A Review of Korean Medicine Treatment for Postherpetic Neuralgia

  • The purpose of this study was to analyze the Korean medicine treatment methods for Postherpetic Neuralgia (PHN) in Korea. There were 5 online databases searched (OASIS, NDSL, RISS, KISS and KMBASE) for studies which were related to PHN. A total of 12 studies were selected. Various treatments such as acupuncture, herbal medicine, and moxibustion were reported treatments for PHN, some of which included Western medicine and Korean medicine treatment. Korean herbal medicine was the most frequently used treatment method, followed by manual acupuncture. Sipjeondaebotanggami was the most frequently used prescription. Poria Sclerotium was the most frequently used principal herb, followed by Angelicae Gigantis Radix, Zingiberis Rhizoma, and Ginseng Radix. GB44, LI4, LR3 were the common acupoints used for the treatment of PHN. CV12 was the most frequently used moxibustion point, and gabapentin was the most frequently used concomitant Western drug. Additional research on Korean medicine treatment of PHN is expected in the future.

Efficacy Comparison of Different Acupuncture Treatments for Hot Flashes: A Systematic Review with Network Meta-Analysis

  • The objective of this study was to conduct a systematic review and network meta-analysis to evaluate and compare the effectiveness of various types of acupuncture for menopausal hot flashes (HF). Randomized controlled trials (RCTs) were retrieved from 8 electronic databases, and the risk of bias was evaluated for the included studies. Pairwise meta-analysis and network meta-analysis were performed using Review Manager and R software for indirect comparison and ranking, respectively. In total, 23 RCTs (2,302 patients) were eligible for systematic review, of which 10 were included in network meta-analysis. Network meta-analysis showed manual acupuncture (MA) had the highest probability of reducing HF frequency and severity, followed by sham acupuncture (SA), electroacupuncture, usual care, or no treatment; furthermore, warm acupuncture significantly improved menopause-specific quality of life compared with MA or electroacupuncture. Compared with hormone replacement therapy, acupuncture had less efficacy in reducing HF frequency but enhanced menopause-specific quality of life. There was no significant difference between MA and SA in mitigating HF. The existing evidence showed that MA could be used for alleviating menopausal HF. However, it is recommended that more high-quality RCTs should be performed.

A Review of Acupuncture Treatment Methods for Polycystic Ovary Syndrome (다낭성난소증후군의 침 치료법에 대한 고찰)

  • Objectives: The purpose of this study is to review the acupuncture treatment for Polycystic ovary syndrome (PCOS) in women. Methods: We searched articles in 3 search engines with keywords related to 'Polycystic ovary syndrome', 'PCOS', and 'Stein-leventhal' in February 2024. Clinical researches and case reports that used acupuncture on PCOS were included. Animal studies and non clinical data were excluded. Data on acupuncture treatment such as methods, site, duration, frequency, and period were analyzed. Results: Of 60 selected articles, there were 51 randomized controlled trials, 5 clinical trials and 4 case reports. Studies were conducted using manual acupuncture, electro acupuncture, auricular acupuncture, thread embedding acupuncture, warm needling and laser acupuncture. Most studies used more than one acupoint, and there were 78 acupoints selected for acupuncture treatment for PCOS. The most commonly used acupoint was 三陰交 (SP6)(n=50). By analyzing the network of acupoints, 關元 (CV4), 氣海 (CV6), 中脘 (CV12), 三陰交 (SP6), 血海 (SP10), 天樞 (ST25), 足三里 (ST36) were located in center of the network. The mean treatment time, number of treatments, and duration were 28.63±4.48 minutes, 34.52±29.26 times, and 98.18±38.25 days. Conclusions: The results of this study could be useful in establishing the evidence for performing standardized acupuncture treatment for Polycystic ovary syndrome.

Proposal of East-West Integrative Medicine Manual for Mild Cognitive Impairment in a Korean Medicine Hospital (일개 한의의료기관 내 경도인지장애의 의한협진 매뉴얼 제안)

  • Objectives: This is one of the manuals of East-West integrative medicine, which was created by the Committee on integrative medicine of Chung-Yeon Korean Medicine Hospital. To support clinical decision making and communication in the East-West cooperative treatment of mild cognitive impairment. Methods: Drafting was based on literature review from the MEDLINE, EMBASE, OASIS, and CNKI databases using search terms such as 'mild cognitive impairment', 'mild neurocognitive disorder', 'acupuncture', 'herbal medicine' and 'traditional Chinese medicine'. An amendment reflecting the goal of consultation and detailed treatment contents was made by reviewing the draft and holding discussion with the rehabilitation specialist of western medicine. The committee then agreed to adopt the manual through the process of review and feedback in addition to face-to-face discussions. Results: This manual contains the diagnosis of mild cognitive impairment, the goal of consultation, and the cooperative treatment contents of the East-West medicine for mild cognitive impairment. Conclusions: This manual provides significant information about the decision making process and contents of treatment in one medical institution for East-West cooperative treatment of mild cognitive impairment.

