Background and Aim: Oxaliplatin hypersensitivity is a well-known adverse reaction but the prevalence varies and data for frequency and clinical features have not been reported for Korea. Here we evaluates the prevalence and risk factors for hypersensitivity reactions to oxaliplatin after chemotherapy. Methods: Clinical information on all patients treated with oxaliplatin was retrospectively reviewed in electronic medical records between August 2009 and July 2010 in Seoul National University Bundang Hospital. Patients who experienced hypersensitivity reactions to oxaliplatin were compared with those who did not. Results: A total of 393 patients received oxaliplatin, with 42 (10.7%) experiencing hypersensitivity reactions including three cases of anaphylaxis. Median cycle of the first hypersensitivity reaction was 8. Reactions correlated with lower dexamethasone doses. Other variables were not significant. Conclusions: The prevalence of hypersensitivity reactions was 10.7%, symptoms being mostly mild and cutaneous. Lower dexamethasone doses could be a predictor for hypersensitivity reactions to oxaliplatin.
This Study was conducted to investigate Muscle Test of Point Selection through CRA(Contact Reflex Analysis) Muscel Diagnosis. So this study used to compare and analyze the effects of Muscel(Deltoid Muscel of Posterior) RMS(Root Mean Squared) and MEF(Median Edge Frequency) Among Groups that do not respond to questionnaire, Tonguibogam Naegyeongpyeon Small Intestine Group, Small Intestine MeridianPathway Primary Symptom and Secondary Symptom Group and Kwanwon(CV4) meridian Principal action Group. The questionnaire is composed of 23 items. The questionnaire was intended to elicit information on assorting into 4 groups. After Survey, Subject had to Muscle test subjects. Muscle experimental methods are as follows: Holding the shoulder without contacting Kwanwon. Holding the shoulder contacting Kwanwon. The first iteration. Group 1,2,3 were increased sEMG RMS compared with First experiment and Second experiment. Group 4(Including Uterus and Intestinal Flora Problem) were decreased sEMG RMS compared with First experiment and Second experiment. This test means that it is similar to diagnosis CRA and Small intestine channel of hand taiyang muscle, not Small Intestine MeridianPathway. It is suggested that subjects with a Small Intestine problem(Uterus and Intestinal Flora Problem) shows the results of decreasing posterior Deltoid Muscular strength. It means that CRA muscle diagnosis is associated with Alarm points diagnosis. But it doesn't consider influence of fat on the surface EMG.
The purpose of this study was to investigate the effects of smartphone use on muscle fatigue and tenderness in the cervical erector spinae (CES) and the upper trapezius (UT) and on the cervical range of motion among subjects with and without neck muscle pain. The subjects were 30 smartphone users in their 20 s who -were assigned to either an experimental group with neck muscle pain or a control group without neck muscle pain. Muscle fatigue and tenderness in the CES and the UT as well as the subjects' cervical range of motion were measured before and after 20-min smartphone sessions in a sitting position. In a between-group comparison of muscle fatigue, the experimental group showed a significantly greater decrease in median frequency in the CES and the right UT after smartphone use (p<.05). Regarding the assessment of muscle tenderness after smartphone use, the experimental group showed a statistically significant decrease in the pressure-pain threshold (PPT) in all muscles (p<.05), whereas the control group showed a significantly decreased PPT in the right CES and the UT (p<.05). The assessment of the cervical range of motion revealed a statistically significant reduction in the cervical flexion-extension and left lateral flexion in the experimental group (p<.05) after smartphone use. However, there was no significant change in the cervical range of motion in the control group (p>.05) after smartphone use. When compared with the control group, the experimental group demonstrated greater changes in cervical extension, lateral flexion, and rotation, except for cervical flexion (p<.05). In conclusion, when smartphone users have pre-existing neck muscle pain, the use of a smartphone further increased muscle fatigue and tenderness in the neck and reduced PPT and the cervical range of motion.
