가족발달단계에 따른 간호요구영역에 관한 연구 (Study of The Area of Nursing Need by the Family Developmental Stage)

  • The Community Health Service considers the family as a service unit and places the emphasis of its service on the health problems and the nursing needs of the family rather than the individual. From the conceptual point of view that tile community health service is both health maintenance and health promotion of the family, the community health nurse should have a knowledge of the growth and development of the family and be responsible for the comprehensive support of normal family development. The community health nurse often is in a position to make a real contribution to normal family development. In order to investigate the relationship between the areas of nursing need and family development, the following objectives were established 1. To discover the general characteristics of the study population by the stage of family development. 2. To discover specific nursing needs in relation to the family developmental stage, and to determine the intensity of the nursing needs and the ability of the family to cope with these needs. 3. To discover overall family health nursing problems in relation to the family developmental stage and determine the intensity of the nursing need and the problem solving ability of family. Definitions : The family developmental stages as classified by Dually were used stage 1. Married couples(without children) stage 2. Childbearing Families (oldest child birth to 30 months of age) stage 3. Families with preschool children (oldest child 2½-to 6 years) stage 4. Families with schoolchildren (oldest child 6 to 13 years). stage 5. Families with teenagers (oldest child 13 to 20 years) stage 6. Families as launching centers (first child gone to last child′s leaving home). stage 7. Middle- aged parents (empty nest to retirement) stage 8. Aging family member (retirement to death of both spouses) The areas of nursing need were defined as those used in the study, "A Comprehensive Study about Health and Nursing Need and a Social Diagram of the Community", by tile Nursing research Institute and Center for population. and Family Planning, July 1974. The study population defiled and selected were 260 nuclear families ill two myron of Kang Hwa Island. Percent, mean value and F- test were utilized in tile statistical analysis of the study result. Findings : 1. General characteristics of the study population by tile family developmental stage ; 1)The study population was distributed by the family developmental stage as follows : stage 1 : 3 families stage 2 : 13 families stage 3 : 24 families stage 4 : 41 families stage 5 : 50 families stage 6 : 106 families stage 7 : 13 families stage 8 : 10 families 2) Most families had 4 or 5 members except for those in stage, 1, 7, and 8. 3) The parents′ present age was older in the higher developmental stage and their age at marriage was also younger in the higher developmental stages. 4) The educational level of parents was primarily less than elementary school irrespective of the developmental stage. 5) More than half of parents′ occupations were listed as laborers irrespective of the developmental stage, 6) More than half of the parents were atheists irrespective of the developmental stage. 7) The higher the developmental stage(from stage 2 to stage 6 ), the wider the distribution of children′s ages. 8) More than half of the families were of middle or lower socio-economic level. 2. Problems in specific areas of nursing need by family developmental stage, the intensity of nursing need and the problem solving ability of the family : 1) As a whole, many problems, irrespective of the developmental stage, occurred in tile areas of Housing and Sanitation, Eating Patterns, Housekeeping, Preventive Measures and Dental care. Problems occurring ill particular stages included the following ; stage 1 : Prevention of Accident stage 2 : Preventive Vaccination, Family Planning. stage 3 : Preventive Vaccination, Maternal Health, Family Planning, Health of Infant and Preschooler. stage 4, 5 : Preventive Vaccination, Family Planning, Health of School Children. stage 6 : Preventive Vaccination, Health of School Children. 2) The intensity of the nursing need in the area of Acute and Chronic Diseases was generally of moderate degree or above irrespective of the developmental stages except for stage 1. Other areas of need listed as moderate or above were found in the following stages: stage 1 : Maternal Health stage 3 . Horsing and Sanitation, Prevention of Accident. stage 4 . Housing and Sanitation. stage 5 : Housing and Sanitation, Diagnostic and Medical Care. stage 6 : Diagnostic and Medical care stage 7 : Diagnostic and Medical Care, Housekeeping. stage 8 : Housing and Sanitation, Prevention of Accident, Diagnostic and Medical Care, Dental Care, Eating Patterns, Housekeeping. 3) Areas of need with moderate problem solving ability or less were as follows : stage 1 : Diagnostic and Medical Care, Maternal Health. stage 2 : Prevention of Accident, Acute and Chronic Disease, Dental Care. stage 3 : Housing and Sanitation, Acute and Chronic Disease, Diagnostic and Medical Care, Preventive Measure, Dental Care, Maternal Health, Health of Infant and preschooler, Eating Patterns. stage 4 : Housing and Sanitation, Prevention of Accident, Diagnostic and Medical Care, Preventive Measure, Dental Care, Maternal Health, Health of New Born, Health of Infant and Preschooler, Health of school Children, Eating Patterns, Housekeeping. stage 5 . Housing and Sanitation, Prevention of Accident, Acute and Chronic Disease, Diagnostic and Medical Care, Preventive Measure, Dental Care, Preventive Vaccination, Maternal Health, Eating Patterns. stage 7, 8 : Housing and Sanitation, Prevention of Accident, Acute and Chronic Disease, Diagnostic and Medical Care, Preventive Measures, Dental Care, Preventive Vaccination, Eating Patterns , Housekeeping. Problem occurrence, the degree of nursing need and the degree of problem solving ability 1 nursing need areas for the family as a whole were as follows : 1) The higher the stages(except stage 1 ), the lower the rate of problem occurrence. 2) The higher the stage becomes, the lower the intensity of the nursing need becomes. 3) The higher the stages (except stages 7 and 8), the higher. the problem solving ability. Conclusions ; 1) When the nursing care plan for the family is drawn up, depending upon the stage of family development, higher priority should be give to nursing need areas ① at which problems were shown to occur ② where the nursing need is shown to be above moderate degree and ③ where the problem solving ability was shown to be of moderate degree. 2) The priority of the nursing service should be Placed ① not on those families in the high developmental stage but on those families in the low developmental stage ② and on those areas of need shown in stages 7 and 8 where the degree nursing need was high and the ability to cope low.

