불규칙한 하악 운동을 하는 환자의 treatment denture를 이용한 총의치 수복 증례 (Fabrication of complete dentures using treatment dentures for a patient with erratic mandibular movements: A case report)

  • 많은 무치악 환자들은 부적합한 의치의 장기간 사용으로 인해 불규칙한 하악 운동을 하는 경우가 빈번하다. 총의치 제작 시 기능과 심미성을 만족하기 위해서는 정확한 하악 운동과 중심위를 채득하는 것이 중요하다. 본 증례는 불규칙한 하악 운동으로 중심위 채득이 어려운 환자를 flat occlusal table을 가진 treatment denture를 이용하여 총의치 수복하였기에 보고하는 바이다.

  • The traumatic removal of the mandibular third molar may promote post chirurgic consequences such as orofacial pain and limited mandibular movements. The aim of this case report is to describe the use of an anterior bite plane and complementary therapies (low level laser therapy and acupuncture) to treat the muscular dysfunction and the painful symptoms. A 33 year-old male patient who had a severe malocclusion and signs and symptoms of temporomandibular dysfunction was submitted to an initial clinical examination: electromyography of the masticatory muscles and IRM of the temporomandibular joint. After treatments, the results showed reduced pain symptoms and an increase of the mandibular movements and adequated electromiographic activities. The authors concluded that the combination therapies may be used as an alternative treatment because it satisfied the functional requirements of the patient and provided an asymptomatic clinical condition.

  • In this paper, we propose a 3D automated measuring system which measures the mandibular movements and the reference plane of the jaw movements. In diagnosis and treatment of the malocclusions, it is necessary to estimate the mandibular movements and the reference plane of the jaw movements. The proposed system is configured with double stereo-cameras, PC, two moving pattern plates(MPPs), two fixed pattern plates(FPPs) and one orbital marker. The virtual pattern plate is applied to calculate the homogeneous transformation matrices which describe the coordinates systems of the FPP and MPP with respect to the world coordinates system. To estimate the parameters of the hinge axis, the Euler's theorem is applied. The hinge axis points are intersections between the FPPs and the hinge axis. The coordinates of a hinge axis point with respect to the MPP coordinates system are set up to fixed value. And then, the paths of the jaw movement can be calculated by applying the homogeneous transformation matrix to fixed hinge axis point. To examine the accuracy of the measurements, experiments of measuring the hinge axis points and floating paths of them are performed using the jaw motion simulator. As results, the measurement errors of the hinge axis points are within reasonable boundary, and the floating paths are very similar to the simulator's moving path.

악관절 폐구성 과두걸림 환자의 하악운동에 관한 연구 (A Study on the Mandibular Movements in the Patients with TMJ Lock Closed)

  • The author examined the patterns and various ranges of mandibular movements in TMJ lock closed patients in the frontal, sagittal and horizontal plane and obtained the following results. 1. In the frontal trajectory, the mean amount of maximum mouth opening was 24.4mm and the opening paths were deviated to the affected side in 87.1% of the patients. The mean amount of maximum laterotrusion to the affected side was 10.4mm and that of non-affected side was 7.5mm. There was a significant difference between them(p<0.001). 2. In the sagittal trajectory, the mean amount of the maximum protrusion was 7.0mm, the mean amount of the maximal retrusion was 1.0mm 3. In the horizontal trajectory, the pattern of laterotrusion showed asymmetry: the mean length of non-affected side was smaller than that of the affected side. Protrusive path were deviated to the affected side in 64.5% of the patients, the mean degree of deviation was 16.4$^{\circ}$. The mandibular movements of TMJ lock-closed patients can be characterized by decreased range of mouth opening, protrusive movement, and laterotrusive movement to the non-affected side and also characterized by deviated opening and protrusive path to the affected side.

Differences in opening and protrusive mandibular movements between Class I and II malocclusions in healthy adolescents

  • Objective: The aim of the study was to compare the opening and protrusive mandibular movements between Class I and Class II malocclusions in healthy adolescents by clinical and axiographic evaluations. Methods: Mechanical axiography was performed on non-orthodontically treated, temporomandibular disorder (TMD)-free adolescents (12 - 16 years) with Class I (n = 38, 16 boys, 22 girls) or Class II (n = 40, 19 boys, 21 girls) malocclusion. Opening and protrusive movements were measured clinically and axiographically. Intergroup comparisons were evaluated by t-tests. Results: In opening movement, the maximum clinical opening capacity was significantly different (p ${\leq}$ 0.05) between the groups. In protrusive movement, the Class II group had significantly greater maximum clinical protrusion (p < 0.001) and maximum axiographic protrusive length (p < 0.01) than the Class I group. No significant difference in the other opening and protrusive axiographic measurements was observed. Conclusions: TMD-free adolescents with Class II malocclusion have increased protrusive capacity compared with TMD-free adolescents with Class I malocclusion; however, the detected differences could be normal variations during adolescence.

