자동차 제조업체 조립공정 근로자의 국소진동 노출 수준 및 작업환경 만족도 평가 (Evaluation of Hand-Arm Vibration Exposure Level and Work Environment Satisfaction of Workers in Automobile Manufacturer Assembly Process)

  • Objectives: This study was conducted to evaluate hand-arm vibration (HAV) exposure levels due to the use of power hand tools and to evaluate the determinants in the automobile assembly process. Methods: The exposure level to HAV was evaluated for 30 work lines in five assembly processes (body, engine, chassis, door, and design) that use air-powered tools and battery-powered tools and operate in circulation for two hours. The 2-hr equivalent energy vibration acceleration, A (2), of the task was measured. The 8-hr equivalent energy vibration acceleration, A (8), was estimated in consideration of the number of tasks that can be performed per day by each process. In addition, a survey on the working environment was conducted with workers exposed to vibration. Results: The geometric mean of the HAV exposure level, A (2), for a total of 30 tasks was 2.51 m/s2, and one case was 10.30 m/s2, exceeding TLV (2hr). The HAV exposure level of A (8) was evaluated from 1.03 m/s2 to 5.36 m/s2. A (2) showed a statistically significant difference (P<0.01) for each process, and the chassis process (GM=3.90 m/s2) was the highest. The larger the tool size and the longer the tool length, the higher was the vibration acceleration when using a battery-powered tool than an air-powered tool (P<0.01). Battery-powered tool users showed higher dissatisfaction on all items than did air-powered tool users. Conclusions: As a result of this study, it is necessary to implement a program to reduce the HAV exposure levels.

15 MV를 이용한 전립샘암 세기조절 방사선치료 시 빔의 개수에 따른 부작용 발생률에 관한 연구 (A Study on the Incidence of Side Effects according to the Number of Beams in Intensity-modulated Radiation Therapy for Prostate Cancer using 15 MV)

  • 본 연구에서는 15 MV를 이용한 전립샘암의 세기조절방사선치료 시 빔의 개수에 따른 광중성자선량의 부작용 발생률을 분석하고자 한다. 전립샘암 세기조절방사선치료의 방사선치료계획설계는 1회 처방선량 220 cGy, 전체 치료 횟수는 33회로 총 7260 cGy로 수립하였다. 실험에 사용된 선형가속기는 Varian의 True Beam STx(Varian, USA)이며, 계획용 표적체적(Planning target volume; PTV)에 15 MV로 광중성자 선량이 발생되도록 하였다. 치료계획의 설계는 Eclipse System (Varian Ver 10.0, USA)을 사용하여 세기조절방사선치료 5, 7, 9 portals로 하였다. 광중성자 선량 측정을 위해 광자극발광선량계 (Optically stimulated luminescence albedo neutron dosimeter, Landauer Inc., USA)를 사용하였다. 세기조절방사선치료 5 portals 시 1,000명당 1.7명, 7 portals 시 1,000명당 1.8명, 9 portals 시 1,000명당 2.0명이 광중성자 선량으로 인하여, 갑상샘에 부작용을 나타낼 확률임을 산출하였다. 본 연구는 세기조절방사선치료 시 2차 피폭선량의 위험성을 연구하여, 향후 방사선의 확률적 영향과 관련된 의미 있는 자료로 활용될 수 있기를 기대한다.

광중성자 발생으로 인한 전립샘암 환자의 유전적 위험과 발암의 확률에 관한 연구 (A Study on the Genetic Risk and Carcinogenesis Probability of Prostate Cancer Patients Due to Photoneutron Generation)

  • 본 연구에서는 고 에너지를 이용한 전립샘암의 방사선 치료 시 발생되는 광중성자의 선량을 광자극발광선량계를 사용하여 측정하였다. 이에 따른 복부에 발생되는 유효선량에 의한 유전적 위험확률을 연구하고자 하였다. 15 MV 에너지 발생이 가능한 의료용 선형가속기 True Beam STx(Varian Medical Systems, USA)와 방사선 치료 계획 시스템(Eclipse, Varian Medical System, USA)을 사용하였다. 선형가속기 Couch 위에 인체팬텀을 설치하고, 광중성자 선량의 측정은 Albedo Neutron Optical Stimulation Luminescence Neutron Detector(Landauer Inc., IL, USA)를 사용하였다. VMAT과 3C-CRT 의 복부에서 광중성자 선량값은 VMAT이 3D-CRT에 비하여 2배 이상 높은 52.8 mSv 였다. 전립샘암의 방사선 치료 시, 광중성자 선량으로 인한 복부에 발생되는 유효선량에 의한 유전적 위험확률은 VMAT는 1,000명 당 3.2명, 3D-CRT는 1.4명이 발생률임을 산출하였다. 전립샘암의 방사선 치료 시 발생할 수 있는 큰 부작용이 있는 복부를 연구함으로써 전립샘암 환자들의 삶의 질과 확률적 영향을 연구하는데에 의미 있는 연구로 활용되어지길 기대한다.

