국소진행성 폐암환자에 대한 전신항암화학요법이 운동부하 심.폐기능에 미치는 영향 : 초기변화를 중심으로 (Early Response of Cardiopulmonary Exercise Test in Patients with Locally Advanced Non-Small Cell Lung Cancer Treated with Systemic Chemotherapy)

  • 연구배경 : 폐암환자는 폐암자체 뿐만 아니라 만성기도질환을 비롯한 기존의 폐질환 및 심질환 등으로 폐기능이 감소한 경우가 많으며, 전신항암치료는 약제에 의한 부작용으로 환자들의 신체활동능력이 급격히 감소되는 것이 일반적이다. 운동은 안정상태에서는 알 수 없는심 폐기능 이상을 확인할 수 있는 좋은 방법으로 심혈관계 및 폐기능 이상은 이들 환자의 삶의 질을 결정하는 중요한 원인으로 작용할 수 있다. 본 연구는 전신항암치료 중인 국소진행성 폐암환자의 심 폐기능 변화를 안정시, 그리고 운동시 각각 측정하여 전신항암치료가 환자의 운동 능력에 미치는 영향을 평가하는 것이 목적이다. 방 법 : 조직화적으로 임상적 병기 IIIA의 비소세포암으로 진단되고 전신항암치료를 받고 있는 11명을 대상으로 치료 전, 그리고 매 주기 전신항암치료 직전 안정시 폐기능검사, Karnofsky performance scale, 체성분검사, 그리고 운동부하 심 폐기능검사를 실시하였다. 결 과 : 안정시 폐기능은 전신항암치료 전 후 유의한 변화는 없었으나 폐확산능의 경우 통계적인 의미는 없었지만 치료 전 기대값의 $89.7{\pm}34.1%$에서 치료 후 기대값의 $71.9{\pm}20.5%$로 상당히 감소하는 경향이 있었다. 호흡근의 기능을 평가하는 최대흡기압 및 최대호 기압 역시 전신항암치료 전 후 차이가 없었다. 전신 항암치료전 후 체성분 및 신체활동능력의 변화는 없었다. 3주기 전신항암화학치료 후 운동부하 심 폐 기능은 최대산소섭취량은 치료 전 기대값의 64.8%(17.9ml/kg/$m^2$)에서 치료 후 44.7% (12.6ml/kg/$m^2$)로 감소하였고 (p<0.05), 산소맥은 치료 전 기대값의 63.5% ($7.0{\pm}1.7ml$/beat)에서 치료 후 46.3% ($5.2{\pm}2.1ml$/beat)로 감소하였다(p<0.05). 결 론 : 전신항암치료 후 짧은 기간 내 체성분 변화와 안정시 폐기능의 저하는 없지만 최대산소섭취량 및 산소맥은 감소하여 운동에 대한 심기능 및 폐기능의 허용한계는 감소한다. 그러나 이러한 결론은 전신항암치료 후 짧은 기간 내의 변화로 장기적인 전향적인 연구가 필요하리라 생각한다.

실내 승마운동이 고령자의 기초체력 및 ADL 기능에 미치는 효과 (Effect of Basal Physical Fitness and ADL Function in the Aged According to Horse Riding Exercise)

  • The purpose of this study was to verify exercise effect of horse riding exercise according to estimate basal physical fitness and activities of daily living(ADL) function in the aged. Participants were nineteen peoples who have no impediment of activity. They performed horse riding exercise using SRider(Neipplus, Co., Korea) at sixty minutes a day. Exercise has progressed three days a week for eight weeks. We measured trunk flexion, sit up, whole body reaction, leg strength and maximal oxygen uptake as basal physical fitness. Also three meter gait, single stance with eyes opened and single stance with eyes closed as ADL function were estimated once a month. The result of legs strength and whole body reaction showed the higher significantly than before the exercise. Moreover, the result of three meter walking ability only increased significantly among the ADL function. This means that horse riding exercise might be activated continuous muscular contraction with maintained tonus of muscle. We thought that continuous movement of horse riding could be lead to isometric muscle contraction in lower limbs. Our study found that horse riding exercise could improve lower strengths and muscle reaction for exercise effect. Also we suggested that horse riding exercise could be adapted to exercise methods that could provide rehabilitation and treatment enough for the aged or disabled person.