특수학교의 보건관리 (Health Management and Services of School-Nurse in Special Schools)

  • 특수학교 보건관리의 방향 설정과 특수학교 양호교사 업무 수행에 있어 질적 향상을 위한 기초 자료를 제시하고자 전국의 102개 특수학교 양호교사를 대상으로 1991년 2월 1일부터 1991년 3월 31일까지 우편 설문 조사를 실시하여 회수된 77개 학교를 대상으로 분석한 결과를 요약하면 다음과 같다. 특수학교의 67.5%가 사립이고, 83.2%가 시 이상 지역에 위치해 있으며, 정신지체학교가 48.1%로 가장 많았다. 특수학교의 평균 학급수는 17.2학급, 평균학생수는 194명, 평균교직원 수는 28명이었다. 양호교사의 평균 연령은 32.7세였고, 97.4%가 전문대학 이상 졸업자였으며, 71.4%가 기혼자였고, 79.2%가 임상이나 보건과 관련된 분야의 과거경력이 있는 것으로 나타났다. 또한 62.3%의 양호교사가 단독 업무를 보고있었으며, 77.9%가 초등에 소속되어 있었다. 대상 특수학교 양호실은 68.9%가 l층에 위치해 있었고, 학교보건 조직은 90.9%가 구성되어 있지 않았으며, 학교보건 인력으로 교의, 치과의, 학교 약사 모두를 위촉하고 있는 곳은 18.2%에 불과했다. 학교보건에 관한 연간 예산은 양호교사의 46.8%가 모르고 있었으며, 학교당 평균 년간지출액은 317,000원으로 그 중 의약품 구입비가 제일 많았다. 학교당 월 평균양호실 이용자수는 71명이었고, 학생 1인당 연간 양호실 이용은 4.4회였으며, 외상으로 인한 이용이 26.6%로 가장 많았다. 양호실 이용자중 1.4%가 의료기관에 의뢰되었는데, 시각장애학교는 고열, 정서장애학교는 골절, 다른 영역학교는 외상으로 가장 많이 의뢰하였다. 특수학교 아동 중 간질 학생수는 956명으로 조사 대상학교 학생수의6.4%를 차지하고 있었다. 신체검사를 2회 이상 실시하고 있는 학교는 22.6% 밖에 되지 않았으며, 98.7%가 보건교육을 실시하고 있고, 성교육은 98.7%가 필요하다고 강조하였다. 보건교육은 개인 위생에 가장 비중을 두고 있었으며, 시각장애 학교는 방송교육, 청각장애 학교는 OHP나 VTR, 다른 영역의 학교는 가정통신문이나 OHP VTR을 가장 많이 사용하는 교육매체였다. 대상 양호교사의 46.8%가 학교보건관리중 보건교육이 가장 어렵다고 하였으며, 중점개선내용으로 49.4%가 특수학교 보건관리에 대한 구체적인 업무 지침이 필요하다고 강조하였다. 사업계획 및 평가, 양호실 관리, 보건교육, 환경관리, 건강관리 등의 양호교사 업무 수행은 비교적 높은 수행율과 자신감을 나타냈으나, 그 중 학교보건 사업의 평가, 체력검사, 보건교육 후 평가, 학교정화구역 관리, 상처 봉합에 대한 수행율과 자신감이 비교적 낮았다. 따라서 특수학교 보건관리의 방향설정과 양호업무의 질적수준 향상을 위하여 학교보건사업에 대한 구체적인 업무지침의 개선과 특수학교 양호교사에 대한 별도교육이 필요한 것으로 생각된다.