Background : The term "mononeuropathy multiplex" means simultaneous or sequential involvement of individual noncontiguous nerve trunks, evolving over days to years. The aim of this study was to delineate the causes, clinical features, and detailed electrophysiological findings in the patients with mononeuropathy multiplex. Methods : We analyzed the medical records of 22 patients with mononeuropathy multiplex confirmed on electrophysiological studies in Inje University Seoul Paik Hospital, Seoul Municipal Boramae Hospital, and Seoul National University Hospital between 1991 to 2000. Results : The number of male and female patients was equal. The mean age was 48 years with a peak incidence in the sixth decade. The etiology could be divided into vasculitis(11 patients) or non-vasculitis group. In vasculitis group, Churg-Strauss syndrome, polyarteritis nodosa, and rheumatoid arthritis were included. The non-vasculitis group included diabetes mellitus, leprosy, and Guillain-Barre syndrome. Ulnar and median nerves were most commonly involved(91%). In descending order of frequency, peroneal, posterior tibial, sural, and radial nerves were also involved. Bilateral involvement occurred most commonly in ulnar nerve. The symptoms and signs of mononeuropathy multiplex were the initial manifestations in 12 patients(55%), which was more frequent in vasculitis group(73%). Nerve conduction abnormalities could be divided into axonal, demyelinating, or mixed type. Most(91%) of the patients in vasculitis group revealed axonal type abnormalities. The location of the nerve lesion was frequently related to potential site of entrapment in demyelinating type. Conclusions : Mononeuropathy multiplex is the presenting features of the etiological disease frequently, especially in vasculitis group. Nerve conduction studies(NCS) reveals not only axonal type but also demyelinating type abnormalities. The etiological diseases were different in each type. Therefore, NCS is very helpful for the early etiological diagnosis and therapeutic implication in the patients with mononeuropathy multiplex.
The purposes of this study were to identify and analyze students' attitudes and satisfaction to the portfolio process and assessment for the Introduction to Clinical Medicine course at Ewha Womans University School of Medicine in Seoul, Korea. The subjects consisted of 64 medical school students. Questionnaires consisting of 20 5-point Likert-type items were developed, including three question domains: 1) orientation, 2) portfolios in general, 3) individualized feedback. The mean and median were found and frequency analysis was performed to identify the common characteristics of the participants. A major finding was that 54.7% of the respondents felt that the self-reflection involved in building the portfolio was a valuable learning experience. Plus, the majority of respondents perceived that the individualized feedback had a positive tone and its contents were specific, practical, and constructive. The students perceived that building and writing portfolios heightened their understanding of exit learning outcomes and enhanced their reflective thinking and self-directed learning skills. Meanwhile, some students perceived that there was too much paperwork in the portfolio process and that the process was time consuming. Furthermore, 32.8% of the respondents said that they had difficulty establishing their learning strategies by themselves and self-directing their learning during the portfolio process. In conclusion, it is expected that building a portfolio can help students not only to enhance their ability to accumulate and use their personal learning resources but also to develop the professional qualities required by doctors, such as self-directed learning, self-reflection, lifelong learning, team work, organizational skills, time management and prioritization, and professional thinking and behavior.
The purpose of this study was to evaluate the clinical and radiographic features of 35 cases of nasopalatine duct cyst by means of the analysis of periapical and/or occlusal radiograms in 35 persons visited the Department of Oral and Maxillofacial Radiology, School of Dentistry, Kyung Hee University and Chonbuk National University. The obtained results were as follows: 1. The incidence of nasopalatine duct cysts was higher in males(74.3%) than in females(25.7%). 2. The nasopalatine duct cysts were the most frequently occurred in the 4th decades(34.3%). The 6th and 7th decades(17.1%, 17.1%) were next in order to frequency followed by the 5th decades(14.3%), the 3rd decades(8.6%), the 2nd decades(5.7%), and 8th decades(2.9%). 3. In the signs and symptoms of nasoplatine duct cysts, 25.7% were swelling, 17.1% were swelling and tenderness, 20.0% were swelling and pain, and 37.2% were asymptom. 4. In the shape of nasopalatine duct cysts, 40.0% were round type, 48.6% were oval type, and 11.8% were heart type. In symmetry of the nasopalatine duct cysts to the median palatine surture, 85.7% were symmetry, 14.3% were asymmetry. 5. In the width of nasopalatine duct cysts, 11.4% were 6-l0㎜, 48.6% were 11-20㎜, 25.7% were 21-30㎜, and 14.3% were 31-40㎜. 6. In the periphery of nasopalatine duct cysts, 82.9% were distinct, 17.1% were relatively distinct. 7. In the change of root, 51.5% were intact, 17.1% were root divergence, 20.0% were root resorption, and 11.4% were root divergence and resorption.

