임플란트 매식조건에 따른 상, 하악골의 응력분포 양상에 대한 3차원 유한요소분석 연구 (EFFECTS OF BONE ENGAGEMENT TYPE&IMPLANT LENGTH ON STRESS DISTRIBUTION: A THREE DIMENSIONAL FINITE ELEMENT ANALYSIS)

  • A finite element analysis has been utilized to analyze stress and strain fields and design a new configuration in orthopedics and implant dentistry. Load transfer and stress analysis at implant bone interface are important factors from treatment planning to long term success. Bone configuration and quality are different according te anatomy of expecting implantation site. The purpose of this study was to compare the stress distribution in maxilla and mandible accord-ing to implant length and bone engagement types. A three dimensional axi-symmetric implant model(Nobel Biocare, Gothenburg, Sweden) with surrounding cortical and cancellous bone were designed to analyze the effects of bone engagement and implant length on stress distribution. ANSYS 5.5 finite element program was utilized as an interpreting toot. Three cases of unicortical anchorage model with 7, 10, 13 mm length and four cases of bicortical anchorage model with 5, 7, 10 and 13 mm length were compared both maxillary and mandibular single implant situation. Within the limits of study, following conclusions were drawn. 1. There is a difference in stress distribution according to cortical and cancellous bone thickness and shape. 2. Maximum stress was shown at the top of cortical bone area regardless of bone engagement types. 3. Bicortical engagement showed less stress accumulation when compared to unicortical case overall. 4. Longer the implant future length, less the stress on cortical bone area, however there is no difference in mandibular bicortical engagement case.

방사선사진과 안모사진을 이용한 하악 비대칭의 평가 (Evaluation of the mandibular asymmetry using the facial photographs and the radiographs)

  • Purpose : To assess the relationship between soft tissue asymmetry and bone tissue asymmetry using the standardized photographs and the posteroanterior (PA) cephalometric radiographs in mandibular asymmetric patients. And to clarify that the lack of morphologic balance among different skeletal components can often be masked by compensatory soft tissue contributions. Methods: Experimental group consisted of 58 patients whose chief complaints were facial asymmetry, they were taken with standardized facial photographs and PA cephalometric radiographs. Control group consisted of 30 persons in the normal occlusion. The reproducibility of the facial photograph was confirmed by model test. The differences of fractional vertical heightand horizontal width from standardized facial photographs and PA cephalometric radiographs were compared and analyzed. Results: The difference of fractional vertical bone height was 0.63 and fractional vertical soft height was 0.58 in control group, 3.10 and 2.01 in asymmetric group, respectively. The difference of fractional horizontal bone width was 0.52 and fractional horizontal soft width was 0.70 in control group, 2.51 and 1.70 in asymmetric group, respectively. Both soft and bone tissue showed significant difference between control and asymmetric group (p<0.05). The difference of bone tissue was greater than that of soft tissue (p<0.05) in the experimental group but, not in control group. Conclusions: Soft tissue components may compensate for underlying skeletal imbalances.

Orthopos$\textregistered$ 파노라마방사선촬영기의 각종 프로그램에 따른 흡수선량 (The absorbed doses from each exposure program of the Orthopos$\textregistered$ panoramic machine)

  • Purpose: The objective of this study was to estimate the radiation absorbed doses in certain critical organs in the head and neck region with 16 imaging programs available on the Orthopos/sup (R)/ panoramic machine. Materials and Methods: A Rando phantom and LiF TLD chips were used for dosimetry. The absorbed doses were measured at the thyroid gland, the submandibular gland, the parotid gland, the mouth floor. the maxillary sinus, the brain, the mandibular body, the mandibular ramus. the 2nd cervical spine and the skin over TMJ area. Results : The overall absorbed doses with imaging programs available on the Orthopos/sup (R)/ panoramic machine were much less than that of standard program (program 1) except program 8, 11, and 16. Generally, the absorbed doses to the bone marrow of the mandibular ramus and the parotid gland were high, but the absorbed doses to the bone marrow in the mandibular body, brain, maxillary sinus, and, especially, the thyroid gland were very low. Conclusion : The modified imaging programs available on the Orthopos/sup (R)/ panoramic machine can be effectively used in aspect of radiation protection.