The Effects of Exercise-based Cardiac Rehabilitation Phase II on Pro- and Anti-inflammatory Markers in Patients with Acute Coronary Syndrome

  • The aim of this study was to evaluate the effect of exercise-based cardiac rehabilitation on pro- and anti-inflammatory markers in patients with acute coronary syndrome (ACS). ACS patients who underwent percutaneous coronary intervention (PCI) and took medicine during phase II of rehabilitation were recruited for study. Subjects were divided into two groups; exercise group (EX, n=21) and a non-exercise group (non-EX, n=13). Supervised exercise program in hospital consisted of treadmill and bicycle exercise was performed three times per week for 6 weeks. Patients of EX received individual counseling, including knowledge of heart disease, risk factor modification, and physical training. Cardiopulmonary fitness, body composition, and biochemical blood factors were analyzed before and after experiment. There was no significant difference in serum levels of hs-CRP and TGF-${\beta}1$ between groups, and between time intervals. But there was a significant decrease in serum levels of IL-18 (P<.001). And there was a significant increase in ratio of IL-18 to IL-10 (P<.01) and serum levels of IL-10 (P<.001). After cardiac rehabilitation, there was significant increase in exercise duration (P<.001), maximal oxygen uptake ($VO_{2peak}$; P<.001) and decrease in submaximal rate-pressure product (sRPP; P<.05) in EX. In conclusion, exercise-based cardiac rehabilitation during phase II in patients with ACS after PCI decreased serum IL-18 (pro-inflammatory) content and ratio of IL-18 to IL-10 in serum (highly related with disease recurrence), and increased serum IL-10 (anti-inflammatory) content. In addition, it led to improved cardiopulmonary fitness.

Effect of the supplementation of fructose and taurine on energy metabolism during exercise

  • The purpose of this thesis is to investigate whether taurine supplementation in combination with fructose improves both energy metabolism and exercise capacity. Eight collegiate female subjects were recruited for the study. Each subject went through threecross-over designs: control(fluid), fructose, and taurine plus fructose supplementation trials. Subjects received taurine supplementation 100 mg/kg a day for two weeks. After the supplementation, all subjects take 10% fructose at 15 min prior to exercise, immediately before exercise, and every 15 min during exercise. Subjects received 150 ml fluid as placebo during the same procedure. The subjects performed submaximal exercise at the exercise intensity of 60% for 45 min and then 80% of maximal oxygen uptake (VO2max) until exhaustion time. A 10ml blood sample was taken for measuring the level of glucose, ammonia, lactate, free fatty acids, and insulin every 15 min during exercise at 60% of VO2max. The blood glucose levels was significantly higher at 45 min and 50 min exercise after supplementation of fructose, and immediately before exercise and 50 min exercise after taurine plus fructose compared to the placebo trial. However, the values tended to be lower in taurine plus fructose supplementation compared to the fructose trial. The levels of both lactate and ammonia were significantly lower compared to the placebo, while the exhaustion time was significantly increased. The level of free-fatty acids was significantly lower at 30, 45, and 50 min after fructoseand fructose plus taurine supplementation compared to the placebo trial. The level of glucagon was significantly lower at 15, 30, 45, and 50 min after fructose and fructose plus taurine supplementation compared to the placebo trial. There was no differences in insulin concentration among three treatments. This thesis concludes that combined taurine and fructose supplementation prior to exercise may improve exercise tolerance time and energy metabolism, lowering the muscle fatigue factors such as lactate and ammonia.

