Preliminary three-dimensional analysis of tooth movement and arch dimension change of the maxillary dentition in Class II division 1 malocclusion treated with first premolar extraction: conventional anchorage vs. mini-implant anchorage

  • Objective: This study aimed to compare the effects of conventional and orthodontic mini-implant (OMI) anchorage on tooth movement and arch-dimension changes in the maxillary dentition in Class II division 1 (CII div.1) patients. Methods: CII div.1 patients treated with extraction of the maxillary first and mandibular second premolars and sliding mechanics were allotted to conventional anchorage group (CA, n = 12) or OMI anchorage group (OA, n = 12). Pre- and post-treatment three-dimensional virtual maxillary models were superimposed using the best-fit method. Linear, angular, and arch-dimension variables were measured with software program. Mann-Whitney U-test and Wilcoxon signed-rank test were performed for statistical analysis. Results: Compared to the CA group, the OMI group showed more backward movement of the maxillary central and lateral incisors and canine (MXCI, MXLI, MXC, respectively; 1.6 mm, p < 0.001; 0.9 mm, p < 0.05; 1.2 mm, p < 0.001); more intrusion of the MXCI and MXC (1.3 mm, 0.5 mm, all p < 0.01); less forward movement of the maxillary second premolar, first, and second molars (MXP2, MXM1, MXM2, respectively; all 1.0 mm, all p < 0.05); less contraction of the MXP2 and MXM1 (0.7 mm, p < 0.05; 0.9 mm, p < 0.001); less mesial-in rotation of the MXM1 and MXM2 ($2.6^{\circ}$, $2.5^{\circ}$, all p < 0.05); and less decrease of the inter-MXP2, MXM1, and MXM2 widths (1.8 mm, 1.5 mm, 2.0 mm, all p < 0.05). Conclusions: In treatment of CII div.1 malocclusion, OA provided better anchorage and less arch-dimension change in the maxillary posterior teeth than CA during en-masse retraction of the maxillary anterior teeth.

  • 목적: 본 연구의 목적은 가철성 다이 시스템으로 제작된 작업 모형과 솔리드 작업 모형을 이용해 치과용 캐드캠 시스템(CAD/CAM)으로 제작된 지르코니아 3본 고정성 치과 보철물의 변연 및 내부 적합성을 평가하고자 하였다. 연구 재료 및 방법: 하악 우측 제1소구치와 하악 우측 제1대구치에 지르코니아 크라운을 위한 치아 삭제 프로토콜을 수행하고, 하악 우측 제2소구치가 없는 레퍼런스 모델을 만들었다. 레퍼런스 모델은 폴리비닐 실록산 인상체를 사용하여 복제되었고, 일반적인 치과 기공 절차에 따라 20개의 작업 모형이 제작되었다. 비교 분석을 위해, 10개의 지르코니아 3본 고정성 치과 보철물은 가철성 다이 시스템에서, 나머지 10개는 솔리드 작업 모형에서 제작되었다. 모든 작업 모형은 치과용 데스크탑 스캐너를 사용하여 디지털화되었고, 캐드 소프트웨어에서 보철물을 설계하였다. 최종 3본 고정성 치과 보철물은 밀링 과정을 통해 제작되었다. 변연 및 내부 적합도 평가는 레퍼런스 모델에 제작된 보철물을 위치시키고, 디지털 평가 방법으로 적합도가 측정되었다. 두 그룹 간의 통계 비교를 위해 Mann-Whitney U 검정이 적용되었다(α = 0.05). 결과: 가철성 다이 그룹은 솔리드 작업 모형 그룹에 비해 소구치와 대구치에서 유의하게 높은 적합도 차이를 보였으며(P < 0.05), 특히 변연 및 교합 간격에서 유의하게 높은 편차를 보였다. 색상 편차 맵에서도 가철성 다이 그룹이 변연 및 교합 영역에서 더욱 높은 편차를 보였다. 결론: 3본 고정성 치과 보철물의 적합도 차이는 가철성 다이 시스템의 작업 모형 상에서 제작된 치과 보철물에서 초래되었으며, 이를 통해 가철성 다이 제작 방법이 치과 고정성 보철물의 정확성에 영향이 있었음을 검증하였다.

