data hiding algorithms are still experimenting on 8-bit depth medical images. We proposed a novel reversible data hiding scheme testing on 16-bit depth CT and MRI medical image. And the peak point and zero point of a histogram are applied to embed secret message k bits without salt-and-pepper.

간호정보시스템에 대한 사용자 만족도 (Users' Satisfaction with Nursing Information System)

  • Purpose: To identify the degree of users' satisfaction with Nursing Information System. Methods: The study is a survey of users' satisfaction with Nursing Information System. The subjects of the study are 300 nurses who use the Nursing Information System at least for 6 months or longer; data was collected from August 26th to September 1st, 2005. The collected data was analyzed on SPSS 12.0. Results: The results showed that there are noticeable differences in the degree of satisfaction among the users using the Nursing Information System depended on the subjects' marital status, working department, job title, types of computer training and other facts, such as individual's ability of using the system finding necessary references. Nursing records (including administration and prescription of medicine) was chosen to be the priority for the recognition of the Nursing Information System; for motivation on establishment of Nursing Information System was the efficiency of work; and for any problems or troubles with the Nursing Information System was the delayed work due to the system-suspension. Conclusion: The study implied that Nursing Information System have improved and effective Nursing practice.

Design and Implementation of a Personal Health Record Platform Based on Patient-consent Blockchain Technology

  • In the 4th Industrial Revolution, the healthcare industry is undergoing a paradigm shift from post-care and management systems based on diagnosis and treatment to disease prevention and management based on personal precision medicine. To optimize medical services for individual patients, an open ecosystem for the healthcare industry that allows the exchange and utilization of personal health records (PHRs) is required. However, under the current system of hospital-centered data management, it is difficult to implement the linking and sharing of PHRs in practice. To address this problem, in this study, we present the design and implementation of a patient-centered PHR platform using blockchain technology. This platform achieved transparency and reliability in information management by eliminating the risk of leakage and tampering/altering personal information, which could occur when using a PHR. In addition, the patient-consent system was applied to a PHR; thus, the patient acted as the user with ownership. The proposed blockchain-based PHR platform enables the integration of personal medical information with scattered distribution across multiple hospitals, and allows patients to freely use their health records in their daily lives and emergencies. The proposed platform is expected to serve as a stepping stone for patient-centered healthcare data management and utilization.

  • 최근의 IT의 발달로 스마트 단말의 보급이 확산되고 있고, 원격진료 및 전자의무기록(EMR) 등의 보편화로 인하여 장소를 가리지 않는 의료 환경으로 디지털화 되어 발전되고 있다. 이러한 정보화 트렌드에 맞춰 스마트 단말을 활용한 모바일 의료정보시스템 구축에 증대되고 있다. 본 논문에서는 최근의 의료정보기술 표준화 동향을 살펴보고, 향후 의료정보시스템 전망을 예측하여 본다. 또한 모바일 의료정보시스템의 보안적 측면을 살펴보고 국내에서의 모바일 기반 의료정보시스템의 필요한 동향 및 표준에 대하여 논의한다.

  • In these days, HIS(Hospital Information System) raise the quality of medical services by effective management of medical records. As computing environment was developed, it is possible to search information quickly. But, standard medical data exchange is not completed between medical clinic and another organ so far. In case of patient transfer, past medical record was not efficiently transmitted. It be feasible treatment delay or medical accident. It is trouble that medical records is transferred by a person and communicate with each other. Extensible Markup Language (XML) is a simple, very flexible text format derived from SGML. Originally designed to meet the challenges of large-scale electronic publishing, XML is also playing an increasingly important role in the exchange of a wide variety of data on the Web and elsewhere. Form in system of company product, relative organs that handle bio-signal data is each other dissimilar and integration and to transmit to supplement bottleneck this research uses XML. In this study, it is discussed about sharing of medical data using XML web technology to standard medical record between hospital and relative organization The data structure model was designed to manage bio-signal data and patient record. We experimented about data transmission and all-in-one between different systems (one make use of MS-SQL database system and the other manage existent bio-signal data in itself form in file in this research). In order to search and refer medical record, the web-based system was implemented. The system that can be shared medical data was tested to estimate the merits of XML. Implemented XML schema confirms data transmission between different data system and integration result.

Application of Standard Terminologies for the Development of a Customized Healthcare Service based on a PHR Platform

  • The personal health record platform can store and manage medical records, health-monitoring data such as blood pressure and blood sugar, and life logs generated from various wearable devices. It provides services such as international standard-based medical document management, data pattern analysis and an intelligent inference engine, and disease prediction and domain contents. This study aims to construct a foundation for the transmission of international standard-based medical documents by mapping the diagnosis items of a general health examination, special health examination, life logs, health data, and life habits with the international standard terminology systems. The results of mapping with international standard terminology systems show a high mapping rate of 95.6%, with 78.8% for LOINC, 10.3% for SNOMED, and 6.5% when mapped with both LOINC and SNOMED.

  • This paper studies medical data classification methods, comparing decision tree and system reconstruction analysis as applied to heart disease medical data mining. The data we study is collected from patients with coronary heart disease. It has 1,723 records of 71 attributes each. We use the system-reconstruction method to weight it. We use decision tree algorithms, such as induction of decision trees (ID3), classification and regression tree (C4.5), classification and regression tree (CART), Chi-square automatic interaction detector (CHAID), and exhausted CHAID. We use the results to compare the correction rate, leaf number, and tree depth of different decision-tree algorithms. According to the experiments, we know that weighted data can improve the correction rate of coronary heart disease data but has little effect on the tree depth and leaf number.

  • There are some limits on cost and efficiency for large amount of data in RDBMS, and NoSQL is starting to gain popularity. In medical institutions, data forms are different between organizations, and that makes difficulty for interoperability between organizations. In this paper we focused on performance issues between RDMBS and NoSQL in medical documents. We had built two different environment and had experiment comparative analysis of NoSQL with RDBMS based on medical data. We used medical HL7 FHIR as a medical data standard. Also YCSB benchmark tool was used for performance comparison. Experiments shows that NoSQL has better performance in large amounts of medical data processing systems that have over 10,000~100,000 records.
