An Intradiscal Granuloma Due to a Retained Wooden Foreign Body

  • We report a patient with a wooden foreign body granuloma in the intervertebral disc space being symptomatic 17 years after a paraspinal penetrant trauma. According to the our result of the search for wooden foreign body granulomas, this is the first case suffered from a wooden foreign body granuloma in the intervertebral disc space that reported in the literature. In this report, we emphasized the importance of rigorous examination and follow up in paraspinal wooden penetrant traumas.

  • The medical library, as a professional information center in relation to medical examination and treatment, research, and medical administration, should be operated under the rational and efficient library management. In that context, this study examines the theoretical underpinnings of the medical library management, and compares and analyzes, through direct visits, webpages, and statistical data, the current library operation of medical libraries in Seoul. This study also tries to make a managemental framework for the medical library by suggesting and discussing the result of this research.

  • As a result of analyzing HyangYakJ ipSeongBang, by focusing on folk medicinal plants, the writer has christened the phrase 'Folk Medical Study' because the medical system of HyangYakJ ipSeongBang was quite different from the usual Korean Medicine or Eastern Medicine. Folk Medicine Study is medical study that emphasizes accessibility of drugs and facility of treatment.   • Chest PA is the basic examination of radiographic imaging. Moreover, Chest PA's demands are constantly increasing because of the Increase in respiratory diseases. However, it is not meeting the demand due to problems such as a shortage of radiological technologist, sexual shame caused by patient contact, and the spread of infectious diseases. There have been many cases of using artificial intelligence to solve this problem. Therefore, the purpose of this research is to build an artificial intelligence dataset of Chest PA and to find a posture evaluation method. To construct the posture dataset, the posture image is acquired during actual and simulated examination and classified correct and incorrect posture of the patient. And to evaluate the artificial intelligence posture method, a posture estimation algorithm is used to preprocess the dataset and an artificial intelligence classification algorithm is applied. As a result, Chest PA posture dataset is validated with in over 95% accuracy in all artificial intelligence classification and the accuracy is improved through the Top-Down posture estimation algorithm AlphaPose and the classification InceptionV3 algorithm. Based on this, it will be possible to build a non-face-to-face automatic Chest PA examination system using artificial intelligence.

A Study on the Status of Medical Equipment and Radiological Technologists using Big Data for Health Care: Based on Data for 2020-2021 (보건의료 빅데이터를 활용한 의료장비 및 방사선사 인력 현황 연구 : 2020-2021년 자료를 기준으로)

  • As we enter the era of the 4th industrial revolution, it is judged that the scope of work of radiologists will be further expanded according to the innovation and advancement of radiation medical technology development. In this study, the current status of medical equipment and radiology technicians was identified, and basic data were provided for the plan for nurturing talents in the field of radiation medical technology in the era of the 4th industrial revolution, as well as career and employment counseling. Data from the second quarter of 2020 and the second quarter of 2021 were analyzed using health and medical big data. As a result of comparing the status of medical equipment by type in 2021 compared to 2020, C-Arm X-ray examination equipment increased by 5.83% to 6,638 units, followed by MRI examination equipment 1,811 units 5.29%, and angiography equipment 725 units 5.22% , general X-ray examination equipment 21,557 units increased 3.99%, CT examination equipment 2,136 units 3.03%, and breast examination equipment 3,425 units increased 3.00%. As a result of a comparison of the total number of radiologists in 2021 compared to 2020, the number was 29,038, an increase of 2.73%. As a result of comparing the status of radiographers by region, the increase was highest in the Gyeonggi region with 5.96%, followed by the Gangwon region with a 5.66% increase and the Chungnam region with a 3.81% increase. In a situation where the number of medical equipment and radiologist manpower is increasing, universities are developing specialized knowledge and practical competency through subject development related to the understanding and utilization of customized artificial intelligence and big data that can be applied in the medical radiation technology field in the era of the 4th industrial revolution. It is necessary to nurture qualified radiographers, and at the level of the association, it is thought that active policies are needed to create new jobs and improve employment.

  • Treating cardiac injuries following blunt trauma to the chest requires thorough examination, accurate diagnosis, and therapeutic plan. We present two cases; pulmonary vein rupture and left atrial appendage laceration, both as a result of blunt chest trauma. Through these cases, our team learned the importance of maintaining hemodynamic stability during the examination of injured cardiac structures. And based on the comprehensive cardiac examination, a decision to surgically intervene with median sternotomy via cardiopulmonary bypass was made, saving lives of the patient. This report introduces how such decision was made based on what supporting evidence and the diagnostic process leading to the initiation of surgical intervention. This report may help with decision-making process when confronted by blunt cardiac injury patients who need cardiac exploration.