  • 치아전위란 인접한 두 치아의 위치가 서로 바뀐 치아 위치 이상을 말하며, 치관과 치근의 위치에 따라 완전전위와 불완전전위로 나눌 수 있다. 하악보다는 상악에 편측성으로 호발하며, 특히 상악 견치에서 자주 발생한다. 전위는 발견 시기와 전위된 양상에 따라 치료방법이 달라질 수 있다. 본 증례는 상악 견치와 제1소구치 간의 전위가 일어난 10세 7개월의 환아와 하악 측절치와 견치 간의 전위가 일어난 8세 11개월 환아와 8세 9개월 환아의 교정치료에 대한 것으로 양호한 치료 결과를 얻었고, 증례 별로 치료방법의 차이점에 관한 고찰을 하였기에 이를 보고하는 바이다.

Cone-beam computed tomography analysis of root and canal morphology of mandibular premolars in a Spanish population

  • Purpose: This study aimed to investigate the clinical anatomy of lower premolar roots in a Spanish population by using cone-beam computed tomography (CBCT), correlating findings with patient gender and tooth type. Materials and Methods: Using 70 CBCT images, we evaluated 126 healthy, untreated, well-developed lower premolars. The number and morphology of roots and root canals, and the foramina number were assessed. Results for gender and tooth type were compared using the chi-squared and ANOVA tests. Results: The average length of teeth and roots was significantly higher in men (p=0.00). All 126 premolars had a single root. One canal was found in 83.3% of the premolars, with no gender or tooth type differences; Vertucci configuration types I and V were the most prevalent. The first premolars showed significantly greater variability than the second premolars (p=0.03). A single apical foramen was found in 89.7% of the premolars, with no differences by tooth type. Women had a significantly higher prevalence of two apical foramina than men (p=0.04). Some degree of curvature was observed in 65% of the premolars, with no differences by gender or tooth type. A root angle of more than $20^{\circ}$ was found in 12.98% of the premolars, without any differences by gender or tooth. Conclusion: All premolars were single-rooted. One canal had the most prevalent morphology. More variability in canal anatomy was found in the first premolars. Curvatures greater than $20^{\circ}$ were found at less than 5 mm from the apex.

Five-year retrospective radiographic follow-up study of dental implants with sandblasting with large grit, and acid etching-treated surfaces

  • Objectives: The purpose of this study is to evaluate five-year radiographic follow-up results of the Korean sandblasting with large grit, and acid etching (SLA)-treated implant system. Materials and Methods: The subjects of the study are 54 patients who have been followed-up to date, of the patients who underwent implant surgery from May 1, 2009 to April 30, 2011. In all, 176 implant placements were performed. Radiographs were taken before the first surgery, immediately after the first and second surgeries, immediately and six months after the final prosthesis installation, and every year after that. Bone loss was evaluated by the method suggested by Romanos and Nentwig. Results: A total of 176 implant placements were performed - 122 in men and 54 in women. These patients have been followed-up for an average of 4.9 years. In terms of prosthetic appliances, there were 156 bridges and 20 single prostheses. Nine implants installed in the maxillary molar area, three in the mandibular molar area and two in the maxillary premolar area were included in group M, with bone loss less than 2 mm at the crestal aspect of the implant. Of these, eight implants were single prostheses. In all, six implants failed - four in the mandible and two in the maxilla. All of these failures occurred in single-implant cases. The implant survival rate was 98.1% on the maxilla and 94.3% on the mandible, with an overall survival of 96.6%. Conclusion: Within the limitations of this study, implants with the SLA surface have a very superior survival rate in relatively poor bone environments such as the maxilla.

