Objectives: The purpose of this study is to examine the factors affecting the rate and duration of breastfeeding. Methods: We analyzed the data from the year 2000 Korea National Fertility Survey that was collected through direct interviews. In particular, the mothers who delivered their last child and the child was under 1 year of age from January 1998 to June 2000 (N=1,066) were analyzed via a logistic model to assess the factors affecting the breastfeeding rate. Among the study subjects, those who had initiated breastfeeding (N=740) were analyzed through Cox's proportional hazard model to evaluate the factors affecting the duration of breastfeeding. Results: The multivariate logistic model showed that the delivery type and the baby's birth-weight have a statistically significant influence on the breastfeeding rate. Women who delivered their babies through Cesarean section were less likely than others to breastfeed. In contrast, the women whose babies weighed 2.5Kg or more were more likely than others to breastfeed. The results obtained from the survival analysis are as follows: the higher the mother's education level, the shorter is the breastfeeding duration. The mother's work status played a significant role in the early termination of breastfeeding. Women aged 35 or older showed a longer breastfeeding duration than the younger age groups, whereas the maternal age was not a significant factor in affecting whether or not a mother would breastfeed. Conclusions: Reducing the cases of operative delivery (Cesarean section) and low weight births, enlightening young and highly educated women on breastfeeding and improving the environment for breastfeeding on the job are important strategies to encourage women to breastfeed.
United Nations (UN) adopted 17 global sustainable development agenda to the year 2030 in the 68th general assembly on september, 2015. The global agendas and goals are important for 3 reasons: (1) to adopt the international standard for determining the health status; (2) to identify areas in need of attention; and (3) to advance international cooperation regarding health issues. In the area of infectious diseases, our goals include the eradication of human immunodeficiency virus infection and acquired immune deficiency syndrome, tuberculosis, and malaria as well as a substantial reduction of hepatitis by the year 2030. In the area of non-communicable diseases, our goal is to reduce premature mortality (${\leq}70years$) at least 30% by the year 2030. Preventive activities such as smoking cessation, alcohol abstinence, nutritional measures, and physical activities, should also be promoted intensively nationwide. It is also necessary to establish stringent policies for control hypertension, diabetes, obesity, and hypercholesterolemia. Additionally, environmental health, injury by traffic accident, mental health, and drug and alcohol abuse are important health policies. Furthermore, in the area of international health and cooperation, maternal and child health remain important areas of support for underdeveloped countries. Education and training towards the empowerment of health professionals in underdeveloped countries is also an important issue. The global agenda prioritize resources(manpower and budget) allocation of international organizations such as UN, World Health Organization, United Nations Development Programme, and World Bank. The global agenda also sets the contribution levels of Official Developmental Assistance donor countries. Health professionals such as professors and researchers will have to turn their attention to areas of vital international importance, and play an important role in implementation strategies and futhermore guiding global agenda.
The purpose of this study was to investigate the current status of postpartum depression intervention program performed in Korea and to evaluate its effectiveness. Of the Korean academic journals reported until November 2018, 13 experimental studies were selected and used for final analysis. The average age of the subjects was 26.9 to 34.4 years, and subjects were puerperal women or couples. The sample size was 6~39 (mean: 20.4) in the experimental group, 5~40 in the control group (mean: 20.0), and the intervention program consisted of 0.5~12 weeks/2~14 sessions/10~120 minutes per session. The design of all the studies was a non-equivalent control group pre-post test design. The main dependent variables, postpartum depression, fatigue, and maternal role self-confidence, were all found to have a statistically significant median level of effect size in the meta-analysis. This study confirms the composition and effects of various experimental studies used to mediate postpartum depression in Korea. This could be used as specific evidence-based data to form an optimal postpartum intervention program.
Objectives: The purpose of this study is to examine whether the maternal and paternal value of children mediates the relationship between the marital relationship satisfaction and the parental sense of competence in the longitudinal dyadic approach. Method: The data were obtained from the Panel Study on Korean Children (PSKC) conducted by the Korea Institute of Child Care and Education. Using the Actor-Partner Interdependence Model(APIM), the author analyzed three waves (from 5th to 7th year) of the data, and the sample size was 1,773. The average age was 34.79 for mothers, and 37.26 for fathers. Their average length of marriage was 95.3 months, and their children were 51.03 months old on average. Results: The marital relationship satisfaction influenced the parental sense of competence with both the actor effect and the partner effect of mothers and fathers. The marital relationship satisfaction also influenced the value of children, with the actor effect from both mothers and fathers and with the partner effect only from mothers. The value of children influenced the parental sense of competence with the actor effect only from mothers and fathers. Also, the value of children mediated the relationship between the marital relationship satisfaction and the parental sense of competence with the actor effect from both mothers and fathers. For the partner effect, only the mediation path from fathers' marital relationship satisfaction to mothers' value of children to mothers' parental sense of competence was significant. Conclusions: These results suggest the importance of highlighting the marital relationship satisfaction and the value of children to improve the parental sense of competence.