최근 5년간(1995-1999) 충남지역자궁경부세포진 검진결과 및 이상소견위험요인분석 (Risk Factors of Cervical Cancer and Results of Cervix Cytology Screening in Chungnam Province, Korea, 1995-1999)

  • 자궁경부암 세포 도말검사법의 사업결과를 평가하고 정확도를 추정하고, 추적조사된 검진자를 대상으로 인구학적 특성 및 분만, 유산 요인과 자궁경부 세포 도말검사 결과와의 관련성을 도출함으로써 자궁경부암 조기발견 사업의 지역보건계획 수립 및 지역보건 정책을 입안시 중요한 기초자료로 활용하고자 시도되었다. 1995년 이후 5년 동안에 충청남도에 거주하는 23세 이상 여성 146,848명을 대상으로 대한가족보건협회 대전 충남지부 및 충청남도의 자궁경부 세포도말 집단 검진을 수진한 여성의 재검 및 2차 정밀검사 등의 결과를 분석하였다. 연도별로 1차 검진결과 이상자 율은 1995년 0.68%, 1996년 0.59%, 1997년도 0.70%, 1998년 0.56%, 1999년 0.62%로 전체적으로 0.63%로 연도별로 큰 차이를 보이지 않았다. 즉시 재검 이상의 이상자율은 0.63%였으며, 3군이상을 자궁경부암 전구병변으로 추정할 경우의 조율유병률은 0.61%였으나 1차검진 및 2차 검사를 종합한 결과에서는 이형성증 이상일 경우 1,000명당 2.2명이었다. 세포도말 검사의 정확성을 판단하기 위하여 위양성율을 추정해 본 결과, 비정상을 자궁경부염 이상의 단계를 양성으로 보았을 때, 즉시재검 판정은 위양성율이 44.0%, 3군은 56.2%, 4군은 26.9%, 5군은 19.0%로 나타났으며, 비정상을 이형성증 이상 단계로 한 경우에는 즉시재검은 68.0%, 3군 75.3%, 4군 46.2%, 5군 47.6% 등으로 다소 높아 여러 요인들이 이러한 결과에 복합적으로 작용하였을 것으로 판단됨으로 이에 대한 개선책이 강구되어야함을 시사하였다. 자궁경부 검사결과 비정상의 위험요인을 도출하기 위하여 다변수 분석을 실시한 결과, 연령, 학력, 행정구역이 유의한 기여변수로 선정되었는 데 기존의 연구결과와 비교해볼 때 상대적으로 대표성이 적고 적은 수의 환자를 대상으로 하였으므로 해석에 다소 제한적일 수 밖에 없어 시험적인 연구의 시도로 해석되어야 할 것이다.

