복합레진의 충전방법에 따른 변연접합성에 관한 주사전자현미경적 연구 (A Scanning Electron Microscopic Study for the Effect of the Filling Techniques of Composite Resin on the Marginal Adaptation)

  • The purpose of this study was to evaluate the marginal adaptation of Class V resin restorations by the filling techniques. The cavities were filled with Heliosit$^{(R)}$ light curing composite resin system in accordance with the following three filling techniques - bulk-pack placement technique, apical to occlusal layering placement technique, and buccolingual layering placement technique. And the interface between the restoration and the tooth structure was observed with scanning electron microscope. The following results were obtained. 1. Marginal adaptation of enamel showed excellent irrespective of the filling techniques. 2. Marginal leakage of dentin was observed irrespective of the filling techniques. 3. No difference of marginal adaptation according to the filling techniques was observed in light curing composite resin.

Comparative analysis of the clinical techniques used in evaluation of marginal accuracy of cast restoration using stereomicroscopy as gold standard

  • This study assessed the effect of preparation design on marginal adaptation and also compared the sensitivity and specificity of clinical evaluation techniques for marginal accuracy of cast restorations to stereomicroscopy. MATERIALS AND METHODS. Three Ivorine molar teeth of different designs were prepared. (A)-A complete crown preparation with buccal shoulder and beveled finish line. (B)-A complete crown preparation with chamfer finish line. (C)-A three-quarter crown preparation with proximal boxes and beveled finish line. Twenty four castings were prepared with eight castings for each design respectively. Each casting underwent examination with an explorer, disclosing media, and a stereomicroscope. Stereomicroscopy at a value less than or equal to 30 microns was used as a gold standard to evaluate the significance of different designs on marginal adaptation. Chi-square tests of independence and Kruskal-Wallis were used to evaluate the effect of preparation design and compare the agreement between examination methods for detection of marginal gap size of greater than or equal to 30 microns (${\alpha}$=.05). Sensitivity and specificity for explorer and disclosing media as compared to stereomicroscope was calculated using statistical formula given by Park. RESULTS. The preparation design did not significantly affect overall marginal adaptation. Examination by explorer and disclosing media at $30 \;{mu}m$ revealed 39% and 10.06% sensitivity and 91% and 82% specificity respectively. CONCLUSION. Preparation designs examined in this study did not significantly affect the marginal adaptation of the castings. Commonly used clinical evaluation techniques using explorer and disclosing media appeared to be inadequate for assessment of marginal accuracy.

레이저선택용융기술에 의해 제작된 금속 코어의 변연 적합도 분석 (An analysis of marginal adaptation of metal cores fabricated by selective laser sintering)

  • Purpose: The purpose of this study were to analysis of marginal adaptation of metal cores fabricated by selective laser sintering. Methods: Main model was prepared and ten stone models were fabricated. Ten single metal cores were fabricated by selective laser sintering(SLS group) and another ten single metal cores using lost wax technique and casting method were manufactured(CAST group). The marginal adaptation of metal cores were analysis using by the silicone replica technique. Silicone replicas were sectioned two times. The marginal adaptations were measured using by digital microscope. Statistical analyses was performed with independent t-test(${\bullet}{\cdot}=0.05$). Results: Means of marginal adaptations were 90.3 ${\ss}{\uparrow}$ for SLS group and 65.1 ${\ss}{\uparrow}$ for CAST group. Two groups were statistically significant differences (p < 0.05). Conclusion: Marginal adaptation of metal cores fabricated by selective laser sintering were ranged within the clinical recommendation.

3D 프린팅 기술로 출력된 치과 보철물의 임상적 적합도 평가 (Evaluation of clinical adaptation of dental prostheses printed by 3-dimensional printing technology)

  • Purpose: The objectives of this study was to evaluate clinical adaptation of dental prostheses printed by 3 dimensional(3D) printing technology. Methods: Ten study models were prepared. Ten specimens of experimental group were printed by 3D printing(3DP group). As a control group, 10 specimens were fabricated by casting method on the same models. Marginal gaps of all specimens were measured to evaluate clinical adaptation. Marginal adaptations were measured using silicone replica technique and measured at 8 sites per specimen. Wilcoxon's signed-ranks test was used for statistical analysis(${\alpha}=0.05$). Results: Means of marginal adaptations were $95.1{\mu}m$ for 3DP group and $75.9{\mu}m$ for CAST group(p < 0.000). Conclusion : However, the mean of the 3DP group was within the clinical tolerance suggested by the previous researchers. Based on this, dental prosthesis fabricated by 3D printing technology is considered to be clinically acceptable.

