심하게 결손된 하악 치조골에서 골유도재생술(GBR) 후 임플란트의 식립: 증례보고 (Implant placement after guided bone regeneration (GBR) in severe defected mandibular alveolar ridge: case report)

  • 하악 구치부에 있어서 치조제 결손은 임플란트 치과학에 있어서 큰 장애물이라 할 수 있다. 이러한 치조제를 재건하는데 있어서 여러 가지 술식이 활용되고 있다. 술식들의 선택 및 필요성은 현저한 질병의 이환률과 관련이 있으며, 2차적인 수술 부위를 요구하는 경우도 있다. 골 유도 재생술(GBR, guided bone generation)의 발전은 결손이 존재하는 치조제의 이환률을 낮추며, 2차적인 수술 부위를 요구하지 않는 경우로 볼 수 있다. 본 증례에서는 심하게 결손된 하악 치조골에 자가골을 제외한 동종골, 이종골, 합성골 입자형 골이식재를 조합하여 골 유도 재생술(GBR)을 진행한 후에 임플란트를 성공적으로 식립을 하였다. 두 증례에서 양호한 식립을 보였음에 보고하는 바이다.

  • Two patients with sufficient vertical bone height but insufficient bone width in the anterior mandibular edentulous area, less than 6mm in the buccolingual aspect, for implant placement were chosen for treatment with a ridge splitting procedure. The surgical technique involving greenstick fracture is described. This ridge splitting procedure could be simple placement of implants into ideal restorable positions in severely atrophic, knife-edged ridges and predictable for narrow edentulous alveolar ridge augmentation associated with implant placement. We experienced two cases to place implant with insufficient bone width in the anterior mandibular edentulous area for overdenture be ridge splitting technique. Thus, we will report two cases and review of the literature.

  • The purpose of this study was to evaluate the stress distribution developed in supporting structures by distal extension removable partial denture with 4-types of direct retainer. The direct retainers examined were Akers clasp, RPI clasp, RPA clasp and RPL clasp in bilateral & unilateral free end case. 3-dimensional photoelastic stress analysis was used to record the isochromatic fringe patterns and to calculate the compressive stress at measuring points. The results were as follows. 1. In bilateral free end case, RPI clasp exhibited the similar stress distribution on distal and mesial alveolar crest but Akers clasp exhibited higher stress concentration on distal alveolar crest than mesial alveolar crest. 2. In bilateral free end case, RPA clasp and RPL clasp exhibited the similar stress distribution on distal and mesial alveolar crest and RPL clasp exhibited higher stress concentration on buccal alveolar crest than lingual alveolar crest. 3. Akers clasp produced high stress concentration on residual alveolar ridge distally, but RPI clasp, RPA clasp and RPL clasp produced even stress distribution on residual alveolar ridge. 4. Removable partial denture in unilateral free end case exerted higher stress on abutment tooth root apex than bilateral distal extension removable partial denture.

  • 심한 치조골의 위축과 흡수로 인한 형태학적 변화는 임플란트의 성공적인 식립과 임플란트의 골유착에 영향을 미친다. 이를 극복하기 위한 다양한 골증대술 중 치조제분할술은 좁은 치조골 폭을 성공적으로 증대시키는 수술방법으로 보고되었다. 또한 다양하게 개발되는 임플란트 디자인과 치조제 팽창 기구 등은 심하게 흡수된 위축된 하악부위에서도 협측골 파절을 최소화할 수 있다. 가철성 부분의치의 사용으로 심하게 흡수된 하악 구치부에 치조제 분할술과 최소 크기의 블록형 골이식술을 이용해 한개의 스크류로 수용부의 고정을 획득한 후 동시적 접근법을 이용한 골이식 증례를 보고 하고자 한다. 보철과와 치주과의 협업으로 환자의 기능과 심미를 회복해준 증례로 사료된다.

좁은 치조골에서 사다리꼴형 디자인으로 개발된 단폭경임플란트의 증례 보고 (Case report of a newly designed narrow-diameter implant with trapezoid-shape for deficient alveolar bone)

  • Long-term survival and prognosis of narrow-diameter implants have been reported to be adequate to consider them a safe method for treating a deficient alveolar ridge. The objective of this study was to perform case report of narrow-diameter implants with a trapezoid-shape in anterior teeth alveolar bone. A 50-year-old male patient presented with discomfort due to mobility of all of the maxillary teeth and mandibular incisors. Due to destruction of alveolar bone, four anterior mandibular teeth were extracted. Soft tissue healing was allowed for approximately 3 months after the extraction, and a new design of implant placement was planned for the mandibular incisor area, followed by clinical and radiological evaluation. Implant placement was determined using an R2GATE surgical stent. The stability of the implants was assessed by ISQ measurements at the first and second implant surgery and after prosthetic placement. At 1 and 3 months and 1 year after implantation of the prosthesis, clinical and radiological examinations were performed. Another 50-year-old male patient presented with discomfort due to mobility of the mandibular central incisors. For the same reason as in the first patient, implant placement was carried out in the same way after extraction. ISQ measurements and clinical and radiological examinations were performed as in the previous case. In these two clinical cases, 12 months of follow-up revealed that the implant remained stable without inflammation or additional bone loss, and there was no discomfort to the patient. In conclusion, computer-guided implant surgery was used to place an implant in an optimal position considering the upper prosthesis. A new design of a narrow-diameter implant with a trapezoid-shape into anterior mandibular alveolar bone is a less invasive treatment method and is based on the contour of the deficient alveolar ridge. Through all of these procedures, we were able to reduce the number of traumas during surgery, reduce the operation time and total treatment period, and provide patients with more comfortable treatment.

