Efficacy and Safety of Geochangmanryeung-dan and Acupuncture on Patient with Spinal Stenosis Treated by Epidural Steroid Injection: a Study Protocol for a Randomized Controlled Pilot Trial (경막외신경차단술로 치료 중인 척추관 협착증 환자에서 거창만령단과 침의 효능 및 안전성 연구(예비임상연구))

  • Objectives : This study was designed to obtain basic data for a further large-scale trial as evaluating the efficacy and safety of Geochangmanryeung-dan(GMD) and acupuncture on patient with spinal stenosis treated by epidural steroid injection. Methods: The study is a randomized controlled pilot clinical trial, conducted over 8 weeks. Twenty participants will be recruited and randomly allocated to 2 groups: an experimental(GMD and acupuncture with epidural steroid injection) group and a control(only epidural steroid injection) group. The epidural steroid injection will be administered once per 2 weeks for 6 weeks(3 times in total). GMD will be administered as a dose of 5 pills, 3 times per day, for 6 weeks. Acupuncture will be performed 2 times per week for 6 weeks(12 times in total). The primary outcome will be measured by visual analogue scale and self-rated walking distance. The secondary outcome will be measured by PainVision, short-form McGill Pain Questionnaire, and Oswestry Disability Index. Both primary and secondary outcomes will be measured at baseline, 2, 4, 6, and 8 weeks. The last assessment(at 8 weeks) will be performed 2 weeks after treatment cessation. Conclusions : This clinical trial, as the pilot study for a future large-scale trial, will provide clinical information for evaluating the efficacy and safety of GMD and acupuncture treatment in combination with epidural steroid injection for the treatment of spinal stenosis.

  • Objectives: This study presented the case of a 49-year-old Korean female with phantom limb pain after below right knee amputation and aimed to assess the effectiveness of Korean medicine treatment. Methods: The patient was treated with scalp acupuncture, electroacupuncture and herbal medicine. We executed a numerical rating scale (NRS), conducted a global assessment (G/A), administrated a Short-Form McGill Pain Questionnaire 2 (SF-MPQ-2) and measured total daily sleep time to evaluate symptom improvement. Results: The patient's G/A scores decreased from 10 to 2 and SF-MPQ-2 points decreased from 20 to 6 after treatment. The total daily sleep time did not changed due to anxiety. Conclusions: This study suggests that Korean medicine treatment could be effective in treating phantom limb pain after amputation. Further studies are needed.

The Effect of Chinemys reevesii Gray Pharmacopuncture for Women with Knee Osteoarthritis (구판(龜板)약침이 여성의 퇴행성 슬관절염에 미치는 영향)

  • Objectives : The aim of this study is to determine whether Chinemys reevesii Gray pharmacopuncture is a clinically effective treatment for women with knee osteoarthritis. Methods : Female patients with knee osteoarthritis were recruited from May $1^{st}$, 2014 to April $15^{th}$, 2015 at the Department of Acupuncture & Moxibustion Medicine of Dong-Eui University Korean Medical Hospital. Patients were randomly assigned to two groups. 4.0 ml of Chinemys reevesii Gray pharmacopuncture was injected at acupuncture points of the experimental group(n = 24) twice a week for three weeks. The control group(n = 24) received 5 % dextrose injections in order to identically stimulate the same acupuncture points of $ST_{35}$, $ST_{34}$, $SP_9$, $SP_{10}$, $GB_{34}$, $LR_8$, $EX-LE_4$, and $EX-LE_2$. The Short-Form McGill Pain Questionnaire(SF-MPQ), the Korean Western Ontario and McMaster Universities Osteoarthritis Index(KWOMAC), and the European Quality of Life-5 Dimensions(EQ-5D) were recorded to evaluate treatment efficacy at first visit, after two weeks and four weeks from the initial visit. Results : 40 patients completed the trial. The experimental group showed more significant improvement than the control group in the scores of SF-MPQ, KWOMAC, and EQ-5D. However, there was no significant difference between groups in a few of the assessment scales. Conclusions : Chinemys reevesii Gray pharmacopuncture can be an effective treatment in control of pain, improvement in function and health-related quality of life for women with knee osteoarthritis.

