  • The purpose of this research was to analyze the relationship between parents' attitudes toward their children's dental health and dental plaque index. Dental plaque index was obtained through oral examination of 118 children(female 40, male 78) with primary dentition The maintenance status of children's dental health though parents' aids was evaluated by means of questionaire to which 79 persons among the 118 children's parents answered. The results were as follows : 1. In children with primary dentition, toothbrushing together with toothbrushing instruction and continuous reinforcement had decreased plaque accumulation progressively. 2. Dental plaque index before toothbrushing showed that dental plaque was accumulated more in the buccal surfaces of teeth in the maxilla and in the lingual surfaces of teeth in the mandible. It was higher in the posterior teeth than in the anterior teeth and in the maxillary teeth than in the mandibular teeth. 3. Dental plaque index of childern was inversely proportional to the level of education of their parents. 4. Those parents who have much knowledge of oral hygiene were more positive toward their children's oral hygiene.

COLORIMETRIC ANALYSIS OF EXTRACTED HUMAN TEETH AND FIVE SHADE GUIDES (발거된 자연치와 5종 Shade Guide의 색채 계측기를 이용한 색상 비교)

  • The tristimulus values of 180 extracted maxillary and mandibular anterior teeth were measured by colorimeter. And it were colnverted to Munsell color order system(Hue, Value, Chroma) and CIE $L^*a^*b^*$ color coordinates. The commonly used Vita, and Bioform shade guides, 2 composite resin shade guides(Prisma APH and Z-100), and a glass-ionomer shade guide(Fuji II) were compared with these teeth. At the middle facial surface, color distributions of teeth were Hue(0.56YR to 9.77Y), Value(2.46 to 7.9), and Chroma(0.14 to 2.02). And the aberaged values and standard deviations for $L^*a^*b^*$ were $63.18{\pm}10.44$, $1.11{\pm}1.66$, and $5.79{\pm}2.36$. The shade guide did not match well with the color space of the human teeth. Especially, the lacks of the Yellow-red Hues and higher values were prominent. Compare with other measurements, the Hues of the teeth measured in this study were broadly distributed(most of ranges in Y and YR were included), while the Value and chroma were shown to the lower.

  • Purpose: The purpose of the study is to evaluate the effectiveness of anterior segmental osteotomy (ASO) in bimaxillary protrusion (BP) patients by comparison between the mandibular soft and hard tissue changes from orthodontic treatment and ASO. Methods: All 44 patients were diagnosed with BP in Kyung-Hee Medical Center. Orthodontic treatment with teeth extractions were underwent by 23 patients (Group A) and 21 patients underwent ASO (Group B). Mandibular soft and hard tissue changes were measured and evaluated, which were based on the vertical and horizontal reference line in lateral cephalometric radiographs. Statistical significance between the changes and correlation between each measurement were analyzed. Results: The amount of B point movement was lesser than that of the lower incisal tip (LIT) retraction, and LIT was tilted lingually in group A. The posterior movement discrepancy between LIT and B point was insignificant, and the inclination of lower incisor was not changed in group B. The soft tissues, including the lower lip, showed a posterior movement and reduction in the depth of mento-labial groove. According to the correlation analysis, the movement of the lower incisor was significantly related to the movement of the lower lip in group A, and the movement of the lower incisor was significantly related to that of the movement of lower lip, B point and Pog in group B. Conclusion: The orthodontic treatment in BP patients results in posterior tilting movement of the lower incisor, but ASO results in the bodily movement of the lower incisor. Consequently, ASO is more effective in BP cases because it ensures the controlled movement of the lower incisors.