측두하악 장애환자의 하악 운동 Simulation에 관한 연구 (A STUDY ON SIMULATION OF THE MANDIBULAR MOVEMENT OF THE PATIENTS WITH TEMPOROMANDIBULAR JOINT DISORDER)

  • To understand the biomechanics of the mandibular movement of the patients with T.M.J. disorder, it is necessary to analyze the mandibular movement by the least moving points of instantaneous center of rotation on computer screen. In this study, two subjects without T.M.J. disorder and with normal occlusion and few patients with T.M.J. disorder were selected for obtaining the processing data. The habitual opening and dosing movements were recorded by image recognition system of video and computer and then the mandibular movement and the least moving point were analyzed using computer program for image anaysis of specialized points. The results obtained were as follows: 1. The least moving points of control group during opening and dosing were dose to the mandibular foramen, but in the test group there were differences between two positions of the least moving points during opening and closing. 2. The variations of the least moving point were in the range of $0.02\sim0.05cm$ of X-coordinate and $0.07\sim0.10cm$ of Y-coordinate for control group, whereas in the range of $0.05\sim0.30cm$ of X-coordinate and $0.08\sim1.65cm$ of Y-coordinate for test group. 3. The index of variation during opening and dosing movements was shown in $0.8\sim1.0$ for the control group and in $0.56\sim2.6$ for the test group. 4. After the treatment of the test group No. 4 by occlusal splint the least moving point was dose to the mandibular foramen and the index of variation was changed from 25 to 05. 5. The tracing shape of the least moving point in the test group represented the irregularities compared to the control group. 6. The mandibular opening and dosing movements were simulated on the computer screen following the movement of the least moving point of instantaneous center of rotation.

전치부 하악운동양태와 과두운동 간의 관계 (RELATIONSHIP BETWEEN MANDIBULAR MOVEMENTS AT INCISAL AREA AND CONDYLAR MOVEMENTS)

  • The author performed this study to investige the relationship between condylar movements recorded with Pantronic and mandibular movements at incisal area recorded with BioEGN. For this study 24 patients with Temporomandibular disorders(TMDs) and 30 dental students without any masticatory symptoms were selected as patients group and control group, respectively. The items recorded with Pantronic(Denar Corp., USA) were immediate side-shift, orbiting path, protrusive path, and PRI. BioEGN(Bioelectric-gnathography, Bioresearch Inc., USA) were sued to measure the amount of mandibular torque movement in frontal and horizontal plane and also the distance of mandibular translation at incisal area. Amount of mandibular rotational torque movement was analyzed by angle and difference between both condyles in frontal and horizontal plane. The collected data were processed with SAS program and conclusion were as follows : 1. Mean value of items recorded with Pantronic were not significantly differed between patients group and control group except the item of pantographic reproducibility index(PRI). The value of PRI was 39.5 in patients group, and 29.5 in control group. 2. The amount of mandibular torque movement was not differed tin early protrusive and early left excursion between patients group and control group, but in early right excursion, patients group showed more value than control group did. 3. The distance on sagittal plane in early eccentric movements were longer in patients group than those in control group, but the distance of maximal eccentric movements were not significantly differed between patients group and control group. 4. Items which showed significant correlation with PRI were progressive side-shift, and horizontal torque movement in early protrusion and right excursion. 5. The angle of protrusive path of affected side was greater than of non-affected side in unilaterally affected patients, but the protrusive angle of preferred chewing side was not differed from that of contralateral side in control group. 6. The amount of torque movement in early protrusion and right excursion were greater in patients with coincidence of affected side and preferred chewing side than in patients without coincidence.

