Modified Halterman Appliance를 이용한 제 1대구치 이소맹출의 치험례 (TREATMENT OF PERMANENT FIRST MOLAR BY MODIFIED HALTERMAN APPLIANCE)

  • Ectopic eruption is defined as abnormal eruption and results in malpositioned teeth and abnormal root resorption of adjacent teeth. Ectopic eruption, first reported by Chapman, occurs in 3% of the population and that mostly in the maxilla. Etiologic factors include narrow maxilla, large maxillary teeth, retarded calcification of the first molar, inclined eruption path of the first molar and retruded position of the maxilla. Impaction of the second molar is rare and occurs mostly in the mandible. Major causes are large teeth and insufficient arch length. Halterman has devised a method of distalizing a ectopically erupting first molar by cementing a band on the second deciduous molar with a hook soldered and a button bonded to the occlusal surface of the first permanent molar. Ectopically erupted posterior teeth should be treated early to maintain normal development of the dentition, harmony of facial growth and occlusal support, a failure to do so could result in severe malocclusion, periodontal damage and continued root resorption of the adjacent teeth. Early detection and treatment is thus vital. The author is submitting this report as he has obtained favorable results in treating a patient who came to the SNUDH Dept. of Pediatric Dentistry complaining of the first molar by using a modified Halterman appliance.

II급 Activator를 이용한 혼합치열기 II급 부정교합아동의 치험례 (TREATMENT OF CLASS II MALOCCLUSION IN THE MIXED DENTITION WITH CLASS II ACTIVATOR: CASE REPORT)

  • Class II malocclusion can be treated via early orthopedic, orthodontic treatment or orthognathic surgery with orthodontic treatment. In the mixed dentition, early orthopedic treatment can be used. Especially, in the case of mandibular retrognathism, the functional appliances can be used, and in the case of maxillary protrusion is combined, they can be used together with headgear. After using activator and activator combined with headgear to the class II malocclusion paitent in the mixed dentition, the results were as follows: 1. Lateral profile was improved, and lower face height was increaed. 2. Overjet was decreased, and molar relationship was changed to class I molar relationship. 3. Growth can be undisturbed, and the aggravation of malocclusion can be prevented to make the 2nd phase orthodontic treatment be much easier.

  • After introduction of Delaire's basic philosophy, to consider the normal and pathologic anatomy, the role of some structures, such as nasal septum, musculature, and tongue, and some functions, such as dental occlusion or nasal respiration, which play important roles in maxillary and particularly premaxillary growth, on the Korean Journal of cleft lip and palate in 2009, Delaire's primary functional cheilorhinoplasty (FCR) in the unilateral and bilateral cleft lip patients was summarized and introduced according to already published literatures and lectures. This paper will be the second publishment of Delaire philosophy with Korean language.

다양한 구강내 결손부 재건을 위한 비순피판의 활용 (USAGE OF NASOLABIAL SKIN FLAPS FOR THE RECONSTRUCTION OF VARIOUS INTRAORAL DEFECTS)

  • The nasolabial flap has been used for reconstruction of moderate size intraoral defects. The nasolabial fold area provides an ample supply of tissue with a good color and texture match. The nasolabial flap classified advancement flap, inferiorly-based flap, superiorly-based flap. The flap is based inferiorly, so that it can easily be rotated to the intraoral defects. The nasolabial flap is chosen for the repair of various intraoral defects because of its simple elevation, proximity to the defect and its rich subcutaneous blood supply of a island flap. The subjects were 6 patients with nasolabial flap, who had reconstruction of moderate size intraoral defects. We have found the inferiorly-based nasolabial flap with a subcutaneous pedicle useful in the primary repair of surgical defects of the buccal mucosa, edentulous mandibular ridge, maxillary alveolus area and soft palate in these patients. There was no complication except one case. Intraoral hair growth was a minor problem of this patient. We thought that the inferiorly-based nasolabial flap is a useful technique for reconstruction of various intraoral defects.

