합곡에 대한 애엽 약침이 흰쥐 대장염 완화에 미치는 효과 (Artemisiae argyi herbal acupuncture of Hapgok (LI4) ameliorates TNBS-induced colitis in rats)

  • Objectives : Ulcerative colitis and Crohn's disease are chronic inflammatory disease of the gastrointestinal tract was identified mainly for diarrhea, stomachache, hemafecia. The source and pathologic mechanism about ulcerative colitis and crohn's disease are still unknown but stress, infection, a genetic factor, environmental factors etc. may be the cause of chronic inflammatory disease of the gastrointestinal tract recently. The purpose of the present study is to evaluate the effects of herbal acupuncture with Artemisiae Argyi for the treatment to intestinal disease in the rat with 2, 4, 6 - trinitrobenzenesulfonic acid (TNBS) induced colitis. Methods : Artemisiae Argyi Herbal acupunctures were infected in HapGok (LI4) which is used to treat for intestinal disease in clinic. All animals were subjected to the injection of saline (300 ${\mu}l$, 500 ${\mu}l$) for a study control and TNBS (300 ${\mu}l$, 500 ${\mu}l$) into the lumen of the colon, 8cm proximal to the anus through the intestine. Artemisiae Agryi' Herbal (20 mg/ml, 0.4 ml) acupunctures were injected to the LI4 (both hands) at the secondary injection time of TNBS in rats. And body weight, RBC count, WBC count, total protein, Paw edema rate, rate of protein leakage into CMC-pouch fluid, IgG levels and IgM levels were observed to study the effects of Artemisiae Argyi' Herbal acupuncture in HapGok (Ll4). Results : Artemisiae Argyi' Herbal acupuncture in HapGok (Ll4) on TNBS-induced colitis inhibit the body weight lose rate but not RBC and WBC count. Furthermore, it inhibited the reduction of total protein concentration, paw edema, rate of protein leakage into CMC-pouch fluid, IgG levels and IgM levels. Conclusions : Herbal acupuncture with Artemisiae Argyi helps recover the TNBS-induced colonic damage and may be an important method for treatment of the colitis.

Result As the study for standardization of EAV's measurement values according to constitution and sex and age, the questionnaire on QSCC and QSCC(I) and EAV were carried out to 603 general patients in east : west promotion health center affiliated to Kyung Hee Medical center. The following conclusions were reached as the result of statistical handling of the data of questionnaire and EAV. 1) Each standard scope of BAZ and CMP was suggested. 2) BAZ and CMP examined about constitution did not show any statistical difference. 3) The result of examination by sex on BAZ and CMP is such as followings. A meaningful difference was showed at the level of p<0.001 in HH, HEL, HFR, and FF in case of male on BAZ. meaningful difference were showed at the level of p<0.001 in R3, R4, R18, L3, L16 and L18 and at the level of P<0.01 In R5, R8, R12, L7, L9, L10, L11, and L13 and at the level of P<0.05 in R1, R2, R6, R11, R13, R14, R16, R17, R19, R20, L1, L2, L4, L5, L14, L17, and L19 in case of male on CMP. There was no statistical difference in R7, R9, R10, R15, L12, and L15. 4) In every measure point of both BAZ and CMP a meaningful difference was notified at the level of P<0.01 by age. 2. Suggestion 1)Environmental, examinee and examiner's factors which may cause error in EAV measurement should be excluded. 2)The factor of change by time and space should be clarified. 3)The correlation between disease and meridian should be learned. And scopes of BAZ and CMP according to disease should be studied.

Genome-Wide Association Analyses on Blood Pressure Using Three Different Phenotype Definitions