Difference in NOS between 2 Hz and 100 Hz EA in cerebral cortex, brain stem and cerebellum of spontaneously hypertensive rats (전침자극이 Spontaneously Hypertensive Rat의 대뇌겉질, 뇌줄기, 소뇌 부위의 Nitric Oxide Synthase 신경세포에 미치는 영향)

  • Background and Objetive : The aim of this study was to investigate the effect of various electroacupuncture stimulation on NADPH-diaphorase in cerebral cortex, brain stem, cerebellum of spontaneously hypertensive rats. Materials and Methods : We evaluated the changes of NADPH-d-positive neurons using a histochemical method. The staining intensity of NADPH-d-positive neurons was assessed in a quantitative fashion using a microdensitometrical method based on optical density by means of an image analyzer. Results and Conculsion : The average optical density of NADPH-d-positive neurons of 100 Hz (bipolar square wave 0.2 ms duration and 100 Hz frequency) electroacupuncture treatment group significantly increased in most cortical areas comparison between the manual acupuncture and 2 Hz (bipolar square wave 0.2 ms duration and 2 Hz frequency) electroacupuncture groups. In the brain stem, the optical density of NADPH-d-positive neuron at only superficial gray layer of the superior colliculus area was same as cerebral cortex. We conclude that the morphological evidence for NADPH-d-positive neurons may be have regional change in cerebral cortex brain stem and cerebellum according to various electroacupuncture stimulations.

Effectiveness of Combined Korean Medicine on Traumatic Spinal Cord Injury: A Case Report

  • Objectives: This study aims to evaluate the clinical effectiveness of a Korean medicine treatment on a traumatic spinal cord injury. Metohds : A 44-year-old male patient who had suffered a traumatic spinal cord injury was treated for two months. The effectiveness of complex Korean medicine treatments was measured using the Numeric Rating Scale (NRS) and the Manual Muscle Test (MMT). Result : We found that the NRS of pain caused by numbness was decreased from 7 to 0-1, and the overall motor measured by the MMT was also improved. The patient's walking state was changed from wheelchair ambulation to walker ambulation. Conclusion : Based on this finding, complex Korean medicine treatments may be effective in treating patients' pain, controlling paresthesia, and aiding the recovery function of activities in daily living. However, because of the limitation of our study, further high-quality research should be conducted.

Effect of Electroacupuncture Stimulation on Activity of Neuronal NOS in Rats (흰쥐의 neuronal NOS 신경세포의 activity에 대한 전침자극 효과)

Narrative Review of the Association between Cervical Region Treatment and Facial Paralysis

  • Facial nerve palsy refers to sudden, unilateral lower motor neuron facial paralysis. This study aimed to determine the importance of neck treatment in the treatment of facial paralysis. A literature search was performed on six online databases and other sources until January 15, 2023. A total of 426 papers were retrieved. After excluding duplicated and inconsistent papers, papers not including cervical treatment, and experimental papers on animals, two papers were finally selected. The type of treatment method, therapeutic effects, assessment of the risk of bias in randomized controlled trials, and non-randomized controlled trials and side effects were evaluated. Chiropractic, manual therapy, facial meridian massage, and acupotomy were applied to the face and cervical spine region. The results showed that each treatment had a significant therapeutic effect through evaluation index measurement methods, such as the visual analog scale and Yanagihara's unweighted regional grading system. This study demonstrated the importance of the cervical spine area in the treatment of facial paralysis. However, this study has many limitations. Thus, high-quality randomized controlled comparative studies on the treatment of the cervical spine area only or studies that include cervical spine area treatment as an interventional treatment while performing oriental or comprehensive treatment are needed.

  • We experienced a patient(53-year-old female) with knee joint effusion, pain and stiffness who had not responded to steroid injection therapy, significantly improved by bee venom acupuncture in clinical symptom. And for a opinion that prohibited bee venom acupuncture on DM because of increasing serum glucose level by increasing serum cortisol level, we also experienced that serum glucose level of DM patient stabilized by bee Venom acupuncture treatments, so we report this case with literature review of articles.

Ananlysis of Correlation between Functional Leg Length Discrepancy caused by Pelvic tilting and Femur head height difference (골반변위와 대퇴골두 높이차이에 대한 상관성 비교 - Gonstead's Technique을 중심으로 -)

  • Objectives : To investigate contributing degree of other factors except pelvic tilting to F.L.L.D by analizing with Gonstead technique on the correlation between femur head height discrepancy on the standing pelvic AP view and F.L.L.D caused by pelvic tilting. Method : We analysed standing pelvis AP X-ray of 70 patients who had visited at the department acupunture and moxibustion in Conmaul oriental medical hospital, during May, 1st, 2004 - July, 30th, 2004, with low back pain or lower extremity pain. We excluded the person with any past history of polio, genetic defect, malunited fracture, growth plate injury, infection and overgrowth attributable to hemangioma, or arteriovenous fistula. Results & Conclusion : The functional leg length discrepancy caused by pelvic tilting and femur head height difference had no statistical difference(p=0.132) but poorly correlated(Pearson ${\nu}=0.05$). In the 94.28% of subjects, the femur head height difference wasn't in accord with F.L.L.D. caused by pelvic tilting. In 47.14% of subjects were expected to have over $3^{mm}$ of leg length discrepancy after pelvic adjustment. The mean of measurement difference between two methods was $3.76{\pm}3.12^{mm}$ and the range was $0{\sim}11.4^{mm}$. Consequently, we must consider not only functional leg length discrepancy caused by pelvic tilting but also anatomical leg length discrepancy, misalignment of ankle, knee or hip joint etc.
