The purpose of this study was to compare differences in endurance time and EMG power spectral characteristics of the masticatory muscles during sustained isometric contraction between patients and controls. 15 CMD patients{8 women and 7 men, aged 15 to 38 years(24.1$\pm$7.5)}, and 15 healthy volunteers{8 women and 7 men, aged 15 to 30 years(24.7$\pm$3.4)} without past history or present symptoms of CMD were included in this study. Sustained isometric contractions of masticatory muscles were perfomeed as long as possible at 50% level of maximum voluntary contraction(MVC) of EMG activity via visual feedback, and the duration of sustained isometric contraction(endurance time) was examined. The author perfomed EMG power spectral analysis in the myoelectric signals of masseter and anterior temporal muscle during sustained isometric contraction in CMD patients with chronic muscle pain and healthy controls. The author came to following conclusions from the results. 1. The endurance time of the patient group was shorter than the control group in sustained isometric contraction of masticatory muscles(p<0.01). 2. MF values of masticatory muscles with sustained isometric contraction during endurance time were decreased following regression line in both groups(p<0.01, r>0.9). 3. The amount of MF shift to lower frequency range exhibited no significant differences between the patients and the control group in sustained isometric contraction during endurance time. 4. SMF to lower frequency range of the patient group was steeper than the control group in sustained isometric contraction during endurance time(p<0.05).

PURPOSE: The purpose of this study was to determine the effect of craniocervical flexion exercise with masticatory muscle contraction in patients with neck pain. Patients with neck pain also experience muscle weakness and limitation of activities of daily living. Craniocervical flexion exercise with masticatory muscle contraction may increase the thickness of the deep cervical flexor muscles, thereby reducing pain and limitations on activities of daily living. METHODS: Twenty-six volunteers participated in this study. The subjects were randomly divided into an experimental group (n=13) and a control group (n=13). Both groups performed craniocervical flexion exercise using a pressure biofeedback unit; the experimental group performed craniocervical flexion exercise with masticatory muscle contraction for 20 minutes, 3 times a week for 4 weeks. Assessment tools included ultrasonography for measurement of muscle thickness and the Neck Disability Index for the level of pain and function. RESULTS: The rate of change in muscle thickness in both groups significantly increased (p<.05), with a significantly greater increase in the experimental group than in the control group (p<.05). The Neck Disability Index score significantly improved (p<.05) in both groups, with significantly greater improvement in the experimental group compared with the control group (p<.05). CONCLUSION: These results suggest that craniocervical flexion exercise with masticatory muscle contraction can be effective in increasing muscle thickness and improving the Neck Disability Index score in patients with neck pain.
The authors performed a experimental study to evaluate the effects of masticatory muscle fatigue on tooth contact and masticatory muscle activity in 26 normal healthy women. The experimental masticatory muscle fatigue was induced by unilateral biting of 5kg force on mandibular first molar. The results were as follows : 1. The initial symptom related to muscle fatigue pain appeared in 85.19 seconds of isometric contraction and the endurance time of isometric contraction was 203.15 seconds. 2. The pain occurred more frequently in masseter region than in temporal region. In masseter pain the incidence was almost equal between both sides, whereas the temporal pain was more in contralateral side. 3. The spontaneity and the symmetry of tooth contact during maximum clenching were reduced after isometric unilateral biting. 4. After induction of experimental muscle fatigue, the EMG activities of masseter muscles of both sides and ipsilateral temporal muscle showed the tendency of decreasing activities. 5. The asymmetry indicies of masseter and temporal muscles were reduced after isometric bilateral biting.
To study the characteristics of EMG power spectrum of masticatory muscles during sustained isometric contraction and recovery at various contraction times, the author analysed the EMG signals of anterior temporal and masseter muscles before, during, and after sustained isometric contraction at 50% level of maximum voluntary contraction (MVC) for 15,30,60 seconds. Twelve normal subjects were included in this study. The author came to following conclusions from the results. 1. MMF of anterior temporal muscle in the contraction period was significantly higher than that of masseter muscle during sustained isometric contraction regardless of isometric contraction times (p<0.05). 2. MMF in the contraction period decreased as the contraction time increased during sustained isometric contraction in both temporal and masseter muscles(p<0.05). 3. SMF in the contraction period increased as the contraction tie increased during sustained isometric contraction in both temporal and masseter muscles(p<0.01). 4. MMF in the first part of recovery period (20 seconds) decreased as the contraction time increased during sustained isometric contraction(p<0.05). However, MMF in the later parts of recovery period (20-120 seconds) showed no significant differences. 5. MMF of anterior temporl muscle in the recovery period was significantly higher than that of masseter muscle after sustained isometric contraction regardless of isometric contraction times (p<0.05). 6. The recovery rate of MF reached 100% in 100 seconds after the isometric contraction regardless of isometric contraction times.
