A Study on Weighting Pathogenic Factor for Oriental OB&GY Questionnaires (한방부인과 진단 설문지의 병기가중치 부여연구)

  • Purpose : This study was investigated, in order to improve that reliability of disease mechanism diagnosis, which were used for the object diagnosis of Oriental medicine in the dept. of Oriental OB&GY, Oriental Medical hospital of Dong-Eui University, amend to a disease mechanism item of Questionnaires and indexes of disease mechanism, we were thought that the results were more pointed to the condition of appropriate disease mechanism, as a result of put a results of Questionnaires and belongs to indexes of disease mechanism together, we suggested to call it a 'weight of disease mechanism', because It was possible to mark a different degrees of indexes of disease mechanism in the same points. Methods : We analyzed the results of Questionnaires about 3354 outpatients who had OB&GY disease in the Oriental Medical hospital of Dong-Eui University from April 2000 to March 2004. Results : 1. weight of disease mechanism is 10 score according to disease mechanism score and the standard of reliability. 2. The standard of reliability is same 11 disease mechanism item except stagnated blood, cold syndrome, dry- ness, phlegm. Conclusion : Weight of disease mechanism which show satisfaction the conditions of standard of reliability, appear the results of Questionnaires, against previous study investigated reliability of Questionnaires make it through the standard of reliability.

A Study on the Kinds(種類), Causes(病因) and Mechanisms(病機) of Malaria(瘧疾) (학질(瘧疾)의 종류(種類)와 병인(病因).병기(病機)에 대한 고찰(考察))

  • Objective : Malaria(瘧疾) is a disease that's main symptom is paroxysm - a cyclical occurrence of sudden coldness followed by rigor and then fever. Since the introduction of the cause and mechanism of malaria(瘧疾) in the "Suwen(素問)", including Cold malaria(寒瘧), Warm malaria(溫瘧), Heat malaria(癉瘧) and Wind malaria(風瘧), there has been over 20 different kinds of malaria, each of which are introduced in multiple medical texts. Method : Through comparison between "Suwen(素問)" and other medical texts, the categories, causes and mechanisms of malaria can be analysed and organized to overview the whole feature of it. Results & Conclusion : External pathogens of malaria(瘧疾) are wind(風), cold(寒), summerheat (暑), dampness(濕), miasmic toxin(瘴), pestilence(疫), ghost(鬼). Internal pathogens of malaria(瘧疾) are dietary irregularities(飮食不節), overexertion and fatigue(勞倦), phlegm(痰), seven emotion(七情). Malaria can be categorized into four groups according to the pathological mechanism that leads to paroxysm. They are latency of disease(伏氣), external contraction(外感), internal damage(內傷), and combination of disease(合病). Malaria-Paroxysm(瘧疾發作) occurs when the three following factors collide strongly : defense qi(衛氣), latent qi(伏邪) and external pathogen(新邪). When collision of the three factors takes place in the interior(裏), the body experiences chills. When it takes place in the exterior(表), the body experiences fever. The cyclical occurrence of Malaria-Paroxysm follows the circulation of defense qi.