Effects of bone-specific physical activity on body composition, bone mineral density, and health-related physical fitness in middle-aged women

  • [Purpose] The study aimed to determine the effects of bone-specific physical activity on body composition, bone mineral density (BMD), and health-related physical fitness in middle-aged women. [Methods] One hundred eighty-six middle-aged women aged 31-49 years participated in this study. The subjects were divided into tertile groups according to the level of physical activity (low-score group, n=62; middle-score group, n=62; high-score group, n=62). Bone-specific physical activity participation was assessed using the bone-specific physical activity questionnaire. Body composition and BMD were measured using dual-energy X-ray absorptiometry. Health-related physical fitness test included isometric muscle strength (grip strength), muscular endurance (sit-ups), flexibility (sit and reach), and cardiorespiratory fitness (maximal oxygen uptake [VO2max]). [Results] The high-score group had a significantly higher fat-free mass (p=.045, partial eta-squared value[ηp2]=.033) than the middle- and low-score groups, whereas the high-score group had significantly lower percent body fat (p=.005, ηp2=.056) than the other two groups. Whole-body BMD (p=.034, ηp2=.036) and lumbar BMD (p=.003, ηp2=.060) were significantly higher in the high-score group than in the low-score group. The high-score group performed significantly better for grip strength (p=.0001, ηp2=.101), sit-ups (p=.0001, ηp2=.108), and VO2max (p=.0001, ηp2=.092) than the other two groups. [Conclusion] The present study suggests that bone-specific physical activity could be useful in improving body composition, BMD, and health-related physical fitness in middle-aged women, significantly enhancing their BMD and health conditions.

융복합을 활용한 휠체어 트레드밀과 암에르고미터 점증부하운동검사 시 심폐기능, 운동자각도 및 젖산농도 비교 (A Comparison of Cardiopulmonary Function, RPE, and Blood Lactate following in Wheelchair Treadmill and Arm Ergometer GXT Test through Convergence)

  • 본 연구는 비장애인 휠체어 농구 선수 11명을 대상으로 휠체어트레드밀과 암 에르고미터의 점증부하운동검사 시 심폐기능, 운동자각도 및 젖산 농도를 비교하는데 목적이 있다. 연구방법으로는 휠체어트레드밀과 암 에르고미터의 점증부하운동검사 시 호흡가스분석기와 무선 심박수 측정기를 사용하여 심폐기능을 알아보았으며, 보그 스케일을 이용하여 운동자각도를 측정하였고, 젖산 분석기를 사용하여 안정 시, 운동직후, 회복기 2분, 4분, 6분, 10분의 혈중 젖산 농도를 측정하였다. 측정된 데이터는 SPSS 18.0 프로그램을 통하여 종속 t-검증(paired t-test)을 실시하여 분석하였고 통계적 유의수준은 .05로 설정하였다. 이 연구를 통하여 얻어진 결과는 다음과 같다. 첫째, 심폐기능 변인 중 최대산소섭취량은 암 에르고미터가 높게 나타났으며, 호흡교환율은 휠체어트레드밀이 높게 나타났으며, 최대 심박수는 휠체어트레드밀이 높게 나타났다. 둘째, 주관적 운동 강도에서는 종료 점에서 암 에르고미터에서 높게 나타났다. 셋째, 혈중 젖산 농도는 운동 직후 암 에르고미터에서 높게 나타났다.

운동부하심폐기능검사상의 분당최대산소섭취량과 체내 지방분포와의 상관관계 (The Relationship of $VO_2$Max/Min in Cardiopulmonary Exercise Test and Fat Distribution)

  • 배경 : 운동부하심폐기능검사는 운동 장애의 조기진단과 환자의 심폐 예비능력을 측정할 수 있는 유용한 방법이다. 또한 비만이 최대운동의 제한인자로 작용함이 알려져 있다. 따라서 본 연구에서는 운동부하심폐기능검사와 체성분분석기를 통하여 비만과 비만의 부위가 최대 운동능력에 미치는 영향을 살펴보고자 하였다. 방법 : 건강한 자원자 42명을 대상으로 하여 체성분분석기 Inbody 2.0 (Biospace Co)와 bicycle ergometer 2900 model(Sensormedic Co, USA)을 이용하여 체생분분석과 운동부하심폐기능검사를 시행하였다. 결과 : 전체 대상군에서 $VO_2$max/min/kg은 비만도와 의미있는 역상관관계를 보였고, 체지방율과도 통계적으로 의미있는 역상관관계를 보였다. 전체 대상군에서는 $VO_2$max/min/kg이 복부지방률과 통계적으로 유의한 상관관계를 보이지 않았지만, 남자대상군에서는 $VO_2$max/min/kg와 복부지방률이 통계적으로 유의한 역상관관계를 보였다. 결론 : 비만이 운동에 제한인자로 작용한다는 것을 확인하였고, 남자에서는 비만 중에는 특히 북부미만이 운동능력에 제한적인 영향을 미친다는 사실을 확인하였다. 여성 대상자에서는 복부비만의 기준을 만족하는 대상자가 적어서 복부비만의 영향평가가 어려웠다. 이상의 결과로 임상에서 운동부하심폐기능검사 결과 판독시에는 비만과 비만부위에 의한 영향을 고려하는 것이 필요할 것으로 판단된다.