  • Hypertension is the most prevalent disease worldwide and is itself a risk factor for cerebral, cardiac, and renal diseases. The inconsistency of candidate genes suggested by previous genomewide association studies (GWASs) may be due to not only differences in study design and genetic or environmental background but also the difference in the power of analysis between continuous traits and discrete traits. We analyzed 352,228 single nucleotide polymorphisms (SNPs) in 8842 unrelated Koreans obtained from Ansan and Ansung cohorts. We performed a series of GWA analyses using three different phenotype models; young hypertensive cases (278 subjects) versus elderly normotensive controls (680 subjects); the upper 25% (2211 hypertensive cases) versus the lower 25% of the SBP distribution (2211 hypotensive controls); and finally SBP and DBP as continuous traits (8842 subjects). The numbers of young hypertensive cases and elderly normotensive controls were not large enough to achieve genomewide significance. The model comparing the upper 25% subjects to the lower 25% of subjects showed a power that was approximate to that of QTL analysis. Two neighboring SNPs of the ATP2B1 gene, rs17249754 (SBP, p=$2.53^{-10}$; DBP, p=$1.28{\times}10^{-8}$) and rs7136259 (SBP, p=$1.30{\times}10^{-9}$; DBP, p=$6.41{\times}10^{-8}$), were associated with both SBP and DBP. Interestingly, a SNP of the RPL6 gene, rs11066280, revealed a significant genomewide association with SBP in men only (p=$3.85{\times}10^{-8}$), and four SNPs located near the MAN2A1 gene showed a strong association with DBP only in elderly men aged 60-70 years (e.g., rs6421827, p=$4.86{\times}10^{-8}$). However, we did not observe any gene variant attaining genomewide significance consistently in the three phenotype models except for the ATP2B1 gene variants. In general, the association signal with blood pressure was stronger in women than in men. Genes identified in GWASs are expected to open the way for prevention, early diagnosis, and personalized treatment of hypertension.

빅데이터 분석을 활용한 실험계획법 기반의 코팅제 배합비율 최적화 모형 (Optimization Model for the Mixing Ratio of Coatings Based on the Design of Experiments Using Big Data Analysis)

  • 코팅제에 대한 연구는 고분자 산업에서 가장 보편화되고 활발하게 연구되고 있는 내용의 하나이다. 코팅제는 전자산업, 의료, 광학 분야 등에서 중요성이 더욱 커지고 있으며, 특히 자동차 및 전자부품의 첨단화에 힘입어 코팅제에 대한 성능과 정밀도 등 기술적인 요구사항이 증가하고 있는 추세이다. 또한 방대한 환경 정보와 상황 정보를 기반으로 한 사물 인터넷과 빅데이터 분석 기술의 도입을 통해 산업 현장에서는 더욱 지능화되고 자동화된 시스템과 처리 기술의 필요성이 높아지고 있다. 이에 본 논문에서는 사물 인터넷 기술과 빅데이터 분석을 활용한 실험계획법 기반의 코팅제 배합 데이터에 대한 최적화 모형을 제안한다. 본 논문에서는 실제 생산현장에서 사용하는 코팅제 배합 기준 데이터와 발생한 오차에 대하여 작업자가 수정한 보정 결과 데이터를 실험계획법을 기반으로 분석하여 최적의 코팅제 배합 기준 데이터를 계산하였다. 또한 빅데이터 분석 기술과 사물 인터넷 기술을 활용하여 기존의 코팅제 배합 기준 데이터만을 적용한 공정이 아니라 제조 환경 정보와 상황 정보를 이용하여 색상과 품질 유지에 가장 중요한 인자를 검색하고 기준값을 보정하는 최적화 모형을 도출하였다. 실험 및 분석을 통해 확보된 기준 데이터는 제조 공정에 적용할 경우 배합의 정확도 향상과 LOT별 작업시간 단축을 가능하게 해주고, 건당 처리시간의 감소로 인한 생산 납품시간 단축, 불량률 감소 등에 따른 원가 절감에 기여할 수 있다. 또한, 다양한 모델링에 대한 제조 공정에서의 표준 데이터를 획득할 수 있다.

한국 농어업인 무릎 퇴행성관절염의 라이프 케어를 위한 유병률과 관련 요인에 대한 조사 (Research on Prevalence and Related Factors for the Life-Care of Knee Osteoarthritis in Korean Agricultural and Fishery Population)

  • 본 연구는 국민건강영양조사 제5기(2010-2012) 원시자료를 이용하여 한국 농어업종사자의 무릎 OA 유병률을 파악하고 관련요인을 알아보기 위해 시행하였다. 국민건강영양조사 5기(2010-2012) 원시자료 중 50세 이상 농어업에 종사하는 50세 이상 성인 1,264명(2010년 440명, 2011년 448명, 2012년 376명)을 연구대상자로 선정하였다. 연구대상자 중 X-ray 판독 결과 Kellgren Lawrence grade가 2 이상이고 최근 3개월 동안 30일 이상 무릎관절통이 있었다고 응답한 215명과 X-ray 판독 결과 Kellgren Lawrence grade가 1 이하이고 최근 3개월 동안 30일 이상 무릎관절통이 없었다고 응답한 601명, 총 816명을 최종 연구대상자로 선정하였고 무릎 OA 유병률과 인구사회학적 또는 건강관련 요인별 유병교차비를 분석하였다. 연구결과 무릎 OA 유병률은 26.3%였고, 인구사회학적 측면에서 여성, 연령이 증가할수록, 교육수준이 낮을 수록, 월소득이 적을수록 무릎 OA 유병률은 증가하였다. 건강관련 요인에서는 비흡연군, BMI가 높을수록 무릎 OA 유병률이 증가하였다.

