Prevalence and Effect of Workplace Violence against Emergency Nurses at a Tertiary Hospital in Kenya: A Cross-Sectional Study

  • Introduction: Workplace violence (WPV) is a major occupational and health hazard for nurses. It affects nurses' physical and psychological well-being and impacts health service delivery. We aimed to assess the prevalence and describe the consequences of WPV experienced by nurses working in an emergency department in Kenya. Methods: We conducted a descriptive cross-sectional study among emergency nurses at one of the largest tertiary hospitals in Kenya. We collected data using a structured questionnaire adapted from the 'WPV in the Health Sector, Country Case Studies Research Instruments' questionnaire. We described the prevalence and effects of WPV using frequencies and percentages. Results: Of the 82 participating nurses, 64.6% were female, 57.3% were married and 65.8% were college-educated (65.8%). Participants' mean age was 33.8 years (standard deviation: 6.8 years, range: 23-55). The overall lifetime prevalence of WPV was 81.7% (n = 67, 95% confidence interval [CI]: 71.6%-88.8%) and the 1-year prevalence was 73.2% (n = 63, 95% CI: 66.3-84.8%). The main WPV included verbal abuse, physical violence, and sexual harassment. Most incidents were perpetrated by patients and their relatives. No action was taken in 50% of the incidents, but 57.1% of physical violence incidents were reported to the hospital security and 28.6% to supervisors. Perpetrators of physical violence were verbally warned (42.9%) and reported to the hospital security (28.6%). Conclusion: Workplace violence is a significant problem affecting emergency nurses in Kenya. Hospitals should promote workplace safety with zero-tolerance to violence. Nurses should be sensitised on WPV to mitigate violence and supported when they experience WPV.

Evaluation of Physicians' Perception of Patient Safety Incidents Including Disclosure Utilizing Hypothetical Clinical Vignettes

  • Purpose:We investigated physicians' responses to a series of clinical vignettes consisting of patient safety incidents, with and without disclosure of patient safety incidents (DPSI). Methods: An anonymous survey was conducted to investigate physicians' responses to the DPSI via online communities of physicians, and additional participants were recruited using a snowballing sampling method. We evaluated physicians' responses to the DPSI using eight hypothetical scenarios (HS) from the following perspectives: thoughts regarding medical errors, revisiting the physician, recommendation, lawsuit, criminal prosecution, trust score, and compensation amounts. We used the chi-square test to evaluate the overall differences in response rates among the scenarios. Statistical analyses were performed using the Student's t-test to compare the trust scores and compensation amounts. Results: A total of 910 physicians participated in this survey. An overall comparison of trust scores among HS showed that HS 1 (unclear medical errors, minor harm, and DPSI) had the highest trust score. In contrast, in the opposite scenario, HS 8 (clear medical errors, major harm, and DPSI not conducted) received the lowest scores. Cases with minor harm to patients (HS 1, 2, 5, and 6) showed lower compensation amounts than the others (HS 3, 4, 7, and 8). Physicians were more likely to think of situations with DPSI as not having medical errors (53.1% vs. 55.2%). In addition, the scenarios with DPSI were evaluated favorably in terms of intention to revisit, recommend, suit, and engage in criminal proceedings. Physicians showed higher trust scores (6.2 vs 5.4) and gave lower compensation amounts ($27.7 million vs $28.1 million), although there was no significant difference in terms of compensation amounts to the physician conducting DPSI. Conclusion: Our study showed overall positive perceptions regarding DPSI among Korean physicians.

대형 기름유출사고와 방제조치에 관한 연구 2. 국내 해양 기름오염사고 분석 (Overview of Major Oil Spill at Sea and Details of Various Response Actions 2. Analysis of Marine Oil Pollution Incidents in Korea)

