술후성상악낭의 임상적, 방사선학적 연구 (CLINICAL AND RADIOLOGICAL STUDY OF THE POSTOPERATIVE MAXILLARY CYST)

  • Post operative maxillary cyst may arise after the surgical treatment for maxillary sinusitis with the symtoms of swelling, pain, and pus discharge in the buccal region. It is examined by Waters' view, panoramic view and other intraoral radiographs, but quite variable radiologically. Most of the cyst is seen round or ovoid shape radiolucency, destruction, expansion and thinning of the lateral wall or posterior wall, and roots of the adjacent teeth may be resorbed. We studied about 117 cases of the post operative maxillary cysts which diagnosed in department of oral and maxillofacial radiology, Seoul National University Hospital. We analyzed and obtained following results. 1. These cysts occured more frequently in male than in female and the incidence is highest in the 4th and 5th decade. 2. Initial radical operation of maxillary sinus were performed mainly between the age of 10 and 45 years, and about 60% of the patients were 15 to 25 years. 3. Pain and swelling on buccal area, pus discharge, and toothache are most chief compaints, seven cases were found at routine examination without symtoms. 4. Most of these cysts were unilocular with smooth and well-defined border. 5. The majority of the cysts occurred in the anterolateral wall of maxillary sinus. 6. Dental changes of the lesional area were loss of lamina dura and root resorption, but about 55% were not changed.

Bilateral postoperative maxillary cysts after orthognathic surgery: A case report

  • Postoperative maxillary cysts are locally aggressive lesions, usually developing as delayed complications many years after radical antral surgery. This report describes a case of bilateral postoperative maxillary cysts following orthognathic surgery performed approximately 21 years previously. The patient complained of stinging pain on her right cheek. Radiographic examination revealed low-attenuation lesions on both maxillary sinuses with discontinuously corticated margins without distinct expansion or bone destruction. The cysts were enucleated with the removal of metal plates and screws for pain relief. Histopathological examination confirmed the diagnosis of postoperative maxillary cysts lined by ciliated, pseudostratified columnar cells. The patient has remained asymptomatic thus far, and there was no evidence of local recurrence at 21 months of postoperative follow-up.

  • Mucocele of the paranasal sinuses is a benign, slow-growing, expansile lesion. Maxillary sinus mucoceles are usually associated with painless bulging of the cheek; however, orbital expansion is rarely observed. Maxillary sinus mucoceles can be classified as primary or secondary according to their etiology. An impediment to sinus ostium ventilation is thought to be the cause of primary mucocele, while sequestering of residual mucosa after surgery in the wound and long-term retention of tissue fluid have been suggested to lead to the formation of secondary mucocele. This report presents 2 cases of primary and secondary mucoceles, with a focus on radiographic features. As primary and superiorly positioned secondary maxillary sinus mucoceles are uncommon and their close proximity to the orbit predisposes the patient to significant morbidity, the authors expect that this report will contribute to a better understanding and diagnosis of maxillary sinus mucocele involving the orbit.

편측성 구순구개열 환자에서의 편측성 분절 구개골 신장술 (Unilateral Segmental Palatal Distraction in Unilateral Cleft Lip and Palate Patient)

  • Patients with unilateral cleft lip and palate (UCLP) usually present unilateral cross bite due to collapse of the maxillary minor segment. Unequal expansion of the palate is needed to resolve this problem in UCLP patient. Unilateral segmental palatal distraction (USPD) after Le Fort I osteotomy and the oblique placed orthodontic expansion screw (Hyrax) can be used to correct the unilateral cross bite. 1his case report describes the effects of USPD of the collapsed maxillary minor segment on patient with unilateral cleft lip and palate.

Alveolar restoration following rapid maxillary expansion with and without corticotomy: A microcomputed tomography study in sheep

  • Objective: This study examined bone microstructure restoration after rapid maxillary expansion (RME) with and without corticotomy over multiple retention periods. Methods: Eighteen male Dorper sheep were randomly distributed into three groups (n = 6 each group): group 1, RME with corticotomy on the buccal and palatal sides; group 2, conventional RME treatment; and group 3, no treatment. Post-RME, trabecular bone microstructure and new bone formation were evaluated by using microcomputed tomography (microCT) and histomorphometry after a 4- or 12-week retention period. Intergroup differences in bone quality and bone remodeling were analyzed by using two-way analysis of variance with Bonferroni post-hoc test. Results: The bone volume fraction (bone volume [BV]/total volume [TV]) values relative to the control in groups 1 and 2 were 54.40% to 69.88% after the 4-week retention period and returned to approximately 80% after the 12-week retention period. The pooled BV/TV values of the banded teeth in groups 1 and 2 were significantly lower than those of the control after the 4-week retention period (p < 0.05). However, after the 12-week retention period, the pooled BV/TV values in group 2 were significantly lower than those in groups 1 and 3 (p < 0.05). Histomorphological analysis showed that the new bone formation area in group 1 was approximately two to three times of those in group 2 and control. Conclusions: Corticotomy significantly enhanced the restoration of bone quality after the retention periods for banded teeth. This benefit might result from the increased new bone formation after corticotomy.