  • 본 교실에서는 조선대학교 구강악안면외과에 내원한 중안면성장부전을 동반한 하악전돌증소견을 보이는 22세 여자 환자의 치료에 있어서 변형 LeFort III 골절단술과 양측 하악지 시상분할골절단술 그리고 자가 장골 이식술을 동시에 시행하여 기능적 심미적으로 만족할 만한 결과를 얻었기에 보고하는 바이다.

Radiotherapy for mandibular metastases from hepatocellular carcinoma: a single institutional experience

  • Purpose: A mandibular metastasis is uncommon in patients with hepatocellular carcinoma (HCC). We report the clinical features of this rare lesion and evaluate the effectiveness of radiotherapy in affected patients. Materials and Methods: We retrospectively reviewed our institutional medical records for HCC patients who received radiotherapy for bone metastasis, and included cases of mandible metastasis. The clinical features of these cases, and the characteristics and outcomes of the treatments were assessed. Results: A total of 1,498 patients with a bone metastasis from HCC were treated with radiotherapy between July 1998 and April 2012 at our institution. We identified 9 patients (0.6%) in this cohort that received radiotherapy for a mandibular metastasis. The condyle was the most common location of mandibular metastasis. The median radiation dose was 40 Gy (range, 27.5 to 60 Gy), with a daily dose of 2-3 Gy. All of these 9 patients died during a median follow-up of 9 months (range, 1 to 19 months). Symptom relief was achieved in 7 of 8 patients who completed radiotherapy. Tumor size reduction was observed in 2 of 4 patients who underwent radiologic evaluation after radiotherapy. Conclusions: Metastasis to the mandible from HCC has a poor prognosis. Radiotherapy can be an effective local treatment option for symptomatic relief in these cases.

Giant osteochondroma of the parapharyngeal space: a case report

  • Osteochondroma is a common benign tumor of the axial skeleton, especially in the distal metaphysis of the femur and the proximal metaphysis of the tibia, that can occur on the facial skeleton (albeit rarely). Osteochondroma is differentiated from chondroma, osteochondromatosis and osteoma. Osteochondroma shows an irregular radiopaque lesion and chondromatic area surrounded by the osteoma. When it develops in the long bone, it has a marked tendency to occur at 10 to 20 years of age and ceases with the end of pubertal growth. However, when it develops in the mandibular condyle, it is prevalent in the third decade and continuous to develop. Tumors that develop in the long bone have a predilection for men, but tumors in the mandible have a predilection for women. In osteochondroma of the mandibular condyle, clinical features presented include occlusal changes, facial asymmetry, headaches, pain and joint noise on the temporomandibular joint, mouth opening limitations, and jaw deviation at the involved site. The first choice of treatment for the massive osteochondroma is surgical removal. A 70-year-old female patient with an osteochondroma on her right mandibular condyle visited our clinic. We surgically removed the mass with favorable results. It is presented here along with a review of literature on osteochondroma.

Repeated injections of botulinum toxin into the masseter muscle induce bony changes in human adults: A longitudinal study

  • Objective: To evaluate soft- and hard-tissue changes in the mandibular angle area after the administration of botulinum toxin type A (BoNT-A) injection to patients with masseteric hypertrophy by using three-dimensional cone-beam computed tomography (3D-CBCT). Methods: Twenty volunteers were randomly divided into two groups of 10 patients. Patients in group I received a single BoNT-A injection in both masseter muscles, while those in group II received two BoNT-A injections in each masseter muscle, with the second injection being administered 4 months after the first one. In both groups, 3D-CBCT was performed before the first injection and 6 months after the first injection. Results: Masseter muscle thicknesses and cross-sectional areas were significantly reduced in both groups, but the reductions were significantly more substantial in group II than in group I. The intergonial width of the mandibular angle area did not change significantly in either group. However, the bone volume of the mandibular gonial angle area was more significantly reduced in group II than in group I. Conclusions: The repeated administration of BoNT-A injections may induce bone volume changes in the mandibular angle area.

