Autogenous bone graft is the useful technique for management of various bone defect in oral and maxillofacial surgery. The most common site for bone graft harvest is the anterior iliac crest. There is usually considerable cancellous bone graft available and it can be obtained with minimal morbidity. However, complications noted in iliac crest grafts include prolonged postoperative pain, hematoma and fracture, gluteal muscle weakness. Occasionally, when large amounts of bone graft are needed and previous harvest procedure had used, iliac bone harvest may be not adequate. Like the iliac crest, the greater trochanter has abundant cancellous bone and is readily accessible with acceptable morbidity. The purpose of this study was to assess the availability of cancellous bone graft from the greater trochanter, compare the quantity with that available from the anterior iliac crest, investigate anatomical hazards, and make recommendations for consistent harvest.
Mucormycosis is a rare, invasive fungal infection that progresses aggressively and requires prompt surgery and appropriate treatment. The number of cases of mucormycosis in coronavirus disease 2019 (COVID-19) patients has recently increased, and patients with uncontrolled diabetes mellitus are particularly at an elevated risk of infection. This report presents a case of mucormycosis-related osteomyelitis of the maxilla in a 37-year-old man with diabetes mellitus. The patient complained of severe and persistent pain in the right maxilla, accompanied by increased tooth mobility and headache. On contrast-enhanced computed tomographic images, gas-forming osteomyelitis of the right maxilla was observed. Destruction of the maxilla and palatine bone then proceeded aggressively. Sequestrectomy was performed on the right maxilla, and the histopathological diagnosis was mucormycosis. Further investigation after the first operation revealed the patient's history of COVID-19 infection.
본 연구는 치과내원환자 334명을 대상으로 치과공포 및 행태에 따른 구강악안면증상을 알아보고자 연구를 시행하였다. 성별과 연령, 치과이용행태 등의 일반적 특성 5문항, 20개문항의 DFS, 스트레스 증상설문지 (Stress Symptom Questionnaire) 20문항을 이용하여 설문조사한 후 얻어진 자료들을 SPSS 12.0을 사용하여 검정하였다. 분석한 결과는 다음과 같다. 1. 여성이 남성보다 공포가 높았고, 연령은 30세 이상에서 공포가 높게 나타났으며, 통계적으로 유의하게 나타났다(p<0.05). 2. 치과진료회피반응, 생리적 반응, 공포유발자극요인 중에서 치료자극반응이 가장 높았으며, 성별과 비교해서는 여성이 모두 높은 점수를 보였으며, 통계적으로 매우 유의하였다(p<0.01). 3. 구강악안면증상부위에 따른 반응정도는 기타 구강외 증상이 62.9%로 가장 높았으며, 항목으로는 뒷머리가 당기거나 목덜미가 뻣뻣하다가 가장 높은 것으로 나타났다. 4. 일반적 특성에 따른 구강악안면증상은 성별에서 혀에서만 유의하였고(p<0.05), 연령은 30세 이상에서 입술이나 볼안쪽, 기타 구강외증상이 더 높게 나타났다. 5. 정기검진을 하는 경우, 스켈링경험이 있는 경우, 마취경험이 있는 경우, 내원횟수가 많은 경우 구강악안면증상이 높았으며, 통계적으로는 스켈링경험이 있는 경우와 내원횟수가 많은 경우가 혀에서만 유의하게 나타났다(p<0.05). 6. 공포정도에 따른 구강악안면증상은 고공포 그룹에서 각 부위별 구강악안면증상이 높게 나타났고, 통계적으로도 모두 유의하게 나타났으며, 공포와 각 구강악안면증상 간에 양의 상관관계를 나타내었으며 통계적으로도 유의하였다(p<0.05).
오늘날 삶의 질에 대한 관심이 고조되면서 일반 대중에게서도 레져 및 스포츠가 활성화됨에 따라 구강악안면영역 외상의 발생빈도도 높아지고 있다. 이러한 구강악안면영역의 외상을 예방하기 위해 구강보호장치 사용의 중요성이 커지고 있으며, 그 중 가장 많이 사용되는 것이 마우스가드이다. 마우스가드는 1) 치아에 가해지는 충격을 흡수하고 변형시켜 치아의 손상을 방지; 2) 입술, 혀, 치은 등 연조직의 열상을 예방; 3) 충격의 발생 시 반대측 치아가 접촉되는 것을 예방; 4) 하악골을 안정화시켜 하악각 또는 하악과두의 파절을 유발할만한 충격을 흡수시킴; 그리고 5) 목 또는 뇌손상을 예방할 수 있다. 이렇듯 마우스가드가 구강악안면영역의 외상방지에 효과적이지만, 운동선수나 일반 대중들에게는 널리 보급되어 있지 않고 그 중요성에 대한 인식률도 저조한 실정이다. 레져나 스포츠 활동 시 나타날 수 있는 구강악안면 외상 예방에 가장 효과적인 마우스가드의 종류, 재료, 제작 시 고려할 사항 등을 알아보고, 사용실태에 대한 고찰을 통해 마우스가드의 중요성에 대한 인식과 대중화에 도움이 되고자 한다.

