Common dental anomalies in Korean orthodontic patients: An update

  • Objective: The aim of this study was 1) to investigate the prevalence and pattern of dental anomalies (DAs), 2) to compare DAs according to the type of malocclusion, and 3) to investigate the correlation between tooth impaction and other DAs in the Korean orthodontic population. Methods: A total of 3,240 orthodontic patients were classified as Class I, Class II, or Class III malocclusion groups. The presence and location of common DAs, including impaction, microdontia, agenesis, supernumerary tooth, transposition, and fusion, were identified by examining diagnostic records. Furthermore, samples were classified as Group 1 without impaction or Group 2 with impaction. The prevalence of other DAs concurrent with impaction was investigated and compared to Group 1. Results: Impaction was the most prevalent DA, followed by microdontia, agenesis, and supernumerary. Class I and Class III groups showed the same order of prevalence, but agenesis was more frequent than microdontia in the Class II group. The prevalence of the four DAs was lowest in the Class III group. Overall, 8.6% of patients were classified into Group 2. The incidence of DAs other than impaction and the prevalence of multiple concurrent DAs were significantly higher in Group 2. Impaction showed a significant relationship with supernumerary tooth, transposition, and fusion. Conclusions: The prevalence and pattern of DAs varied depending on the type of malocclusion. As there was a higher risk of other DAs in patients with impacted teeth, early detection of the impacted tooth and a detailed diagnosis of other possible DAs may be essential.

Characterization of facial asymmetry phenotypes in adult patients with skeletal Class III malocclusion using three-dimensional computed tomography and cluster analysis

  • Objective: To classify facial asymmetry (FA) phenotypes in adult patients with skeletal Class III (C-III) malocclusion. Methods: A total of 120 C-III patients who underwent orthognathic surgery (OGS) and whose three-dimensional computed tomography images were taken one month prior to OGS were evaluated. Thirty hard tissue landmarks were identified. After measurement of 22 variables, including cant (°, mm), shift (mm), and yaw (°) of the maxilla, maxillary dentition (Max-dent), mandibular dentition, mandible, and mandibular border (Man-border) and differences in the frontal ramus angle (FRA, °) and ramus height (RH, mm), K-means cluster analysis was conducted using three variables (cant in the Max-dent [mm] and shift [mm] and yaw [°] in the Manborder). Statistical analyses were conducted to characterize the differences in the FA variables among the clusters. Results: The FA phenotypes were classified into five types: 1) non-asymmetry type (35.8%); 2) maxillary-cant type (14.2%; severe cant of the Max-dent, mild shift of the Man-border); 3) mandibular-shift and yaw type (16.7%; moderate shift and yaw of the Man-border, mild RH-difference); 4) complex type (9.2%; severe cant of the Max-dent, moderate cant, severe shift, and severe yaw of the Man-border, moderate differences in FRA and RH); and 5) maxillary reverse-cant type (24.2%; reverse-cant of the Max-dent). Strategic decompensation by pre-surgical orthodontic treatment and considerations for OGS planning were proposed according to the FA phenotypes. Conclusions: This FA phenotype classification may be an effective tool for differential diagnosis and surgical planning for Class III patients with FA.

Acute Malocclusion Caused by Articular Disc Perforation: A Case Report

  • Patients with temporomandibular disorder often present with acute occlusal change and properly managed with conservative treatment. If such change is caused by unusual etiology, differential diagnosis may be challenged. This article describes the diagnosis of a patient exhibiting pain and acute posterior open bite on the ipsilateral side after chewing hard food. After initial conservative treatment failed to resolve the complaint, magnetic resonance imaging was ordered and confirmed partial perforation of articular disc. Disc perforation itself is usually chronic in nature, but sudden macrotrauma may also cause the disorder. However, occlusal discrepancy caused by disc perforation is rare and seldom reported. We present a case of acute malocclusion caused by disc perforation with a review of related literature.

