안면 중앙부 골절에 관한 임상적 연구 (CLINICAL STUDY OF MIDFACE FRACTURE)

  • The purpose of this study was to investigate the epidemiology of midface fractures. We observed clinically 71 patients with midface fracture who visited the Department of oral and maxillofacial-surgery, Chosun University, Dental Hospital, from 1991 to 1994. The results obtained were as follows. 1. There was the highest age incidence in the third decade(29.6%). 2. There was the highest incidence in the summer(36.6%). 3. The most etiologic factor of midface fracture was traffic accidents(43.6%) and next factor was fall down(38.0%). 4. The highest incidence fracture was zygomatic arch(38.7%) and next fracture was ZMC fracture(31.5%). 5. Most midface fracture was treated within 2 weeks(86.7%). 6. Midface fractures was most frequently combined with mandibular fracture(15.5%), head injuries(14.1%), orthhopedic injuries(8.5), thorax & abdomen(5.6%). 7. The highest complication was the nerve injury(8명), and next complications were infection(3명), epiphora(3명) etc.

근관 치료 후 수복 방법이 파절 강도와 파절 양상에 미치는 영향 (The Effect of Restorative Method on Fracture Strength and Fracture Pattern in Endodontically Treated Teeth)

  • The purpose of this study was to evaluate interaction of restorative method according to amount of remaining clinical crown length. For the study, sixty healthy mandibular premolars with no dental caries were chosen. Specimens with clinical crown length of 1 mm, 2 mm, 3 mm above the cemento-enamel junction(CEJ) were prepared and root canal treatment was conducted using Ni-Ti rotaty files. Thirty specimens were restored with $LuxaCore^{(R)}$ and thirty were restored with casting posts. All specimens were restored with full coverage crowns and the fracture strength was teseted with the MTS universal testing marchine. The fracture pattern was observed by measuring the distance between CEJ and fracture line. From the results above, the fracture strength seems to be influenced more by amount of remaining clinical crown length than restorative method. Good prognosis could be expected, when more than 2mm of clinical crown length is retained. In the future, the further studies on restorative method, to increase fracture strength of teeth will be necessary.

하악 우각부 골절 시 2개의 miniplate를 이용한 관혈적 정복술에 대한 전향적 예비 임상연구 (The prospective preliminary clinical study of open reduction and internal fixation of mandibular angle fractures using 2 miniplates)

  • Introduction: The placement of a single miniplate is not sufficient to achieve rigid fixation in mandibular angle fractures. It often causes difficulties in reducing the intermaxillary fixation (IMF) period. Consequently, the placement of 2 miniplates is preferable. The intraoral approach in an open reduction and internal fixation (ORIF) of a mandibular angle fracture with 2 miniplates is often challenging. Accordingly, an alternative of transbuccal approach is performed. However, this method leaves a scar on the face and can result in facial nerve injury. This clinical study suggests a protocol that can maintain rigid fixation without a transbuccal approach in mandibular angle fractures. Materials and Methods: The subjects were 7 patients who sustained fractures of the mandibular angle and treated at Department of Oral and maxillofacial surgery, Sacred Heart Hospital, Hallym University. ORIF under general anesthesia was done using the intraoral approach. One miniplate was inserted on external oblique ridge of the mandible, and the other was placed on lateral surface of the mandibular body with contra-angle drill and driver. A radiographic assessment and occlusal contact point examination was carried out before surgery, and 2, 4 and 6 weeks after surgery. Results: The mean operation time was 80 minutes. Regarding the occlusion state, the number of contact points increased after surgery. Paresthesia and infection were reported to be complications before surgery. Conclusion: The placement of 2 miniplates using contra-angle drill for ORIF of mandibular angle fractures allows early movement of the mandible without IMF. We propose this approach to reduce the patients’discomfort and simplify the surgical procedure.

폐경후 골다공증 여성환자에 있어서 파노라마상 하악골 지표에 관한 비교연구 (COMPARATIVE STUDY OF PANORAMIC MANDIBULAR PARAMETERS IN POSTMENOPAUSAL OSTEOPOROTIC WOMEN)

  • Osteoporosis has recently been recognized as a major health problem in the elderly population. The disorder is manifested as a loss of bone mass accompanied by structural alteration of bone and increased incidence of fracture. Mandible also may be affected. So, I evaluated panoramic views of 66 postmenopausal women for finding the possibility of useful diagnostic mandibular parmeters of osteoporosis. To know the correlationship between skeleton and mandible, the average of the bone mineral density of lumbar from 2nd to 4th by the dual energy X-ray absorptiometry(DEXA, LUNAR DPZ. USA), and age and mandibular parameters, that is, the number of residual teeth, alveolar ridge resorption ratio, panoramic mandibular index (PMI), mandibular cortical width (MCW), angular cortical thickness (ACT), ramus cortical thickness (RCT), morphology of mandibular inferior cortical (MIC) were compared. And I divided the all tested women to the osteoporotic group and non-osteoporotic group by the use of T-score -2.0, which was derived from skeletal bone mineral density (BMD). To find the correlationship of the each group with mandibular parameters, t-test and discriminant analysis were done. The results of the t-test were that all parameters were highly related with 2 groups (p<0.05). Especially ACT, MIC, age have had even higher correlationship than others (p<0.001). The results of the discriminant analysis by the use of these ACT, MIC and age were that the discriminant function was Z = -2.973+(-1.447)$\times$(ACT)+1.131$\times$(MIC score)+(0.052)$\times$(age), the cutting score was 0.257 and the classification accuracy was 84.8%. Therefore I suggest that the consideration of the angular cortical thickness (ACT), the age of patient and the morphology of mandibular inferior cortical(MIC) may help find the osteoporosis.

