  • The anterior maxillary incisor is the most traumatized region in the mouth and trauma is frequent between the ages of 8-10. Traumatic loss of teeth, can lead to many complications in children. Thus, as possible to keep traumatic teeth, but if you need extractions There may be. Complications occur and early tooth loss is frequent. Complications of early loss of central incisors are esthetic compromise, loss of vertical and horizontal width, height, contour of alveolar bone, tilting of adjacent teeth, arch length loss. Alveolar bone loss may affect normal function and stability, and results in esthetic problem for future prosthesis restoration. The 9-year-old girl and 6-year-old boy got early loss of upper central incisor. The amount of alveolar bone resorption was measured using cone beam computed tomograph and cast analysis.

INCIDENCE OF SINUS SEPTA AND ACCURACY OF PANORAMIC RADIOGRAPH (상악동 중격의 발생율과 파노라마 방사선 사진의 정확도)

  • Purpose: The purpose of this study was to determine the incidence of antral septa and the accuracy of panoramic radiograph in identifying maxillary sinus septa. In addition, when panoramic radiograph led to a false diagnosis of more than majority, we analyzed findings of the panoramic radiograph. Patients and methods: This study included 180 patients who were radiographically examined before the surgery was done using both panoramic and computerized tomographic radiographs(CT scan), Samsung Medical Center from April 2003 to March 2006, and examined the incidence of antral septa, the false diagnosis rate of panoramic radiograph, and radiographic characters in case of false diagnosis. Only those bone lamellae were considered as septa that showed a height of at least 3.0mm. It was thus possible to exclude the alveolar recess. One oral and maxillofacial surgeon(OMFS) determined the presence or absence of sinus septa in CT scan, and five OMFS determined the presence or absence of sinus septa in panoramic radiograph. Results: The septa were observed in 81 of 360 sinuses(22.5%). All septa were oriented in a buccopalatal plane. More or less greater prevalence was observed in the second molar region(38.2%) but no predominant location was found. Panoramic radiograph led to false diagnosed septa in 361 of 1800 cases(20.1%). In case of false diagnosis of more than majority, superimposed image on zygomatic process was 44.1%, faint image in the region not related to sinus lifting 26.5%, faint image in the region related to sinus lifting 17.6%, and misconception for superimposed image 11.8% Conclusion: In this study we could get clinico-anatomical information of septum in the maxillary sinus. Compared to CT scan, panoramic radiograph can not clearly differentiate the sinus septa, but we consider that panoramic radiograph may improve its usefulness if we take additional modified panoramic radiograph and clinical exam.

Long-term evaluation of implant placed in sites grafed by lateral window approach on maxillary sinus;a 10-year retrospective study (측방 접근법에 의한 상악동 거상술을 이용하여 식립된 골내 임플란트의 10년간 후향적 연구)

  • Between 1997 May and 2007 May, One hundred and seventeen patients were treated. There were 129 cases of sinus elevation using a lateral window opening procedure and 258 implants placed simultaneously or delayed. The cumulative survival rate of the implants calculated. The implants were evaluated according to surgical site, quality and quantity of bone, graft material, membrane used, the length and diameter of the implant and complications. 1. The 10-year cumulative survival rate of the implants by sinus augmentation using lateral window approach was 96.90%. 2. There was no difference in the survival rate between the implant placed simultaneously with sinus elevation (one stage) and the procedure performed in the delayed procedure (two stage). 3. There was no difference in the survival rate according to the type and amount of graft materials. 4. There was no difference in the survival rate according to the implant site, bone quality and quantity. 5. There was no difference in the survival rate when the $CollaTape^{(R)}$ or Gore-Tex was placed in the window of the lateral wall. 6. There was no difference in the survival rate of the implant length and diameter. 7. The survival rate was as low as 75.00% when there were more than two complications. Implant placement with sinus augmentation using the lateral window approach is a predictable treatment method. Although the vertical height of residual ridge is insufficient and the quality of bone is poor, the normal survival rate of the implants would be expected if an appropriate graft material and membrane is used with greater effort to prevent complications.

A Cephalometric Study of Lateral Morphologic Features in Adult Cleft Lip and Palate Patients (구순 구개열 환자의 성장후 측모형태에 관한 두부계측방사선학적 연구)

  • Background: Cleft lip and palate deformity have unknown patterns of maxillofacial growth and development. The maxillofacial growth can be affected either by congenital or environmental factors such as infection and trauma. Surgical repair of cleft lip and palate may interfere the subsequent growth and development of maxillofacial region. The purpose of this study is to evaluate the characteristics of maxillofacial growth patterns in adult cleft lip and palate patients. Materials and Methods: The material for this study consisted of 17 adult male patients with cleft lip and palate. Cephalometric tracing and measurements were done by one investigator. The relationship between 17 cleft lip and palate patients and Korean norms were evaluated statistically. Results: There were statistically differences in Na. perpendicular to point A, SNA angle, effective maxillary length, maxillofacial differencial. Wit's appraisal and upper incisor to point A(p < 0.01). Pogonion to Na. perpendicular also statistically differed(p < 0.05). Other measurements didn't statistically differ. Conclusion: It was evident that in adult cleft lip and palate patients, maxilla was retruded and short. Careful cleft lip and palate repair and treatment are recommended for facilitating normal growth of maxilla.