  • 저자들은 잔존 치조골의 흡수 및 위축으로 통상적인 임플란트 시술이 어려운 증례에서 하악골의 하악지에서 자가골을 채취하여 이를 증례에 따라 블록형 혹은 입자형으로 골 이식술을 시행하여 다양한 증례에 적용하여 비교적 만족할 만한 임플란트 식립 수술이 가능하였던 바 이를 문헌고찰과 함께 보고하며, 본 연구에서는 다양한 술식의 임상적 활용에 대해서만 보고하였으나 향후 이러한 증례들에 대하여 보다 장기적인 추적조사와 골 이식된 부위의 골 조직의 재형성 식립된 임프란트의 보철적인 기능과 장기적인 예후에 대한보다 체계적인 연구가 필요하리라 사료된다.

The preliminary study for three-dimensional alveolar bone morphologic characteristics for alveolar bone restoration

  • Background: The concept of the ideal morphology for the alveolar bone form is an important element to reconstruct or restore the in maximizing esthetic profile and functional alveolar bone restoration. The purpose of this preliminary study is to evaluate the normal alveolar bone structure to provide the standard reference and guide template for use in diagnosing for implant placement, determining the correct amount of bone augmentation in actual clinical practice and producing prostheses based on three-dimensional imaging assessment of alveolar bone. Methods: This study was included 11 men and 11 women (average age, 22.6 and 24.5 years, respectively) selected from among 127 patients. The horizontal widths of alveolar bone of maxilla and mandible were measured at the crestal, mid-root, and root apex level on MDCT (multi-detector computed tomography) images reconstructed by medical imaging software. In addition, tooth dimensions of the central incisors, canines, second premolars, and first molars of maxilla and mandible, including the horizontal width of the interdental alveolar bone crest, were also measured and statistically analyzed. Results: The horizontal alveolar bone width of the palatal side of maxilla showed a distinct increment from the alveolar bone crest to the apical region in both anterior and posterior areas. The average widths of the maxillary alveolar ridge were as follows: central incisor, 7.43 mm; canine, 8.91 mm; second premolar, 9.57 mm; and first molar, 12.38 mm. The average widths of the mandibular alveolar ridge were as follows: central incisor, 6.21 mm; canine, 8.55 mm; second premolar, 8.45 mm; and first molar, 10.02 mm. In the buccal side, the alveolar bone width was not increased from the crest to the apical region. The horizontal alveolar bone width of an apical and mandibular border region was thinner than at the mid-root level. Conclusions: The results of the preliminary study are useful as a clinical guideline when determining dental implant diameter and position. And also, these measurements can also be useful during the production of prefabricated membranes and customized alveolar bone scaffolds.

Comparison of anterior maxillary and mandibular alveolar parameters in African American and Caucasian women: A retrospective pilot study

  • Purpose: The primary objective of this study was to analyze the thickness and height of alveolar bone around the maxillary and mandibular incisors. Additionally, this study aimed to compare bone parameters between Caucasian (CC) and African American (AA) female patients. Materials and Methods: In this retrospective pilot study, 50 female subjects(25 CC and 25 AA) were included. The inclusion criteria were AA or CC women between the ages of 18 and 50 with a normo-divergent facial pattern and Angle's class I, end-on class II, or mild class III malocclusion. The distance from the cementoenamel junction (CEJ) to the buccal and lingual alveolar crest; the alveolar ridge thickness at the mid-root and apex; and the buccal and lingual bone thickness at 3, 6, and 9mm from the CEJ were measured. Results: No significant difference was found (P>0.05) in the cortical bone thickness at 3mm, 6mm, or 9mm from the alveolar crest between CC and AA populations for most measurements. A significant difference in bone thickness was found (P<0.05) for the lingual surface of the central incisor, with maxillary bone thickness found to be higher than mandibular bone thickness. The measurements of lingual thickness were larger than those of buccal thickness for both races. Conclusion: There were no differences in maxillomandibular anterior alveolar bone measurements between normo-divergent adult AA and CC women, except for a few parameters at varying locations. However, future studies can be planned based the current pilot study data, which may provide valuable information.

Implant placement with inferior alveolar nerve repositioning in the posterior mandible

  • This case report presents inferior alveolar nerve (IAN) repositioning as a viable approach for implant placement in the mandibular molar region, where challenges of severe alveolar bone width and height deficiencies can exist. Two patients requiring implant placement in the right mandibular molar region underwent nerve transposition and lateralization. In both cases, inadequate alveolar bone height above the IAN precluded the use of short implants. The first patient exhibited an overall low alveolar ridge from the anterior to posterior regions, with a complex relationship with adjacent implant bone level and the mental nerve, complicating vertical augmentation. In the second case, although vertical bone resorption was not severe, the high positioning of the IAN within the alveolar bone due to orthognathic surgery raised concerns regarding adequate height of the implant prosthesis. Therefore, instead of onlay bone grafting, nerve transposition and lateralization were employed for implant placement. In both cases, the follow-up results demonstrated successful osseointegration of all implants and complete recovery of postoperative numbness in the lower lip and mentum area. IAN repositioning is a valuable surgical technique that allows implant placement in severely compromised posterior mandibular regions, promoting patient comfort and successful implant placement without permanent IAN damage.

  • The 25-year old unmarried female patient has lost her mandibular molars bilaterally and her alveolar ridge has been atrophied for long term teeth missing. As rejected the removable partial denture, she was practiced the vitallium subperiosteal implants pertinent to atrophied alveolar ridge. Now, the results are satisfiable to both of the author and the patient. But it is necessary to observe the prognosis for some years.