  • This study is about researching DongEuiBoGam by analysing with pattern identification of modern Traditional Korean medical patholgy as more logical, systematic and standardized theory. Disease pattern mechanisms of essence, spirit, qi and blood in NaeGyungPyeb of DongEuiBoGam are these. In Essence, this explain mechanism of disease patterns those are seminal emission, dream emission, spermatorrhea, white ooze. These disease pattern's mechanisms are kidney yang deficiency, kidney yin deficiency, heart yang deficiency, heart yin deficiency, heart qi deficiency, spleen qi deficiency and so on. On viewpoints of viscera and bowels they are related with heart, kidney, spleen. And most of them are deficiency from deficiency-excess Pattern Identification. Classifying disease pattern of qi is about upward, downward movement and more concentrated deficiency than excess pattern. Fright palpitations can be classified heart deficiency with timidity, heart blood and qi deficiency, heart qi deficiency, heart blood deficiency, heart qi movement stagnation, water qi intimidating the heart, phlegm-fire harassing the heart, phlegm clouding the pericardium, and so on. Palpitations can be classified heart blood deficiency, heart yin deficiency, heart deficiency with timidity, heart spleen blood deficiency, spleen qi deficiency, phlegm-fire harassing the heart, intense heart fire, and so on. Forgetfulness can be classified heart spleen blood deficiency, heart spleen qi deficiency, kidney essence deficiency, heart qi deficiency, non-interaction between the heart and kidney, etc. for deficiency pattern, phlegm clouding the pericardium for excess pattern. In Blood just say inside bleeding pattern's category, there are nose bleeding, flopping syncope, qi counterflow, blood vomiting, hemoptysis, spitting of blood, bloody stool, hematuria, and so on. Like these, this study identify pattern of disease in DongEuiBoGam by mechanism of disease theory.

Comparative Study on Etiological Cause, Pathogenesis Mechanism of "Shanghanlun" and "Wenbingtiaobian" ("상한론(傷寒論)"과 "온병조변(溫病條辨)"의 병인병기론적 비교 연구)

  • We can understand "Shanghanlun(傷寒論)" and "Wenbingtiaobian(溫病條辨)" which are major books on externally contracted diseases well by making a comparative study of their similarities and differences. After studying etiological causes and characteristics of disease, disease pattern, syndrome differentiation, transmutation rules, following conclusions are derived. While cold is an etiological cause of Cold damage and harms Yang qi, heat is an etiological cause of Warm disease and harms Yin qi. Cold damage and Warm disease have something in common in the respect of damage to fluid and humor and Yang qi. Exuberant heat symptom of Yang brightness disease and lesser yin heat transformation pattern have similar damage to fluid and humor as Warm disease does. Warm disease can reach qi collapse syndrome through damage to Yang qi following fluid and humor damage. In the respect of water qi, as Cold damage makes water-dampness retain easily due to cold congealing, dampness-draining diuretic medicinal and warm yang medicinal are used together. As warm disease damages fluid and humor, yin-tonifying medicinal is used and dampness-draining diuretic medicinal can be used in the case of Warm disease with dampness. In the respect of disease pattern, cold syndromes arise mostly by Cold damage except heat syndrome of grater yang disease, chest bind syndrome, stuffiness syndrome, reverting yin disease and yang brightness disease. Warm disease is classified as pure heat syndrome and heat syndrome with bowel excess, damage to yin, qi collapse or damage to blood.

A Study on The Disease Mechanism and Treatment of The So-eumin Exterior Disease of "Dong-uisusebowon(東醫壽世保元)" -On Ikgi(益氣) and Seung-yang(升陽)- ("동의수세보원(東醫壽世保元)" 소음인(少陰人) 표병(表病)의 병기(病機) 및 치법(治法)에 대한 고찰(考察) - 익기(益氣)와 승양(升陽)을 중심으로 -)

  • This paper studies the main treatment methods, Ikgi(益氣) and Seung-yang(升陽) applied frequently by Lee Jema(李濟馬) on the exterior disease of So-eumin(少陰人), one of the four constitutions. The methodology of this paper is to understand the pharmacology of "Dong-uisusebowon(東醫壽世保元)" by examining the formulas applied by Lee Jema. I have examined the organization of formulas in "Dong-uisusebowon(東醫壽世保元)" beforehand to categorize the formulas applicable in this study according to an objective standard. I have analyzed the prescriptions applied to So-eumin exterior disease. As a result, I could see that in the case of Ulgwang(鬱狂) syndrome, Ikgi(益氣) and Seung-yang(升陽) methods were mainly applied, but as the disease progressed, Seung-yang(升陽) was withdrawn while Ikgi(益氣) was stressed. Likewise, in the case of Mang-yang(亡陽), both methods were mainly adopted, but as the patients got worse, the level of Seung-yang(升陽) was maintained and that of Ikgi(益氣) was elevated with the addition of Buja(附子). Through this process, we could verify the overall action of Ikgi(益氣), Seung-yang(升陽) and Buja(附子). Originally, the two methods of Ikgi(益氣) and Seung-yang(升陽) are intimately related, but by analyzing the overall functions of the two methods, we could see that Seung-yang(升陽) and Buja(附子) support the Yang gi of the interior of So-eumin, while Ikgi(益氣) resolves inner stagnation of Yang gi and emits the cold pathogen of the exterior. Also, in the course of treatment, Ikgi(益氣) could only be realized after securing Seung-yang(升陽).