This paper aims to reveal the merits and demerits of the exhibition by examining whether the subject intended at the exhibition planning stage was finally persuasively implemented throughout the work and exhibition, along with the theoretical verification of the way the exhibition dealing with the history of the women's independence movement from the psychoanalytic feminist point of view. To this end, a more fundamental approach to the theme of the Women's Independence Movement calls for the search for a feminine language that can capture women's unique identity rather than a masculine language such as the existing independence movement exhibition method, and for finding such feminine language, a feminine speech, art and poetic language, maternal genealogy, and women's solidarity are presented, along with theories. This paper, which expounds the role of art works in exhibitions dealing with history through theoretical verification of actual exhibition cases, has significance as communication between theory and field.
Background: With the establishment of international guidelines and changes in insurance policies in Korea, the role of targeted ultrasonography has increased. This study aimed to identify the rates and clinical course of anomalies detected using prenatal targeted ultrasonography. Methods: This study was a retrospective analysis of all pregnancies with targeted ultrasonography performed in a single secondary medical center over 5 years. Results: Fetal anomalies were detected by targeted ultrasonography in 137 of the 8,147 cases (1.7%). The rates of anomalies were significantly higher in female fetuses (2.0% vs. 1.3%). In cases of female fetuses, the rate of anomalies was significantly higher in the advanced maternal age group (2.4% vs. 1.2%). In cases of male fetuses, the rate of anomalies was significantly higher in nulliparous (2.4% vs. 1.5%) and twin (5.7% vs. 1.9%) pregnancies. Pulmonary anomalies were significantly more common in the multiparity group (17.6% vs. 5.8%). Among the 137 cases, 17.5% terminated the pregnancy, 16.8% were diagnosed as normal after birth, and 42.3% were diagnosed with anomalies after birth or required follow-up. Conclusion: Through the first study on the rates and clinical course of anomalies detected by targeted ultrasonography at a single secondary center in Korea, we found that artificial abortions were performed at a high rate, even for relatively mild anomalies or anomalies with good prognosis. We suggest the necessity of a nationwide study to establish clinical guidelines based on actual incidences or prognoses.
Objectives: Young mothers are vulnerable to postpartum depression due to role transition-related stress. Understanding the causes underlying these stressors is essential for developing effective interventions. Methods: This study analyzed the 2018 Indonesian Basic Health Research data. The Mini International Neuropsychiatric Interview was used to assess postpartum depression symptoms in mothers aged 15-24 years with infants aged 0-6 months. In 1285 subjects, the risk factors for postpartum depression were evaluated using multivariate logistic regression. Results: The overall prevalence of depression in the 6 months postpartum was 4.0%, with a higher prevalence in urban areas (5.7%) than in rural areas (2.9%). Urban and rural young mothers showed distinct postpartum depression risk factors. In urban areas, living without a husband (odds ratio [OR], 3.82; 95% confidence interval [CI], 1.24 to 11.76), experiencing preterm birth (OR, 4.67; 95% CI, 1.50 to 14.50), having pregnancy complications (OR, 3.03; 95% CI, 1.20 to 7.66), and having postpartum complications (OR, 5.23; 95% CI, 1.98 to 13.80) were associated with a higher risk of postpartum depression. In rural areas, postpartum depression was significantly associated with a smaller household size (OR, 3.22; 95% CI, 1.00 to 10.38), unwanted pregnancy (OR, 4.40; 95% CI, 1.15 to 16.86), and pregnancy complications (OR, 3.41; 95% CI, 1.31 to 8.88). Conclusions: In both urban and rural contexts, postpartum depression relates to the availability of others to accompany young mothers throughout the postpartum period and offer support with reproductive issues. Support from the family and the healthcare system is essential to young mothers' mental health. The healthcare system needs to involve families to support young mothers' mental health from pregnancy until the postpartum period.