  • Dental endosseous implants require sufficient alveolar bone volume and quality for complete bone coverage and initial stability. But, atrophy or resorption of alveolar bone height and width according to patient's age and period of tooth loss can prevent ideal implant placement. Bone graft procedure has been proposed before or simultaneously with the placement of dental implants in patients with insufficient alveolar bone volume. While allografts, xenografts, and alloplastic bone grafts have been proposed and studied for alveolar ridge augmentation, the use of autogenous bone grafts represents the 'gold standard' for bone augmentation procedures. Conventional bone grafts are usually harvested from distant sites such as the ilium or ribs. Recently there is a growing use of intraoral bone grafts from intraoral donor sites such as mandibular symphysis, mandibular ramus and maxillary tuberosity. We recommend that the mandibular ramus is a safe autogenous bone graft donor site for bone harvesting with low morbidity. We report various effective autogenous bone graft procedures from mandibular ramus for the implant placement on various atrophic alveolar ridges.

  • The purpose of this study is to investigate the cephalo-facio-dental relationships in the craniofacial complex and their changes with age, and to use them for diagnostic and treatment purposes in the orthodontics The author studied on the changes of the cephalo-facio-dental relationships, using serial lateral cephalometric roentgenograms of 46 boys and 47 girls aged from 6 to 11 years of normal Korean children Following results were obtained 1 Means and Standard deviation of Korean children were obtained. 2 In the evaluation of the craniofacial vertical proportions, lower anterior face was larger than the upper, and upper posterior face was larger than the lower at all ages 3 The growth change was more prominent in the anterior craniofacial vertical proportion than in the posterior, and growth increment in the upper anterior facial height dimension was larger than m the lower anterior. 4 In the evaluation of the craniofacial horizontal proportion, ANS, Pog, Go and 6 were all situated posterior to their reference ares, and point B was always situated anterior to the arc passing by point A. 5. Anteroposterior growth change was the most prominent in the mandible, and there was no significant difference between the horizontal growth increment in the cranial base and that in the maxilla 6 Growth increment in the horizontal direction was larger in the mandibular apical base than in the maxillary apical base 7 The upper central incisor and the upper first molar were gradually anterior positioned against their reference ares with age increase 8 The length of mandibular corpus was larger than that of cranial base from the seven years old, and the difference was increased as the age increased 9 With age, there was slight difference in the angular relationships formed by craniofacial reference planes and axial inclinations of upper and lower permanent teeth.

  • Orthodontic treatment of cleft patients is difficult as the growth is different from that of normal ones. So it is very important to know the characteristic features of the craniofacial morphology and growth pattern in unilateral cleft lip and palate patients. The materials for this study consisted of 55 normal males and 50 unilateral cleft lip and palate ones who received cheiloplasty and palatoplasty previously. The cleft subjects were divided into 4 groups according to their ages kto find out the growth pattern of hard and soft tissue, and to compare the features with those of normal ones. Each cephalogram analysed by McNamara method and others. The obtained results were as follows 1. In the unilateral cleft lip and palate subjects, forward growth of the maxilla was smaller than that of normal ones from 9 years old. So the maxilla was retruded. The maxillary incisors were severely retruded in all age groups. 2. The mandibular overall length and its anteroposterior position did not show any significant differences between two groups. But the height of ramus was very short and the mandible had vertical growth tendency to compensate for undergrowth of the maxilla in cleft subjects after 12 years of age. 3. Horizontal growth of the soft tissue in middle face was smaller than that of any other facial region from 9 years old. The vertical growth rate of upper lip was decreased as growing old. 4. In cleft subjects, the upper and lower facial component angle and the facial convexity angle were large. So their facial profile changed to straight or concave as growing old.

