상악 전치부 치은선의 평균치에 관한 연구 (Study of Normative Gingival Proportion in Anterior Maxilla)

  • Tooth is the most important element in esthetic consideration on facial area. Tooth alignment which is in harmony with gingiva, lips, and face is also key element. The purpose of this study was to give a clinical discipline for restoration of gingival contour, which contains a ratio of maxillary 6 anterior teeth, research for gingival contour etc., in case of rehabilitation of maxillary anterior teeth. 300 Dankook university dental school students ,who is their twenties and free from periodontitis, participate in this study. The result was from each 70 males and females who had normal occlusion and tooth alignment. Length, width and length/width ratio of maxillary 6 anterior teeth were 0.74-0.81 in male, 0.81-0.84 in female. There was significant difference between male and female. The most deepest position of gingiva in maxillary central incisor and canine was located in distal part of teeth and maxillary lateral incisor was middle portion. In maxillary central incisors, gingival line of the most deepest point appeared significant difference between male and female (p<0.01) whereas there was no difference in lateral incisors and canines. Distance between interdental papilla apex and the most deepest portion appeared significant difference between male and female. Distance of the deepest position of gingiva is statistically significant except maxillary central incisors and lateral incisors. Standard deviation and mean of the width of labio-lingual were measured in cervical area and there was a significant difference both male and female.(p<0.01) From this result, we could get the mean of maxillary anterior gingival line and these results have great value in clinical guidance in studying maxillary anterior teeth.

상악전치에 발생한 치근단낭종 치험례 (A Case Report of the Radicular Cyst in Maxillary Anterior Teeth Treated by Enucleation)

  • A radicular cyst is a slowly-growing epithelial sac lining a pathologic cavity in the bone at the apex of a tooth. Large radicular cyst revealed by radiographic examination almost always appear to involved several teeth. The author observed a 31 year old woman with a labial swelling and palatal mucosal expansion of the maxillary anterior region. Radiographic examination indicated that the lesion extended from the left maxillary lateral incisor to the right lateral incisor. Cyst consisted of tan to gray yellow slightly firm to partly hard plaque like tissue measuring 3.4 x 2.2 x 0.3㎝. The author has treated the cyst by means of enucleation with apicoecomies of the involved teeth except the right central incisor. Recovery proceed satisfactorily and there were no clinical or radiologic signs of recurrence 8 months later.

Interdisciplinary rehabilitation of a root-fractured maxillary central incisor: A 12-year follow-up case report

  • Single-tooth implantation has become a common treatment solution for replacement of a root-fractured maxillary incisor in adults, but the long-term esthetic results can be unfavorable due to progressive marginal bone loss, resulting in gingival recession. In this case report, a maxillary central incisor with a root fracture in its apical one-third was orthodontically extruded and extracted in a 21-year-old female. Implant surgery was performed after a 3-month healing period, and the final crown was placed about 12 months after extraction. After 12 years, favorable osseous and gingival architectures were visible with adequate bone height and thickness at the buccal cortical plate, and no gingival recession was seen around the implant-supported crown. Although modern dentistry has been shifting toward simplified, clinical procedures and shorter treatment times, both general dentists and orthodontists should be aware of the possible long-term esthetic advantages of orthodontic extrusion of hopelessly fractured teeth for highly esthetically demanding areas and should educate and motivate patients regarding the choice of this treatment solution, if necessary.

Evaluation of changes in the maxillary alveolar bone after incisor intrusion

  • Objective: This study was performed to investigate the changes in alveolar bone after maxillary incisor intrusion and to determine the related factors in deep-bite patients. Methods: Fifty maxillary central incisors of 25 patients were evaluated retrospectively. The maxillary incisors in Group I (12 patients; mean age, $16.51{\pm}1.32years$) were intruded with a base-arch, while those in Group II (13 patients; mean age, $17.47{\pm}2.71years$) were intruded with miniscrews. Changes in the alveolar envelope were assessed using pre-intrusion and post-intrusion cone-beam computed tomography images. Labial, palatal, and total bone thicknesses were evaluated at the crestal (3 mm), midroot (6 mm), and apical (9 mm) levels. Buccal and palatal alveolar crestal height, buccal bone height, and the prevalence of dehiscence were evaluated. Two-way repeated measure ANOVA was used to determine the significance of the changes. Pearson's correlation coefficient analysis was performed to assess the relationship between dental and alveolar bone measurement changes. Results: Upper incisor inclination and intrusion changes were significantly greater in Group II than in Group I. With treatment, the alveolar bone thickness at the labial bone thickness (LBT, 3 and 6 mm) decreased significantly in Group II (p < 0.001) as compared to Group I. The LBT change at 3 mm was strongly and positively correlated with the amount of upper incisor intrusion (r = 0.539; p = 0.005). Conclusions: Change in the labial inclination and the amount of intrusion should be considered during upper incisor intrusion, as these factors increase the risk of alveolar bone loss.

