Abducens Nerve Palsy after Lumbar Spinal Fusion Surgery with Inadvertent Dural Tearing

  • Abducens nerve palsy associated with spinal surgery is extremely rare. We report an extremely rare case of abducens nerve palsy after lumbar spinal fusion surgery with inadvertent dural tearing, which resolved spontaneously and completely. A 61-year-old previous healthy man presented with chronic lower back pain of 6 weeks duration and 2 weeks history of bilateral leg pain. He was diagnosed as having isthmic spondylolisthesis at L4-5 and L5-S1, and posterior lumbar interbody fusion was conducted on L4-5 and L5-S1. During the operation, inadvertent dural tearing occurred, which was repaired with a watertight dural closure. The patient recovered uneventfully from general anesthesia and his visual analogue pain scores decreased from 9 pre-op to 3 immediately after his operation. However, on day 2 he developed headache and nausea, which were severe when he was upright, but alleviated when supine. This led us to consider the possibility of cerebrospinal fluid leakage, and thus, he was restricted to bed. After an interval of bed rest, the severe headache disappeared, but four days after surgery he experienced diplopia during right gaze, which was caused by right-side palsy of the abducens nerve. Under conservative treatment, the diplopia gradually disappeared and was completely resolved at 5 weeks post-op.

  • Oxidative stress damages all cellular constituents, and therefore, cell has to possess various defense mechanisms to cope. Saccharomyces cerevisiae, widely used as a model organism for studying cellular responses to oxidative stress, contains three glutathione peroxidase (Gpx) proteins. Among them, Gpx3 plays a major defense role against oxidative stress in S. cerevisiae. In this study, in order to identity the new interaction proteins of Gpx3, we carried out two-dimensional gel electrophoresis after immunoprecipitation (IP-2DE), and MALDI-TOF mass spectrometry. The results showed that several proteins including protein disulfide isomerase, glutaredoxin 2, and SSY protein 3 specifically interact with Gpx3. These findings led us to suggest the possibility that Gpx3, known as a redox sensor and ROS scavenger, has another functional role by interacting with several proteins with various cellular functions.

  • For detecting of Faulty Insulator, a new Lateral Information Propagation Networks (LIPN) has been proposed. Energized insulator is reduced the rate of insulation extremely, and taken the results dirty and injured. It is necessary to be actions that detect the faulty insulator and exchange the new one. And thus, we have designed the LIPN to be detected that insulators by the real time computation method through the inter-node diffusion. In the network, a node corresponds to a state in the quantized input space. Each node is composed of a processing unit and fixed weights from its neighbor nodes as well as its input terminal. Information propagates among neighbor nodes laterally and inter-node interpolation is achieved. Through several simulation experiments,real time reconstruction of the nonlinear image information is processed.

  • Miller Fisher syndrome is characterized by ophthalmoplegia, ataxia and areflexia and develops after respiratory tract viral infection. Other events are GI tract infection, vaccination, digitalis intoxication, insect bite and delivery. Diagnosis of Miller Fisher syndrome can be made with clinical history taking, cardinal symptoms and normal findings of CT or MRI. We have experienced a case of Miller Fisher syndrome and treated with herbal medicine, eletro-acupuncture at paralytic external ophthalmic muscles. We enforced electro-acupuncture for 10 minutes daily. We used the PG-306 electro-acupuncture products(Suzuki Iryoki Co. Japan) and applied the low consequence wave of 1-8Hz. In 3 months, all the main symptoms disappered and the patient improved in health. Based on this experience, herbal medicine and eletro-acupuncture can be applied to the Miller Fisher syndrome.

  • Extrahepatic bile duct injury is commonly associated with hepatic, duodenal, or pancreatic injuries, and isolated extrahepatic bile duct injury is rare. We report a patient who presented with an isolated extrahepatic bile duct injury after blunt trauma. A 50-year-old man was referred to our hospital after having suffered a fall down injury. His laboratory findings showed hyperbiliribinemia with elevated aspartate aminotransferase and alanine aminotransferase level. Initial abdominal computed tomography (CT) showed a mild degree of hemoperitoneum without evidence of abdominal solid organ injury. On the 3rd day of hospitalization, the patient complained of dyspnea and severe abdominal discomfort. Follow-up abdominal CT showed no significant interval change. Owing to the patient's condition, Emergency laparotomy revealed a large amount of bile-containing fluid collection and about 1 cm in size laceration on the left lateral side of the common hepatic duct. Primary repair of the injured bile duct with T-tube insertion was performed On postoperative day (POD) 30, endoscopic retrograde cholangiopancreatography showed minimal bile leakage and endoscopic sphincteroplasty and endoscopic retrograde biliary drainage were performed. On POD 61, the T-tube was removed and the patient was discharged.