STABILITY OF SIMULTANEOUS IMPLANTATION WITH AUTOGENOUS ILIAC BONE GRAFT IN THE SEVERELY ATROPHIC MAXILLA (심하게 흡수된 상악골에서 자가 장골 이식술과 동시에 시행한 임프란트 치료의 안정성에 대한 연구)

  • A severely atrophic maxilla may disturb the proper implant placement. The various bone graft techniques are required for simultaneous or delayed implantation in the cases of atrophic alveolar ridges. We present 11 consecutive patients treated with simultaneous implantation using the autogenous inlay and/or onlay bone grafts from iliac crest to the floor of the maxillary sinus and the alveolar crest. In the cases of atrophic maxilla, a total 69 implants were simultaneously placed with autogenous iliac bone graft. 40 fixtures were inserted in the sinus floor simultaneously with subantral block bone graft, the other 29 fixtures were placed in the anterior or premolar areas with block or particulate bone graft. The vertical alveolar bone height was measured with Dental CT at the preoperation and 6 months postoperation. Moreover, the implant stability quotients (ISQ) were measured by $Osstell^{TM}$ during second implant surgery at 6 months later of first implantation. All implants were obtained successful osseointegration with the grafted bone. The mean vertical increases were 3.9mm in the anterior ridges and 12.8mm in the posterior ridges. During the second implant surgery, mean ISQ were 62.95 in the anterior ridge and 61.32 in the posterior ridge. We concluded that the simultaneous implantation with autogenous iliac bone graft were stable and available methods for severely atrophic maxilla.

ROENTGENOCEPHALOMETRIC STUDY ON THE TEETH AND SKULL (치아 및 두개골에 대한 두부방사선 계측학적 연구)

  • For the purposes of augmentation of the aid for case analysis and diagnosis of malocclusion, a roentgenocephalometric study was made from 84 Korean adolescences. The Subjects consist of 42 males and 42 females aged from 17 to 20 years with normal occlusion and acceptable facial appearance. The author measured 18 angles and 14 linear distances as suggested by Jarabak. The following results were obtained. 1) Each linear measurement of the males' skull was greaten than that of the females. 2) The posterior to anterior facial height was $69.2\%$ in the males and $67.1\%$ in the females. 3) In the relationship of upper lip to esthetic line, the lip of females was more behind than that of males. 4) Saddle angle was $124.7^{\circ}$, articular angle was $148.7^{\circ}$, genial angle was $119.4^{\circ}$ and upper and lower genial angles were $45.1^{\circ}\;(N-Go-a^{\circ})$ and $74.2^{\circ}\;(N-Go-Me^{\circ})$. 5) The ratio of mandibular body to anterior cranial base was about 1:1. 1. 6) The angulations of $SNA^{\circ},\;SNB^{\circ}\;and\;SNP^{\circ}$ were as follows; $SNA^{\circ},\;80.3^{\circ},\;SNB^{\circ},\;79.8^{\circ},\;SNP^{\circ},\;81.1^{\circ}$. 7) The angle of the sella-nasion plane to the mandibular plane $(SNG^{\circ}Me^{\circ})$ was $32.0^{\circ}$ and that of the occlusal plane to the mandibular plane was $18.2^{\circ}$. 8) The angle of the maxillary central incisor to the sellanasion plane $(1-SN^{\circ})$ was $105.6^{\circ}$. That of the mandibular central incisor to the mandibular plane $(1-GoMe^{\circ})$ was $94.0^{\circ}$, and the interincisal angle $(1\;to\;1^{\circ})$ was $127.6^{\circ}$. 9) The linear distance from incisal edge of upper central incisor to facial plane was 8.0mm and that of lower central incisor was 4.6mm. 10) In the relationship of the lower lip to the esthetic line, the lower lip was 0.2mm front of the esthetic line.

Evaluation of factors influencing the success rate of orthodontic microimplants using panoramic radiographs

  • Objective: The purpose of this study was to investigate factors influencing the success rate of orthodontic microimplants (OMIs) using panoramic radiographs (PRs). Methods: We examined 160 OMIs inserted bilaterally in the maxillary buccal alveolar bone between the second premolars and first molars of 80 patients (51 women, 29 men; mean age, $18.0{\pm}6.1years$) undergoing treatment for malocclusion. The angulation and position of OMIs, as well as other parameters, were measured on PRs. The correlation between each measurement and the OMI success rate was then evaluated. Results: The overall success rate was 85.0% (136/160). Age was found to be a significant predictor of implant success (p < 0.05), while sex, side of placement, extraction, and position of the OMI tip were not significant predictors (p > 0.05). The highest success rate was observed for OMIs with tips positioned on the interradicular midline (IRML; central position). Univariate analyses revealed that the OMI success rate significantly increased with an increase in the OMI length and placement height of OMI (p = 0.001). However, in simultaneous analyses, only length remained significant (p = 0.027). Root proximity, distance between the OMI tip and IRML, interradicular distance, alveolar crest width, distance between the OMI head and IRML, and placement angle were not factors for success. Correlations between the placement angle and all other measurements except root proximity were statistically significant (p < 0.05). Conclusions: Our findings suggest that OMIs positioned more apically with a lesser angulation, as observed on PRs, exhibit high success rates.
