Mediastinal Parathyroid Cyst -1 case report - (종격동 부갑상선 낭종 -1례 보고-)

  • Mediastinal parathyroid cyst is a very rare disease and is usually found incidentally. Surgical excision is the treatment of choice and recurrence is very rare when complete excision is done. A 71-year-old man was referred to our department because of 6$\times$5cm sized right superior mediastinal mass found incidentally on chest X-ray Surgical excision was performed and pathologic findings were confirmed as mediastinal parathyroid cyst. The patient has been followed up postoperatively without recurrence for 4 months up to now. We report a case of mediastinal parathyroid cyst.

  • As the majority of incidentally detected lesions in the anterior mediastinum is small nodules with soft tissue appearance, the differential diagnosis has typically included thymic neoplasm and prevascular lymph node, with benign cyst. Overestimation or misinterpretation of these lesions can lead to unnecessary surgery for ultimately benign conditions. Diagnosing mediastinal cysts using MRI serves as a problem-solving modality in distinguishing between surgical and nonsurgical anterior mediastinal lesions. The pitfalls of MRI evaluation for anterior mediastinal cystic lesions are as follows: first, we acknowledge the limitation of T2-weighted images for evaluating benign cystic lesions. Due to variable contents within benign cystic lesions, such as hemorrhage, T2 signal intensity may be variable. Second, owing to extensive necrosis and cystic changes, the T2 shine-through effect may be seen on diffusion-weighted images (DWI), and small solid portions might be missed on enhanced images. Therefore, both enhancement and DWI with apparent diffusion coefficient values should be considered. An algorithm will be suggested for the diagnostic evaluation of anterior mediastinal cystic lesions, and finally, a management strategy based on MRI features will be suggested.

  • The recommended approach for dumbell trmor is a one-stage removal by a team work of both neurosurgeons & thoracic surgeons. Recently we removed a case of dumbell tumor in a 69 years old woman with standard posterolateral thoracotomy and concomitant laminectomy as single stage operation. Postoperative course was uneventful and there were no postoperative neurologic signs.

  • As reported in the literature, extraskeletal mesenchymal chondrosarcoma is a rare malignant tumor of soft tissue, and it has a unique, distinctive histologic picture and poor prognosis.The common sites are the orbit, the cranial dura mater, the neck, the thigh, the leg, the chest wall, and the retroperitoneum. Radical excision of the tumor seems to be the primary treatment.We report experience with a very rare case of extraskeletal mesenchymal chondrosarcoma in the posterior mediastinum.

Recurrent Solitary Fibrous Tumor of the Mediastinum (재발성 고립성 섬유성 종양의 외과적 치험)

  • 장막내의 고립성 섬유성 종양은 드물며 대부분 장측 또는 벽측 흉막에서 발생하지만 종격동, 심막, 복막, 폐실질 , 안과 그리고 뇌척수막에서도 발생하기도 한다. 이 종양은 다양성으로 인하여 진단하는데 어려움이 있으며, 특히 종격동이나 흉부 외의 장소에서 발생하는 경우에는 더욱 그러하다. 이 종양의 임상적 양상을 예측하기 어려우며 조직학적으로 양성의 소견을 보이면서, 임상적으로는 악성의 양상을 띠기도 한다. 악성의 임상양상을 보이는 경우는 약 13∼23% 정도로 보고되고 있다. 저자는 53세의 여자 환자에서 종격동의 고립성 섬유성 조양을 수술한 후, 4차례 재발한 드문 증례를 수술 치험하였기에, 문헌 고찰과 함께 보고하는 바이다.