Objectives: Headache is the most common complaint of patients suffering from temporomandibular joint disorders (TMDs). Thus, temporomandibular joint (TMJ) examinations maybe necessary in patients with headache. Considering the high prevalence of bruxism and TMDs in patients with headache the effects of conservative TMD treatment on headache should be assessed. Materials and Methods: Patients were questioned about headaches in the past three months. Those responding affirmatively to this question were examined for TMD and bruxism. After the examinations, 219 patients remained in the study and received self-management instructions. Patients were requested to modify oral habits except when eating or sleeping. The degree of pain (visual analogue scale), headache disability index (HDI), frequency of headaches (FH) per month and TMD intensity were evaluated. Results: The median levels of pain, HDI, FH, and TMD intensity were 8, 44, 8, and 7, respectively, before modifying oral habits and decreased to 4, 24, 2, and 3, respectively, after intervention. These decreases were statistically significant. Conclusion: Having patients maintain free space between the teeth and relax muscles can be an efficient method to treat headache and TMD, especially when repeated frequently.
Objective : An effective intervention has not yet been established for patients with acute occlusion of the internal carotid artery (ICA). The aim of our study was to investigate the feasibility, safety, and efficacy of emergent stent placement of carotid artery to improve neurologic symptoms and clinical outcome. Methods : Of 84 consecutive patients with severe ICA stenosis who were admitted to our institution from March 2006 to May 2009, 10 patients with acute ICA occlusion (11.9%) underwent emergency carotid artery stent placement. We reviewed their records for neurologic outcome using the National Institutes of Health Stroke Scale (NIHSS) score, before and at 7 days after stent placement; clinical outcome using the modified Rankin Scale score (mRS) and Glasgow Outcome Scale (GOS); frequency of procedure-related complications; and recurrence rate of ipsilateral ischemic stroke within 90 days. Results : Carotid lesions were dilated completely in all patients. Median NIHSS scores before emergency stent placement and at 7 days were 16.6 and 6, respectively, showing significant improvement. Eight patients (80%) had favorable outcomes (mRS score 0-2 and GOS 4-5). Complications occurred in two patients (20%): stent insertion failed in one and an intracerebral hemorrhage occurred in the other. Ipsilateral ischemic stroke did not recur within 3 months. Conclusion : Emergency carotid artery stent placement can improve the 7-day neurologic outcome and the 90-day clinical outcome in selected patients with acute cerebral infarction.

A microsporidium, from cabbage white bntteflies, Pieris rapae, collected in Korea, was purified and characterized according to its gene structure, spore morphology and pathogenicity. From the observation of the isolate by SEM and TEM, the endospores, exospores and nuclei, about 12 polar filament coils of the polar tube and posterior vacuoles were all identified. The nucleotide sequence was determined for a portion of genomic DNA which spans the V4 variable region of the small subunit rRNA gene. Comparison with the GenBank database for 15 other microsporidia species suggests that this isolate is most closely related to Nosema species. The pathogenicity against cabbage white butterflies was quantified by inoculating variable doses of spores to the second instar larvae. Peroral inoculation at a dosage of 10$\^$8/ spores/ml resulted in the death of all larvae prior to adult eclosion, but at lower spore dosages of 10$\^$4/-10$\^$5/ spores/ml, many adults successfully emerged. The median lethal dose (LD$\_$50/) was deter-mined to be 4.6$\times$10$\^$6/ spores/ml and the isolate also transmitted transovarially to the progeny eggs at a frequency of 92%.
최근 마을 중심부를 통과하는 일반국도 구간에서 교통량 증가와 지나치게 높은 차량속도로 인해 교통사고 위험성이 높아지고 있다. 마을 내 넓은 도로와 중앙분리대, 좁은 보도 및 화단은 보행자에게 매우 위험하며 지역 주민들의 생활환경까지 파괴하고 있다. 이러한 문제는 국도 마을통과구간이 소규모 마을과의 조화를 고려하지 않고 단순히 도로 기능 향상에 초점을 맞추어 2차로에서 4차로로 확장된데 그 원인이 있다. 이를 해결하기 위해 우회도로 건설 및 기능 재분류를 통해 설계속도를 낮추는 방법이 있으나 많은 비용과 시간을 투자해야 하는 어려움이 있다. 따라서 본 연구는 국도 마을통과구간에 대한 안전성 향상을 위해 적은 투자로 높은 효율을 낼 수 있는 Positive Guidance 기법을 활용하여 사례분석을 실시하고 개선방안을 도출하였다. 분석 결과, 국도 마을통과구간에서 가장 큰 문제점은 차량의 높은 속도였으며 감속을 위한 시설 및 정보조차 적절히 제공되고 있지 않았다. 이에 대한 해결책으로 감속시설의 설치와 적절한 정보제공의 필요성을 밝히고 개선대안을 제시하였다.