  • Jinguiyaolue, written by Zhong-Jing Zhang(張仲景) in East-Han Dynasty of China, is believed to be the professional book about divers diseases of Korean Medicine(韓醫學). As based its theoretical framework upon Nei-Jing(內經) and Nan-Jing(難經), Jinguiyaolue(金匱要略) not only suggests the basic theory about the cause of disease, mechanism of disease(病機), diagnosis and treatment and prevent of divers disease but also constructs foundation for the clinical medicine. Thus, it can be said that Jinguiyaolue(金匱要略), along with Shang-Han-Lun(像寒論) by Zhong-Jing Zhang(張仲景), will be quite worthy in that the book deals with the fundamental theory of Korean Medicine(韓醫學).

  • The cause and mechanism of eye diseases are follow in biblography. The etiological factor of eye diseases are nearly caused by heat. It is important to distinguish deficiency and substance. In external etiologic factors wind-fever is most of all in six exogenous factors. In internal etiologic factors xu(deficiency) of kidney, blood and fire of seven emotions are lots. In factors other than the internal and external, eating greasy food and hyper-caloric food are a lot of case. In relation with the yin-yang andxu-shi, ther are a lot of eye diseases symptom as dark-dizziness due to the yin-xu of hepatic-kidney. According to eight principles of differentiation of syndromes, syndromes of exterior, heat, shi, almost fall into the category of yang and syndromes of interior, cold, xu, nearly fall into the category of yin. In interior treatment, inducing wind and clearing heat, using Qi and tonifying blood, tonifying and suing of hepatic-kid ney are useful and often used treatment.

  • Objective : Ankylosing Spodylitis(AS) is a chronic inflammatory disease that is notorious for its difficulty to cure. This study aims to interpret the mechanism of AS in the Traditional Korean Medical(TKM) perspective, and to understand the role of the spine. Method : Clinical manifestations of AS, epidemiological data and clinical cases were interpreted by traditional methodology. Result : The young age group of patients suffering from this condition suggests that the cause of this condition is not solely linked to deficiency of the Kidney[腎], which is a general cause for bone disease. Its symptoms are linked to obstruction in the upper body, which results in disturbance of Jeong(精) collection in the lower body. Based on other accompanying symptoms, together with its character as an autoimmune disease, a strong link to the spirit of the heart[心神] can be suggested. Conclusion : AS is closely linked not only to essence of kidney[腎陰], but to the spirit of the heart[心神] as well. From the TKM analysis of the spine, we can understand it as the basic pathway for the fundamental Gi(氣) circulation.

Study on the Mechanism of Wasting-thirst in Viewpoint of 'Viscera and Bowels' and 'Three Yin and Three Yang' (장부(臟腑)와 삼음삼양(三陰三陽)으로 살펴본 소갈(消渴) 병기(病機) 연구)