Horseshoe Expander의 확장 효과에 관한 연구 (Effects of Horseshoe Expander)

  • 이 연구는 Horseshoe Expander를 이용하여 확장 전과 후의 악구강계의 변화를 분석하고자 시행되었다. 상악궁 확장이 필요한 것으로 진단된 33명을 연구자료로 사용하였다. 완만 상악골 확장술(SME)을 적용한 19명을 대상으로 연구대상자의 정모 및 측모 두부 방사선 사진과 모형을 확장 전과 후로 채득하였다. 또한, 급속 상악골 확장장치(RME)를 적용한 13명을 대상으로 확장 후의 치축각의 변화를Horseshoe Expander군과 비교하였다. 이 자료를 계측, 통계 처리하여 분석한 결과, 다음과 같은 결론을 얻었다. 1. 정모 두부 방사선 규격사진 분석에서 양측 상악 중절치의 치근 및 절단연의 중앙점 사이의 거리, 상악 폭경, 비강 폭경이 유의성있는 증가를 보였고, 상하악 폭경은 감소를 보여 결국 삼각형 모양의 확장 양상을 나타냈다. 2. 측모 두부 방사선 규격사진 분석에서 McNamara line에 대한 상악 중절치의 거리, 상악깊이, 상악 높이 항목이 증가를 나타냈다. 또한, FH평면에 대한 상악 중절치, 구개평면, 교합평면, 하악 평면의 각도가 증가를 나타냈으나, McNamara line에 대한 하악 중절치의 거리항목은 유의성이 없었다. 3. 모형 분석에서, 구개용적, 상악 견치와 제1대구치의 폭경 및 상악궁 길이 항목이 유의성 있게 증가하였고, 교합 접촉점의 감소가 유의성을 나타냈다. 4. 상악 제1대구치간 폭경변화(치성 변화)대 상악폭경변화(골격성 변화)가 약 2.2:1의 비율을 보였다. 5. Horseshoe Expander군과 급속 상악골 확장장치군(RME군)의 확장 전,후 치축각 비교에서는, Horseshoe Expander군이 급속 상악골 확장장치군(RME군)보다 상악 제2소구치, 제1대구치에서 더 큰 상관계수를 가짐으로써 협측 경사이동이 적은 것으로 나타났다. 따라서, Horseshoe Expander는 치성 변화가 적고, 급속 상악골 확장장치 (RME)에 비하여 협측 경사이동이 적으므로 상악궁 확장에 효과적인 장치로 사료된다.

Maxillary sinus volumetric changes in jet aircraft pilots: A multislice computed tomography pilot study

  • Purpose: This study evaluated maxillary sinus volume changes in military jet aircraft pilot candidates before and after the training program, in comparison with a control group, considering the effects of pressurization, altitude, and total flight hours, through multislice computed tomography. Materials and Methods: Fifteen fighter pilots were evaluated before initiating the training program and after the final approval. The control group consisted of 41 young adults who had not flown during their military career. The volumes of each maxillary sinus were measured individually before and at the end of the training program. Results: When comparing the initial and final volumes in the pilots, a statistically significant increase was observed both in the left and right maxillary sinuses. When evaluating the average total volume of the maxillary sinuses(i.e., the average volume of the right and left maxillary sinuses together), a significant increase in the volume of the maxillary sinuses was observed in the pilot group when compared to the control group. Conclusion: The maxillary sinus volumes in aircraft pilot candidates increased after the 8-month training program. This may be explained by changes in the gravitational force, the expansion of gas, and positive pressure from oxygen masks. This unprecedented investigation among pilots might lead to other investigations considering paranasal sinus alterations in this singular population.