수복물의 미세누출도와 변연적합도 측정에 사용되는 방법 사이의 상관 관계 평가 (ESTIMATION OF RELATION BETWEEN TECHNIQUES OF DYE PENETRATION FOR MICROLEA-KAGE AND SEM EVALUATION FOR MARGINAL ADAPTATION OF THE RESTORATION)

  • 본 연구에서는 색소 침투법을 이용한 5급 복합레진 수복물의 미세누출도와 주사전자 현미경을 이용한 변연적합도를 측정하고 두 방법 사이의 상관성을 평가하고자 하였다. 발거된 사람의 대구치 20개의 협면에 고속의 330번 버를 이용하여 5급 와동을 (백아법랑경계부를 중앙으로 가로 4 mm, 세로 3 mm, 깊이 1.5 mm) 형성하고, 평가시 참고점으로 사용하고자 1/2번 원형 버로 6개의 함몰부를 만들었다. 제조사의 지시에 따라 단일 단계의 올인원 접착제인 Xeno III로 치면을 처리한 다음, 미세혼합형 복합레진인 Esthet X로 단일 충전하였다. 시편을 식염수에 1일간 보관한 후 Sof-Lex 시스템으로 마무리 및 연마하였다. 이후 5도와 55도에서 1000회 열순환시킨 다음, 50%질산은 용액을 이용하여 침투시켰다. 실리콘 고무 인상재와 폴리우레탄 모형재로 수복물 주변의 레진 복제물을 제작하고 금도금한 다음 주사전자현미경으로 외부 및 내부 변연부의 변연적합도를 측정하였다. 또한 참고점을 기점으로 수복물을 3분한 다음, 미세누출도를 측정하였다. 내부 및 외부 변연적합도와 미세누출도 평가법 사이의 상관관계는 Spearman's rho test를 이용하여 다음과 같은 결과를 얻었다. 1. 미세누출도와 각각의 외부 및 내부 변연 적합도는 유의한 상관관계를 보였다 (p <0.01). 2.내부와 외부의 변연 적합도 사이에는 유의한 상관관계를 보이지 않았다.

기성금관수복 유구치의 변연 적합도에 관한 연구 (A STUDY ON MARGINAL ADAPTATION OF READY-MADE STAINLESS STEEL CROWN TO THE PRIMARY MOLAR)

  • In Class II amalgam restoration in deciduous molar, failure rate and incidence of recurrent caries are high as children become older. In order to preserve deciduous molars till the physiologic exfoliation time, stainless steel crown is a choice of the treatment. As a result of a careless treatment, such as overhanging margin, poor marginal adaptation, poor proximal contour and inadequate mesiodistal width give rise to interfering eruption of the adjacent teeth, recurrent caries and chronic gingival irritation and insufficient arch length respectively. In this study, 252 s.s. crowned teeth extracted due to physiologic exfoliation or periapical lesion. The purpose of this study is to analyze the marginal adaptation of stainless steel crown to the deciduous molar in order to obtain better clinical result. The results were as follows : 1. Between the length of s.s. crown and the marginal gap of crown, positive correlations were shown. 2. Largest amount of marginal gap was shown at buccal side in upper deciduous molars and lower first deciduous molar, lingual side in lower second deciduous molar. But no significant diffrence were found statistically compared to second most largest one. 3. Incidence of exposed restoration and recurrent caries were higher in proximal surface than buccal/lingual surface. And extension of restoration below the margin of s.s. crown gives rise to higher rate of recurrent caries. 4. Defect of contour was found in 34%, frequently found in lower 1st deciduous molar and upper 1st deciduous molar. 5. Marginal polishing defects were found in 23%. 6. Ledge was formed in 10% especially in lower 1st deciduous molar and lower 2nd deciduous molar. 7. 16% of the teeth had wear facet due to traumatic occlusion, 7% of them had occlusal perforation.

치과용 3D 프린팅 시스템에 의해 출력된 보철물의 품질 평가 (An evaluation of quality of dental prostheses printed by dental 3-dimensional printing system)

The marginal adaptations were evaluated using by digital microscope. Statistical analyses were performed with Mann-Whitney test(${\alpha}=0.05$). Results: $Mean{\pm}standard$ deviations of all marginal adaptations were $92.1(20.0){\mu}m$ for Metal group and $69.7(12.3){\mu}m$ for Resin group. Two groups were no statistically significant differences(p>0.05). Conclusion: Marginal adaptation of single crowns printed by 3D-printing system were ranged within the clinical recommendation.