Background: Parental attention is crucial for preventing childhood oral diseases. Mothers play a significant role in maintaining their families' oral health, and their educational level influences their children's oral health behaviors. This study investigates the impact of mothers' educational levels on adult oral health behaviors using data from a national survey. Methods: This study employed a cross-sectional analysis of secondary data. The data used were obtained from the 8th Korea National Health and Nutrition Examination Survey. Descriptive statistics were calculated to identify participant characteristics. Next, t-tests and one-way analysis of variance were conducted to examine the effects of the explanatory variables on the distribution of the dependent variable. Finally, logistic regression analysis was used to investigate the influence of the explanatory variable on the dependent variable, using "no education" as the reference value, and calculate the odds ratios. Results: Children of mothers with a college education or higher had a 1.13 times higher likelihood of receiving oral examinations than those whose mothers had no education. Children whose mothers graduated from college or higher had a 2.23 times higher probability of receiving preventative dental treatment than those whose mothers had no education. Children whose mothers graduated from college or higher had a 1.92 times higher probability of receiving scaling than those whose mothers had no education. Children whose mothers graduated from high school had a 1.35 times higher probability of receiving scaling than those whose mothers had no education. Conclusion: Developing oral health programs is important for low-educated and low-income parents to change theirs and their children's oral health behaviors/attitudes. This will help reduce oral health disparities among adults raised by parents of higher and lower socioeconomic statuses. Therefore, a comprehensive approach is essential for adults to maintain good oral health, regardless of variations in their parental educational levels during childhood.
Membrane type matrix metalloproteinases(MT-MMPs) have been suggested to play an important role during structural remodeling of various tissue. Expression patterns of MT1-MMP and MT2-MMP mRNAs were investigated in oocytes, embryos, ovary and oviduct of mouse during their differentiation or periovulatory period using RT-PCR technique. Both cDNA products of MT1- and MT2-MMP of immature oocytes were barely discernable with a minimum amount but the expressions were distinct in mature oocytes regardless that they were matured in vivo or in vitro. MT2-MMP was not expressed by 2-cell embryos but was expressed by 4-cell stage embryos. From the morula stage untill hatched blastocyst stage, embyos showed intesnse expression of MT2-MMP with a sudden increase at blastocyst stage. While mouse ovarian tissues showed both expression of MT1- and MT2-MMP, there was no stage-specific difference throughout the estrous cycle. Mouse oviducts also exhibit constant amount of both MT1- and MT2-MMP expressions throughout periovulatory period, i.e., before or after ovulation. These observations lead to suggest that the differential expressions of maternal MT1- and MT2-MMP during meiotic resumption of mouse oocytes and embryonic expression of MT2-MMP particularly at blastocyst stage might play a role in the differentiation of mouse oocytes and/or embryos. The precise function of MT1- and MT2-MMP with regards to their participation in the remodeling of ovarian and oviductal tissues remains in a question.
The pregnancy and abortion process involves a complex mechanism with various immune cells present in the implantation sites and several hormones associated with pregnancy, such as leptin, ghrelin and nesfatin-1. However, the mechanism underlying spontaneous abortion by maternal T helper 17 (Th17) present in the implantation sites and nesfatin-1, which is of anorexigenic hormones, is not fully understood so far. Therefore, the purpose of this study was to examine the possible roles of Th17 cells present in the implantation sites and nesfatin-1 expressed in the uterus on spontaneous abortion using the $CBA/j{\times}DBA/2$ mouse model. Th17 transcription factor, ROR-${\gamma}t$ mRNA expression was significantly increased in the abortion sites compared with the implantation sites of abortion model mice on day 14.5 and 19.5 of pregnancy. In addition, the expression levels of IL-17A mRNA were significantly higher in abortion sites than in implantation sites on day 14.5 and 19.5. Moreover, the nesfatin-1/NUCB2 protein and mRNA levels were increased in abortion sites compared with levels in implantation sites of both normal pregnant and abortion model mice on day 14.5 of pregnancy. Interestingly, nesfatin-1/NUCB2 serum levels were not changed throughout the whole pregnancy in abortion model mice, but its serum level was dramatically increased on day 14.5, and then rapidly decreased on day 19.5 in normal pregnant mice. In this study, we showed for the first time the expression of nesfatin-1/NUCB2 mRNA and protein in implantation sites during pregnancy. The present results suggest that Th17 cells in the uterus may play an important role in the period of implantation and for maintenance of pregnancy. Furthermore, the present results suggest that Th17 cells in implantation sites may be a key regulator for maintenance of pregnancy and provides evidence that activation of these cells may be regulated by nesfatin-1/NUCB2. Further study is needed to elucidate the role of nesfatin-1 expressed in the uterus during pregnancy.