B형 간염 바이러스의 ntC1731T 및 G1806A의 core 프로모터 돌연변이에 의한 HBV 유전자 발현 증가 분석 (Core Promoter Mutation of ntC1731T and G1806A of Hepatitis B Virus Increases HBV Gene Expression)

  • B형간염바이러스(HBV)의 만성 감염은 간경화와 간세포암 발생 빈도를 현저히 높인다. HBV 감염의 임상적 결과는 숙주 유전적 요인과 바이러스의 유전자 변이, 그리고 환경적 요인 등에 결정된다. HBV 복제를 위한 HBV의 pre-genomic RNA 전사는 바이러스의 core promoter 활성화에 의해 조절된다. Core promoter 돌연변이는 급성간 질환과 간세포암 발생에 연관되어 있다. 본 연구팀은 미얀마의 HBV 감염 환자들로부터 바이러스 유전자를 획득하여 core promoter 부위의 유전자 변이들을 파악하였다. Core promoter의 상대적 유전자 활성 차이를 분석하기 위해서 core promoter를 luciferase reporter에 재조합한 시스템을 제작하였다. 분석한 core promoter의 유전자 변이들 중에서 C1731T와 G1806A 돌연변이가 HBV core promoter의 전사 활성화를 증가에 관여하였다. 돌연변이 부위를 중심으로 전사 인자들의 가능한 결합 부위 변화를 컴퓨터 프로그램 분석을 통해 조사한 결과, C/EBPβ와 XBP1 반응 부위가 새롭게 생성되었음을 도출하였다. C/EBPβ의 세포 내 발현은 C1713T 돌연변이를 가진 core promoter의 전사 활성을 증가시켰으며, XBP1 발현은 G1806A 돌연변이를 함유한 M95 promoter를 활성을 증가시켰다. HBV 감염의 치료는 약제 내성과 백신 회피 돌연변이 발생으로 문제점을 가지고 있는 상황에서, 이 연구 결과는 HBV core promoter 돌연변이의 분자생물학적 그리고 임상학적 중요성을 제공한다.

서울대도시권역의 설정과 지역구조에 관한 연구 (A study on the establishment and regional strunture of Seoul metropolitan region)

  • 본 논문은 지속적인 서울시의 성장이 그 주변지역의 성장을 수반하면서 서울대도시 권화되어가는 과정을 밝히고 서울대도시권역을 설정한 후, 서울대도시권역 내에서 기능적 특성에 따라 지역을 분류하려는 것이 그 목적이었다. 1990년 시점에서의 서울대도시권역의 경계가 어느 정도 확대되었는가를 측정하기 위하여 도시화 정도를 측정하는 지표와 서울과 의 기능적 연계성을 측정하는 지표를 토대로 하여 그 권역을 설정하였다. 그 결과 서울대도 시권역은 수도권의 거의 전 지역을 포함하고 있는 것으로 나타났다. 또한 서울대도시권역에 포함된 지역들은 서울이 갖고 있는 기능을 일부 분담하면서 기능적인 면에서 특화되거나 전 문화되어 기능적 특성에 따라서 신종주거기능 탁월지역, 제조업기능 탁월지역, 서비스기능 탁월지역, 잠재적 개발가능지역의 4개 그룹으로 지역을 분류할 수 있었다.

산업장 교대근무 근로자의 건강증진행위 예측요인 (Predictive Factors of Health promotion behaviors of Industrial Shift Workers)