정상교합 및 III급부정교합의 두개악안면 골격요소에 관한 두부방사선계측학적 연구 (A ROENTGENOCEPHALOMETRIC STUDY ON MORPHOLOGIC FACTORS OF NORMAL OCCLUSION AND CLASS III MALOCCLUSION)

  • There are variations in regional cranial and facial balance as a normal developmental process and regional imbalances often tend to compensate each other to provide functional equilibrium. This study was designed to analyse the patterns of morphologic harmony and inharmony inherent in normal occlusion and malocclusion. The subjects consisted of 92 individuals with normal occlusion and 60 Class III malocclusion patients. Their lateral cephalograms were traced and analysed using the counterpart analysis described by Enlow. The normal occlusion group was divided into Normal Types A and B according to the relative positions of Points A and B. The following conclusions were reached: 1 The normal occlusion consisted of $28.3\%$ of Normal Type A and $69.6\%$ of Normal Type B. 2. The Normal Type A and B differed from each other in the morphology of the cranial base, the mandibular ramus and corpus, and the functional occlusal plane. The Normal Type B showed considerable mandibular protrusion effect in the effective dimension and alignment of the above factors. 3. Most normal individuals showed some degree of disharmony among morphologic factors but the deviations were relatively small. 4. The Normal Type B was less balanced than the Normal Type A. 5. More regional imbalances were involved in Class III malocclusion and the imbalances were more severe.

Correction of Class III malocclusion with alternate rapid maxillary expansions and constrictions using a hybrid hyrax-mandibular miniplate combination and simultaneous orthodontic treatment: A case report

  • In this report, we describe the successful use of alternate rapid maxillary expansions and constrictions with a hybrid hyrax-mandibular miniplate combination and simultaneous orthodontic treatment for the management of severe Class III malocclusion due to maxillary hypoplasia in an 11-year-old girl. The devices were removed after 20 months of treatment, and the family was instructed about a careful control and retention program that should be followed in accordance with the patient's growth. The final result included the correction of Class III malocclusion with adequate function and excellent facial esthetics, which restored the patient's self-esteem and provided personal motivation. The outcomes showed good stability after 24 months of retention. The decrease in the duration of active treatment is the most important finding from the present case. Considering that facial esthetics in adolescence is a determining factor for the development of a personality and interpersonal relationships, we recommend the use of this protocol for growing patients, who will exhibit not only an improved physical appearance but also a better quality of life.

가철식 Lip Pad가 부가된 Reverse Twin Block 장치 (THE REVERSE TWIN BLOCK APPLIANCE WITH REMOVABLE LIP PADS)

  • 항시 장착이 가능한 reverse twin block 장치에 기능의 장애를 받지 않고도 lip pad의 효과를 첨가하기 위하여 가철식으로 lip pad를 제작하여 III급 부정교합의 치료에 적용해 본 결과 다음과 같은 결론을 얻었다. 1. Reverse twin block 장치는 장착한 상태에서 lip pad의 착탈을 통하여 정상적인 저작, 발음 등의 기능을 할 수 있으며 심미적이므로 III급 부정교합의 치료에 있어 상하악이 분리되어 있지 않은 다른 기능성 장치가 가진 단점을 극복할 수 있었다. 2. 본 증례에서는 저발육된 상악에 대한 효과적인 작용을 하는 lip pad를 가철식으로 제작하여 기능의 장애를 받지 않고도 양호한 치료효과를 얻을 수 있었다.

  • Purpose: This article examined the affecting factors of a malocclusion status of a patient for orthodontic treatment, orthodontics medical expenses and oral health behavior on orthodontic satisfaction. Methods: This paper conducted a survey from the 15th of July to 30th of September 2012 for the patients who were under orthodontic treatment at three dental clinics where are in Deagu, and distributed a total of 210 questionnaires and analyzed 194 questionnaires, excepting for some questionnaires that were answered unfaithfully. Results: This study classified the related factors into a feeling of satisfaction with treatment and mental satisfaction for finding orthodontic satisfaction. There were the effect of the right tooth-brushing method, a periodic scaling and orthodontics medical expenses on a feeling of satisfaction with treatment, and power of explanation was 16.7%. Conclusion: There were the effect of a malocclusion status, matters that requires attention during orthodontic treatment, a periodic scaling during orthodontic treatment and orthodontics medical expenses on mental satisfaction, and power of explanation was 16.9%. Based on the result above, this paper concluded that preventive treatment and early treatment should be emphasized through developing a program for regular oral examination suited to each medical type, including the method for improving the medical treatment condition and care service for increasing orthodontic satisfaction, which the dental medical-service providers could consider the patients and secure trust.

