Purpose : To evaluate the morphologic measurement and microvasculature of the normal human meniscus in Korean and to compare the difference of microvasculature according to the age and anatomical location. Materials and methods : Menisci of 21 cadavers divided into 3 groups according to the age. Each meniscus was divided into 10 parts of which the width and area of microvasculature were measured. Results : The average width of the medial and lateral menisci were $10.49{\pm}02.92mm$ and $10.55{\pm}2.05mm$, respectively. The average area of the microvasculature measured $20.29{\pm}8.44\%$ in the medial meniscus and $18.99{\pm}7.03\%$ in the lateral meniscus. The microvasculature of the medial meniscus was most abundant in the anterior horn and least abundant in the posterior horn (p<0.05). The lateral meniscus showed the most vessels in the posterior horn (p<0.05). The microvasculature of popliteal hiatus was not avascular but hypovascular. The area of microvasculature decreased with increasing age in all parts of the medial meniscus and body and posterior horn of the lateral meniscus (p<0.05). Conclusion : Differences in the anatomical distribution of the vessels were noted of both the medial and lateral menisci. The area of microvasculature generally decreased proportionally with increasing age in both medial and lateral meniscus.

Although the lateral discoid meniscus has been observed frequently, the medial discoid meniscus is an extremely rare anomaly. The incidence of lateral discoid menisci ranges from 1.4% to 15.5% and the incidence of medial discoid menisci ranges from 0.06% to 0.3%. We present 1 patient of bilateral medial discoid menisci with tear on left side and 1 patient of unilateral medial discoid meniscus with tear, that were diagnosed by magnetic resonance imaging (MRI) and arthroscopy. We performed partial meniscectomy on 2 cases of symptomatic medial discoid menisci under arthroscopy and meniscus-related symptoms were relieved. We report 3 cases of 2 patients with medial discoid meniscus.
Most of discoid menisci are lateral and medial discoid meniscus is very rare. There is no report about simultaneous medial and lateral discoid meniscus in one knee joint. A 15-year-old male patient was diagnosed as having a complete medial discoid meniscus with horizontal tear and intact incomplete lateral discoid meniscus by means of magnetic resonance image and arthroscopy. The patient was treated by arthroscopic partial meniscectomy for both discoid menisci.
A 4 years old male mixed breed dog, which weighed 25.5 kg, referred to Veterinary Teaching Hospital at College of Veterinary Medicine in Chonnam National University, because of chronic hindleg lameness. Click sound by flexion and extension of stifle joint, positive reaction by cranial drawer movement, fat pad sign and tibial position in cranial drawer position on the lateral radiographic view, medial patellar luxation (grade II), and lameness score 2.5 at standing and 2 at walking in right hindleg were showed. Under general anesthesia with enflurane, after medial arthrotomy, it was performed to remove remnants of cranial cruciate ligament and torn medial menisci and joint closed. In modified retinacular imbrication technique, one lateral fabellar/tuberosity suture, one medial fabellar/tuberosity suture, and one imbrication suture adjacent to the patella were placed. At 9 days after operation, lameness score 0 was observed and general condition was excellent.