  • Industrial shift workers feels suffer mental stresses which are caused by unfamiliar day sleep, noisy environment, sleeping disorder by bright light, unusual contacts with family, difficulty in meeting with friends or having formal social meetings and other social limitations such as the use of transportation. Such stresses influence health of the workers negatively. Thus the health promotion policy for shift workers should be made considering the workers' ways of living and shift work specially. This study attempted to provide basic information for development of the health promotion program for industrial shift workers by examining predictive factors influencing health promotion behaviors of those workers. In designing the study, three power generation plants located in Pusan and south Kyungsang province were randomly selected and therefrom 280 workers at central control, boiler and turbine rooms and environmental chemistry parts whose processes require shift works were sampled as subjects of the study. Data were collected two times from September 17 to October 8, 1999 using questionnaires with helps of safety and health managers of the plants. The questionnaires were distributed through mails or direct visits. Means for the study included the measurement tool of health promotion behavior provided by Park(1995), the tool of self-efficacy measurement by Suh(1995), the tool of internal locus of control measurement by Oh(1987), the measurement tool of perceived health state by Park(1995) and the tool of social support measurement by Paek(1995). The collected data were analyzed using SPSS program. Controlling factors of the subjects were evaluated in terms of frequency and percentage ratio Perceived factors and health promotion behaviors of the subjects were done so in terms of mean and standard deviation, and average mark and standard deviation, respectively. Relations between controlling and perceived factors were analyzed using t-test and ANOVA and those between perceived factors and the performance of health promotion behaviors, using Pearson's Correlation Coefficient. The performance of health promotion behaviors was tested using t-test, ANOVA and post multi-comparison (Scheffe test). Predictive factors of health promotion behavior were examined through the Stepwise Multiple Regression Analysis. Results of the study are summarized as follows. 1. The performance of health promotion behaviors by the subjects was evaluated as having the value of mean, $161.27{\pm}26.73$ points(min.:60, max.:240) and average mark, $2.68{\pm}0.44$ points(min.:1, max.:4). When the performance was analyzed according to related aspects, it showed the highest level in harmonious relation with average mark, $3.15{\pm}.56$ points, followed by hygienic life($3.03{\pm}.55$), self-realization ($2.84{\pm}.55$), emotional support($2.73{\pm}.61$), regular meals($2.71{\pm}.76$), self-control($2.62{\pm}.63$), health diet($2.62{\pm}.56$), rest and sleep($2.60{\pm}.59$), exercise and activity($2.53{\pm}.57$), diet control($2.52{\pm}.56$) and special health management($2.06{\pm}.65$). 2. In relations between perceived factors of the subjects(self-efficacy, internal locus of control, perceived health state) and the performance of health promotion behaviors, the performance was found having significantly pure relations with self-efficacy (r=.524, P=.000), internal locus of control (r=.225, P=.000) and perceived health state(r=.244, P=.000). The higher each evaluated point of the three factors was, the higher the performance was in level. 3. When relations between the controlling factors(demography-based social, health-related, job-related and human relations characteristics) and the performance of health promotion behaviors were analyzed, the performance showed significant differences according to marital status (t=2.09, P= .03), religion(F=3.93, P= .00) and participation in religious activities (F=8.10, P= .00) out of demography-based characteristics, medical examination results (F=7.20, P= .00) and methods of the collection of health knowledge and information(F=3.41, P= .01) and methods of desired health education(F=3.41, P= .01) out of health-related characteristics, detrimental factors perception(F=4.49, P= .01) and job satisfaction(F=8.41, P= .00) out of job-related characteristics and social support(F=14.69, P= .00) out of human relations characteristics. 4. The factor which is a variable predicting best the performance of health promotion behaviors by the subjects was the self-efficacy accounting for 27.4% of the prediction, followed by participation in religious activities, social support, job satisfaction, received health state and internal locus of control in order all of which totally account for 41.0%. In conclusion, the predictive factor which most influence the performance of health promotion behaviors by shift workers was self-efficacy. To promote the sense, therefore, it is necessary to develop the nursing intervention program considering predictive factors as variables identified in this study. Further industrial nurses should play their roles actively to help shift workers increase their capability of self-management of health.