How adjustment could affect internal and marginal adaptation of CAD/CAM crowns made with different materials

  • PURPOSE. Recently introduced hybrid and reinforced glass ceramic computer-aided design/computer-aided manufacturing (CAD/CAM) materials have been used for full-coverage restorations. However; the effect of adjustment and type of materials on internal and marginal adaptation are unknown. This study aimed to evaluate and compare the marginal and internal adaptations of crowns made of three different CAD/CAM materials before and after adjustment. MATERIALS AND METHODS. One acrylic resin maxillary first molar was prepared and served as the master die. Thirty-six restorations were fabricated using CAD/CAM system (CEREC Omnicam, MCXL) with three materials including lithium disilicate (IPS e.max CAD), zirconia-reinforced lithium silicate (Suprinity), and hybrid ceramic (Enamic). Internal and marginal adaptations were evaluated with the reference point matching technique before and after adjustment. The data were analyzed using mixed ANOVA considering α=.05 as the significance level. RESULTS. The effect of adjustment and its interaction with the restoration material were significant for marginal, absolute marginal, and occlusal discrepancies (P<.05). Before adjustment, Suprinity had lower marginal discrepancies than IPS e.max CAD (P=.18) and Enamic (P=.021); though no significant differences existed after adjustment. CONCLUSION. Within the limitations of this study, crowns fabricated from IPS e.max CAD and Suprinity resulted in slightly better adaptation compared with Enamic crowns before adjustment. However, marginal, axial, and occlusal discrepancies were similar among all materials after the adjustment.

In-vitro evaluation of marginal and internal fit of 3-unit monolithic zirconia restorations fabricated using digital scanning technologies

  • PURPOSE. This study aimed to compare the marginal and internal fit of 3-unit monolithic zirconia restorations that were designed by using the data obtained with the aid of intraoral and laboratory scanners. MATERIALS AND METHODS. For the fabrication of 3-unit monolithic zirconia restorations using impressions taken from the maxillary master cast, plaster cast was created and scanned in laboratory scanners (InEos X5 and D900L). The main cast was also scanned with different intraoral scanners (Omnicam [OMNI], Primescan [PS], Trios 3 [T3], Trios 4 [T4]) (n = 12 per group). Zirconia fixed partial dentures were virtually designed, produced from presintered block, and subsequently sintered. Marginal and internal discrepancy values (in ㎛) were measured by using silicone replica method under stereomicroscope. Data were statistically analyzed by using 1-way ANOVA and Kruskal Wallis tests (P<.05). RESULTS. In terms of marginal adaptation, the measurements on the canine tooth indicated better performance with intraoral scanners than those in laboratory scanners, but there was no difference among intraoral scanners (P<.05). In the premolar tooth, PS had the lowest marginal (86.9 ± 19.2 ㎛) and axial (92.4 ± 14.8 ㎛), and T4 had the lowest axio-occlusal (89.4 ± 15.6 ㎛) and occlusal (89.1 ± 13.9 ㎛) discrepancy value. In both canine and premolar teeth, the D900L was found to be the most marginally and internally inconsistent scanner. CONCLUSION. Within the limits of the study, marginal and internal discrepancy values were generally lower in intraoral scanners than in laboratory scanners. Marginal discrepancy values of scanners were clinically acceptable (< 120 ㎛), except D900L.

CAD/CAM으로 제작한 코누스내관의 변연적합 (MARGINAL ADAPTATION OF THE CONICAL INNER CROWN FABRICATED WITH CAD/CAM)

  • This study was to evaluate the fabrication method and marginal adaptation of the conical inner crown fabricated with CAD/CAM. The informations on abutment teeth were transferred to a computer with a micro contact digitizer, which had a $50{\mu}m$ accuracy on the master die. A conical inner crown was designed on a computer and a real crown was machined based on this design using CAM. The marginal fit of a computer-machined conical inner crown was assessed using electron microscopy Measurement of the marginal gap between the conical inner crown and the abutment was performed on four different locations (mesial, distal, buccal, and lingual surfaces) of the finish line. The evaluation was based on 10 test specimens. The results were as follow. 1 The mean marginal gap between the conical inner crown and abutment tooth was $83.2{\pm}43{\mu}m$, 28.9% of the specimen showed marginal gap over $100{\mu}m$. 2. The fabrication method using CATRS and CAM provided clinically acceptable marginal fitness compared to conventional casting method (P<0.05).