  • For the purpose of evaluating the effect of both direct retainer design and bony absorption degree around abutment of indirect retainer on the supporting tissue of abutment of indirect retainer, dislodging force was transmitted to unilateral distal extension RPD bases. Analysis of stress distributed within the supporting tissue around abutment of indirect retainer was carried out. Using three-dimensional photoelastic stress analysis method and the conclusion is a follows. 1. According to the extent of force which the direct retainer of the most distal abutment tooth, the amount of force transmitted to the abutment tooth of indirect retainer was small. 2. Of all the cases, Mandibular first premolar which was used abutment tooth of indirect retainer, buccal, mesial and distal sides represented compression stress and lingual side represented tensile stress. 3. The more bone resorption of abutment tooth of indirect retainer, the more distortion of buccal and distal side of abutment tooth was existed and the extent of compression stress which was existed and distal side to abutment tooth was large. 4. When the alveolar bone around the abutment with indircet retainer is normal. The amount of force transmitted on abutment with indirect retainer was small in the order of Akers clasp, RPA clasp, RPI clasp. 5. When the alveolar bone around the abutment with indirect retainer has been absorbed 20% and 30%, the amount of force transmitted on abutment with indirect retainer was small in the order of RPA calsp, RPI clasp, Akers clasp. 6. When denture is displaced, shape of the direct retainer reciprocating abutment affect much the function of indirect retainer.

En-masse retraction with a preformed nickel-titanium and stainless steel archwire assembly and temporary skeletal anchorage devices without posterior bonding

  • Objective: To evaluate the therapeutic effects of a preformed assembly of nickel-titanium (NiTi) and stainless steel (SS) archwires (preformed C-wire) combined with temporary skeletal anchorage devices (TSADs) as the sole source of anchorage and to compare these effects with those of a SS version of C-wire (conventional C-wire) for en-masse retraction. Methods: Thirty-one adult female patients with skeletal Class I or II dentoalveolar protrusion, mild-to-moderate anterior crowding (3.0-6.0 mm), and stable Class I posterior occlusion were divided into conventional (n = 15) and preformed (n = 16) C-wire groups. All subjects underwent first premolar extractions and en-masse retraction with preadjusted edgewise anterior brackets, the assigned C-wire, and maxillary C-tubes or C-implants; bonded mesh-tube appliances were used in the mandibular dentition. Differences in pretreatment and post-retraction measurements of skeletal, dental, and soft-tissue cephalometric variables were statistically analyzed. Results: Both groups showed full retraction of the maxillary anterior teeth by controlled tipping and space closure without altered posterior occlusion. However, the preformed C-wire group had a shorter retraction period (by 3.2 months). Furthermore, the maxillary molars in this group showed no significant mesialization, mesial tipping, or extrusion; some mesialization and mesial tipping occurred in the conventional C-wire group. Conclusions: Preformed C-wires combined with maxillary TSADs enable simultaneous leveling and space closure from the beginning of the treatment without maxillary posterior bonding. This allows for faster treatment of dentoalveolar protrusion without unwanted side effects, when compared with conventional C-wire, evidencing its clinical expediency.

Qualitative and quantitative evaluation of root injury risk potentially burdening insertion of miniscrew implants

  • Objective: Microscrew implants (MSls) offer many advantages, but some complications are known to occur during their insertion. One of the most commonly reported complications is root injury. Our aim was to identify factors associated with root injury and to evaluate their qualitative and quantitative values. Methods: Thirty-five orthodontists placed MSls (AbsoAnchor$AbsoAnchor^{(R)}$, Dentos Co. Ltd, Daegu, Korea) in the upper jaw of typodonts, labially between the second premolar and the first molar, in low and high vertical positions. Root contacts were counted, and distances between MSI apices and roots were measured. Fear level of the orthodontists was surveyed before and after the experiment. Wilcoxon's test, chi-square test, and Mann-Whitney test were used for statistical analysis. Results: Overall root contact rate of MSI insertion was 23.57%. The root contact rate was significantly higher in MSls inserted at $90^{\circ}$ (45.71%) than at $30^{\circ}$ (1.43%). The distance between the dental root and MSI also increased significantly in MSls inserted at $30^{\circ}$. Mean fear level before MSI insertion (4.6) significantly decreased after insertion (3.2); the causative factors were risk of injury to dental root and maxillary sinus or mandibular canal. Conclusions: Root injury is relatively rare, and oblique angulation reduces the risk of root and MSI contact.