A Case Report of Rhabdomyolysis Induced by a Combination of Exercise, Alcohol, and Medication (운동, 알코올, 약물 등 복합적 요인으로 유발된 횡문근융해증 환자 치험 1례)

  • Rhabdomyolysis is a syndrome that evokes either myopathy or cola-colored urine with elevated creatine kinase and myoglobin levels. The most common complication of rhabdomyolysis is acute renal failure (ARF). A large quantity of fluid must be injected promptly the first time symptoms appear, whereas sequela of symptoms has remained in the long run. In this report, a 58-year-woman was diagnosed with ARF due to rhabdomyolysis induced by exercise, alcohol, and medication. A treatment of acupuncture and herbal medicines was carried out for over 2 months. To evaluate the condition of the patient's lower limb, a short form of the McGill Pain Questionnaire, Visual Analogue Scale (VAS), European Quality of Life-5 Dimensions (EQ-5D), and Manual Muscle Test (MMT) points were used. As a result, clinical symptoms and quality of life improved noticeably after treatment with Korean medicine. This suggests that Korean medicine has an effect on rhabdomyolysis.

Two Cases of Korean Medicine Therapy for Fibromyalgia Syndrome (섬유근통 증후군에 대한 치험 2례)

  • Objectives : The purpose of this study was to report the effects of Korean medicine therapy on patients with fibromyalgia syndrome. Methods : In this study, we used Korean medicine treatments, including acupuncture, bee venom therapy, chuna manual therapy, and others, to treat hospitalized patients diagnosed with fibromyalgia syndrome. Improvements in clinical symptoms were evaluated using a numeric rating scale (NRS), the short form McGill Pain Questionnaire (SF-MPQ), and changes in the number of tender points. Results : After Korean medicine treatment, the NRS scores reduced from 10 to 7 (Patient 1) and from 10 to 5 (Patient 2). The SF-MPQ scores decreased from 33 to 26 (Patient 1) and from 25 to 18 (Patient 2). The number of tender points reduced from 18 to 15 (Patient 1) and from 12 to 11 (Patient 2). Conclusions : In both patients, the NRS scores, SF-MPQ scores, and number of tender points significantly improved. The results suggest that Korean medicine treatments, including acupuncture, bee venom therapy, chuna manual therapy, and others, may be effective for treating fibromyalgia syndrome.

  • 연구 배경 최근 한국 및 중국에서 근골격계 질환에 대한 도침술의 활용이 늘고 있다. 하지만 현재까지의 임상연구는 대부분 증례보고 형태에 그치거나, 충분한 근거가 확보되지는 않은 실정이다. 우리는 요천추신경병증환자의 치료에 있어 도침치료와 통상한의치료와의 비교를 통해 도침치료의 유효성, 안전성, 경제성 평가를 시행하기 위해 본 연구를 기획하게 되었다. 연구 방법 본 연구는 예비임상 연구로, 다기관에서 진행되며, 무작위대조군, 활성 대조군을 통한 2개군 병행집단 연구로 평가자 맹검을 시행하게 된다. 이 연구에서는 총 50명의 요천추신경병증 환자를 대상으로 2개군으로 균등하게 분배하여 도침술 또는 통상한의치료를 시행하게 된다(각군당 25명). 도침술 그룹의 경우 협척혈에 도침치료를 시행 받게 되며, 통상 한의치료군은 연구자 판단하에 도침술을 제외한 적절한 한의치료를 시행 받게 된다. 두 개의 그룹은 주당 2회씩 총 3주간 치료를 받게 된다. 일차 유효성 평가는 요통으로 인한 기능장애를 평가하기 위한 Oswestry disability index를 통해 시행한다. 이차 유효성 평가지표로는 numeric rating scale, European Quality of Life 5-Dimension 5-Level, short-form McGill Pain Questionnaire, Roland-Morris Disability Questionnaire scores를 시행하게 된다. 고찰 본 예비임상연구의 결과는 추후 있을 요천추신경병증에 대한 도침치료 및 한의통상치료 비교효과 연구의 유효성, 경제성평가 본 임상연구를 위한 기초 정보 및 가능성을 확인하고 적절한 대상자수 산정에 도움이 될 것이다.