Firefighting and Cancer: A Meta-analysis of Cohort Studies in the Context of Cancer Hazard Identification

  • Objective: We performed a meta-analysis of epidemiological results for the association between occupational exposure as a firefighter and cancer as part of the broader evidence synthesis work of the IARC Monographs program. Methods: A systematic literature search was conducted to identify cohort studies of firefighters followed for cancer incidence and mortality. Studies were evaluated for the influence of key biases on results. Random-effects meta-analysis models were used to estimate the association between ever-employment and duration of employment as a firefighter and risk of 12 selected cancers. The impact of bias was explored in sensitivity analyses. Results: Among the 16 included cancer incidence studies, the estimated meta-rate ratio, 95% confidence interval (CI), and heterogeneity statistic (I2) for ever-employment as a career firefighter compared mostly to general populations were 1.58 (1.14-2.20, 8%) for mesothelioma, 1.16 (1.08-1.26, 0%) for bladder cancer, 1.21 (1.12-1.32, 81%) for prostate cancer, 1.37 (1.03-1.82, 56%) for testicular cancer, 1.19 (1.07-1.32, 37%) for colon cancer, 1.36 (1.15-1.62, 83%) for melanoma, 1.12 (1.01-1.25, 0%) for non-Hodgkin lymphoma, 1.28 (1.02-1.61, 40%) for thyroid cancer, and 1.09 (0.92-1.29, 55%) for kidney cancer. Ever-employment as a firefighter was not positively associated with lung, nervous system, or stomach cancer. Results for mesothelioma and bladder cancer exhibited low heterogeneity and were largely robust across sensitivity analyses. Conclusions: There is epidemiological evidence to support a causal relationship between occupational exposure as a firefighter and certain cancers. Challenges persist in the body of evidence related to the quality of exposure assessment, confounding, and medical surveillance bias.

Artificial Neural Network with Firefly Algorithm-Based Collaborative Spectrum Sensing in Cognitive Radio Networks

  • Recent advances in Cognitive Radio Networks (CRN) have elevated them to the status of a critical instrument for overcoming spectrum limits and achieving severe future wireless communication requirements. Collaborative spectrum sensing is presented for efficient channel selection because spectrum sensing is an essential part of CRNs. This study presents an innovative cooperative spectrum sensing (CSS) model that is built on the Firefly Algorithm (FA), as well as machine learning artificial neural networks (ANN). This system makes use of user grouping strategies to improve detection performance dramatically while lowering collaboration costs. Cooperative sensing wasn't used until after cognitive radio users had been correctly identified using energy data samples and an ANN model. Cooperative sensing strategies produce a user base that is either secure, requires less effort, or is faultless. The suggested method's purpose is to choose the best transmission channel. Clustering is utilized by the suggested ANN-FA model to reduce spectrum sensing inaccuracy. The transmission channel that has the highest weight is chosen by employing the method that has been provided for computing channel weight. The proposed ANN-FA model computes channel weight based on three sets of input parameters: PU utilization, CR count, and channel capacity. Using an improved evolutionary algorithm, the key principles of the ANN-FA scheme are optimized to boost the overall efficiency of the CRN channel selection technique. This study proposes the Artificial Neural Network with Firefly Algorithm (ANN-FA) for cognitive radio networks to overcome the obstacles. This proposed work focuses primarily on sensing the optimal secondary user channel and reducing the spectrum handoff delay in wireless networks. Several benchmark functions are utilized We analyze the efficacy of this innovative strategy by evaluating its performance. The performance of ANN-FA is 22.72 percent more robust and effective than that of the other metaheuristic algorithm, according to experimental findings. The proposed ANN-FA model is simulated using the NS2 simulator, The results are evaluated in terms of average interference ratio, spectrum opportunity utilization, three metrics are measured: packet delivery ratio (PDR), end-to-end delay, and end-to-average throughput for a variety of different CRs found in the network.

