A Case of Cognitive & Behavioral Therapy and Oriental Medical Treatment On Social Phobia (한방치료와 인지행동 치료를 병행한 사회공포증 1례(例)에 대한 임상보고)

  • The purpose of this study was to evaluate the effects and limitations of cognitive & behavioral therapy that applied to the social phobia. In this case, a female patient, 25 old, who had been treated distantly by her friends, was trained to catch her dysfunctional belief and maladaptive automatic thoughts. In this training, we used homework and counselling for 2months. And we diagnosed her state as SimDamHeoGup(心膽虛怯), so we used acupuncture treatment and herbs, for example HaChulBoShimTang(夏朮補心湯) and GuiBiWonDamTang(歸脾溫膽湯). In result, she could recover social relationship slowly. Conclusionally, cognitive & behavioral therapy can be very effective methods to treat social phobia, because patients with social phobia have maladaptive automatic thoughts, based on dysfunctional beliefs like 'I'm infelicitous' or 'I'm ineffective'.

  • Cognitive-behavioral approach can be clinically applied to coping with stress, because cognitions are playing a central mediating role in the occurances of stress and stress reactions. In other words, cognitive distortions can be associated with causing and/or maintaining psychopathology. The goal of cognitive-behavioral approach is to help the patients identify and alter cognitive distortions and maladaptive assumptions. This approach is aimed not at curing but rather at helping the patients to develop better coping strategies to deal with their life and work. The cognitive-behavioral techniques often used in this approach include problem solving, hypothesis-testing, self-monitoring, cognitive challenges, generating alternatives to automatic cognitive distortions, self-instruction, attribution and reattribution, and techniques to control or suppress thoughts. This approach is considered to be helpful for treatment and prevention of psychiatric disorders including psychosomatic disorders, in which stress can greatly affect their onset and course.

  • Panic disorder is one of the anxiety disorder, characterized by panic attacks which are discrete episodes of fear accompanied by somatic symptoms such as shortness of breath, palpitations, chest pain, choking, dizziness, trembling and/or faintness. We experienced a 41 year-old male who complained of sudden enervation, unstable blood pressure and anxiety about self's symptoms. We bad given herbal medicines aoh cognitive & behavioral therapy. Results from studies to date suggest the cognitive behavioral therapy(CBTl are useful for depression, anxiety disorder, phobia. He understood his symptom's meaning and tried to overcome fear related to symptoms through exposure training. We concluded that cognitive behavioral therapy can be very effective methods to treat panic disorder, because patient with panic disorder has maladaptive automatic thoughts, based on dysfunctional beliefs like "I'm too weak, I have some problem."