하악골운동 추적을 위한 단순기록장치 고안에 관한 연구 (A STUDY ON THE DESIGN OF SIMPLE RECORDING DEVICE TO TRACE THE MANDIBULAR MOVEMENTS)

  • The purpose of this study was to improve limitations and disadvantages of the mechanical pantograph and the Visi-Trainer, and to design the reliable and reproducible device mandibular movement tracking device (MMTD) that is more simple, convenient and save the chair time than the mechanical pantograph and Visi-Trainer. MMTD was consist of head frame, horizontal bar, condylar path tracking stylus holders, anterior path tracking stylus holder, two condylar path plastic recording plates, one anterior path plastic recording plate, toggles and open occlusal clutch. To prove the reliability and reproducibility of MMTD, a five adults were selected and mandibular condylar movement was recorded one time by the mechanical pantographic tracing and MMTD. The border movement recording of the mandibular incisor (frontal, sagittal and horizontal) was also recorded by Visi-Trainer and the MMTD. The obtained results were as follows; 1. The condylar movement path (sagittal, horizontal) of the MMTD was not coincidence with that of mechanical pantograph. 2. Measurements of the angulation which established between working and balancing path records by use of the mechanical pantograph and MMTD, there was no significance. 3. In a view of MMTD's reproducibility, there was revealed almost similar recording pattern. 4. The border movement recording of the mandibular incisor by the MMTD and the Visi-Trainer showed almost same reliability and reproducibility. 5. The subjects were able to his original mandibular movements by use of open occlusal clutch in the MMTD.

중심교합위와 후방교합위 간의 편위와 습관성 하악개폐운동과의 관계에 관한 연구 (A Study on the Relationship between the Deviation of the Intercuspal Position from the Retruded Contact Position and the Habitual Mandibular Closing and Opening Movements)

  • The purpose of this study was to investigate the relationship between the mandibular positions and the habitual mandibular opening and closing movements in normal subject using the newly developed electric mandibular movement analyzing device(Saphon Visi-Trainer C-II.Tokyo Shizaisha Inc. Tokyo, Japan) One of the cause of the temporomandibular joint dysfunction has been considered abnormal mandibular positions especially the abnormal deviation between intercuspal position from retruded contact position. The subjects in this study were consisted of 55 young adults in 20 decade who had normal occlusion, no prosthodontic restorations and no history of disturbance or pain on temporomandibular joint and masticatory muscles. The results were as follows : 1. The deviations of intercuspal position from retruded contact position were 0.23mm in frontal view and 1.02mm in sagirral view. 2. The larger the deviation of intercuspal position from the retruded contact position, the higher the proportion of uncoincidence of the habitual nandibular closing and opening trajectories was, and the proportion was higher in sagittal view than frontal view, 10mm opening than 5mm opening. 3. Correlation between the deviation of intercuspal position from retruded contact position and uncoincidence rate of the habitual mandibular closing and opening trajectories was higher in frontal view than sagittal view.

악관절음 환자의 하악 비틀림회전운동에 관한 연구 (A Study on Mandibular Rotational Torque Movement in Subjects with Temporomandibular Joint Sounds)

  • The purpose of this study was to investigate the magnitude of mandibular rotational torque movements in subjects with TMJ sounds, and to analyse correlation between quantitative characteristics of TMJ sounds and mandibular rotational torque movement. Twenty dental college students with TMJ clicking and twenty students without any TMD signs and symptoms were examined by mean of SonoPak and Rotate program of BioPAK system(Bioresearch Inc. MilWaukee, wisconsin, USA) in this study. Mandibular rotational torque movements were recorded and analysed during maximum mouth opening, protrusion, and lateral excursion in frontal and horizontal planes. The obtained results were as follows: 1. On maximum mouth opening, mandibular rotational angle and distance of clicking group were significantly greater than those of control group in frontal plane. (P<0.05). 2. During maximum mouth opening closing, maximum mandibular rotational angle and distance of clicking group were significantly greater than those of control group in frontal plane. (P<0.01). 3. On protrusion, mandibular rotational angle and distance of clickin group were significantly greater than those of control group in horizontal plane. (P<0.05). 4. On lateral excursion, there was no significant difference in mandibular rotational angle and distance between clicking group and control group in frontal and horizontal planes. 5. There were significant correlations between peak amplitude of TMJ sounds and maximum mandibular rotational distance during maximum mouth opening (r=-.481) and mandibular rotational distance on maximum mouth opening (r=-.455) in horizontal plane. 6. There were significant correlations between Above 300/(0-300)Hz ratio of TMJ sounds and mandibular rotational angle (r=-.499) and distance (r=-.457) on maximum mouth opening in frontal plane.