Myofascial pain is a condition associated with regional pain and muscle tenderness characterized by the presence of myofascial trigger points. In this case report, a subject complaining of nighttime bruxism was clinically assessed, and a latent trigger point of the anterior temporalis muscle was identified with manual palpation. A surface electromyographic (SEMG) exam of the anterior temporalis muscle harboring the latent trigger point demonstrated several SEMG features, including post-contraction irritability, delayed relaxation following contraction and accelerated muscle fatigue. It was concluded that a SEMG exam may detect abnormal masticatory muscle function and, therefore, assist in the evaluation of myogenous temporomandibular disorders.
Purpose: This study investigated the association between the objective indicator of masticatory function assessment and the masseter muscle thickness (MMT) using ultrasound imaging. Methods: A total of 99 subjects (males: 24, females: 75, mean age: 76) were analyzed. The maximum bite force (MBF) was measured with a pressure-sensitive sheet and an image scanner. The mixing ability index (MAI) was calculated by image analysis after asking the subjects to chew a wax specimen. The MMT during rest and clenching were obtained with a diagnostic ultrasound system, and the difference in MMT during rest and MMT during clenching was defined as the difference in masseter muscle thickness (DMMT). Multiple regression analysis was performed to determine the independent variables affecting MBF and MAI. Results: The MBF showed correlation with the number of remaining teeth (β=0.346, p=0.002) and DMMT (β=0.251, p=0.011). The MAI correlated with only the number of remaining teeth (β=0.476, p<0.001). Conclusions: The DMMT reflects the state of masseter muscle contraction, and can be used as a predictor as well as the number of teeth when assessing masticatory function.
The purpose of this study was to investigate short-term masticatory muscle reactions in response to simulated noise and music sound. Hypothesis of this study was that loud noise would cause increased stiffness and decreased elasticity of the masticatory muscles compared to low level of noise or identical sound level of music. Fifteen male volunteers were recruited for the study. The sound levels of noise and music used here were 60 dB and 100 dB. The experiment comprised 4 sessions, Session 1 with 100 dB of noise for the 1st day of experiment: Session 2 with 100 dB of music for the $2^{nd}$ day: Session 3 with 60 dB of noise for the $3^{rd}$ day: Session 4 with 60 dB of music for the $4^{th}$ day. Stiffness and elasticity on the anterior temporalis and superficial masseter muscles were measured with tactile sensor before and 2, 4 and 6 minutes after exposure of sound. The study indicated that, in short-term exposure of sound, there was no significant difference between noise and music at both 60 and 100 dB of sound level, but that there were partially significant differences between 60 and 100 dB of sound level regardless of sound type. This suggest that high level of sounds like 100 dB used in this study, in spite of short term exposure of several minutes, would lead to masticatory muscle contraction, especially in the masseter muscles.
본 연구는 정상인의 저작근에서 지속적인 등척성 수축후 근전도 power spectrum과 압력통각 역치의 회복 특성을 구명하기위해 시행되었다. 악안면 영역의 동통과 긴으장애의 벙력 및 현증이 ㅇ벗고 정상적인 교합관계를 가진 26명의 정상인(남자 14명, 여자 12명) 을 대상으로 최대근활성도의 70% 수준에서 인내가능 할 때까지의 등척성 수축과 수축후 회복기 동안의 근전도 power spectrum과 압력통각 역치를 컴퓨터를 이용한 근전도 시스텀과 전자식 압력통각계를 사용하여 분석한 결과 다음과 같은 결론을 얻었다. 1. 지속적인 등척성 수축 시작 시점의 중간주파수에서 남녀간에는 통계적으로 유의한 차이가 관철되지 않았지만, 근육간 비교에서는 전측두근에서 교근보다 유의하게 높게 나타났다(p<0.05). 2. 지속적인 등척성 수축전 암력통각 역치는 전축두근에서 교근보다 유의하게 높았고(p<0.001), 남성에서 여성보다 유의하게 높게 나타났다(p<0.05). 3. 지속적인 등척성 수축시 중간주파수와 압력통각 역치의 변화량은 성별간, 근육간에 우의한 차이가 관찰되지 않았다. 4. 지속적인 등척성 수축말기 중간주파수는 수축초기에 비하여 유의하게 감소하였으며(p<0.001), 이의 회복은 모든 근육에서 60초와 120초 사이에서 이루어 졌다. 5. 지속적인 등척성 수축후 압력통각역치는 수축전에 비해 유의하게 감소하였으며(p<0.001), 수축전 수준으로의 회복은 여자군의 전측두근과 교근에서는 30분후에 남자구느이 전측두근과 교근에서는 50분 후에 이루어졌다. 6. 지속적인 등척성 수축후 주관적인 동통이 소실된 시각은 여자군의 전축두근에서는 28분, 교근에서는 42분으로 남자군의 전측두근에서는 30분, 교근에서는 40분으로 나타났다.