Customized maxillary incisor position relative to dentoskeletal and soft tissue patterns in Chinese women: A retrospective study

  • Objective: To provide reliable prediction models based on dentoskeletal and soft tissue variables for customizing maxillary incisor positions and to optimize digitalized orthodontic treatment planning. Methods: This study included 244 Chinese women (age, 18-40 years old) with esthetic profiles after orthodontic treatment with fixed appliances (133 in group I: 1° ≤ The angle between the nasion [N]-A point [A] plane and the N-B point [B] plane [ANB] ≤ 4°; 111 in group II: 4° < ANB ≤ 7°). Dental, skeletal, and soft tissue measurements were performed on lateral cephalograms of the participants. Correlation and multiple linear regression analyses were used to determine the influence of dentoskeletal and soft tissue variables on maxillary incisor position. Results: The ideal anteroposterior position of the maxillary incisor varied between sagittal skeletal patterns. The position of the maxillary incisor correlated with the sagittal discrepancy between the maxilla and the mandible (ANB), protrusion of the midface, nasal tip projection, development of the chin, and inclination of both the maxillary and mandibular incisors. Distance from the maxillary central incisor to nasion-pogonion plane predicted using multiple linear regression analysis was accurate and could be a practical measurement in orthodontic treatment planning. Conclusions: Instead of using an average value or norm, orthodontists should customize a patient's ideal maxillary incisor position using dentoskeletal and soft tissue evaluations.

A COMPARATIVE STUDY OF THE ANGLES BETWEEN CROWN AXIS AND ROOT AXIS IN MESIODISTAL DIRECTION BY USING ORTHOPANTOMOGRAM

  • Orthopantomogram is commonly used to evaluate root parallelism. "Good parallelism" between roots is widely accepted as one of the guidelines of a successful orthodontic treatment. In case there was a large angle between crown axis and root axis, and if we valued only the position of crown in establishing occlusal relationship without considering of the situation of root, the problem of root arrangement between adjacent teeth would be occurred. The estimate of root parallelism in mesiodistal direction before and after orthodontic treatment must be emphasized. The intent of this study was to determine the clinical importance and correlation of the angle between crown axis and root axis. Orthopantomograms of 105 orthodontic patients being treated in Yonsei university were used in this study. Twenty-eight teeth in both maxilla and mandible were selected and analyzed quantitively to evaluate the angle between crown axis and root axis, and obtain the correlationship among the individual teeth. The results are as follows: 1. Among the teeth presenting normal distribution, the maxillary right canine showed the largest mean value( $5.73{\pm}4.42^{\circ}$), which was composed of the crown-root angles, and the mandibular left lateral incisor showed the smallest mean value( $0.60{\pm}3.76^{\circ}$). 2. The crown-root angles of the maxillary incisors and the first molars, and the mandibular central incisors and the first molars didn't show normal distribution and the ranges of these angles were dispersed. 3. Significant differences were present between the crown axis and the root axis except for lower first premolars. (p<0.05) 4. No significant difference was present for the crown-root angle between right and left side, (p<0.05) 5. No significant difference was present for the crown-root angle between male and female except for lower left first premolar. (p<0.05) 6. In the upper right quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, lateral incisor and canine. In the upper left quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor. In the lower right quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, first molar and second molar. In the lower left quadrant, significant correlations were present between crown-root angles of the central incisor and lateral incisor, lateral incisor and canine, first molar and second molar. Onlay technique에 의한 매복중절치의 교정적 치험예 (A CASE OF PALATALLY IMPACTED INCISOR TREATED BY ONLAY TECHNIQUE WITH SURGICAL EXPOSURE)