임상 간호원을 위한 실무교육 과정으로서의 가족계획 (Family Planning as a Part of the Nursing-Staff In - Service Education Program)

  • When Korean family planning services began as a part of the National Policy in 1962, the annual population growth was 3.0%. This growth rate has been decreased to 2.0% during last ten year period. And it seems imperative that all hospitals, as well as related organizations, should participate in family planning in order to contribute to achieving the National goal of 1.5% population growth by 1976, the end of the Third Five Year Economic Development Plan. Nurses should be considered the most important human resources in charge of the core of family planning services in any setting. For the family planning services in the general hospital setting, nurses as a core members contribute much as change agent, motivators, counsellors, educators etc. A nurse can work with patients and their relatives when she is equipped with relevant knowledge and skills. Fur the more family planning cannot be ignored even in hospital setting where more comprehensive nursing care is needed Thus, the general objective of this study is to provide baseline data for better programming of In-service education in family planning so that effective hospital family planning nursing services can be made a part of comprehensive nursing care contributing to the national population program and human welfare. In order to meet the general objective, this study has the following specific objectives : 1. To find out the general characteristics of the clinical nurses working in Y Hospital 2. To evaluate their attitudes and practices of family planning 3. To assess their knowledge, attitudes and practices of population and family planning as professional nurses. 4. To examine and compare data collecting methods for the planning of an In-service Educational Program 5. To explore the contents to be included in this In-service Education Program. The study population randomly selected one hundred nurses working in Y Hospital A cross-sectional survey with questionnaires developed for this study was chosen for the study method. To collect reliable data, the questionnaires were distributed to and answered by the study population in a controlled situation. X²test and t-test was employed in analyzing the data. The findings of this study are as follows: 1. Y Hospital nurses had a lower ideal number of children (X=2.02) and showed no strong preference for male children, and 74% of them expressed the desire to use permanent methods of birth control 2. of this thirty Y Hospital nurses who were married 66.7% stated they were already practicing contraceptive methods. Most of them preferred male methods of contraception. 3. According to objective evaluation about knowledge of various aspects of population and family planning, respondents from collegiate programs significantly knew better the subjects on the average than did respondents from diploma programs of nursing. 4. There was a marked difference in the results of self-evaluation and objective evaluation in their family planning knowledge. It was found that the self-evaluation family planning knowledge seemed to be unreliable. Accordingly, the objective test methods appeared to be more reliable in the evaluation of knowledge levels. 5. The subject areas needed to be included in In-service education for the Hospital family planning services in Y Hospital are 1) rhythm methods, 2) tubal-legation, 3) family planning effects of contraceptives, 4) population growth, 5) demographic traction, 6) population structure and 7) infant mortality facts. In addition, 1) various oral contraceptives, 2) basal temperature method, 3) laparoscopic female sterilization, 4) interfering factors of family planning, 5) anatomy and physiology of the female reproductive organs were additional areas to be taught to respondents from 3-year diploma schools of nursing. Demographic transition was one subject area in which the four-year graduates need further study. 6. Population problems guidance and counselling in family planning instruction in the theory and practice of contraceptives should be included in future In-service Education Programs in order to provide more effective hospital Family Planning Services, stated 77.0% of the respondents.

국립병원 간호사의 직무만족과 관련요인 연구 (A Study of Job Satisfaction and Related Factors of the National Hospital Nurses)