Cytologic examination sputum and bronchial secretion bronchofiberscopic sampling procedures in lung cancer (기관지경검사에서 객담세포검사의 암양성률에 대한 연구)

  • The value of exfoliative sputum cytology in diagnosis of lung cancer has been accepted with bronchoscopic technique and results has been much improved by foreign investigators, but there was not presented accurate reports for sputum cytologic evaluation in Korea. We tried to find indicators of cytologic tests result in our hospital. During the period between May, 1980 and August, 1982, 400 patients, tested at Department of Thoracic and Cardiovascular Surgery of Kosin Medical College, had diagnostic bronchofiberscopic examination, and the cytologic study of sputum and bronchial secretions were performed. The sputum or bronchial secretion during bronchofiberscopic examination were obtained with various methods and the name of specimen were labeled as I, ASPIRATION SPUTUM, which was collected initially endobronchial sputum as introducing of scope, II, WASHING SPUTUM, which was collected washing bronchial secretion, III, BRUSHING SPUTUM, which was washing solution of brushing instrument and endobronchial sputum after brushing of lesions, IV, POST-SCOPIC SPUTUM, which was expectorated sputum as soon as removing of scopic instrument, V, ALL SPUTUM CYTOLOGY & CELL BLOCK, which was collected all specimen of above procedures. The diagnostic results of bronchofiberscopic examination was disclosed 174 cases [43.5%] of proved lung cancer, 47 cases [11.8%] of suspected lung cancer in grossly, and 179 cases [44.8%] of others finding except cancer. Patient with bronchofiberscopically grossly evidence of lung cancer which were not confirmed histologically or cytologically were excluded from this cytologic study. Histologic and cytologic correlation in proven lung cancer, 174 cases was revealed that number of cytologic positive patients were 45 cases [38.7%] among the 117 cases of proved squamous cell carcinoma, 12 cases [38.7%] among hislogically unknown cancer 34 cases and 6 cases [33.3%] among small cell undifferentiated carcinoma 18 Gases. Total cytologic positive result was presented as 67 cases [38.3%]. The other type of lung cancer, histologically, could not comparison because of small cases. The sequence of positive cytologic result in I-V specimen were disclosed as II, WASHING SPUTUM 57.6%, and V, ALL SPUTUM & CELL BLOCK 41.4%. The I, III & IV result were 28.6%, 22.2% and 26.1% respectively.

  • The purpose of this study was to spatial organization feature and area composition of the over 500beds general hospital that will be expected construct in Seoul, Gyeonggi-do area after 2000year. From this study, we can suggest architectural planning for the efficiency operating of emergency medical facilities that can be used as preliminary data. The Spatial organization of emergency medical facilities are categorize into 4 session as medical examination and treatment, diagonosticradiology, staff, public and the others. We can find spatial organization system and area composition that compare area composition of 6 hospital the subject of research in a emergency medical facilities. The result of this study were as follows : spatial distribution of its field, grasping of spatial organization factor and recognize its merits and demerits should be researched in advance. Through this kind of recognizing emergency system, realistic medical local and condition architectural planning for spatial organization will be operate.

  • A medical information management system for small to medium sized clinics and hospitals is developed. The system is designed to process, analyze and manage each patient's clinical record using database technique. The structure of the database was determined and implemented through careful and rigorous study of medical practices in Korea and, therefore, reflects the needs of information management in Korean medical community. Furthermore, a sophisticated inference engine that can deduce possible diseases from the result of medical examination is added to the system to provide doctors with a guideline in medical diagnoses.

A Comparison of Medical Care Services by Type of Medical Care Facility -In cases of normal spontaneous vaginal delivery and acute appendicitis- (의료기관 종류별 진료내역 비교 -정상분만과 급성 충수염을 중심으로-)

  • To find out the differences in medical practice according to the environment of patient care and managerial situation of the medical care institutions, normal spontaneous vaginal delivery and acute appendicitis were selected, which is suitable for comparison because of their high comparability. A total of 473 cases of normal spontaneous vaginal delivery and 408 cases of acute appendicitis was sampled from the claims to Korea Medical Insurance Cooperations during January to June 1984. Complicated cases were excluded from population and sampling was restricted from 40 to 60 percentile for total charges by the type of medical care facility in order to rule out the influence originating from case mix. Important items representing type and quantity of medical care service were compared by type of facility. Major findings are as follows : 1. University hospital shows the highest in charges per case and decrease in order of general hospital, and clinic. 2. In case of normal spontaneous vaginal delivery, average length of stay shows statistically significant difference by type of facility. 3. Charge amount for each service item affected by practice pattern shows statistically significant difference mostly by type of facility. It is suggested that medical practice pattern is different by type of facility for medical services. 4. Difference in total medical expenditure by type of facility is affected more with charges for materials, consumables and drugs than with fee for service activity. 5. In administering drugs to patients, hospital and clinic show higher injection rate than university and general hospital. 6. Clinical Laboratory tests were common in order of uninalysis, hemoglobin, hematocrit, white blood cell count, urine microscopic examination in cases of normal spontaneous vaginal delivery; white blood cell count, urinalysis, hemoglobin, hematocrit, urine microscopic examination, white blood cell differential count, in cases of acute appendicitis. 7. The result for Laboratory test and Radiologic study shows extreme difference by type of facility. Test rate is lowest in clinic and increase hospital, general hospital, and university hospital in order, both in type and frequency.