수복재료와 임플랜트 종류에 따른 임플랜트 및 지지조직의 응력분포 (THE STRESS ANALYSIS OF SUPPORTING TISSUE AND IMPLANT ACCORDING TO CROWN RESTORATIVE MATERIALS AND TYPE OF IMPLANT)

  • This study was aimed to analyze the stress distribution of implant and supporting tissue in single tooth implant restoration using Branemark $system^{(R)}$(Nobel Biocare, Gothenberg, Sweden) and Bicon system(Bicon Dental Implants, Boston, MA). Two dimensional finite element analysis model was made at mandibular first premolar area As a crown materials porcelain, ceromer, ADA type III gold alloy were used. Tests have been performed at 25Kgf vertical load on central fossa of crown portion and at 10Kgf load with $45^{\circ}$ lateral direction on cusp inclination. The displacement and stresses of implant and supporting structures were analyzed to investigate the influence of the crown material and the type of implant systems by finite element analysis. The results were obtained as follows : 1. The type of crown material influenced the stress distribution of superstructure, but did not influence that of the supporting alveolar bone. 2. The stress distribution of ceromer and type III gold alloy and porcelain is similar. 3. Stress under lateral load was about twice higher than that of vertical load in all occlusal restorative materials. 4. In Bicon system, stress concentration is similar in supporting bone area but CerOne system generated about 1.5times eater stress more in superstructure material. 5. In Branemark models, if severe occlusal overload is loaded in superstvucture. gold screw or abutment will be fractured or loosened to buffer the occlusal overload but in Bicon models such buffering effect is not expected, so in Bicon model, load can be concentrated in alveolar bone area.

  • Statement of problem : Dental implant which has been developed gradually through many experiments and clinical applications is presently used to various dental prosthetic treatments. To conduct a successful function of implant prosthesis in oral cavity for a long time, it is important that not only structure materials must have the biocompatibility, but also the prosthesis must be designed for the stress, which is occurred in occlusion, to scatter adequately of load support. Therefore, it is essential to give the consideration about the stress analysis of supporting tissues for higher successful rates. Purpose : Recently, many implant manufacturing company produce various taper design of root form implant, the fixture is often selected. However, the stress analysis of taper form fixture still requires more studies. Material and method : This study we make the element models that five implant fixture; Branemark system(Nobel Biocare, Gothenberg, Sweden), Camlog system(Altatec, Germany), Astra system(Astra Tech, Sweden), 3i system(Implant Innovations Inc, USA), Avana system(Osstem, Korea) were placed in the area of mandibular first premolar and prosthesis fabricated, which we compared with stress distribution using the three-dimension finite element analysis under two loading condition. Results : This study compares the aspect of stress distribution of each system with the standard of Von mises stress, among many resulted from finite element analysis so that this research gets the following results. 1. In all implant system, oblique loading of maximum Von mises stress of implant, alveolar bone and crown is higher than vertical loading of those. 2. Regardless of loading conditions and type of system, cortical bone which contacts with implant fixture top area has high stress, and cancellous bone has a little stress. under the vertical loading, maximum Von mises stress is more higher in order of Branemark, Camlog, Astra, 3i, Avana. under the horizontal loading, maximum Von mises is more higher in order of Camlog, Branemark, Astra, 3i, Avana.
