The Effects of Group Exercise, Manual Therapy and Home Exercise on Pain, Range of Motion and Function in Patient with Adhesive Capsulitis

  • Purpose: The purpose of this study was to determine the effects of group exercise, manual therapy, and a home exercise program on VAS (visual analogue scale), ROM (range of motion) of shoulder, and functional outcome in adhesive capsulitis. Methods: The study was conducted with 45 patients. All subjects were randomly assigned to 3 groups: group exercise (n=15), manual therapy (n=15), home exercise (n=15). The subjects performed an intervention program 3 times a week for 4 weeks a total of 12 times. Outcome measurements were VAS, ROM, and Constant-Murley score outcome measures. Measurements were taken four times, at baseline, 4 weeks later, 8 weeks later, and 12 weeks later. Results: VAS, ROM of the shoulder, and functional outcomes showed improvement in all groups at the final follow-up (p<0.05). VAS and ROM showed greater improvement with group exercise and manual therapy than home exercise, however, there were no significant differences between the two groups (p>0.05). Functional outcomes showed the greatest improvement in group exercise at the final follow-up. This improvement was greater than with manual therapy or home exercise (p<0.05). Conclusion: Group exercise and manual therapy were more effective for improving pain, ROM, and function than home exercise in adhesive capsulitis. In particular, group exercise is more effective in functional recovery than other physiotherapy interventions.

척추도수교정이 요통에 미치는 영향 (The Effect of spinal manipulation on the Lumbar Spine with LBP patients)

  • Purpose: Many people are suffering from Low back pain due to HIVD and muscular problems, lack of joint functions on lumbar spine. In this study we compared the change of the herniation index, Oswestry LBP disability index (OLDI), visual analog scale (VAS), lumbar flexion range of motion (ROM) between the pre-experiment and after 4 weeks treatment by maitland manual therapy. Method: we selected and managed both the 15 people. They are $46.80{\pm}15.46$ years old people with HIVD and Stenosis. We treated for the people with HIVD and Stenosis by manual therapy(maitland manipulation method) during 4 weeks. And then we compared with pre-experiment and after 12 weeks through measuring the herniation index change by using computor themograpy (CT), LBP OLDI, VAS, lumbar flexion ROM. Results: The changes in the herniation index, Oswestry lumbar Disability Index, VAS, lumbar flexion ROM between the pre-experiment and after 4 weeks treatment by maitland manual therapy, there was a statistically significant difference. Although there was a significant difference after 4 weeks in OLDI, VAS, lumbar flexion ROM. But disc herniation index was no significant difference. Conclusion: Manual therapy is very effective for Lumbago due to the HIVD and spinap stenosis patients. OLDI, VAS and lumbar flexion ROM were increased. But disc herniation index was no significant difference. We suggest the combination treatment between manual exercise and spinal traction.

무릎뼈 높이에 대한 도수측정방법의 신뢰도와 타당도 (Reliability and Validity of the Manual Measurement Method for Patellar Height)

  • This study was to determine the reliability and validity of manual measurements of patellar height to standard radiographic measurements in 30 knees of 15 subjects. Patellar height was measured using manual and radiographic methods. The manual measurements were performed by two examiners using digital vernier calipers with the subject sitting and the knees in $30^{\circ}$ of flexion. The radiographic measurements were performed in the same position. The reliability of the manual measurements was assessed by means of intraclass correlation coefficients [ICC(3,1)], and the validity was investigated using the Pearson's product-moment correlation coefficient and an independent t-test. The intra- and inter-rater reliabilities of the manual measurement of patellar height were excellent (ICC=.86 and .88 respectively). The validity of patellar height measured manually compared to the radiographic method was good (Pearson's r=.69). In conclusion, the manual method is an objective, qualitative measurement of patella height.