Comparison of Mechanical Stability between Fibular Free Flap Reconstruction versus Locking Mandibular Reconstruction Plate Fixation

  • Background: The fibular free flap has been used as the standard methods of segmental mandibular reconstruction. The objective of mandibular reconstruction not only includes restored continuity of the mandible but also the recovery of optimal function. This paper emphasizes the advantage of the fibular free flap reconstruction over that of locking mandibular reconstruction plate fixation. Methods: The hospital charts of all patients (n=20) who had a mandibular reconstruction between 1994 and 2013 were retrospectively reviewed. Eight patients had plateonly fixation of the mandible, and the remaining 12 had vascularized fibular free flap reconstruction. Complications and outcomes were reviewed and compared between the 2 groups via statistical analysis. Results: Overall complication rates were significantly lower in the fibular flap group (8.3%) than in the plate fixation group (87.5%; p =0.001). Most (7/8) patients in the plate fixation group had experienced plate-related late complications, including plate fracture or exposure. In the fibular flap group, no complications were observed, except for a single case of donor-site wound dehiscence (1/12). Conclusion: The fibular free flap provides a more stable support and additional soft tissue support for the plate, thereby minimizing the risk of plate-related complications. Fibular free flap is the most reliable option for mandibular reconstruction, and we believe that the flap should be performed primarily whenever possible.

The effect of the dental root on single mandibular bone fractures

  • Background: This study was conducted to determine the effect of the distributional relationship between dental roots and the mandibular bone on single mandibular bone fractures, which are common craniofacial fractures. Methods: This was a retrospective, single-center study in Seoul, Korea. Patients with single mandibular fractures in the symphysis, parasymphysis, body, and angle area, with tooth structure involvement were included. The control group included patients with simple, bone-level lacerations without fractures. In total, 94 patients (72 males and 22 females) were included in the treatment group, and 125 (71 males and 54 females) were included in the control group. The height of the mandibular bone and the dental root were measured with panoramic radiography. The central incisor represented the symphysis area, the canine represented the parasymphysis area, the first molar represented the body area, and the second molar represented the angle area. Results: In the treatment group, symphysis fractures occurred in 16 patients (17%), parasymphysis fractures in 36 patients (38%), body fractures in 17 patients (18%), and angle fractures in 25 patients (27%). The ratios of the dental roots to the total height of the mandibular bone in the treatment group were 30.35%, 39.75%, 39.53%, and 36.27% for symphysis, parasymphysis, body, and angle areas, respectively, whereas in the control group, they were 27.73%, 39.70%, 36.76%, and 35.48%. The ratios of the treatment group were significantly higher than those of the control group. Conclusion: The results show that the higher ratio of the dental root to the height of the mandibular bone increases the fracture risk.

감염된 하악골 골절의 치험 (TREATMENT OF INFECTED MANDIBULAR FRACTURES)

  • Treatment of infected mandibular fractures is confronted with various difficult problem, e.g. eradication of infection and osseous union even in the presence of devastating infection. To solve this problem various methods were proved including exteranal fixation, IMF and plating with some success. Author treated 3 cases of infected mandibular fractures with champy's miniplate. 1. Satisfactory union was obtained in 3 cases. 2. In One case, pus drainage continued even after internal fixation with miniplate, but it was easily controlled using proper antibiotic therapy & drainage. 3. In Case 3, autogenous bone graft was implanted into the infected fracture site to fill defect.

하악골 골절의 견고고정에 사용된 monocortical titanium miniplate와 관련된 감염증에 관한 연구 (Complications Associated with Monocortical Titanium Miniplate used in Rigid Fixation of Mandibular Fractures)

  • Eighty-nine patients with mandibular fracture were treated by open reduction and internal fixation using the monocortical titanium miniplate(Leibinger Co.). Postsurgical intermaxillary fixation was carried out for 2 to 18 days according to the patient's status. Seven patients developed infections postoperatively(7.9%). Five patients were favorably treated by incision and drainage and/or saucerization. But two patients were not controlled by early surgical intervention and should have been followed by plate removal, saucerization and secondary reconstruction including the bone graft. This article reports the postoperative infection associated with miniplate fixation of mandibular fractures and discuss the incidence, cause, treatment and prognosis with careful case analyses.

구강의 횡문근육종

  • The authors have observed clinically and histopathologically on two cases of rhabdomyosarcomas occurred in the oral cavity. The results were as follow: 1. A case of adult pleomorphic rhabdomyosarcoma, involving the right mandibular third molar area of 52 year old male, revealed pathological fracture of the right mandibular angle area. Microscopically, sections of this tumor showed cross striations by staining of phosphotungstic acid hematoxylin. 2. A case of embryonal alveolar rhabdomyosarcoma, involving the right buccal area of one year and ten months old female, revealed severe bone destruction on the right mandibular body area. Microscopically, sections of this tumor did not show cross striations.

하악과두에 발생한 호산구육아종 (Eosinophilic granuloma of the mandibular condyle)

  • The present study reports a case of eosinophilic granuloma of the mandibular condyle. Eosinophilic granulomas on the mandibular condyle are very rare, but there are several common clinical and radiographic presentations. The clinical presentations involve swelling on preauricular area, limitation of opening, TMJ pain, etc. The radiographic presentations involve radiolucent lytic condylar lesion with or without pathologic fracture. Sometimes new bone formations are observed. The purpose of the article is to add new cases to the literatures.