A Study on Relationship between Physical Elements and Tennis/Golf Elbow

  • Objective: The purpose of this research was to assess the agreement between job physical risk factor analysis by ergonomists using ergonomic methods and physical examinations made by occupational physicians on the presence of musculoskeletal disorders of the upper extremities. Background: Ergonomics is the systematic application of principles concerned with the design of devices and working conditions for enhancing human capabilities and optimizing working and living conditions. Proper ergonomic design is necessary to prevent injuries and physical and emotional stress. The major types of ergonomic injuries and incidents are cumulative trauma disorders (CTDs), acute strains, sprains, and system failures. Minimization of use of excessive force and awkward postures can help to prevent such injuries Method: Initial data were collected as part of a larger study by the University of Utah Ergonomics and Safety program field data collection teams and medical data collection teams from the Rocky Mountain Center for Occupational and Environmental Health (RMCOEH). Subjects included 173 male and female workers, 83 at Beehive Clothing (a clothing plant), 74 at Autoliv (a plant making air bags for vehicles), and 16 at Deseret Meat (a meat-processing plant). Posture and effort levels were analyzed using a software program developed at the University of Utah (Utah Ergonomic Analysis Tool). The Ergonomic Epicondylitis Model (EEM) was developed to assess the risk of epicondylitis from observable job physical factors. The model considers five job risk factors: (1) intensity of exertion, (2) forearm rotation, (3) wrist posture, (4) elbow compression, and (5) speed of work. Qualitative ratings of these physical factors were determined during video analysis. Personal variables were also investigated to study their relationship with epicondylitis. Logistic regression models were used to determine the association between risk factors and symptoms of epicondyle pain. Results: Results of this study indicate that gender, smoking status, and BMI do have an effect on the risk of epicondylitis but there is not a statistically significant relationship between EEM and epicondylitis. Conclusion: This research studied the relationship between an Ergonomic Epicondylitis Model (EEM) and the occurrence of epicondylitis. The model was not predictive for epicondylitis. However, it is clear that epicondylitis was associated with some individual risk factors such as smoking status, gender, and BMI. Based on the results, future research may discover risk factors that seem to increase the risk of epicondylitis. Application: Although this research used a combination of questionnaire, ergonomic job analysis, and medical job analysis to specifically verify risk factors related to epicondylitis, there are limitations. This research did not have a very large sample size because only 173 subjects were available for this study. Also, it was conducted in only 3 facilities, a plant making air bags for vehicles, a meat-processing plant, and a clothing plant in Utah. If working conditions in other kinds of facilities are considered, results may improve. Therefore, future research should perform analysis with additional subjects in different kinds of facilities. Repetition and duration of a task were not considered as risk factors in this research. These two factors could be associated with epicondylitis so it could be important to include these factors in future research. Psychosocial data and workplace conditions (e.g., low temperature) were also noted during data collection, and could be used to further study the prevalence of epicondylitis. Univariate analysis methods could be used for each variable of EEM. This research was performed using multivariate analysis. Therefore, it was difficult to recognize the different effect of each variable. Basically, the difference between univariate and multivariate analysis is that univariate analysis deals with one predictor variable at a time, whereas multivariate analysis deals with multiple predictor variables combined in a predetermined manner. The univariate analysis could show how each variable is associated with epicondyle pain. This may allow more appropriate weighting factors to be determined and therefore improve the performance of the EEM.

8체질의학과 질병생태학의 공통성에 관한 연구: 의료지리학적 접근을 중심으로 (A Search of the Community between Eight Constitution Medicine and Disease Ecology: A Perspective from Medical Geography)

  • 8체질의학과 질병생태학은 질병의 원인을 병균 이외의 유전적 요인, 환경적 요인, 문화적 요인의 상호관계, 즉 인간과 환경의 관계에서 접근한다는 점에서 상당한 유사성을 공유한다. 본 연구에서는 한의학의 8체질의학과 지리학의 질병생태학 간의 공통성을 실증적으로 검토하는 것을 목표로 두었다. 한의원을 내방하는 환자 647명의 임상 데이터와 전라남도 영광군의 사례를 분석하고 검증한 결과 도출된 내용은 다음과 같이 요약된다: 첫째, 각 시도별로 체질의 지리적 분포는 상이하게 나타나며, 이러한 분포 양상은 크게 해양형과 내륙형으로 분류할 수 있다. 둘째, 체질에 따라 잘 걸릴 수 있는 질환과 태생지의 상호관련성은 비교적 큰 것으로 파악되었다. 그렇지만 특정한 질환이 모든 체질에게 동일하게 발생하는 것이 아니라 지리적 환경의 특성에 영향을 받는 식생활이나 생활습관에 따라 사람들에게 차별적으로 발생하는 것으로 보인다. 셋째, 유전, 자연환경, 문화 등 요인의 상호연관성을 면밀하게 분석하기 위하여 전라남도 영광군의 사례를 실험적으로 분석해 보았다. 그 결과 자신의 체질에 맞는 식습관이나 생활습관을 가지고 있는 경우에는 비교적 건강한 상태를 유지하고 있었지만, 그렇지 않은 경우에는 다양한 질환에 노출되어 있었다. 8체질의학과 질병생태학의 공통성에 관한 연구는 아직 초기 단계에 있으므로 앞으로 이러한 공통성을 규명하는 방법과 노력이 다양하게 개발되고 투여되어야 할 것이다.