Split orthodontic airway plate: An innovation to the utilization method of conventional orthodontic airway plate for neonates with Robin sequence

  • Since the emergence of neonatal infant orthodontics for treatments of cleft lip and palate with or without Robin sequence (RS) in Europe in the 1950s, advancements in design and scope of its application have been remarkable. As the first institution to adopt orthodontic airway plate (OAP) treatment in the United States in 2019, we saw a need for innovation of the original design to streamline the most labor-intensive and time-consuming aspects of OAP utilization. A solution is introduced using a systematic split expansion mechanism to re-size the OAP periodically to accommodate the neonate's maxillary growth. To date, seven RS patients have received this modified treatment protocol at our institution. Each patient completed full treatment using only one OAP. This innovative utilization method is aptly named the split orthodontic airway plate (S-OAP). Details of the S-OAP and its modifications from conventional OAP are reported.

Orthodontic treatment in a patient with Moebius syndrome: A case report

  • Moebius syndrome (MBS) is a congenital neurologic disorder that causes cranio-facial abnormalities. It involves paralysis of the VI and VII cranial nerves and causes bilateral or unilateral facial paralysis, eye movement disorder, and deformation of the upper and lower limbs. The orofacial dysfunctions include microstomia, micrognathia, hypotonic mimetic and lip muscles, dental enamel hypoplasia, tongue deformity, open bite or deep overbite, maxillary hypoplasia, high arched palate, mandibular hyperplasia or features indicating mandibular hypoplasia. This case report presents a 7-year-old male patient who was diagnosed with MBS at the age 2 years. The patient displayed typical clinical symptoms and was diagnosed with Class II malocclusion with a large overjet/overbite, tongue deformity and motion limitation, and lip closure incompetency. Treatment was initiated using a removable appliance for left scissor bite correction. After permanent tooth eruption, fixed appliance treatment was performed for correction of the arch width discrepancy and deep overbite. A self-ligation system and wide-width arch form wire were used during the treatment to expand the arch width. After 30 months of phase II treatment, the alignment of the dental arch and stable molar occlusion was achieved. Function and occlusion remained stable with a Class I canine and molar relationship, and a normal overjet/overbite was maintained after 9.4 years of retainer use. In MBS patients, it is important to achieve an accurate early diagnosis, and implement a multidisciplinary treatment approach and long-term retention and follow-up.

Is conservative treatment (enucleation using modified Carnoy's solution) of odontogenic keratocyst in the maxilla good prognosis?

  • Odontogenic keratocysts (OKCs) located in the maxillae have rarely been reported in the literature. Standard treatment modalities for OKC range from marsupialization to marginal resection. However, most of the studies on OKC treatment have been related to mandibular OKCs. The anatomical structure and loose bone density of the maxillae and the empty space of the maxillary sinus could allow rapid growth of a lesion and the ability to tolerate tumor occupancy in the entire maxilla within a short period of time. Therefore, OKCs of the maxillae require more aggressive surgery, suchas resection. As an alternative, this report introduces a modified Carnoy's solution, a strong acid, as an adjuvant chemotherapy after cyst enucleation. This report describes the clinical outcomes of enucleation using a modified Carnoy's solution in patients with large OKCs on the posterior maxillae. In three cases, application of a modified Carnoy's solution had few side effects or morbidity. Each patient was followed for four to six years, and none showed any signs of recurrence. In conclusion, adjuvant treatment with a modified Carnoy's solution can be considered a treatment option capable of reducing the recurrence rate of OKC in the maxillae.