  • By this time, a few of previous studies of factors related to separation from their jobs and job satisfaction only have dealt with the separation rate. the cause of separation and related factors that induce job satisfaction and incentive factors, the actualities of morale some suggestions for reduction of the separation rate. This study is attempted to determine factors that have effect on job satisfaction of national hospital nurses. and to proide information and materials for the development of the administration of nursing through the appreciation of factors influencing on job satisfaction between isolated ward nurses and general ward nurses working at national hospitals. 185 nurses of national hospitals responsed th this study, and were divided into two groups. Group 1: 57 nurses working at isolated wards for tuberculosis patients and Group 2 : 128 nurses at general wards. Relevant data were collected from August, 5, 1992 through August 20, 1992. The questionnaire consisted of 8 genalized items and 4 items concerning job satisfaction. The collected data were processed with SPSS, and the relationship among vaviables was analyzed by means of $X^2-test$, Pearson Correlation, Multiple Regression. The results of this study are as follows: 1. There is no significant difference between two groups in terms of generalized items. Age distributions show $44.3\%$ under the category of less than 34. and $55.7\%$ under the category more than 35, $19.3%$ was single and $74.6\%$ was married respectively. 2. $79.4\%$ of the nurses have the desire to have in-service education under the satisfactory physical environments such as welfare system, accommodating structures and facilities, instruments or management systems of the hospital, but under the category of unsatisfactory circumstances, $60.3\%$ have the intention of having in- service education. The concern in terms of in-service education shows statistically significant difference between two groups $(X^2=8.85,\;p<0. 05)$. This result accepts the hypothesis that good physical environments could intensify interests in service education. 3. The extent of satisfaction related to psychological environments is heightend according to good physical environments. In result, the hypothesis that the extent of satisfaction in terms of physical environments could raise satisfaction about psychological environment is accepted. 4. In the light of the extent of satisfaction about physical environments, $33.3\%$ of isolated ward nurses are satisfied with physical environments, but only $11.7\%$ of general ward nurses are satisfied. $(X^2=10.88,\; p<0.01)$. This result shows that the satisfaction degree about phusicalenvironments of isolated war nurses was higher than that of general ward nurses in spite of high physical and psychological risks due to exposure to infection. Consequently. the hypothesis was rejected that the satisfaction degree about physical environments would be lower in isolated ward nurses than in general ward nurses. 5. The fact that $87.7%$ of isolated ward nurses took interest in service education and $53.19\%$ of general ward nurses took interest in service education demonstrats that isolated ward nurse have more interest in service education than gerneral ward nurses. The result shows that the hypothesis is accepted that isolated ward nurses would have mor interests in service education than general ward nurses. 6. In the extent of satisfaction about physical environments such as morale human relationship promotion, there is no significant difference between two groups in terms of statistics. The hypothesis is rejected that satisfaction about psychologic environments would be high in isolated ward nurses than in general ward nurses. In conclusion. factors influencing on job satisfaction are considered to have effect on. another, and also interdisciplinary amelioration of factors accompanied with systematic inter cooperative investigation is necessary.

조기진통 산부의 간호 요구에 대한 산부와 간호사의 지각 차이에 대한 연구 (A Study of Mothers' and Nurses' Perception of the Nursing Needs of Women Experiencing Premature Labor)

  • The purpose of this study was to compare the nurses' perceptions with the women's about the women's needs in premature labor and to find out how to accomplish effective nursing intervention for the women to cope with their crises in premature labor. The objects of this study consisted of 33 hospitalized mothers in post partum because of premature labor, who were delivered of new born, in four university hospitals, a general hospital and a lower general hospital which has a neonatal intensive care unit in Seoul during 17 days(Oct. 4, 1994 to Oct. 20, 1994) and 32 nurses who have worked at delivery rooms in the hospitals above for 6 months or more, accomplishing nursing intervention. The results of this investigation were as follows : 1) General characteristics of women : Their mean age was 29.4 years(SD 4.37) and the mean of their gestational periods 3 days over 34 weeks(SD 2.48). Most of them were the married(93.9%) and held high school degree or more(93.9%). And some of them were the deligious(68.7%), multiparae(59.4%) and had their jobs(21.2%). The also had hospitalized experience due to premature labor(51.5%), experienced in premature delivery(33.3%), the prental care(78.6%), cesearean section(63.6%), and pregnancies less than twice(62.5%). The most mothers were included in 34 weeks and a day to 37 weeks(60.6%). It was also confirmed that most of the sustaining presences in labor were their husbands(81.2%). 2) General characteristics of nurses : Their mean age was 27.6 years(SD 4.50), and their carrier with the service on the average 6 months over 3 years long(SD 3.63). Numbers of them were staff nurses(84.4%), maiden ladies(71.9%), graduated from junior colleges of nursing(71.9%), and had no experiences in delivery(78.1%).Besides, none of them had experiences in premature labor. 3) The whole mean of needs perceived by pregnant women was identified as 3.086 points and the degree of their perception was given much weight in order of interdependence need(3.14), self concept need(3.11), physical physiologic need(3.09), and role function need(2.74). So it was proved that they regarded interdependence need as being the most important need in crises caused premature labor. 4) The whole mean of needs felt by the nurses was 3.092 points and the degree of their feelings was given much weigh in order of physical physiologic need(3.22), self concept need(3.15), interdependence need(3.06) and role function need(2.75). So it was ascertained that they felt physical physiologic need the most important, differing from the women in it. 5) There were few differences on the degrees of the perceptions that the nurses and the women showed on each of the four parts. 6) On each part of the questionnaires, it was examined that the women felt the need for the fetus to be the most important generally while the nurses perceived the need with the women to be more important than that. 7) The primiparae(40.6%) felt role function need be more important than multiparae(59.4%) (p<0.05), and the women from universities and above(39.4%) perceived self concept need to be more important than those holding the high school degrees and below(60.6%). (p<0.05) 8) The nurses having experience in delivery(21.9%) perceived interdependence need and role function need to be more important than important than those having no experiences(78.1%). (p<0.05) So far most of the nurses have concentrated their efforts on nursing for safety for the women in premature labor hospitalized in delivery rooms. But the women are not satisfied with just it because of having perceived the need for the fetus more important above all. In nursing for the future, therefore, every nurse caring for the women should offer them all the informations that they will want for their fetuses to adapt them to the crises effectively, understanding such their needs and making most of honest and simple terms for them, I insist. Through this study, I'm sure that inquiring into the need of women in premature labor minetely will show the way of more effective nursing interventions in clinic. And I suggest that the various kinds of studies about the more objects be done for the generalization of the results of this study in the future.