  • The patient, a girl of 19 years in good health, had a class I malocclusion. The maxillary left centra1 incisors and both lateral incisors had already erupted. But the space for the right central incisor was partially closed by the mesial drifting of the neighboring teeth. The caused a shift in the midline and a cross-bite relation on the incisors. X-ray examination revealed the presence of the right central incisor in the alveolar bone and odontoma just above the crown of the right central incisor. After enough space for the impacted incisor was created in the dental arch with a open-coil spring the rectangular incision was made. Removing the odontoma uncovered the flat surface of the labial aspect of the incisor. During the tooth had erupted of its own accord, any unnecessary force had been imposed on the tooth. When it was decided that the tooth should be brought out by the mechanical device, the gold cast onlay with hook was used and run a light elastic between this hook and the main arch wire. Finally the tooth was brought down to the arch level. The result was excellent. Fortunately the esthetic problem and any detrimental effects on the psychological make-up could be avoided.

과잉치를 동반한 상악 중절치 전위와 상악 견치 매복에서 자가이식에 의한 치험례 (AUTOTRANSPLANTATION OF TRANSPOSITIONED MAXILLARY CENTRAL INCISOR WITH MESIODENS AND IMPACTED MAXILLARY CANINE : A CASE REPORT)

  • Autotransplantation is a procedure which transplants teeth from the original position to other positions in the same individual. It is classified surgical reposition by intraalveolar autotransplantation and transalveolar autotransplantation. The prognosis for successful autotransplantation is dependent on a number of factors such as root development, surgical technique, patient's age, endodontic treatment, time and type of splinting, preservation of periodontal ligament and storage medium. The most important factor is preservation of periodontal ligament. The cause of the failure of transplantation include damage of the transplant during removal from deep palatal malposition, poor regeneration of the bone around the transplant and chronic periodontal infection. In case I, Impacted maxillary canine for which surgical exposure and orthodontic treatment was impossisle was transplanted. After 2 weeks, It showed periapical radiolucency and external root resorption. So, endodontic treatment was done. One year later, permanent filling was done with gutta percha. In case II, Transpositioned maxillary central incisor was transplanted after extraction of impacted mesiodens. Pulp vitality was maintained during 5 months without other clinical symptons.

일부 여대생의 구강 및 안모상태와 치은노출(Gummy smile)과의 상관성 (The correlation among the oral & facial states and the gummy smile in female college students)

  • Objectives : The author has studied about correlation of gingival exposure upon smiling and oral facial status that reduce facial aesthetic. Methods : The subjects in this study are 91 female vulunteers who were in aged $21.4{\pm}1.89$ in Suwon. Objectives should be normal oral and facial status without the prosthodontic, orthodontic appliance or conqenital missing tooth, and agree to be examined the oral status and impression taking. 1.Measure the length of gingival exposure upon smiling. 2.Measure of the size on central incisor. 3.Measure of Facial. SPSS(SPSS 10.0 for windows, SPSS Inc, Chicago, USA) was utilized for calculating the correlation coefficient between gingival exposure upon smiling and facial status. Regression analysis was calculated in order to predict the R square for gingival exposure upon smiling. Results : 1.Correlation coefficient between the gingival exposure and length of maxillary central incisor was calculated as reversed correlation(r=-.302, p<0.01), and between the gingival exposure and the ratio of the length of central incisor/width of central incisor was revealed as reversed correlation(r=-.250, p<0.05) on smiling. 2.There was correlation between the gingival exposure and the facial height(r=.351, p<0.01), the lower facial height(r=.454, p<0.01) and the upper lip height(r=.274, p<0.01) upon smiling. 3.There was correlation between the gingival exposure and the ratio of the facial height/facial width(r=.358, p<0.05), the ratio of the upper facial height/facial width(r=.214, p<0.05), and the ratio of the lower facial height/facial height(r=.383, p<0.01) upon smiling. 4.The equation of the regression analysis for gingival exposure upon smiling could be estimated as gingival exposure upon smiling=-5.139+.279${\times}$lower facial height-.615${\times}$maxillary central incisal length-.05${\times}$nasolabial angle. Conclusions : Considering these results, it recommended that treatment planning should be designed in consideration of such factors as the length of maxillary central incisor, facial height, upper lip height and lower facial height, in order to promote the easthetic problems of face on smiling.