Upper airway dimensions and craniofacial morphology: A correlation study using cone beam computed tomography

  • Objective: To determine the correlation between dentoskeletal parameters related to craniofacial morphology and the upper airway (UA) volume. Methods: Cone-beam computed tomography images of 106 randomly selected orthodontic patients were analyzed using NemoFab Ortho software. The dentoskeletal variables assessed were anterior facial height (AFH), posterior facial height (PFH), PFH/AFH ratio, hyoid position, maxillary width (MW), and palatal depth. The UA volume (evaluation in anatomical regions and as a whole) was also assessed using the same software. We also evaluated potential differences in UA variables between age and sex groups. The correlation between the dentoskeletal parameters and UA volume was calculated using the Pearson correlation coefficient (R). Analysis of variance and Student's t test were performed to assess differences between age and sex for UA variables. Statistical analyses were performed using SPSS software (version 26 for Windows). Results: This study found that PFH, AFH, and MW were the dentoskeletal parameters most strongly correlated with UA volume. However, the ANB angle did not show any significant correlation with UA volume. Additionally, differences in UA volumes were observed between age groups. Sex differences were found in both the "8-12" and "≥ 16" age groups for oropharyngeal and pharyngeal volumes. Conclusions: In conclusion, our findings indicate a significant correlation between UA volume and dentoskeletal parameters, particularly those related to facial height and MW.

Early outcomes of cleft and palatal width following anterior palate repair (vomerine flap) in infants with wide cleft lip and palate

  • Background Anterior palatal repair performed during cleft lip repair using a vomerine flap may assist in recruiting additional soft tissue for subsequent completion of palatoplasty, especially in patients with a wide cleft. We present our early results in the hope of triggering a re-evaluation of this technique regarding its advantages for maxillary growth through further studies of patients with a wide cleft. Methods A retrospective analysis of patients with complete unilateral and bilateral cleft lip and palate was performed, including cleft and palatal measurements taken during initial surgery (lip repair together with anterior palate repair) and upon completion of palatoplasty. Results In total, 14 patients were included in this study, of whom nine (63.3%) had unilateral cleft lip and palate and five (37.5%) had bilateral cleft. All patients had a wide cleft palate. Lip and anterior palate repair was done at a median age of 3 months, while completion of palatoplasty was done at a median age of 10.5 months. Measurements taken upon completion of palatoplasty showed significant cleft width reduction in the mid-palate and intertubercle regions; however, the palatal arch distances at nearby landmarks showed non-significant marginal changes. Conclusions Anterior palate repair using a vomerine flap significantly reduced the remaining cleft width, while the palatal width remained. Further research is warranted to explore the long-term effects of this technique in wide cleft patients in terms of facial growth.

안모의 성장유형에 따른 경부견인 헤드기어의 효과에 대한 두부방사선계측학적 연구 (Cephalometric study of the effect of cervical pull headgear based on facial growth patterns)

  • 이 연구의 목적은 II급 부정교합의 치료를 위해 경부견인 헤드기어(cervical pull headgear)를 사용할 때, 치료에 불리한 요인으로 작용한다고 언급되는 효과들이 실제로 발현되는지의 여부와 안모의 성장유형에 따라 그 차이가 존재하는 지를 알아 보고자 함에 있다. 이를 위해서 삼성서울병원 치과교정과에서 II급 부정교합으로 진단되어 경부견인 헤드기어로 치료받은 환자중, 성장기에 있으면서 비발치 치료를 받은 환자를 연구대상으로 선별하였다. 그리고 연구대상을 "후안면/전안면 고경비" 에 따라 수직성장군과 수평성장군으로 분류하였다. 수직성장군 15명(남자 5명, 여자 10명)과 수평성장군 11명(남자 4명, 여자 7명)의 치료전, 후의 두부방사선규격사진상에서 SN-PP angle, SN-GoGn angle, SN-MP angle, PFH/AFH ratio, SN to 6, PP to 6 등을 계측하고, 비교분석하여 다음의 결과를 얻었다. 1)수직성장군에서 구개평면이 전하방으로 경사되었다. 2)수평성장군에서 후안면/전안면 고경비가 증가하였다. 3)수직성장군과 수평성장군 모두에서 하악평면각은 변화하지 않았다. 4)상악대구치의 정출양에 있어서 수직성장군과 수평성장군의 차이는 없었다.
