제3대구치와 하악관과의 위치 관계에서 치과용 Cone beam CT의 유용성 (POSITIONAL RELATIONSHIP OF THE MANDIBULAR CANAL AND IMPACTED THIRD MOLARS BY USING DENTAL CONE BEAM COMPUTED TOMOGRAPHY)

  • We sought to evaluate the relationship between the mandibular canal and impacted mandibular third molars by using dental cone beam computed tomograph(CBCT) for third molar surgery. A total of 111 patients(177 teeth) offered the images through CBCT and panoramic radiography for the extraction of the mandibular third molars. In CBCT, the accurate relationship between the third molar and the mandibular canal were evaluated. In panoramic radiographies, we evaluated the impacted level and superimposition sign of the mandibular third molar with the mandibular canal, and also, the radiopacity of the white line in the canal. Data were statistically analyzed and estimated by $X^2$-test. In CBCT finding, high prevalence of contact between the mandibular canal and roots occured in the deep impacted third molars, narrowing mandibular canals, bending mandibular canals and cases where the radiopacity of white line of canals were "absence" on panoramic images. It showed statistical significance (P<0.05). When evaluating the mandibular canal and the roots through the panoramic radiography for third molar extraction, it could be difficult to diagnosis accurately. Thus, it is required to have an accurate diagnostic approach through CBCT that could evaluated the location between mandibular canal and root.

Effect of a vertical incision on postoperative swelling after an impacted mandibular third molar extraction: two cases with split-mouth designed magnetic resonance imaging analysis

  • This study examined the effects of a vertical incision on postoperative edema after third molar extraction. The study design was that of a comparative split-mouth approach. Evaluation was performed via magnetic resonance imaging (MRI). Two patients with homogeneous bilateral impacted mandibular third molars were enrolled. These patients underwent facial MRI within 24 hours after simultaneous extraction surgery. Modified triangular flap and enveloped flap incisions were made. Postoperative edema was evaluated by MRI and was assessed according to anatomical space. The two pairs of homogeneous extractions demonstrated that vertical incisions were associated qualitatively and quantitatively with extensive postoperative edema. The edema associated with these incisions spread toward the buccal space, beyond the buccinator muscle. In conclusion, a vertical incision with mandibular third molar extraction was related to edema in the buccal space and the fascial space, which contributed to clinical facial swelling.

  • Distal caries of the second molar is common indication for the mandibular third molar surgery and there are no universally acceptable predictive criteria for distal caries of the second molar. To analyze the correlation of the distal caries of the second molar and the eruption state of the mandibular third molar using panoramic radiographs statistically and propose the acceptable guideline for preventive extraction of the mandibular third molar. 786 patients who were extracted the mandibular third molar from 2002 to 2006 at Samsung medical center were examined. The presence and absence of distal caries of mandibular second molar, age, gender, angulation, impaction degree, distance between distal cementoenamel junction of the second molar and mesial cementoenamel junction of the mandibular third molar were assessed. 79.6% of third molars had a mesial angulation of between $40^{\circ}$ and $80^{\circ}$. The mean age of third molar removal for distal caries of second molar was $33.86{\pm}9.81$. The prophylactic removal of a mesio-angular third molar about $40^{\circ}$ and $80^{\circ}$ could prevent distal cervical caries forming in the mandibular second molar.

Surgical extraction of mandibular third molar in pterygomandibular space: a case report

  • Impacted mandibular third molars are located between the second mandibular molar and mandibular ramus. However, ectopic mandibular third molars with heterotopic positions are reported in the subcondylar or pterygomandibular space. The usual cause of malposition is a cyst or tumor, and malposition without a pathology is rare. This case report described an impacted mandibular third molar in the pterygomandibular space without any associated pathology.