간헐적 견인과 도수 견인이 요추 추간판 탈출에 미치는 효과 (The Effects of Intermittent Traction and Manual Traction on Lumbar Herniated Disc)

  • Purpose: The purpose of this study was to investigate the effects of intermittent lumbar traction and manual traction on a $L_{4-5}$ herniated disc. Method: The subjects were randomly divided into the intermittent lumbar traction group (n=10) and the manual traction group (n=10). The intermittent traction group had traction-treatment applied for 12 times in the first two weeks, and then 6 times in the next two weeks. The time the traction was applied was for 30 minutes. The manual traction group had flexion-distraction therapy applied for 12 times in first two weeks, and for 6 times in next two weeks. The time the traction was applied was 3 to 6 minutes per treatment. Results: The change of the muscle test (MT), the disc herniation index (DHI) and the sagittal $T_2$ weighted MRI was measured at pretreatment and 4weeks and 12weeks after treatment. Conclusion: We found that intermittent lumbar traction and manual traction could improved the MT and DHI and this could improve the rehabilitation of patients with lumbar herniated disc.

멀리건 도수치료가 여성 퇴행성 슬관절염 환자의 통증과 근 기능평가에 미치는 효과 (The effect of mulligan manual therapy on pain and muscle assessment questionnaire in female elders with osteoarthritis of the knee)

  • 본 연구는 퇴행성 슬관절염 여성 30명을 대상으로 물리치료와 멀리건 도수치료를 병용하여 4주간 실시한 후 운동전, 2주 후, 그리고 4주 후에 통증과 근 기능평가 측정에 미치는 효과에 대해 알아보기 위해 실시하였다. 본 연구의 측정은 시각적 상사 척도 검사, 근력 검사, 근 기능 검사, 그리고 협응력/균형감을 검사하였다. 반복측정자료 분산분석 결과, 물리치료와 멀리건 도수치료를 병용한 실험군에서 중재기간에 따라 효과적 이었으며, 시간과 그룹 간 상호작용도 있었다. 퇴행성 슬관절염의 통증과 근 기능평가를 개선시키기 위해 물리치료와 멀리건 도수치료 병용 시 효과적이란 것을 검증하였다.

  • The PT program provides relief to most patients with symptom of TOS. So the purpose of our study is to evaluate a effects of PT program in patients with TOS. PT program included orthopaedic manual therapy with the aim of restoring the function of the upper thoracic aperture is to be recommended, and long-term following is advisable. 1. Mean treatment duration was over an 11.4 day(range 4~24). 2. At the follow-up evaluation, 88.1% of the patients were satisfied with the effects of their therapy. 3. 73% of the patients returened to work after PT and 88% of the patients carried through the recommendations given at discharge during follow up. 4. Normalized grip strength and Tinel' sign predicted patient satisfaction(p< .001) and return to work(p< .001). 5. Return to work was more often successful if the work was sedentary rather than heavy (p< .05).

요통 환자에 대한 물리치료방법의 적용 시간을 중심으로 한 기술적 연구 (Physical Therapy Session Duration in patients with low Back Pain: Descriptive Research)

  • Objective: The purpose of study was to compare physical therapy duration in relation tohealth care organization system in patients with low back pain. Subjects: Subjects of this study were 759 patients who are receiving physical therapy at 56 health care organization. Methods: Data were collected by questionnaire that had been completed by patients and physical therapist for two months. Physical therapy procedures consisted of modality application, manual therapy, active therapeutic exercise, and patient education. Physical therapy session duration was investigated for each physical therapy procedure. Data were analyzed in relation to the university hospital, hospital, and clinic. Results: The mean age of subjects was $42.84{\pm}15.46$. There were no significant differences in age among three groups. The number of patients diagnosed with herniated disc were 244(32.15%) and with mechanical low back pain was 187(24.64%). The mean treatment duration per day was 53.22 minutes, and the longest treatment duration was 61.28 minutes at the university hospitals (p<0.001). The mean modality application duration was 42.17 minutes, and the longest application duration was 46.26 minutes at the university hospitals (p<0.001). The mean duration for manual therapy was 5.11 minutes, and the longest treatment duration was 5.97 minutes at clinics. The mean duration for active therapeutic exercise was 4.16 minutes. It was 7.60 minutes at the university hospitals, and 2.48 minutes at clinics. There was a significant difference in active therapeutic exercise duration between university hospitals and clinics(p<0.001). For modalities, hot packs(89.33%) and interferential current therapy(60.87%) were mostly applied. For manual therapy, Soft tissue mobilization(32.93%) and manipulation(14.10%) were mostly applied. In general, treatment application duration was longer at University hospital (p<0.05). For therapeutic exercise, exercise without equipment(18.84%) and muscle strengthening (16.73%) were mostly performed. The longest treatment duration for therapeutic exercise was 7.60 minutes at the university hospital(p<0.05). Conclusion: physical therapy session duration for low back pain was 53.22 minutes. Modality application constitutes 79%, manual therapy 10%, active therapeutic exercise 8% of total treatment duration. It is concluded that patients do not participate actively in treatment procedures.

