턱벼각 절골술 후 발생한 괴사근막염의 치험례 (A Case of Necrotizing Fasciitis of Lower Leg Following Mandible Angle Ostectomy)

  • Mandible angle ostectomy is the most widely-operated facial contour surgery. We experienced a rare case of necrotizing fasciitis of the lower leg following mandible angle ostectomy. A 20 years old female visited our department 5 days after mandible angle ostectomy. At the time, she was diagnosed as sepsis and on her 2nd admission day, she was diagnosed as necrotizing fasciitis on her left leg. She was treated by fasciotomy and necrotized tissue removal. Necrotizing fasciitis is a rare complication of any kinds of operation but once it develops, it causes fatal results, especially to cosmetic surgeries. Early diagnosis and aggressive operation is known as only treatment for the disease.

The effect of mechanical properties of bone in the mandible, a numerical case study

  • Bone properties are one of the key components when constructing models that can simulate the mechanical behavior of a mandible. Due to the complexity of the structure, the tooth, ligaments, different bones etc., some simplifications are often considered and bone properties are one of them. The objective of this study is to understand if a simplification of the problem is possible and assess its influence on mandible behavior. A cadaveric toothless mandible was used to build three computational models from CT scan information: a full cortical bone model; a cortical and cancellous bone model, and a model where the Young's modulus was obtained as function of the pixel value in a CT scan. Twelve muscle forces were applied on the mandible. Results showed that although all the models presented the same type of global behavior and proximity in some locations, the influence of cancellous bone can be seen in strain distribution. The different Young's modulus defined by the CT scan gray scale influenced the maximum and minimum strains. For modeling general behavior, a full cortical bone model can be effective. However, when cancellous bone is included, maximum values in thin regions increase the strain distribution. Results revealed that when properties are assigned to the gray scale some peaks could occur which did not represent the real situation.

  • Background: This clinical case presented a novel method of segmental mandible reconstruction using 3D-printed titanium implant with pre-mounted dental implants that was planned to rehabilitate occlusion. Case presentation: A 53-year-old male who suffered osteoradionecrosis due to the radiation after squamous cell carcinoma resection. The 3D-printed titanium implant with pre-mounted dental implant fixtures was simulated and fabricated with selective laser melting method. The implant was successfully inserted, and the discontinuous mandible defect was rehabilitated without postoperative infection or foreign body reaction during follow-ups, until a year. Conclusions: The 3D-printed titanium implant would be the one of the suitable treatment modalities for mandible reconstruction considering all the aspect of mandibular functions.

  • Ameloblastic fibro-odontoma is a rare benign mixed odontogenic tumor with histologic characteristics of ameloblastic fibroma and complex odontoma. As with ameloblastic fibroma, it may be asymptomatic or found because of painless swelling and delayed eruption of associated tooth. This report presents a case of ameloblastic fibro-odontoma in the posterior mandible of a 14-year-old girl and analyses its clinical features and radiographic features on plain X-ray film and CT images.   • I had treated a patient, 58 years old male, with squamous cell carcinoma in mandible. In the patient, radical operation was made by hemimandiblectomy, neck dissection and gnathoplasty with resin made mandible. The patient was radically treated, and the results were excellent in point of view of mandibular functions and esthetics.

  • Schwannoma(neurilemmoma) is a benign neoplasm that originates from the Schwann cells. Schwannoma most commonly arises in the soft tissue of the head and neck. Intraoral lesions are uncommon, however, and intraosseous schwannoma are even rare. The most common site of occurrence for these unusual lesions is the mandible. This article documents a case of an intraosseous schwannoma located in the mandibular ascending ramus. The lesion was surgically removed and no radiographic evidence of recurrence was observed after 1 year.

  • Twenty three year old soldier, was referred to me because of extreme mandibular prognathism. Eight years before, the patient had become aware of some protrusion of his mandible. Up until this time, his jaws seemed to have been developing normally. The oblique osteotomy of the mandible was performed. Risdon cable wiring was used in the upper and lower jaw in order to immobilize the mandible by intermaxillary wiring. The patient recovered from the operation, and was discharged from the 1st Army Hospital after 6 months with good functional mandibular relationship. The appearance of the patient was greatly improved.

  • Myofibroma is a benign tumor composed of fibroblasts and myofibroblasts, occasionally occurring most commonly as a solitary lesion of soft tissue, skin, or bone in children younger than 3 years of age. Solitary lesion of myofibroma is exceedingly rare in adult jaws. This report describes a rare case of myofibroma in the mandible that occurred in a 41-year-old Korean woman.

  • The effect of irradiation of x-ray to developing rat mandible in the gestation stage was focused on the study of mandible development and the side effect of x-ray irradiation. The author studied the effect of x-ray irradiation with the gestated rat and their off'springs. 100 rads, 200 rads, 300 rads and 400 rads of x-ray was irradiated in regular order schematically at the lower left abdomen of gestated rat. 18½days after conception, their off'springs were sacrificed and exaimined their developing mandible with histological findings. The results were as followed. 1. In the 100-200 rads irradiated rat off'springs, bony trabeclulation was revealed irregular shape. In combine with this finding, osteoblast and fibroblast were appeared shrunken of their nucleus and location of eccentric position. 2. In the 300-400 rads irradiated rat off'springs, decrease of fibroblast and osteoblast appearance in the periosteum were prominently observed and empty lacunae were frequently appeared in their bone matrix. 3. The advent of osteoclast and resorption of cortical bone were appeared in proportion to increasing of x-ray irradiation.

하악에 전이된 위 선양암종 (Metastatic gastric adenocarcinoma to the mandible)

  • Metastatic tumors to the jaw bones are uncommon. The most common metastatic tumors to the jaw bones are the breast, lung and kidney. In the jaw bones, the common location of the lesions is the mandible, and the posterior area of the mandible is more commonly affected. The radiographic appearance is quite variable. In this report, a very rare case of metastatic gastric adenocarcinoma to the mandible is presented. The patient had undergone a gastrectomy 3 years ago.