Microglial cell activation is known to contribute to neuropathic pain following spinal sensory nerve injuries. In this study, I investigated its mechanisms in the case of trigeminal sensory nerve injuries by which microglial cell and p38 mitogen-activated protein kinase (p38 MAPK) activation in the medullary dorsal horn (MDH) would contribute to the facial pain hypersensitivity following mandibular nerve transection (MNT). And also investigated the changes of trigeminal ganglion neurons and ERK, p38 MAPK manifestations. Activation of microglial cells was monitored at 1, 3, 7, 14, 28 and 60 day using immunohistochemical analyses. Microglial cell activation was primarily observed in the superficial laminae of the MDH. Microglial cell activation was initiated at postoperative 1 day, maximal at 3 day, maintained until 14 day and gradually reduced and returned to the basal level by 60 days after MNT. Pain hypersensitivity was also initiated and attenuated almost in parallel with microglial cell activation pattern. To investigate the contribution of the microglial cell activation to the pain hypersensitivity, minocycline, an inhibitor of microglial cell activation by means of p38 MAPK inhibition, was administered. Minocycline dose-dependently attenuated the development of the pain hypersensitivity in parallel with inhibition of microglial cell and p38 MAPK activation following MNT. Mandibular nerve transection induced the activation of ERK, but did not p38 MAPK in the trigeminal ganglion. These results suggest that microglial cell activation in the MDH and p38 MAPK activation in the hyperactive microglial cells play an important role in the development of facial neuropathic pain following MNT. The results also suggest that ERK activation in the trigeminal ganglion contributes microglial cell activation and facial neuropathic pain.
Temporomandibular ankylosis is defined as a situation in which the condyle is fused to the fossa by bone or fibrous tissue. Conditons such as trauma, infection, or systemic disease may predispose to various types of ankylosis, bringing about different levels of limitation in mandibular movement. Most patients with temporomandibular ankylosis are associated with limitation of maximal mouth opening, deviation of the chin toward the affected side, impaired occlusion, chronic pain, compromised oral hygiene, severe facial asymmetry & impeded mandibular molar eruption occurring in childhood. Several techniques to release ankylosis have been described in the literature, showing variable and often unsatisfactory results. The most frequently used operations are gap arthroplasty, interpositional arthroplasty, and exicision and joint reconstruction with autogenous or alloplastic materials. We have managed the two patients of TMJ ankylosis. They had previously TMJ surgery and we treated with gap arthroplasty & active physial therapy. We have obtained favorable results and report these cases with literatures review.
Cholesterol granuloma is an unusual clinical entity described as an inflammatory granulation in response to the deposit of cholesterol crystals. It can develop in any portion of air cells within the temporal bone as a result of a lack of aeration and inadequate drainage, especially in the middle ear cavity. Here, we report very unusual three cases of cholesterol granuloma developed in mandible. In the first case a 68-year-old male with a large mass arising from the mandible was observed. Panoramic radiograph and computed tomography scans revealed a huge expanding lesion in the mandible. In the second case a 47-year-old female with a cystic lesion in the mandible was observed. And in the third case a 19-year-old male complaining atypical facial pain had a large lesion in the mandibular ramus. The histopathologic examinations of the cases showed numerous cholesterol crystals surrounded by multinucleated foreign body giant cells.

Fractures of the mandible occur with a greater frequency in the elderly. This study reports three cases of edentulous atrophic mandible fracture in elderly patients treated with open reduction technique. Three patients who presented with edentulous atrophic mandible fractures underwent surgical management using open reduction and internal fixation. After treatment, clinical evaluations and postoperative complications were examined with postoperative x-ray. Patients were followed with clinical and radiographic examinations. In the postoperative clinical evaluation, two male patients healed well, but one female patient complained of pain and swelling. In radiographic examinations, no union delay or lack of fusion was observed in the edentulous area. Open reduction technique is a viable treatment option for the edentulous atrophic mandible fractures in geriatric patients.
As carotid-cavernous fistula is the major complication that can be occurred uncommonly after maxillofacial injuries, it is abnormal arteriovenous communication between cavernous sinus and internal carotid artery. Such an arteriovenous communication is most often the result of injuries, but need not be associated with bony fracture. It usually begins soon after an injury, but it may be delayed for as long as several months. It begins undramatically with eye pain, headache and slow protrusion of eye ball. A bruit may be heard above the eye with stethoscope. Close examination will reveal dilatation of superficial veins of the eyelid and forehead and periorbital edema. There will be complete or partial ophthalmoplesia of the affected eye. Compression of the common carotid artery on the ipsilateral side will reduce or obliterate the bruit. The lesion in the cavernous sinus is them demonstrable by angiography.
Osteochondroma is the one of the most benign tumors of the axial skeleton, but is rarely found in the facial bones. Typical facial features of condylar osteochondroma include striking facial asymmetry, malocclusion with openbite on the affected side, and prognathic deviation of the chin and crossbite to the contralateral side. In this case, twenty four year-old female showed facial asymmetry, chin deviation, openbite on the affected side but have no symptoms of pain or dysfunction. Concomitantly she had maxillary occlusal cant and hemimandibular hypertrophy. Panoramic radiograph showed radiopaque mass on right mandibular condyle extended along the lateral pterygoid muscle. Computed tomogram demonstrated enlarged condylar head and bony spur on posteromedial side of condyle and 99Tc bone scintigraphy showed a focal hot image. These findings were correspond with osteochondroma. The lesion was treated with condylectomy and residual facial asymmetry was corrected with 2-jaw orthognathic surgery. Herein, we report a case of osteochondroma of the mandibular condyle and accompanying facial asymmetry.