  • Skull-base osteomyelitis is a rare disease affecting the medulla of the temporal, sphenoid, and occipital bones. In general, it occurs due to external ear canal infections caused by malignant external otitis. Skull-base osteomyelitis usually affects elderly diabetic patients. The patient, a 58-year-old man, was referred for evaluation and management of the left jaw. Clinical examination of the patient revealed pain in the left jaw and mouth-opening deflection to the left. The maximum active mouth opening was measured to about 27 mm. Panoramic, CT, and CBCT revealed bone resorption patterns in the left condyle. Through control of diabetes, continued pharmacological treatment, arthrocentesis, and occlusal stabilization appliance therapy were carried out. The extent of active mouth opening was increased to 45 mm, and pain in the left jaw joint was alleviated. This was a case wherein complications caused by failure to control diabetes induced skull-base osteomyelitis. There is a need for continued discussion about the advantages and disadvantages of arthrocentesis with lavage for patients with skull-base osteomyelitis and other treatment options.

  • Tarsal tunnel syndrome is often misdiagnosed it shares clinical features with other diseases of the lower extremity such as lumbar radiculopathy and arthritis. Failure to diagnose and treat neuropathies effectively can cause permanent neuropathic pain. We report a case of a ganglion cyst detected early using ultrasonography and MRI in a 51-year-old man who complained of rapidly aggravating pain, numbness and paresthesia on the great toe and the first metatarsal area in the sole, symptoms that resembled tarsal tunnel syndrome. Surgical treatment could be performed early with no permanent sequelae. We could also avoid repeated steroid injections or prescription medications.

  • A virtual prototyping (VP) system provides realistic 3D images of product. A virtual prototype, however, can only be operated using traditional input device like keyboards or mice and cannot present physical interface of the product. This paper describes a haptic dial system for virtual prototyping, which integrates haptic effects with virtual user interface to facilitate product development. A user can interface the virtual prototype with the motor-driven haptic dial with feeling like operating mechanical dial. This system will help product designers sketch user interfaces during the whole stage of product design.

  • Percentage differential characteristic relaying(PDR) has been recognized as the principal basis for power transformer protection. Second harmonic restraint PDR has been widely used for magnetizing inrush in practice. Nowadays, relaying signals can contain 2nd harmonic component to a large extent even in a normal state, and 2nd harmonic ratio indicates a tendency of relative reduction because of the advancement of material. Further, as the power system voltage becomes higher and more underground cables are used, larger 2nd harmonic component in the differential current under internal fault is observed. And then, conventional 2nd harmonic restraint PDR exposes some doubt in reliability. It is, therefore, necessary to develop a new algorithm for performance improvement of conventional protective relaying. This paper proposes an advanced protective relaying algorithm by using voltage-current trend and flux-differential current slope characteristic. To evaluate the performance of the proposed algorithm, we have made comparative studies of PDR, fuzzy relaying and DWT relaying. The paper is constructed power system model including power transformer, utilizing the WatATP, and data collection is made through simulation of various internal faults and inrush. As the results of test, the new proposed algorithm was proven to be faster and more reliable.

Inhibitory Effect on Melanin Synthesis of Radix Codonopsis Lanceolatae (더덕 추출물의 멜라닌 생성 억제 효과)

  • The aim of this study was to investigate the effect of Codonopsis lanceolatae on the melanogenesis of HM3KO human melanoma cells biologically. The cells were treated for 3 days and 5 days with Codonopsis lanceolatae at several concentrations. The effects on tyrosinase activity and melanin contents were examined. And DOPAchrome tautomerase(TRP-2) activity was also examined to search a pathway influencing this inhibitory effect. 1. It was examined mushroom tyrosinase activity was significantly inhibited by Codonopsis lanceolatae. 2. Treatment with Codonopsis lanceolatae did not affect cell viability at the highest concentration, 1 ㎎/ml, and suppressed melanin contents as a time and dose dependent manner. 3. It was investigated treatment with Codonopsis lanceolatae inhibited tyrosinase, a key enzyme forming melanin, activity in a dose-dependent manner. 4. Treatment with Codonopsis lanceolatae did not affect DOPAchrome tautomerase(TRP-2) activity. 5. There was not a morphological change by Codonopsis lanceolatae microscopically. These results suggest that Codonopsis lanceolatae is a candidate for an efficient whitening agent which supresses melanogenesis by a Raper-Mason pathway.