  • Purpose : The purpose of this study was to determine radiologic variables affecting extraction of impacted mandibular third molar. Materials and Methods : This study was investigated on 44 consecutive extractions of unilateral impacted mandibular third molars (Male;34, Female;10, Mean age;24.98, Age range;19-35). Fourteen radiologic variables, which were actual value except only one, were evaluated to establish their relation to the operation time. The Pearson correlation coefficient and Stepwise procedures for regression analysis were carried out. Results : Eight variables (depth, angulation, ramus width, relationship to the second molar, distal ramus depth, mesial ramus depth, mesial periodontal ligament width and follicle) showed a statistically significant relation to the operation time(Pearson correlation coefficient, p<0.05). Two variables, depth and mesial periodontal ligament width, showed the most powerful and just relation to the operation time($r^2$ = 0.349, p<0.05). Conclusion : This study shows that our results can help general practioner to estimate the operation time of extraction of impacted mandibular third molars by measuring two radiologic variables ; depth and mesial periondontal ligament width.

방사선 사진을 이용한 하악 제3대구치와 하치조신경의 관계에 대한 연구 (THE STUDY OF EVALUATION TO RELATIONSHIP BETWEEN THE INFERIOR ALVEOLAR NERVE AND THE MANDIBULAR THIRD MOLAR BY USING RADIOGRAPHIC IMAGE)

  • Purpose: This study was performed to evaluate relationship between the inferior alveolar nerve injury and the findings of panoramic and tomographic images for preventing inferior alveolar nerve injury after the 3rd molar extraction. Material and Method: From April, 2005 to June, 2005, The 190 patients who visited in the Department of Oral and Maxillofacia Surgery, Chonnam National University Hospital and the panoramic radiographies were taken for extraction of the mandibular third molar, was selected. Among 215 mandibular third molars, Scanora tomographic imagings were taken in the 90 teeth which were overlaped to the mandibular canal in the panoramic imagies. In panoramic radiographies, the angulation, the level, the root morphology, and the superimposition sign of the mandibular third molars with the mandibular canal were evaluated. In the tomographic radiographies, the location and distance of the mandibular third molar from the canal were also evaluated. The relationships between these findings and the inferior alveolar nerve injury were examined. Results: In the panoramic findings, the inferior alveolar nerve injuries were occurred in the darkened roots (5 molars, 7%), the uncontinuous radiopaque image (3 molars, 7%), and the depositioned mandibular canal (2 molars, 10%). In the tomographic findings of 90 molars, 20 molars also had the superimposition imagies. Five molars in those molars (25%) had the inferior alveolar nerve injury after extraction. There were 10 patients who had the inferior alveolar nerve injury. The sensory was began to be recovered in 9 patients, except 1 patient, within 2 weeks, then fully recovered within 3 months. Conclusion: These results indicate that the depth mandibular third molar and the superimposition sign may be related with the risk of the inferior alveolar nerve injury after extraction.

  • Subcutaneous emphysema and pneumomediastium is a relatively uncommon phenomenon. Most case of pneumomediastium are caused by iatrogenic injury on the cervical region and chest during tracheostomy. It is also well known that emphysema may occur secondary to dental treatment using high-speed air turbine handpiece, but there have been few cases of emphysema extended to involving the mediastinum. These complications are reported to occur mainly in patients after dental procedures, in particular during mandibular third molar extraction. Early recognition and conservative treatment of these problems is essential in preventing life-threatening complications such as airway obstruction, mediastinitis, pneumothorax and cardiac failure. As we report a case of 25-year-old woman with subcutaneous emphysema and pneumomediastium after mandibular third molar extraction using high-speed air turbine handpiece.

치과병원 환자의 하악 제3대구치 맹출 양상과 치관주위염과의 관계에 대한 연구 (A Study of Relationship between Pericoronitis and Eruption State of the Mandibular Third Molar among Patients at Dental Hospital)

  • 본 연구는 하악 제3대구치의 맹출 양상이 치관주위염의 발생에 미치는 영향을 조사하고자 양산시에 소재한 N치과병원에 하악 제3대구치 발거를 주소로 내원한 환자 200명을 대상으로 조사연구를 실시하였다. 하악 제3대구치의 좌 우측 분포도, 경사도, 매복정도, 하악지 전연에서의 위치, 하악 제2대구치와 제3대구치 사이의 거리는 $X^2$-test, 치관주위염 유 무와 제 변수와의 상관성은 Pearson 상관관계를 실시하여 다음과 같은 결론을 얻었다. 1. 하악 제3대구치에 대한 치관주위염의 발생은 연령과 상관관계가 있는 것으로 나타났다. 2. 하악 제3대구치의 경사도에 있어서 치관주위염이 가장 호발된 군은 근심경사군이었다. 3. 하악 제3대구치의 매복정도와 치관주위염의 발생은 Level a, Level c, Level b순으로 나타났다. 4. 하악 제3대구치의 하악지 전연에서의 위치는 Class II, I, III의 순으로 치관주위염이 호발하였다. 5. 하악 제2대구치의 원심 백악법랑경계에서 하악 제3대구치의 근심 백악법랑경계까지의 거리가 좁을수록 치관주위염이 호발하였다.

