Color distribution of maxillary primary incisors using a dental spectrophotometer in Korean children (치과용 분광광도계를 이용한 상악 유전치의 색조 분포에 관한 연구)

  • The purpose of this study was to analyze the color distribution of the maxillary primary central and lateral incisors using a dental spectrophotometer. Color measurements of maxillary primary central and lateral incisors of 32 pediatric patients aged 2 to 6 years were performed using SpectroShade Handy Dental Type 713000 (Serial No. HDL2678, MHT, Verona, Italy) by one researcher in the same clinic. CIE L*, a*, b* values of total surfaces and cervical, middle, incisal region were recorded. L* values were higher and a* values were lower in the primary central incisors than those in primary lateral incisors, but b* values didn't show statistical difference. L* values of the middle region were the highest, and a* and b* values decreased from the cervical region to the incisal region. There were significant color differences between each region, and ΔE*ab between each region were greater than ΔE*ab between the primary central and lateral incisors. For esthetic restorations of primary incisors, it is necessary to consider the color differences between each region rather than those between the primary central and lateral incisors.

Color Comparison of Maxillary Primary Anterior Teeth and Various Composite Resins using a Spectrophotometer (분광광도계를 이용한 상악 유전치부와 다양한 복합레진 색조의 비교)

  • The purpose of this study was to compare the color of maxillary primary central incisors, lateral incisors, canines with the color of various composite resins using a spectrophotometer VITA Easyshade®V. One researcher measured the color of sound maxillary primary anterior teeth of 100 children aged 1 to 6 and specimens which were made by composite resins with a total of 10 types and 31 shades, and CIE L*, a*, b* values were obtained. There was no clinically recognizable color difference between the maxillary primary central and lateral incisors. However, the maxillary primary canines showed difference in color above the clinical acceptability threshold with the primary central and lateral incisors. These findings showed no significant color differences between men and women, and no significant color changes with age. A1 shade is the most common in all maxillary primary anterior teeth. Composite resins, which are thought to be similar to the color of maxillary primary anterior teeth were selected from 9 types of maxillary primary central incisors, 6 types of primary lateral incisors, 6 types of primary canines.

  • Physiologic root resorption occur from the apex of the primary teeth close to the permanent teeth towards the apex. Pathologic root resorption occur on surfaces of root due to causes such as trauma, replantation, orthodontic treatment, delayed or irregular eruption of teeth, or growing cysts or tumors. In children, the most frequently affected teeth from trauma are the maxillary primary central incisors. After such an event, root resorption initiate from the traumatized pulp or periodontium. In this case report, periapical radiographs were used to evaluate the features of pathologic root resorption by studying patients with history of trauma. The following results were noted. 1. Pathologic root resorption was observed in various patterns in all of the 10 traumatized maxillary primary central incisors. 2. Though teeth with pathologic root resorption were treated with pulpectomy, the resorption process continued to progress.

  • This study was performed to investigate the changes of dento-alveolar relationship in relation to the eruption of in cisors using roentgenocephalometry. The Subjects consisted of 76 females from 4 to 8 years with normal occlusion. The following conclusions were obtained: 1. When maxillary and mandibular permanent central incisors were erupted, maxillary alveolar prognathism was increased, while mandibular alveolar prognathism was not changed. 2. Axial inclination of permanent incisors was greater than that of primary incisors.

  • The purpose of this study is to determine the developmental relationship between the maxillary primary central incisors and their permanent successor. The auther took 315 cases of lateral cephalogram of the children (males were 171, females were 141) Angular change of the teeth and horizontal and vertical linear change were observed. The obtained results were as follows: 1. The inclination of the long axes of both incisor teeth was relatively stable and labio-version of both incisore was significant at 7 years of age. 2. The distance between the incisal edge of the permanent central insisor and the resorbing apex of the primary maxillary central incisor remained within 2mm of each other, 3. Vertical growth of the maxillary anterior portion was greater than horizontal growth from 6 to 7 years of age. 4. There was not a significant sexual difference.

  • Trauma to the primary incisors may cause developmental or eruptional disturbance of the permanent successors. Two cases illustrate developmental disturbances of arrested root formation, dilaceration, and eruptional disturbances of impaction, ectopic eruption of permanent successors caused by traumatic injury to deciduous incisors. The patient of the first case suffered trauma at the age of 4 years 7 months, causing alveolar bone fracture including the maxillary right primary central and lateral incisors that were immedi-ately extracted. The second patient had trauma episodes at the age of 3 years. Avulsion of the maxillary primary right central and lateral incisors were occurred due to trauma. After such trauma, regular follow-up including radiographs is necessary to detect early any possible interference with normal eruption of permanent successors.