THE STUDY BY USING THE COMPUTERIZED TOMOGRAPHY IMAGING IN ORDER TO ACCESS TO MANDIBULAR FORAMEN WHILE INFERIOR ALVEOLAR NERVE ANESTHESIA (하치조신경 마취시 하악공으로의 접근을 위한 전산화단층촬영을 통한 방사선적 연구)

  • Purpose : This study was performed to provide an anatomical information of the mandibular ramus for the successful inferior alveolar nerve block. Three dimensional images were reconstructed from the computerized tomography (CT) and the anatomical evaluation of the mandibular ramus was done. Materials and methods : Sixty-four patients who had been taken the facial CT scans from 2000, Jan to 2003, June was selected. The patients who had the anterior or posterior teeth misssing, edentulous ridge, and jaw fracture were excepted. In the occulusal plane, the lingual surface angle (LSA) between the mid-sagittal plane and the mandibular molar lingual surface from the 2nd premolar to the 2nd molar, the inner ramal surface angle (IRSA), the maximum inner ramal surface angle (MxIRSA), and the outer ramal surface angle (ORSA) to the-mid sagittal plane were measured. The inner ramal surface angle in the ligular tip level (IRSA-L) and the outer ramal surface angle in the ligular tip level (ORSA-L), the ramal length (RL), and the anterior ramal length (ARL) were also measured in the lingular tip level. Results : In the lingular tip level, the mean IRSA-L and ORSA-L were $28.6{\pm}6.3^{\circ}$ and $17.9{\pm}4.9^{\circ}$ respectively. The larger was the IRSA, the larger was the ORSA. In the lingular tip level, the mean ramal length was 35.8${\pm}$3.4 mm. The larger was the IRSA-L, the shorter was the ramal length. On the lingular tip level, the mean anterior ramal length from anterior ramus to lingular tip was 19.6${\pm}$3.3 mm. when the ramal length was longer, the anterior ramal length was also longer. On the lingular tip level, there was positive correlation vetween the IRSA and the ORSA, negative correlation between the IRSA and the ramal length, and positive correlation between the ramal length and the lingular tip level to the anterior ramus. There was no statistical meaning of data between sex and age. Conclusion : In the clinical view of the results so far achieved, if the direction of needle is closer to posterior it is able to contact bone on lingular tip when the internal surface of ramus is wided outer.

A Study on Clinical Crown Angulation and Inclination of Females in the Twenties with Normal Occlusion (20대 여성 정상교합자에서 임상치관의 순·설측 경사도와 근·원심 경사도에 관한 연구)

  • Purpose: This study is to present a standard value for clinical crown angulation and inclination required in laboratory process and see if the value can be used for actual laboratory process. Methods: In order to find out a standard value for clinical crown angulation and inclination, this study made a study model of normal occlusion of 21 females in twenties. The clinical crown angulation and inclination of both six-maxillary and six-mandibular anterior teeth are measured by Set-up Model Checker. From the measured value above, the mean and standard deviation of the twelve teeth are obtained, and then the mean of the teeth between right and left side is calculated. Results: Each clinical crown angulation of maxillary central incisor, lateral incisor, and canine is like this; $1.0^{\circ}{\pm}1.3^{\circ}$, $3.0^{\circ}{\pm}1.3^{\circ}$, and $5.0^{\circ}{\pm}1.4^{\circ}$. In case of mandibular, each degree is like this; $0.6^{\circ}{\pm}1.1^{\circ}$, $1.5^{\circ}{\pm}1.1^{\circ}$, and $4.1^{\circ}{\pm}1.1^{\circ}$. Each clinical crown inclination of maxillary central incisor, lateral incisor, and canine is like this; $6.1^{\circ}{\pm}1.8^{\circ}$, $4.5^{\circ}{\pm}1.9^{\circ}$, and $-6.2^{\circ}{\pm}1.4^{\circ}$. In case of mandibular, each degree is like this; $0.3^{\circ}{\pm}1.5^{\circ}$, $0.3^{\circ}{\pm}1.8^{\circ}$, and $-7.5^{\circ}{\pm}1.8^{\circ}$. Conclusion: As the result, the mean value for clinical crown angulation and inclination can be referred to actual laboratory process. However, the mean value is different from those of the precedent study and an unsatisfied one for adopting the standard value.