In advanced gout, monosodium urate crystals are deposited in the joints and surrounding tissues, resulting in gouty tophi. A 45-year-old man was admitted to hospital due to pain and a limited range of motion in his right knee. Magnetic resonance imaging revealed a mass, and arthroscopic surgery and biopsy were then performed. Gouty tophi invading the anterior horns of the medial and lateral menisci were identified and surgically removed. To the best of the author's knowledge, there is no case report of intrameniscal gouty tophi invading both the medial and lateral menisci. This paper presents this case along with a review of the relevant literature.
Purpose: The results of meniscal repair of peripheral tear of meniscus with meniscal arrow was analysed. Materials and Methods: Peripheral tears of posterior 1/2 of menisci of 25 cases including 18medial, 6 lateral and 1 both menisci were repaired with meniscal arrows. One to six meniscal arrows were used for each meniscus according to the length of tears. Concurrent tears of ACL were noted in14 cases. The follow-up period was 3.4 years in average. The findings of physical examination and MRI which were taken once or twice in 1$\~$5 years after operation were evaluated. Results: Healing of the tear was achieved in all cases. However, a vertical longitudinal tear of body occurred in two medial menisci along the line of insertion of arrows. An arrow migrated subcuta-neously in two cases, respectively. Conclusions: The meniscal arrow was effective in the treatment of the peripheral tear of menicus. However, there was a chance of occurrence of longitudinal tear of the meniscus in case of insertion of arrows in a row. Migration of the arrow may occur.
The anterior horn of the medial meniscus is attached to the anterior surface of the tibia. The anterior horns of the medial and lateral menisci are connect with the transverse ligament. The posterior horn of the medial meniscus is firmly attached to the posterior aspect of the tibia just anterior to the insertion of the posterior cruciate ligament. The authors incidentally found an anomalous insertion of posterior horn of medial meniscus into the anterior horn area during arthroscopic examination in the symptomatic knee joint, which had been ruptured anterior cruciate ligament, and report with literature review.
Most of bucket handle meniscal tears are associated with anterior cruciate ligament (ACL) deficiency. Lateral meniscus lesions are more common with acute ACL deficiency, where medial meniscus lesions are more associated with chronic ACL deficiency. We reported an ACL deficient knee with bucket handle tears of medial and lateral meniscus of the same knee. The report suggests the need for increased awareness of the possible presence of this. Additionally, we discuss injury mechanism, clinical symptoms, specific signs on Magnetic Resonance Imaging (MRI), and treatment options.

Recent studies suggest that most meniscal cysts can be treated surgically by arthroscopic management of the meniscal tear and arthroscopic cyst evacuation. But arthroscopic cyst decompression may sacrifice a substantial amount of meniscal tissue that is not torn in order to expose the "stalk" of the cyst. Nowadays, the trend is changing as preserving the involved meniscus to prevent from inevitable degenerative changes after meniscectomy. The purpose of this report is to describe a new surgical technique that minimizes loss of meniscal tissue in hopes of maximizing residual meniscal function. We experienced 10 patients with meniscal cysts that were consisted of four lateral cysts and six medial cysts. Menisci were torn in all cases. Arthroscopic partial meniscectomy and decompression of cysts were performed in 9 cases, and arthroscopic partial meniscectomy and open cystectomy in 1 case. The procedures were consisted of injection of the methylene blue into the cyst, partial meniscectomy of the meniscal tear until the dye was seen in orifice of the cyst, and decompression of cyst through cystic opening. This article serves to confirm the relationship between torn menisci and cysts, to re-evaluate the occurrence ratio of the meniscal cyst on the medial to lateral meniscus, and to assess the efficacy of arthroscopic partial meniscectomy and decompression of cyst as a potentially meniscal sparing procedure.

Purpose : The purpose of this study was to compare the pattern of displaced bucket-handle tear of lateral and medial menisci and the treatment modality and results from accompanying injury. Materials and Methods : Patients who were diagnosed for displaced bucket-handle tear of medial meniscus (group I: 52patients, 52cases) and lateral meniscus (group II: 31patients, 32cases) from September 1998 to December 2002. The mean ages were 25years $(16\~66)$ for Group I and 29years $(18\~63)$ for Group II, and the average follow-up period were 18months $(12\~44)$ and 13months $(6\~46)$, respectively. The zone of meniscus tear and the existence of accompanying injury were verified through intraoperative arthroscopy and discoid type meniscus was additionally examined for group II. The assessment was made according to the physical examination and clinical pattern at the postoperative last follow-up, and the 2nd look arthroscopy was performed in 19cases $(23\%)$ for the cases accompanying anterior cruciate ligament (ACL) injury in both groups. Results : There were 38cases $(73\%)$ in group I, 5cases $(16\%)$ in group II for associating ACL injury. We observed discoid type meniscus (19cases, $59\%$) in group II. At last follow-up clinical success in repair cases of group I and II are 22cases $(85\%)$, 2cases $(67\%)$, in resection cases are 26cases $(100\%)$, 26cases $(93\%)$ respectively. Reoperation is performed 2cases in repair cases of group I. One case is re-rupture, the other case is newly developed tear in white-white zone. Reoperation is performed 2cases due to remnant meniscal tear in resection cases of group II. All of 2cases are discoid type menisci. Conclusion : Displaced bucket-handle tear of medial & lateral menisci would be substantially different an aspect, considering on difference would help to select proper treatment modality.