운동 강도의 차이가 제2형 당뇨병 환자와 비만인의 초과산소 섭취량, 안정 시 대사량 및 생화학적 변인에 미치는 영향 (Effects of Different Exercise Intensity on Excess Post Exercise Oxygen Consumption (EPOC), Resting Metabolic Rate (RMR), and Biochemical Variables in Obese and NIDDM Patients)

  • The purpose of this study was to evaluate the effect of various exercise intensity on Excess post exercise energy expenditure (EPEE), Resting Metabolic Rate (RMR),thyroid hormonal changes and biochemical variables in obese and NIDDM patients. The subject of the present study were divided into four groups and four periods: trained (T; n=10), control (C; n=10), obese (O; n=10) and NIDDM (N; n=10) group. And the periods were divided as follows; Resting (RE), Maximal (MA), High intensity (HI), and Low intensity (LI). There was significant difference in RMR among different intensity of exercise. in the T (p<0.05) not in the C, O, and N groups. however, there was no significant different percent body fat among all groups. In the energy expenditure, there was significant different among C, O, N groups compare to T in HIEE (high intensity exercise energy expenditure), LIEE (low intensity exercise energy expenditure), HIEEPE (high intensity exercise energy expenditure post exercise) and LIEEPE (low intensity exercise expenditure post exercise). In the hormonal level, there was significant different in T4 level in the T group at LI period and there was also significant difference in T4, Free T3, & Free T4 levels in T group at LI period, however there was no significant different in the O and N groups except LI period. In the fatigue variables, there was significant different in lactate and ammonia levels in the N group in the period of HI compare to C. The present cross-sectional study was design to investigate the relationship between exercise intensity and RMR in four groups. The focus of this investigation was to compare RMR in aerobically trained (T), control (C), obese (O) and NIDDM (N) group. The relationship among RMR, exercise intensity and percent body fat would best be investigated using Meta Lyzer 3B, MMX3B and body composition analyzer. Each subject completed measurement of percent body fat, RMR, hormone in the period of maximal oxygen uptake exercise (MA), high intensity exercise (HI), and low intensity exercise (LI). From the results, High and Low intensity of exercise, there was a trend for an increased RMR (kcal/day) in the trained groups and control group (in case of LI) not for the obese and N groups. This is best explained not by the reduced percent body fat but by the highly induced energy expenditure (during exercise and post exercise energy expenditure) and increased T4, Free T3, and Free T4 hormonal levels in the low intensity exercise for the T group and sometimes C group.

Muscle oxygenation, endocrine and metabolic regulation during low-intensity endurance exercise with blood flow restriction