  • When inflammatory products are found in both periodontal and pulpal tissues simultaneously, a periodontal-endodontic combined lesion is established. The treatment of periodontal-endodontic combined lesions includes root canal therapy and periodontal regenerative procedure for resolution of both the apical and marginal inflammatory lesions. The present study reports the treatment of periodontal-endodontic combined lesions in the mandibular anterior area with root canal therapy, followed by guided tissue regeneration therapy. Teeth with severe bone destruction in each case could be preserved, without extraction, over a 3-year period. Therefore, it appears that treatment of periodontal-endodontic combined lesions in the mandibular anterior area using guided tissue regeneration technique after root canal therapy may provide clinical advantages.

Prevalence and Clinical Features of Molar-Incisor Hypomineralization in Adolescents in Yangsan (양산시 거주 청소년의 MIH 유병률과 임상적 특성)

  • This study aimed to investigate the prevalence and distribution of enamel hypomineralization, including molar-incisor hypomineralization (MIH), among adolescents and assess their correlation with esthetic satisfaction. A total of 1371 adolescents between the ages of 14 and 16 years in Yangsan city were evaluated for enamel hypomineralization, including MIH, according to the European Academy of Paediatric Dentistry (EAPD) criteria. In a parallel survey, esthetic satisfaction about anterior teeth and its correlation with incisor enamel hypomineralization were analyzed. The prevalence of MIH was 13.8% (n = 189), while that of hypomineralization in any permanent tooth was 23.2% (n = 318), which was substantially greater compared to the national prevalence of MIH. Mandibular first molars exhibited the highest prevalence of hypomineralization, followed by maxillary central incisors and mandibular second molars. Among anterior teeth, the most frequently affected site was the incisal edge of maxillary central incisors. A high degree of hypomineralization in anterior teeth was associated with a high demand for esthetic treatment.

  • Implant-supported overdenture is a reliable treatment option for the patients with edentulous mandible when they have difficulty in using complete dentures. Several options have been used for implant-supported overdenture attachments. Among these, bar attachment system has greater retention and better maintainability than others. SFI-Bar$^{(R)}$ is prefabricated and can be adjustable at chairside. Therefore, laboratory procedures such as soldering and welding are unnecessary, which leads to fewer errors and lower costs. A 67-year-old female patient presented, complaining of mobility of lower anterior teeth with old denture. She had been wearing complete denture in the maxilla and removable partial denture in the mandible with severe bone loss. After extracting the teeth, two implants were placed in front of mental foramen, and SFI-Bar$^{(R)}$ was connected. A tube bar was seated to two adapters through large ball joints and fixation screws, connecting each implant. The length of the tube bar was adjusted according to inter-implant distance. Then, a female part was attached to the bar beneath the new denture. This clinical report describes two-implant-supported overdenture using the SFI-Bar$^{(R)}$ system in a mandibular edentulous patient.

Recurrent unicystic ameloblastoma in mandibular anterior teeth area (개발된 단낭성 법랑아세포종의 증례보고)

  • The unicystic ameloblastoma (UA) is a variant of the solid or multicystic ameloblastoma, a less encountered variant of the ameloblastoma. It appears more frequently in the second or third decade with no sexual or racial predilection. It is almost exclusively encountered asymptomatic ally in the posterior mandible. We report a case of a 43-year old patient with UA, who had previously undergone a surgical treatment on the same site about 1year ago, this lesion recurred and presented as an exophytic gingival lesion in the anterior mandibular region.

Adenomatoid odontogenic tumor associated with an unerupted mandibular lateral incisor: a case report

  • Adenomatoid odontogenic tumor (AOT) is a rare, benign odontogenic tumor that predominantly appears in the second decade of life in female patients. Most AOTs occur in the anterior part of the maxilla and are usually associated with impacted anterior teeth. There are three types of AOT, follicular, extrafollicular, and peripheral, which are classified based on the location of the lesion and its association with the impacted tooth. We report a rare case of AOT associated with an impacted right mandibular lateral incisor in an 11-year-old female patient.
