A study on maritime casualty investigations combining the SHEL and Hybrid model methods

  • This paper reviews the analysis of a given scenario according to the Hybrid Model, and why accident causation models are necessary in casualty investigations. The given scenario has been analyzed according to the Hybrid Model using its main five components, fallible decisions, line management, psychological precursors to unsafe acts, unsafe acts, and inadequate defenses. In addition, the differences between the SHEL and the Hybrid Model, and the importance of a safety barrier during an accident investigation, are shown in this paper. One unit of SHEL can be linked with another unit of SHEL. However, it cannot be used for the analysis of an accident. Therefore, we must use an accident causation model, which can be a Hybrid Model. This can explain the "How" and "Why" of accident, so it is a suitable model for analyzing them. During an accident investigation, the reason we focus on a safety barrier is to create another safety barrier or to change an existing safety barrier if that barrier fails. Hence, the paper shows how a sea accident can be investigated, and we propose a preventive way of avoiding the accident through combining the methods of different models for the future.

해양사고 온톨로지 구축 및 데이터 관리방안 연구: 서해남부해역 선박사고 상황보고서 분석을 중심으로 (A Study on Marine Accident Ontology Development and Data Management: Based on a Situation Report Analysis of Southwest Coast Marine Accidents in Korea)

  • 매년 해양활동이 증가하며 해양사고 발생빈도가 높아지고 있다. 이에 따라 해양안전을 위한 각종 연구 활동과 정책이 실행되고 있다. 하지만 이러한 노력에도 불구하고 매년 해양사고가 증가하고 있어 이들의 실효성에 대한 문제가 제기되고 있다. 문헌연구 결과에 따르면, 통계연보를 활용한 선행연구는 통계제공항목 간 비교를 통해 두드러지는 항목에 대한 예방책을 제시하고 있다. 2000년대 이후에는 대형 해양사고가 반복적으로 발생하면서 '사고대응'에 대한 사례연구(case study)가 진행되고 있다. 국내 해양안전을 위한 정책수립 연구과정에서 통계연보나 사고사례를 주 자료로 활용하고 있으나 현재 자료는 사후결과 요약정도의 수준이다. 따라서 본 연구에서는 해양사고 사례분석 및 개선방안 관련 문헌연구를 통해 현행 해양관련 연구와 정책의 한계를 탐색하였다. 또한 자료 활용 한계를 개선하기 위한 방안의 일환으로 선박사고 상황보고서 속성분석, 텍스트 마이닝을 통해 해양사고 정보 분류체계인 온톨로지(ontology)를 수정 보완하였다. 해당 항목은 '신고자, 신고수단, 구조세력, 대응 조치사항, 대응취약성, 적재물, 유류유출경위, 피해유형, 사고처리결과'이며, 이 항목들은 분류체계 표준용어를 활용해 향후 지속적으로 수집 활용할 수 있다. 마지막으로 온톨로지를 실질적으로 활용하기 위한 데이터 수집 및 품질확보 방안을 제시했다. 결과적으로 현재 해양안전이 직면한 문제를 명확히 파악하고 '품질이 확보된 충분한 정보'를 활용한다면 보다 다양한 연구와 실효성 있는 정책 실현이 가능할 것이다.

소형 승강기 제동장치의 거동해석 (Behavior Analysis of Small Size Elevator Brake)

  • Elevator is very important transportation facility of modern society. But there have been frequent elevator accidents recently. In the view of safety management of elevator, the corresponding study must be done since accidents related elevator include not only actual accident but also possible accident in order to protect user from danger. A 3D design was developed using SolidWorks and behavior analysis using FEM Code.

소형선박 운용자의 냉각수 관리에 관한 고찰 (A Study on the Management of Engine Coolant in Small Fishing Vessels)

  • Majority of marine accidents that occur on fishing vessels are engine accidents. This comprises more than 26 % of the total annual fishing vessel marine accident cases. Large numbers of engine accidents happen in the cooling water system, which are mostly caused by negligence on regular check-up and repair. Notably, small-sized ships have higher engine accidents occurrence rate compared to medium-and large-sized ships. Based on the Report of the Korea Ship Safety Technology Authority, engine accident cases reached 3,032 out of the total 3,081 cases. This study researches on the differences between the small-sized ship pilot, an operator of a vessel engine of less than 200 tons, and a 6th level marine engineer, in terms of the relationship between management forms and what causes the marine accidents in association with the cooling water system. It also studies and analyzes the differences in frequency of the accident occurrence between the two groups. ${\chi}^2$ qualification was imposed through the SPSS statistical analysis program and it got qualified at the significance level of 5%. The research shall be utilized as one of the base line data for the reduction of marine accidents.

  • 해양사고는 많은 원인이 서로 복잡하게 상호작용을 하여 발생하고 있다. 이러한 해양사고의 분석은 선박의 안전 운항상의측면에서 매우 중요하다고 할 수 있다. 따라서 본 연구의 목적은 시스템다이내믹스법을 이용하여 해양사고 원인과 개선책에 대한 모델을 구축하고, 요소의 개선에 대한 효과를 분석하고자 한다. 본 연구의 수행을 위해 해양사고 원인과 개선책의 요소를 브레인스토밍법에 의해 추출하고, 인과지도상의 정량적, 정성적, 피드백루프로 변환하였다. 그리고 표준모델과 4가지 정책모델에 대해 23년간($1997\~2020$) 시뮬레이션을 수행하였다.

A Study on the Analysis and Identification of Seafarers' Skill-Rule-Knowledge Inherent in Maritime Accidents

  • The purpose of this study is to classify the deficient abilities of seafarers into SRK (Skill, Rule, and Knowledge) and analyze and identify the SRK by the type of accident and ship. Experimental data used the SRK cumulative frequency for 1,606 marine accident records and two-way ANOVA and t-test were used for the analysis tools. The results of two-way ANOVA showed that it is possible to identify the deficient abilities by using the cumulative frequency of SRK in both accident and ship types. As a result of the t-test, the adoption of the null hypothesis (H=0) that the mean of two pairs is equal and the rejection of the null hypothesis (H=1) were 29.2 % and 70.8 %, respectively. For the ship type, H=0 is 33.3 % and H=1 is 66.7 %. Through this study, it was found that about 70 % of the deficient abilities of seafarers inherent in maritime accidents can be identified using the proposed method.

Comparison Analysis between the IWRAP and the ES Model in Ulsan Waterway

  • According to the Marine Traffic Safety Law, revised in 2009, Marine Traffic Safety Audit is introduced to secure the safe navigation, to prevent the marine accident and to maximize the efficiency of the port. In this audit system, marine traffic safety assessment is the most important scheme because the primary purpose of the audit system is to identify potential risk elements affecting safe navigation. Even though the reliability of audit result depends on the selection of assessment models, there are no independent assessment models for Korean coastal waters and most of models used in Korea currently are developed by foreign countries. Therefore, the development of the independent assessment model for Korean coastal water is required. This study, prior to the development of independent assessment model, aims to provide a basic data by comparing two foreign assessment models in Ulsan port area with marine accident statistics data.