  • [Purpose] The present study investigated the effect of endurance exercise with blood flow restriction (BFR) performed at either 25% maximal oxygen uptake (${\dot{V}}O_2$ max) or 40% ${\dot{V}}O_2$ max) on muscle oxygenation, energy metabolism, and endocrine responses. [Methods] Ten males were recruited in the present study. The subjects performed three trials: (1) endurance exercise at 40% ${\dot{V}}O_2$ max without BFR (NBFR40), (2) endurance exercise at 25% ${\dot{V}}O_2$ max with BFR (BFR25), and (3) endurance exercise at 40% ${\dot{V}}O_2$ max with BFR (BFR40). The exercises were performed for 15 min during which the pedaling frequency was set at 70 rpm. In BFR25 and BFR40, 2 min of pressure phase (equivalent to 160 mmHg) followed by 1 min of release phase were repeated five times (5 × 3 min) throughout 15 minutes of exercise. During exercise, muscle oxygenation and concentration of respiratory gases were measured. The blood samples were collected before exercise, immediately after 15 min of exercise, and at 15, 30, and 60 minutes after completion of exercise. [Results] Deoxygenated hemoglobin (deoxy-Hb) level during exercise was significantly higher with BFR25 and BFR40 than that with NBFR40. BFR40 showed significantly higher total-hemoglobin (total-Hb) than NBFR40 during 2 min of pressure phase. Moreover, exercise-induced lactate elevation and pH reduction were significantly augmented in BFR40, with concomitant increase in serum cortisol concentration after exercise. Carbohydrate (CHO) oxidation was significantly higher with BFR40 than that with NBFR40 and BFR25, whereas fat oxidation was lower with BFR40. [Conclusion] Deoxy-Hb and total Hb levels were significantly increased during 15 min of pedaling exercise in BFR25 and BFR40, indicating augmented local hypoxia and blood volume (blood perfusion) in the muscle. Moreover, low-and moderate-intensity exercise with BFR facilitated CHO oxidation.

운동 트레이닝을 통한 심폐체력 반응의 차이가 복부비만 여성의 심혈관계 위험요인과 아디포싸이토카인에 미치는 영향 (Effects of different cardiorespiratory fitness response to exercise training on cardiovascular disease and adipocytokine in abdominal obesity women)

  • 본 연구는 복부비만 여성을 대상으로 운동 트레이닝을 통해 심폐체력의 반응의 차이가 심혈관계 위험요인 및 아디포싸이토카인 분비 및 유전자 발현에 미치는 영향을 알아보는 데 그 목적이 있다. 본 연구는 운동 트레이닝 실시 후 심폐체력 향상의 유무를 이용하여 복부비만 여성 총 48명을 운동-반응 집단(n=34)과 운동-비반응 집단(n=14)으로 나누었다. 운동 트레이닝은 주당 1200kcal를 소모하도록 설계되었으며, 중등도-활발한 운동강도, 걷기나 간단한 조깅의 형태로 12주간 실시하였다. 측정 변인으로 실험 참가 전, 후에 신체조성, 혈압, 심폐체력, 혈중지질, 글루코스, 인슐린 그리고 유리지방산을 측정하였으며, 혈중 아디포싸이토카인과 복부지방 아디포싸이토카인 유전자 발현을 분석하였다. 본 연구의 결과 운동 트레이닝 후 두 집단 모두 비만지표에서 통계적으로 유의하게 감소하였지만, 허리둘레(p=0.040), 체지방률(p=0.031) 그리고 중성지방(p=0.023)과 수축기 혈압(0.046)은 운동-반응 집단과 운동-비반응 집단에서 상호작용 효과를 보였다. 혈중 렙틴은 두 집단에서 상호작용 효과(p=0.022)가 나타났으나 복부지방 렙틴 유전자는 두 집단 모두에서 트레이닝 후(p<0.001)에 유의하게 감소하였다. 이상의 결과를 종합해보면 12주간 운동 트레이닝 실시 후 심폐체력의 향상을 보인 운동-반응 집단은 운동-비반응 집단에 비해 심혈관계 질환 위험요소와 아디포싸이토카인 분비 및 유전자 발현에 더 효과적인 것으로 나타났다. 또한 운동 트레이닝 실시 후 심폐체력의 개선이 없었던 운동-비반응 집단에서도 비만지표와 혈중 렙틴 및 렙틴 유전자 발현이 감소되는 것을 살펴봤을 때 규칙적인 운동 자체가 비록 심폐체력의 개선이 없더라도 인간의 건강상의 이점을 주는 것으로 나타났다. 따라서 심폐체력이 향상되지 않더라도 규칙적인 운동 은 비만 관련 위험요인의 감소 및 예방에 좋은 영향을 주는 것으로 사료된다.