  • 하악 4전치 수복은 치아가 작고 공간도 좁아서 지대주 선택에 어려움이 따른다. 치주적으로 좋지 못한 상황에서 치아를 발거하는 경우가 대부분이고, 이러한 경우 치간공극을 크게 열어주는 것이 구강위생 관리 측면에서 유리하다1. 커스텀 지대주로 아무리 작은 직경으로 포스트 부위를 만든다 해도, 스크류가 들어가는 공간 때문에 작은 직경으로 지대주의 포스트 부위를 만드는 것에는 한계가 있다. One body implant2의 경우, post 부위를 넓은 정면으로 설정하는 것 보다는, 보다 d 얇은 측면(90도 회전면)으로 설정하는 것이 치간공극 형성을 위해 유리하다. Internal bone level implant 활용 시 two piece abutment를 활용하는 것 보다, one-piece abutment를 활용한다면 더 효과적으로 치간공극을 위한 공간을 확보할 수 있다.

Radiographic analysis of odontogenic cysts showing displacement of the mandibular canal

  • Purpose: To assess the radiographic findings of odontogenic cysts showing displacement of the mandibular canal using computed tomographic (CT) and panoramic images. Materials and Methods : CT and panoramic images of 63 odontogenic cysts (27 dentigerous, 16 odontogenic keratocysts, and 20 radicular cysts) were analyzed to evaluate the following parameters: the dimension and shape of the cysts, and the effect of the cysts on the mandibular canal and cortical plates. Results: Of the 63 cysts examined in the study, 35 (55.6%) showed inferior displacement of the mandibular canal and 46 (73.0%) showed perforation of the canal. There were statistically significant differences between CT and panoramic images in depicting displacement and perforation of the mandibular canal. Cortical expansion was seen in 46 cases (73.0%) and cortical perforation in 23 cases (36.5%). The radicular cysts showed cortical expansion and perforation less frequently than the other cyst groups. Conclusion: Large cysts of mandible should be evaluated by multiplanar CT images inorder to detect the mandibular canal and cortical bone involvement.

좁은 치조골에서 사다리꼴형 디자인으로 개발된 단폭경임플란트의 증례 보고 (Case report of a newly designed narrow-diameter implant with trapezoid-shape for deficient alveolar bone)

  • Long-term survival and prognosis of narrow-diameter implants have been reported to be adequate to consider them a safe method for treating a deficient alveolar ridge. The objective of this study was to perform case report of narrow-diameter implants with a trapezoid-shape in anterior teeth alveolar bone. A 50-year-old male patient presented with discomfort due to mobility of all of the maxillary teeth and mandibular incisors. Due to destruction of alveolar bone, four anterior mandibular teeth were extracted. Soft tissue healing was allowed for approximately 3 months after the extraction, and a new design of implant placement was planned for the mandibular incisor area, followed by clinical and radiological evaluation. Implant placement was determined using an R2GATE surgical stent. The stability of the implants was assessed by ISQ measurements at the first and second implant surgery and after prosthetic placement. At 1 and 3 months and 1 year after implantation of the prosthesis, clinical and radiological examinations were performed. Another 50-year-old male patient presented with discomfort due to mobility of the mandibular central incisors. For the same reason as in the first patient, implant placement was carried out in the same way after extraction. ISQ measurements and clinical and radiological examinations were performed as in the previous case. In these two clinical cases, 12 months of follow-up revealed that the implant remained stable without inflammation or additional bone loss, and there was no discomfort to the patient. In conclusion, computer-guided implant surgery was used to place an implant in an optimal position considering the upper prosthesis. A new design of a narrow-diameter implant with a trapezoid-shape into anterior mandibular alveolar bone is a less invasive treatment method and is based on the contour of the deficient alveolar ridge. Through all of these procedures, we were able to reduce the number of traumas during surgery, reduce the operation time and total treatment period, and provide patients with more comfortable treatment.