상악 전방견인시 악안면골의 초기반응에 관한 Laser Holography연구 (A LASER HOLOGRAPHIC STUDY ON THE INITIAL REACTION OF MAXILLOFACIAL COMPLEX TO MAXILLARY PROTRACTION)

  • In case of skeletal Class III malocclusion with underdeveloped maxilla, the extraoral orthopedic force for the stimulation of maxillary growth or anterior reposition of the maxilla has been used clinically for the improvement of facial skeletal relationship. The purpose of this investigation was to examine the initial reaction of maxillofacial complex to the maxillary protraction by using extraoral orthopedic force. The dried human skull was used and this investigation was done by means of double exposure holographic interferometry. The protraction forces placed on the canine or the first molar were parallel, $10^{\circ}$ downward, $20^{\circ}$ downward to the occlusal plane. Fringe pattern of each protraction condition was compared and analized. The results were as follows: 1. Each maxillofacial bone displaced saparately. 2. More displacement was shown at the area of the teeth and the alveolar bone. 3. A counterclockwise rotation of the maxilla wa decreased by downward protraction and especially 20 degree downward protraction from the canine showed least rotation. 4. On the zygomatic arch, outward bend was observed and this effect was decreased by downward protraction. 5. On the zygomatic bone, the counter clockwise rotation was increased by the downward protraction. 6. When maxillary expansion was applied at the same time, outward and upward displacement with counterclockwise rotation was observed on the maxilla. 7. The lateral pterygoid plate of sphenoid bone was affected by maxillary protraction.

Modified Protraction Headgear를 이용한 상악골 전방 견인시 악안면골의 초기반응에 관한 Holographic Interferometry 연구 (A STUDY OF HOLOGRAPHIC INTERFEROMETRY ON THE INITIAL REACTION OF MAXILLOFACIAL COMPLEX TO THE MAXILLARY PROTRACTION USING THE ANTENNA TYPE MODIFIED PROTRACTION HEAD GEAR)

  • The majority of the commonly used protraction headgears for the protraction of small and/or retropositioned maxilla not allow a change in the point of force application or direction of the force delivery to attain predictable results because of the position of the upper and lower lips to avoid discomfort to the patient. The purpose of this study was to investigate the initial reaction of maxillofacial complex according to the change of force variables such as direction and point of force application with designing an antenna type-modified protraction head gear. A macerated human skull with well aligned upper teeth was used to experimental model and the investigation was done by double exposure holographic interferometry. Fringe patterns of each protraction conditions were compared and analized. The results were as follows. (Frontal view) 1. The Counterclockwise rotation of the maxilla was showed by parallel protraction to occlusal plane and the fringe was decreased in number as higher point of force application. 2. Generally, the number of fringe was increased in 500gm of protraction force than in 300gm. 3. When apply the protraction force to the maxilla with rapid palatal expansion, the direction of fringe patterns was differed from the protraction without expansion. 4. In most of cases, the counterclockwise rotation was decreased in case of the direction of the force is $20^{\circ}$ downward to occlusal plane compared to the parallel direction. 5. At the point of force application is 15mm above and the direction of force is 20 downward to occlusal plane , the translation of the maxillary complex was showed. (Lateral view) 6. The direction of fringe patterns of the facial bones were differed each other by the sutures, and showed almost parallel when apply the 300gm and 500gm of protraction force. 7. In case of rapid palatal expansion with protraction of the maxilla, the fringe patterns between the maxillary area and the area from the posterior of the maxillary first molar to the pterygomaxillary fissure were differed. In case without rapid palatal expansion, the changes of direction and point of the force application did not affect to the direction and the number of the fringe patterns.

경부고정(頸部固定) headgear 사용시(使用時) 안면두개골(顔面頭蓋骨)의 변위(變位)에 관(關)한 장력계측법(張力計測法) 및 유한요소법적(有限要素法的) 연구(硏究) (A FINITE ELEMENT AND STRAIN GAUGE ANALYSIS ON THE DISPLACEMENT OF CRANIOFACIAL COMPLEX WITH CERVICAL HEADGEAR)

  • This paper was undertaken to observe the displacement of craniofacial complex with cervical headgear and to compare narrowing or widening effect of palate by use of contraction or expansion face-bow, respectively. The 3-dimensional finite element method(FEM) was used for a mathematical model composed of 597 nodes and 790 elements and an electrical resistance strain gauge investigation was performed to validate the finite element model. The outer bow of cervical headgear was adjusted to be placed below the occlusal plane by $25^{\circ}$ and met the midsagittal plane by $40^{\circ}$, and was loaded 1kg on each right and left hook toward posterior direction. The results were as follows 1. Generally, the maxillary teeth and facial bone were displaced in posterior, medial and downward direction. 2. It was the maxillary 2nd bicuspid that moved bodily. 3. The craniofacial complex rotated in a clockwise direction around the rotating axis which lay from the most posterior and lowest point connecting nasal crest of maxillary bone and vomer, progressively toward a more posterior, lateral and upward direction, anterior and upper area of pterygomaxillary fissure, base of medial pterygoid plate and laterally to the contact area of zygomatic arch with squamous part of temporal bone. 4. No contraction effect was observed by contraction face-bow when compared to the standard face-bow. 5. In case of expansion face-bow, the areas of maxillary 2nd bicuspid, molars and palate were expanded remarkably.