Unilateral maxillary central incisor root resorption after orthodontic treatment for Angle Class II, division 1 malocclusion with significant maxillary midline deviation: A possible correlation with root proximity to the incisive canal

  • Root resorption can be caused by several factors, including contact with the cortical bone. Here we report a case involving a 21-year-old female with Angle Class II, division 1 malocclusion who exhibited significant root resorption in the maxillary right central incisor after orthodontic treatment. The patient presented with significant left-sided deviation of the maxillary incisors due to lingual dislocation of the left lateral incisor and a Class II molar relationship. Cephalometric analysis demonstrated a Class I skeletal relationship (A point-nasion-B point, 2.5°) and proclined maxillary anterior teeth (upper incisor to sella-nasion plane angle, 113.4°). The primary treatment objectives were the achievement of stable occlusion with midline agreement between the maxillary and mandibular dentitions and appropriate maxillary anterior tooth axes and molar relationship. A panoramic radiograph obtained after active treatment showed significant root resorption in the maxillary right central incisor; therefore, we performed cone-beam computed tomography, which confirmed root resorption along the cortical bone around the incisive canal. The findings from this case, where different degrees of root resorption were observed despite comparable degrees of orthodontic movement in the bilateral maxillary central incisors, suggest that the incisive canal could be an inducing factor for root resorption. However, further investigation is necessary to confirm this assumption.

Facial and occlusal esthetic improvements of an adult skeletal Class III malocclusion using surgical, orthodontic, and implant treatment

  • The aim of this clinical report is to describe the complex treatment of an adult Class III malocclusion patient who was disappointed with the outcome of a previous oral rehabilitation. Interdisciplinary treatment planning was performed with a primary indication for implant removal because of marginal bone loss and gingival recession, followed by orthodontic and surgical procedures to correct the esthetics and skeletal malocclusion. The comprehensive treatment approach included: (1) implant removal in the area of the central incisors; (2) combined orthodontic decompensation with mesial displacement and forced extrusion of the lateral incisors; (3) extraction of the lateral incisors and placement of new implants corresponding to the central incisors, which received provisional crowns; (4) orthognathic surgery for maxillary advancement to improve occlusal and facial relationships; and finally, (5) orthodontic refinement followed by definitive prosthetic rehabilitation of the maxillary central incisors and reshaping of the adjacent teeth. At the three-year follow-up, clinical and radiographic examinations showed successful replacement of the central incisors and improved skeletal and esthetic appearances. Moreover, a Class II molar relationship was obtained with an ideal overbite, overjet, and intercuspation. In conclusion, we report the successful esthetic anterior rehabilitation of a complex case in which interdisciplinary treatment planning improved facial harmony, provided gingival architecture with sufficient width and thickness, and improved smile esthetics, resulting in enhanced patient comfort and satisfaction. This clinical case report might be useful to improve facial esthetics and occlusion in patients with dentoalveolar and skeletal defects.

A Morphometric Study of Primary Anterior Zirconia Crowns in Korean Tooth Models (한국 유치 모델에서 유전치 지르코니아 크라운의 형태계측학적 연구)

  • The purpose of this study was to provide clinical recommendations for restoration with selection of the most similar zirconia crown by 3-dimensional analysis of the shape of the maxillary primary central and lateral incisors in Korean individuals and prefabricated zirconia crowns. The average shape of the sound maxillary primary central and lateral incisors in 300 children was reproduced by 3-dimensional scanning. Zirconia crowns of 4 manufacturers (NuSmile $ZR^{(R)}$ Crown, Cheng $Crowns^{(R)}$, Kinder $Krowns^{(R)}$, and EZ $Pedo^{(R)}$ Crown) were scanned 3-dimensionally, and coordinates for comparison of the shape were measured to evaluate the similarity between the teeth and crowns. The most similar crowns were selected by comparing the mesiodistal length, crown height, crown shape ratio, distance between the same coordinates of a tooth and crown, the radius of curvature of the labial surface, and the volume. As a result of analysis, Cheng $Crowns^{(R)}$ size 3 and NuSmile $ZR^{(R)}$ Crown size 2 were the most similar crowns in the maxillary primary central and lateral incisors, respectively. Scanning the inner surface of the crowns and evaluating the amount of tooth reduction required suggested that an overall lesser amount of tooth reduction compared to that presented by the manufacturer's guidelines should be performed.

Color Distribution Analysis of Maxillary Primary Central Incisors using a Spectrophotometer (분광광도계를 이용한 상악 유중절치의 부위에 따른 색조 분포 연구)

  • The objective of this study was to analyze the color distribution of the maxillary primary central incisors and the relationship between the color of each tooth region by dividing tooth into incisal, middle and cervical region using a spectrophotometer. The subjects were 105 sound maxillary primary central incisors from 3 to 6 years old children. Color measurements were performed by one dentist using same protocol. CIE $L^*$, $a^*$, $b^*$, $C^*$, $h^*$ data and the best matched shades with coverage error were obtained for each region and whole labial surface. There was a significant difference in CIE $L^*$, $a^*$, $b^*$ according to regions and the difference in color between each region was more than the clinical acceptance. By correlating the relationship between the color of each region with $L^*$, $a^*$, $b^*$ values, it is possible to extrapolate the color of other region with one color or to be used as data for making restorative materials and prostheses that are more aesthetically pleasing for primary teeth.