Definitions of Chronic Obstructive Pulmonary Disease and Chronic Obstructive Pulmonary Disease Exacerbation: A Modified Delphi Survey

  • Background: The Global Initiative for Chronic Obstructive Lung Disease (GOLD) update 2023 proposed new definitions of chronic obstructive pulmonary disease (COPD) and COPD exacerbation. However, an agreement on the definitions has not been made, either internationally or domestically. This study aimed to reach an agreement between experts on the new definitions of COPD and COPD exacerbation in South Korea. Methods: A modified Delphi method was used to make an agreement on the definitions of COPD and COPD exacerbation proposed by the GOLD update 2023. We performed two rounds of the survey including 15 Korean experts on COPD, asthma, and tuberculosis. Results: More than two-thirds of the experts agreed on 12 of the 13 statements related to the definitions of COPD and COPD exacerbation in the two rounds of the survey. The experts agreed on the definitions of COPD and COPD exacerbation that should be revised in line with the definitions proposed by the GOLD update 2023. However, the experts showed an uncertain opinion on the statement that the definition of COPD includes patients with persistent airflow obstruction due to bronchiectasis. Conclusion: Based on this Delphi survey, experts' agreement was made on the definitions of COPD and COPD exacerbation proposed by the GOLD update 2023.

위 MALT 림프종의 진단 및 치료에 대한 고찰 (Diagnosis and Treatment of Gastric MALT Lymphoma)

  • 위에 발생하는 암 중 위 림프종이 1-6% 가량을 차지하고 있고, 이 중 50% 가량이 MALT 림프종이다. MALT 림프종의 60-70% 가량은 조기, 국소병기에서 진단된다. 위 MALT 림프종은 일반적으로 그 진행이 매우 느리며 상복부 통증, 소화불량, 오심, 구토 등의 비특이적인 증상을 보인다. 진단은 조직학적으로 진단하며, 헬리코박터필로리 연관 만성 위염에서 보이는 염증성 병변과의 감별을 위해 Wotherspoon score를 이용한다. 여러 연구를 통해 위 MALT 림프종이 헬리코박터필로리 감염과 관련이 있다는 것이 밝혀져 있고, 3세염색체증, 18세염색체증, t(11;18), t(1;14), t(14;18), t(3;14) 등 여러 유전적 이상을 가질 수 있는 것으로 알려져 있다. 적절한 치료 방침을 결정하기 위해 병기를 결정하는 것은 매우 중요하며, Lugano International Conference classification을 주로 사용한다. 위 MALT 림프종의 1차 치료는 헬리코박터필로리 감염 유무 및 병기와 관계없이 헬리코박터필로리 제균치료이다. Stage I/II1 환자의 경우 제균치료로 60-90%의 완전 완화율을 보인다. 제균치료에 반응이 없는 경우에는 방사선치료, 항암화학요법, 면역치료 등을 시행해 볼 수 있고, 75-100%의 완전완화율을 보인다. 향후 헬리코박터필로리의 감염율이 감소하면서 위 MALT 림프종의 발생도 감소할 것으로 기대된다. 또한 향후 치료방침 확립을 위해 지속적인 연구가 필요할 것으로 생각한다.

어깨관절의 벌림 위치에 따른 아래등세모근 운동이 둥근어깨와 아래등세모근의 근활성도에 미치는 영향 (The Effect of Lower Trapezius Muscle Exercise According to the Abduction Position of the Shoulder Joint on Round Shoulder Posture and Muscle Activity of the Lower Trapezius Muscle)