Bite force is created by the force of adjacent teeth accompanied with tension of masticatory muscle. The bite force value is greater in male than in female and ha maximum value at first molar. Masseter muscle is associated with bite force and during muscle contraction the electric signal is expressed in EMG form. The aim of the study is to assess recovery time for masseter muscle activity and according to each part of bite force after open reduction with internal fixation when mandibular angle fracture and subcondyle fracture occurred. And to determine the appropriate period for mandibular fracture patients to have normal masticatory activity. 30 patients with normal bite condition was selected for control group and from April, 2007 to September, 2007, 20 patients who visited our department of oral and maxillofacial surgery of Dankook University, were selected for the study and were diagnosed as mandibular angle fracture and subcondyle fracture. For control group, the bite force for incisors, canine, premolars and molars and activity of the masseter muscle was measured and compared for 1, 2, 3, 4, 6 and 8 weeks. That was divided as fracture side and normal side. Mann-Whitney U test was performed for significant difference and the following result was obtained. 1. The maximum voluntary bite force for incisors, canine, premolars and molars portion were 0.113 kN, 0.182kN, 0.295kN and 0.486kN and the masseter muscle activity was 0.192 volts in the control group. 2. The maximum bite force at fracture side was recovered by 4th weeks for incisors, 6th weeks for canine and premolars and 8th weeks for molars and the masseter muscle activity was recovered by 6th weeks in the experimental group. 2. The maximum bite force at normal side was recovered by 4th weeks for incisors, 6th weeks for canine, premolars and molars and the masseter muscle activity was recovered by 3rd weeks in the experimental group. 3. The method for internal fixation by 2.0mm miniplates at both superior and inferior border had no complications according for twenty patients and had a satisfactory recovery. According to the result, patient with mandibular angle fracture and subcondyle fracture, 8 weeks was required for bite force recovery. Therefore, patients with open reduction and internal fixation under general anesthesis, it can be assumed that 8 weeks was needed after operation in order to have normal bite force and masseter muscle recovery.
As people prefer to use right or left hand, some have preferred chewing side while others do not. Totally, 82 volunteers composed of students and staffs from Dental Hospital College of Dentistry Yonsei University participated in this study for the investigation of influence of preferred chewing habit, that has lasted for more than a year, on electromyographic(EMG) activity of masticatory muscles and bite force. Among the 82 volunteers, 46 had preferred chewing habit while the other 36 did not. Prior to the investigation, those with factors that could affect the study, such as, general disease, irregular dentition and malocclusion, were screened and excluded by questionnaire and clinical examination. The results were as follows: 1. There was no significant difference in EMG activities between chewing side and non-chewing side of preferred chewing subjects at rest as well as maximal voluntary contraction(MCV)(p>0.05). 2. Asymmetrical coefficient of temporal and masseter muscle EMG activities between preferred chewing subjects and non-preferred chewing subjects at rest was not significantly different(p>0.05). 3. Asymmetrical coefficient of masseter EMG activity was significantly higher(p<0.05) than that of non-preferred chewing subjects at MCV, whereas that of anterior temporal muscle showed no difference(p<0.05). 4. In preferred chewing subjects, there was no significant difference in average bite force and occlusal contact area between chewing side and non-chewing side(p>0.05). 5. There was no significant difference in Asymmetrical coefficients of average bite force and occlusal contact area between preferred chewing subjects and non-preferred chewing subjects (p>0.05). Consequently, preferred chewing habit can be considered as physiological asymmetry with normal function rather than to have influence on EMG muscle activity of masticatory muscles, average bite force and occlusal contact area. Objective standardization to differentiate preferred chewing subjects and non-preferred chewing subjects should be established in the further study.