2급 부정교합에서 교정 전 Trainer를 이용한 조기치료 효과와 예후관찰 (The effects and follow-up of early preorthdontic trainer treatment on class II malocclusions)

  • TRAINER for Kids ($T4K^{TM}$, Myofunctional. Research Co, Australia)는 주로 구강악습관으로 혀내밀기, 구호흡, 혀의 하방위치, 혀, 손가락 빨기에 의한 변형 등에 의한 부정교합이 야기되었거나 발생하고 있는 어린이들에게 쓰이고 있는 기성 근기능 교정장치이다. Trainer 장치는 기성품으로서 작은 부피와 부드러운 질감으로 어린 환자들의 장착 동의율을 높이는 데 장점을 가진다. 장치에 포함된 lip bumper는 과도한 하순의 힘을 차단하여 주고, 하순 내측 전정을 자극하는 요소가 포함되어 있다. 부적절한 혀의 위치를 교정할 수 있는 요소는 환자들이 장치를 장착하는 동안 부가적인 혀 운동을 할 필요성을 감소시킨다. 본 연구의 목적은 원광대학교 치과대학 산본치과병원에 2010년 1월과 6월에 내원한 혼합치열기의 II급 부정교합 환자에게 약 6~10개월간 Trainer를 착용한 후 동일한 환자를 3년간 정기적으로 예후관찰(follow up)한 결과 값을 가지고 이 장치의 임상적 효용 가치에 대해 분석하기 위해 진행되었다. 연구 결과, 첫째, 구강 내 장치의 일종인 Trainer를 혼합치열기 2급 부정교합 환자에게 사용한 결과 치료 전 존재하였던 상악 전치부 전돌 증상이 개선되어 안모의 심미적인 개선효과가 있었다. 둘째, 정상 교합에 비교하여 과도한 수직, 수평 피개 교합이 개선되었다. 셋째, 치료기간 및 예후 관찰기간동안 SNB 값은 증가하고, ANB 값은 감소하여 하악이 전방으로 성장한 것을 알 수 있었다. 그러므로, Trainer system은 혼합치열기 2급 부정교합 어린이에서 조기 교정 전치료로 임상에 활용 가능한 방법이다.

The effect of orthognathic surgery on the lip lines while smiling in skeletal class III patients with facial asymmetry

  • Background: The aim of this study was to examine the relationship between improvements in lip asymmetry at rest and while smiling after orthognathic surgery in patients with skeletal class III malocclusion. Methods: This study included 21 patients with skeletal class III malocclusion and facial asymmetry. We used preoperative and postoperative CT data and photographs to measure the vertical distance of the lips when smiling. The photographs were calibrated based on these distances and the CT image. We compared preoperative and postoperative results with the t test and correlations between measurements at rest and when smiling by regression analyses. Results: There were significant correlations between the postoperative changes in canting of the mouth corners at rest, canting of the canines, canting of the first molars, the slope of the line connecting the canines, and the slope of the line connecting first molars. The magnitude of the postoperative lip line improvement while smiling was not significantly correlated with changes in the canting and slopes of the canines, molars, and lip lines at rest. Conclusions: It remains difficult to predict lip line changes while smiling compared with at rest after orthognathic surgery in patients with mandibular prognathism, accompanied by facial asymmetry.

New approach of maxillary protraction using modified C-palatal plates in Class III patients

  • Maxillary protraction is the conventional treatment for growing Class III patients with maxillary deficiency, but it has undesirable dental effects. The purpose of this report is to introduce an alternative modality of maxillary protraction in patients with dentoskeletal Class III malocclusion using a modified C-palatal plate connected with elastics to a face mask. This method improved skeletal measurements, corrected overjet, and slightly improved the profile. The patients may require definitive treatment in adolescence or adulthood. The modified C-palatal plate enables nonsurgical maxillary advancement with maximal skeletal effects and minimal dental side effects.