간호사의 보상적합도와 직무몰입 ${\cdot}$ 조직몰입정도간의 관계 연구 (The Relationship between the Nurse's Reward Fit and Job Involvement${\cdot}$Organizational Commitment)

  • This study surveyed nurses' value of reward and recognition level of organizational reward, and measured the fit of both. It also looked into the relationship between the reward fit and attitude of nurses toward their job and organization (job involvement${\cdot}$organizational commitment). It was planned to suggest the alternative of a future reward system. The sample consisted of 625 nurses of 8 private University Hospitals. Data for this study was collected from Mar. 25 to Apr. 17 by structured questionnaire. This study examined the differences of nurses' value of reward by their demographic characteristics, and looked into the relationship between the reward fit and job involvement${\cdot}$organizational commitment. Four instruments and a demographic questionnair were used to collect the data. Developed for myself and repaired by panel of judges, the value of reward scale and organizational reward scale consisted of 34 items on five points Likert-type scale. Developed by Kanungo and repaired by panel of judges, the job involvement scale measured overall job involvement on 7 items. The organizational commitment scale was developed by Mowday et al and repaired by panel of judges on 10 items. The data was analyzed by frequency, percentage, ranking, one-way ANOVA, Pearson's correlation coefficient, Chronbach alpha coefficient, t-test, SNK test, factor analysis with SPSS/PC+ progra,.Major findings are as follows 1. The mean of nurses' value of reward is 4.2435 and job content rewards are seen as the most important(M=4.5532). The following orders are seen as follows; financial rewards(M=4.4181), human realtion rewards(M=4.4130), establishment ${\cdot}$ facilities rewards(M=4.1632), professional rewards(M=4.1117), social status or prestige rewards(M=3.9228), career rewards(M=3.8816). Of 34 indivisual reward factors, the retainment allowance is seen to be thought of as the most important thing. 2. The mean of nurses' actual reward is 2.6035. The actual reward responded to the most extremely offered is job content rewards. The following orders are seen as follows ; human relation rewards(M=2.9420), financial rewards(M=2.7682), professional rewards(M=2.4601), social status or prestige rewards(M=2.3696), career rewards(M=2.3466), establishment ${\cdot}$ facilities rewards(M=1.9364). Significant value of reward differences were found among nurses by their demographic characteristics such as married status, tenure, academic career. 7. The fit of reward was significant related to job involvement and organizational commitment. When generalizing the result of this study, the value of reward, which nurses consider important and appropriate offers a reward that corresponds to the nurses' value of reward. This increases nurses' job and organization devotion further, as well as hospital effectiveness. It appears that nurses have recognized that the present reward offered in hospitals doesn't come up to their expectations so I think it is urgent to plan and perform the new reward system which is in accord with the nurses' reward fit.