The Ability of FDG Uptake Ratio and Glut-1 Expression to Predict Mediastinal Lymph Node Metastasis in Resected Non-small Cell Lung Cancer (절제된 비소세포암에서 FDG 섭취비와 Glut-1 발현 정도를 이용한 종격동 림프선 전이 여부 예측)

  • Background: This study was designed to evaluate the FDG uptake ratio of mediastinal node and primary tumors using integrated PET/CT imaging combined with Glut-1 expression of the primary tumor in order to predict the N2 status more accurately in NSCLC patients. Material and Method: Patients who underwent integrated PET/CT scanning with a detectable mSUV for both primary tumors and mediastinal lymph nodes were eligible for this study. The FDG uptake ratio between the mediastinal node and the primary tumor was calculated. Result: The average mSUV of primary tumors and mediastinal nodes were, respectively, $7.4{\pm}2.2$ and $4.2{\pm}2.2$ in N2-positive patients and $7.6{\pm}3.7$ and $2.8{\pm}6.9$ in N2-negative patients. The mean FDG uptake ratio of mediastinal node to primary tumor were $0.58{\pm}0.23$ for malignant N2 lymph nodes and $0.45{\pm}0.20$ for benign lymph nodes (p<0.05). Models which combined Glut-1 expression with an FDG ratio have better diagnostic power than models that use the FDG uptake ratio alone. Conclusion: In some patients with a previous history of pulmonary tuberculosis or other inflammatory lung diseases, an FDG uptake ratio combined with Glut-1 expression may be useful in diagnosing mediastinal node metastasis more exactly.

  • The initial presentation of distant metastases in patients with papillary thyroid carcinoma is quite rare. Most distant metastases are solid nodular lesions. A 67-year-old man who complained of severe dyspnea underwent surgery due to a large mediastinal cystic mass compressing the trachea and great vessels. Pathologically, the cystic mass was a metastatic thyroid papillary carcinoma. The thyroid evaluations were compatible with a well differentiated thyroid carcinoma. An occult thyroid carcinoma presenting as a large mediastinal cystic lesion is extremely rare.

  • Mediastinal teratoma is a tumor that thoracic surgeons made an operation much less commonly than other mediastinal masses and most of them are asymptomatic. But very rarely, this tumor invades the pleura and pericardium resulting in pleural effusion, pericardial effusion and cardiac tamponade in severe cases. The mechanism of invasion and perforation of the tumor is unknown and tumor-consisting tissue factor is suspected of a cause. In this case, we operated on a patient whose anterior mediastinal teratoma invaded and perforated pericardium and pleura resulting in pericardial effusion and pleural effusion. The patient was improved and discharged with no problem after resection of mass and involved pericardium.

Primary Mediastinal Liposarcoma -1 Cases Report- (원발성 종격동 지방육종 -1예 보고-)

  • The primary mediastinal liposarcoma is a very rare tumor. It mainly causes respiratory symptoms, but can be asymptomatic. The most favorable treatment of primary mediastinal liposarcoma is a surgical removal, whether it is complete or not, regardless of the size or histologic type of the tumor. The survival is determined by the histologic type of tumor. A 34-year- ld man was admitted because of asymptomatic mediastinal tumor which was progressively growing over 6 years. The tumor was completely resected and the microscopic findings of the tumor were compatible with well-differentiated liposarcoma. The patient recovered and discharged without complication on the 7th postoperative day.

Three Regional Lymph Node Dissection in Thoracic Esophageal Cancer Surgery (흉부식도암 수술에서의 3영역 림프절 적출술)

  • Extended lymph node dissection, which includes dissection of the cervical and superior mediastinal nodes[three-field dissection , has been performed to improve the long-term survival since 1982 in Japan. Recently, the 5-year survival rate after three-field dissection has been reported to be better than 40%. During the period, from April to June, 1995, 4 patients among 7 operable esophageal cancer patients underwent subtotal esophagectomy with systematic dissection of regional lymph nodes including superior mediastinal and cervical lymph nodes at St. Mary`s Hospital. The esophagogastric anastomoses were made in the neck and the ascending routes of gastric tube were posterior mediastinal route. The cancer stage of them were stage IIA & IIB and it was possible to operate on a curability II & III basis. The numbers of resected lymph nodes with the three field dissection were 40-55. Postoperative complications were transient recurrent laryngeal nerve paralysis and atelectasis in 2 patients respectively but there was no anastomotic leak nor stenosis.