경골 Pilon 골절 후 가동술(mobilization)과 복합운동 적용사례 - 증례 보고 - (The Mobilization and Combined Exercises Application after Fibula Pilon Fracture Operation: Case Study)

  • Purpose: The purpose of this study was to evaluate the effects of mobilization application for the lower extremity after fibula Pilon fracture operation patient. Methods: The subjects was 62 years old male who was injury of Lt. fibula shaft Fx., 3 cuneiform & cuboid Fx., 2.3.4 metatarsal bone Fx., We were compared to result of physical therapy between pre and post exercise for 2weeks. Results: The results of this study were summarized below; The mobilization application of the Lt. lower extremity was significantly differences of the ROM at pre and post therapy after 2 weeks, especially in knee flexion ($40^{\circ}$). The increased of accessary movement was evaluated to increased of the physiologic movement about the joints of the lower extremity. Conclusion: We consider that factors of therapy result were not only fracture types, operation and reduction methods for the fibula Pilon fracture but also the ability of physical therapist's manual techniques.

종아리 근경련 환자에 대한 도수치료 사례 연구 (Case Study; Manual Therapy for Patients with Calf Cramps)

  • Purpose: The first purpose of this study was to determine the effect of reciprocal inhibition that influence changes in pain when applied to patients with culf cramps. The second purpose of this study was to determine the effect of manual therapy on iliaosacral joint that influence changes number of occurrences when applied to patients with frequent nocturnal culf cramps. Methods: The first study using reciprocal inhibition techniques to 20 patients with calf cramps and saw the change in pain. The second study using manual therapy on iliaosacral joint to 2 patients with frequent nocturnal culf cramps and saw the change in number of occurrences during the week. Results: The pain and number of occurrences were significantly difference between pre-treatment and post treatment. Conclusion: Based on the results of this study, we found that reciprocal inhibition decreased pain and that manual therapy on iliaosacral joint decreased number of occurrences.

Effects of Home Exercise Program and Manual Therapy on Shoulder Function and Quality of Life in Patients with Adhesive Capsulitis

  • Purpose: This study was conducted to identify the effects of home exercise programs and manual therapy on shoulder function and quality of life in patients with adhesive capsulitis. Methods: A total of 31 patients were enrolled in the study. All subjects were randomly assigned to a home exercise group (n=15) or a manual therapy group (n=16). Both groups performed each intervention program three times a week for four weeks. After four weeks, both groups performed a home exercise program continuously until week 12. Shoulder function was evaluated using the Korean Shoulder Scoring (KSS) system, while quality of life was evaluated using the WHO Quality of Life-BREF (WHOQOL-BREF) before and 4, 8, and 12 weeks after the intervention. Data were analyzed using two-way repeated measures analysis of variance (ANOVA). Results: There were significant differences in KSS before intervention and 4, 8, and 12 weeks after intervention for both the home exercise group and manual therapy group (p<0.001). There was also a significant difference in KSS at 4, 8, and 12 weeks when compared to the home exercise group and manual therapy group (p<0.001). Moreover, there was a significant difference in WHOQOL-BREF before and 4, 8, and 12 weeks after intervention in the home exercise group and manual therapy group (p<0.001). Conclusion: Home exercise and manual therapy improved shoulder function, but manual therapy led to a greater improvement in shoulder function better than home exercise. Home exercise and manual therapy improved quality of life, but there was no significant difference between groups.