  • Purpose : The purpose of this study is to investigate the effect of each lower trapezius muscle exercise performed according to the abduction position (Y type - shoulder joint abduction 145 °, T type - shoulder joint abduction 90 °, and MPC type - shoulder joint 45 ° abduction) of the shoulder joint on the muscle activity of the round shoulder and lower trapezius muscle. Methods : This study was conducted on 31 adult men and women. Through random assignment, they were assigned to the Y group, T group, and MPC group. A 4-week intervention was performed for each group of 31 subjects who participated in the experiment, and shoulder height and lower trapezius muscle activity were measured before and after the intervention. Shoulder height measurement is a test to measure rounded shoulder posture. When the value is low, it means that rounded shoulder posture is improved. The muscle activity of the lower trapezius muscle was measured using the %MVIC method, and when the value is high, it means that the lower trapezius muscle is active. All measured data were verified using dependent t-tests for before and after comparisons and one-way analysis of variance for comparisons between groups. Results : The results of this study showed a significant decrease after intervention only in shoulder height. Muscle activity of the lower trapezius muscle decreased after intervention, but did not show a significant difference. Both variables showed no significant differences between groups. Conclusion : The results of this study show that three lower trapezius muscle exercises were performed on subjects in rounded shoulder posture. All three groups showed a significant decrease in the shoulder height value, a method of measuring rounded shoulder posture, and no significant differences between groups could be confirmed. Therefore, all three exercises can be considered effective in reducing shoulder posture.

Efficacy and safety of losartan in childhood immunoglobulin A nephropathy: a prospective multicenter study

  • Purpose: Angiotensin-converting enzyme inhibitors and angiotensin II receptor blockers (ARBs) are frequently employed to counteract the detrimental effects of proteinuria on glomerular diseases. However, the effects of ARBs remain poorly examined in pediatric patients with immunoglobulin A (IgA) nephropathy. Herein, we evaluated the efficacy and safety of losartan, an ARB, in pediatric IgA nephropathy with proteinuria. Methods: This prospective, single-arm, multicenter study included children with IgA nephropathy exhibiting proteinuria. Changes in proteinuria, blood pressure, and kidney function were prospectively evaluated before and 4 and 24 weeks after losartan administration. The primary endpoint was the difference in proteinuria between baseline and 24 weeks. Results: In total, 29 patients were enrolled and received losartan treatment. The full analysis set included 28 patients who received losartan at least once and had pre- and post-urinary protein to creatinine ratio measurements (n=28). The per-protocol analysis group included 22 patients who completed all scheduled visits without any serious violations during the study period. In both groups, the mean log (urine protein to creatinine ratio) value decreased significantly at 6 months. After 24 weeks, the urinary protein to creatinine ratio decreased by more than 50% in approximately 40% of the patients. The glomerular filtration rate was not significantly altered during the observation period. Conclusions: Losartan decreased proteinuria without decreasing kidney function in patients with IgA nephropathy over 24 weeks. Losartan could be safely employed to reduce proteinuria in this patient population. ClinicalTrials.gov trial registration (NCT0223277)

계층적 라우팅 경로를 제공하는 에너지 균등분포 클러스터 센서 네트워크 (Energy Balancing Distribution Cluster With Hierarchical Routing In Sensor Networks)

  • 효율적인 에너지 관리는 제한된 자원을 가지는 센서 네트워크에서 매우 중요한 요소이며, 클러스터 기법은 그러한 관점에서 많은 연구가 이루어지고 있다. 그러나, 클러스터 헤더의 에너지 사용이 집중되는 문제가 발생할 수 있으며, 클러스터 헤더가 전 영역에 골고루 분포되지 않고 특정 영역에 집중되는 경우, 클러스터 멤버의 전송 거리가 크거나 매우 불균등한 상태가 될 수 있다. 전송거리는 에너지 소모의 문제와 직결될 수 있다. 특정 노드의 에너지가 빨리 고갈되는 것은 센서 네트워크 생존 기간을 줄이고, 전체 센서 네트워크의 효율이 저하되므로 센서 노드들의 균등한 에너지 소모는 매우 중요한 연구과제이다. 본 연구에서는 센서 클러스터 기법에서 클러스터 헤더와 센서 노드가 에너지를 균등하게 사용하기 위한 요소들을 분석하고, 클러스터 헤더가 센서 네트워크 전역에 골고루 분포하는 균등분포 클러스터링을 제안한다. 제안하는 클러스터 기법은 멀티홉 라우팅을 사용하여 원거리 전송으로 인한 센서 노드의 에너지 소모를 줄인다. 기존 연구에서 멀티홉 클러스터 기법은 클러스터 구성과 라우팅 경로 설정의 2단계 과정을 통해서 멀티홉 클러스터 경로를 설정하는 반면, 제안하는 방식은 클러스터 헤더를 선출하는 과정에서 클러스터 라우팅 경로를 설정하여 제어 메시지 과정을 최소화한다.