하악 제3대구치 발치를 위한 예방적 항생제의 효용성 (THE EFFICACY OF PROPHYLACTIC ANTIBIOTICS FOR MANDIBULAR THIRD MOLAR EXTRACTION)

  • This study analyzed the incidence of wound infection after the operation of mandibular third molar extraction in relation with antibiotic prophylaxis with the object of young and healthy patients. The study object was 1,177 mandibular third molars of 850 men of 20 to 25 years old without any specific systemic disease. Three methods of preventive antibiotic medication were selected according to the preventive antibiotic medication previously reported; three experimental groups were selected based on them, and the antibiotic used was amoxicillin($^{(R)}Kymoxin$, Yuhanyanghaeng, Seoul). The group 1 includes the patients that took the antibiotic orally before the operation(one hour earlier, 500mg) and for three days after the operation(250mg per time, three times/day), the group 2 is the ones that took the same antibiotic orally only once about one hour before the operation(500mg), and the group 3 did not take any antibiotics before and after the operation. And to compare the difficulties and the degrees of extraction during operations which can be possibly related to the wound infection after the operations, the mandibular third molars' impacted depths and extraction methods were investigated as well. To check if the wound was infected, observations with an internal of one week were performed twice after the operation, and the meaningfulness of the infection incidence was verified through Chi-square test using SPSS program(SPSS Inc., IL, USA). There was no statistically significant difference between the antibiotic medication methods and the wound infection incidence after the operation among the experimental groups. As examining the relations between the mandibular third molar operation methods and the wound infection incidence after the operation, there existed a statistically meaningful difference in the infection incidence according to the operation methods(p=0.020). And there was no statistically significant difference in the wound infection incidence according to the impacted depth of the mandibular third molar. Therefore, it is thought that there exists little necessity of prophylatic antibiotics medication when extracting the mandibular third molar of young and healthy men without any systemic disease in general; however, in case when it is expected that the possibility of infection will be high or the wound on the tissue will be severe, it is sure that the prophylactic antibiotics medication will be necessary.

Comparison of the effects of articaine and bupivacaine in impacted mandibular third molar tooth surgery: a randomized, controlled trial

  • Background: The aim of this randomized, triple-blind trial was to determine the anesthetic, analgesic, and hemodynamic effects of articaine and bupivacaine in the extraction of impacted mandibular third molar teeth. Methods: Twenty-six patients who underwent removal of bilaterally symmetric mandibular third molars were randomly assigned to articaine and bupivacaine groups in a split-mouth design. The onset of anesthetic action, intraoperative comfort, total amount of solution used, duration of postoperative anesthesia and analgesia, rescue analgesic use, postoperative pain, intraoperative bleeding, and hemodynamic parameters were evaluated. Results: In the articaine group, the onset of anesthetic activity was faster, intraoperative comfort was greater, and effective anesthesia required less local anesthetic solution. The bupivacaine group showed a significantly longer duration of postoperative anesthesia and analgesia, in addition to lower visual analog scale values at 6 and 48 hours postoperatively. There were no significant differences between the two solutions regarding rescue analgesic medication use, intraoperative bleeding, or hemodynamics. Conclusion: Articaine showed greater clinical efficacy than bupivacaine in intraoperative anesthesia, achieving faster onset of anesthetic action and greater patient comfort while also requiring less reinforcement during surgery. However, bupivacaine was superior in terms of postoperative anesthesia, reducing postoperative pain due to its residual anesthetic and analgesic effects. Both anesthetic solutions led to similar hemodynamics at low doses in mandibular third molar surgery