  • The applicability of Diabetes Mellitus(DM) in Oriental Medicine might be placed under the category in Wasting-thirst(消渴, WT). However, because of WT having so various concepts, classification, and names in Oriental Medicine, this resulted in confusion in applying WT treatment to DM. WT occurs mainly due to dryness-heat(燥熱) which by depleting 'Fluid and Humor'(津液) makes symptoms of thirst(口渴) swift digestion with rapid hungering(消穀善饑) frequent urination(小便頻數) according to the "Internal Classic"(內經). WT was also called Sodan(消癉), Bidan(脾癉), or Sojoong(消中) in the "Internal Classic", which was caused by not only the disorder of 'five viscera' (五臟), but also the failure of 'three yin and three yang'(三陰三陽). However, the doctors of those days had a tendency to manage WT (including DM) with herbal drugs of dryness-heat or with drugs that recover the pancreas and the kidney, that have a tendency to appreciate that WT is the disease of 'five viscera'(五臟). Accoding to the "Internal Classic"(內經) and "Jinguiyaolue"(金匱要略), WT was appreciated that WT is also due to the disorder of Meridian including 'three yin and three yang'(三陰三陽) theory. Disorder of 'yang brightness'(陽明) or 'reverting yin'(厥陰) would cause Wasting-thirst(消渴), so the 'three yin and three yang'(三陰三陽) theory should be considered in WT treatments. Furthermore, 'yang brightness'(陽明) and 'reverting yin'(厥陰) all belong to 'the door'(闔) in the bolt-door-pivot(關闔樞) theory, and the mean of 'the door'(闔) is the function of storage, so we could understood WT is the wasting disease due to the disorder of 'the door'(闔) system in Meridian. WT should also be understood by the point of the bolt-door-pivot(關闔樞) theory in addition to the 'three yin and three yang'(三陰三陽) theory.

  • This paper was written into condsideration records about the definition of "Ong-Jeo", the difference between "Ong" and "Jeo", the Western medical approach to "Ong-Jeo", and the etiology and pathologic mechanism of "Ong-Jeo". After this study, I report the following results from it. 1. "Ong" is an acute diapyesis disease which is found is in our skin and muscle and flesh. The chrateristics of this disease is that the affacted site is shine, no head, and the size is 3-4 chon. The prossess of the diesease shows that it is very quick, and very easily swell, and becomes pus easily, easily become to ulcer, easily converge. But this don't damage to the muscle and bone. 2. "Jeo" is the disease which damages bone-skeleton, muscle and flesh, and even destroy the stationary tissue. Jeo can be divided into two. One is called "Yudujeo" and it is acute diapydesis disease. The characteriscs of this disease is the miliary abscess, swell, has fever, and has an ache which is feeled spaned. And this is diffused into periphrey tissue and is diffused into deep site. After the ulcer, this becomes to shape the cellula. The size is more than 3-4 chon and this disease goes into chuk if this is serious. The other one is called "Mudujeo" and this disease is the ulcerative one in our joint and skeleton. The characteristic of this disease shows that the color of skin be not changed, and swell diffusely, and is not easily vanished, not easily becomes ulcerative, and not easily becomes converged. 3. "Ong-Jeo" is caused by the bacteriunm named by the "Golden and Yeollw Staphylococcus" in the Western medicine. "Ong" can be applicable to the carbuncle, acute diapyesis lymphadenitis, and some of cellulitis. "Jeo" can belong to cellulitis, and Mudujeo can belong to suppurative arthritis, suppurative osteomyelitis, tuberculous arthritis and osteomyelitis, and tuberculous lymphadenitis. 4. The etiologies of "Ong-Jeo" can be divided three, which are internal, external and other etiology which can not be clasiffied by two etiologies above. The internal etiology is seven emotion, and the external etiology is the six eumsa, unki, chunhang and so on. Other etiology is inadequate absorption of food, and excessive bang-sa. 5. The etiology of "Ong" is suppurative one which is choked between our skin and muscle and flesh, and is congested, become to hot, and finally erodes the muscle and flesh because of the inbalance of cirrculation in the enegy and blood. "Jeo" is the same as the Ong, but this is the suppurative disease which damages the muscle, flesh, and skeleton, and even damages into five Zang, the internal intestine.