  • 해양 기름오염 방제조치에 관한 연구의 일환으로 정량적 기초자료를 확보하기 위하여 최근 10년간(2003~2012년) 국내 연안에서 발생한 기름오염사고에 관한 통계자료를 수집하여 연간 기름오염사고 건수와 연간 기름 유출량을 사고 원인(Cause)별, 오염원(Source)별, 해역(Sea area)별로 분석하였다. (1) 사고 원인별로 분석한 결과, (1) 부주의로 인한 사고 건수(1,429건)와 해양사고로 인한 사고 건수(790건)가 각각 전체 건수(2,833건)의 50.4 %와 27.9 %를 차지하였다. 그리고 해가 거듭될수록 해양사고로 인한 사고 건수는 감소하는 경향을 보인 반면에 부주의로 인한 사고 건수는 증가하는 경향을 보였다. (2) 해양사고로 인한 유출량(17,400 kL)이 전체 기름 유출량(17,877 kL)의 97.3 %를 차지하였고 부주의로 인한 기름유출량(294 kL)이 1.7 %를 점유하였다. (2) 오염원별로 분석한 결과, (1) 어선에 의한 사고건수(1,210건)가 전체 건수(2,833건)의 42.7 %, 기타 선박에 의한 사고 건수(620건)가 21.9 %, 화물선에 의한 사고 건수(367건)가 13.0 %, 유조선에 의한 사고 건수(261건)가 9.2 %를 차지하였다. 해가 거듭될수록 어선에 의한 사고 건수는 감소하는 경향을 보였지만 여전히 높은 수치를 보인 반면에 화물선과 기타 선박에 의한 사고 건수는 완만하게 증가하는 경향을 보였고 유조선에 의한 사고 건수는 연간 13~37건으로 해마다 크게 변동하였다. (2) 유조선의 기름 유출량(15,488 kL)이 전체 기름 유출량(17,877 kL)의 86.7 %를, 기타 선박의 기름 유출량(898 kL)이 5.0 %를, 화물선의 기름 유출량(733 kL)이 4.1 %를, 어선의 기름 유출량(590 kL)이 3.3 %를 차지하였다. (3) 해역별로 분석한 결과, (1) 남해에서의 사고 건수(1,613건)와 기름 유출량(3,804 kL)은 전체 사고 건수의 56.9 %와 전체 기름 유출량의 21.3 %를, 서해에서의 사고 건수(700건)와 기름 유출량(13,501 kL)은 전체 사고 건수의 24.7 %와 전체 기름 유출량의 75.5 %를, 동해에서의 사고 건수(520건)와 기름 유출량(572 kL)은 전체 사고 건수의 18.2 %와 전체 기름 유출량의 3.2 %를 차지함으로써 사고 건수에서는 남해가 최고였고 기름 유출량에서는 2006년까지는 남해가 최고(1위)였으나 2007년 유조선 Hebei Spirit호 기름유출사고 이후에는 서해가 최고(1위)였으며, 동해에서의 사고 건수와 기름 유출량이 모두 남해나 서해보다 적은 값으로 나타났다. (2) 해가 거듭될수록 동해와 남해에서의 사고 건수와 기름 유출량이 모두 감소하는 경향을 보인 반면에 서해에서는 사고 건수가 연도별 증감의 변동 폭이 적어 거의 일정한 경향을 보였으며 기름 유출량은 연도별로 증감의 변동 폭이 컸다.

국내 저비용 항공사 이용승객의 안전에 관한 인식 연구 (A Study of Passenger's Safety Perception on Domestic Low-Cost Carriers)

  • This paper examines the safety perception of passengers on low cost carriers which has made several accidents and incidents at the early stage of their business. The concept of low cost carrier is becoming more important to airline industry in Korea. This study achieves the first selecting attribute among airline service factors on domestic flights and try to indicate that how much the passengers trust in the safety of low cost carriers. It is also sets the comparative safety awareness between low cost carriers and two full service national carriers in domestic market. The safety perception of aircraft type can be a major factor for LCC choice in Korea due to frequent accidents and incidents for last two years. More than 60% of LCC passengers have increasingly considered the safety after recent accidents. The low cost airlines' images of the passengers showed that the first image of the LCC was the fare and the second image was safety. This study indicates that most of the LCC travelers will use the international low cost airlines when LCC extend their destination to adjacent countries. For the accomplishment of the low cost carriers' competitiveness in domestic market, they have to try to get rid of bad image for safety and pay extra attention to find out better services other than the safety and develop other strategies to compete against existing full service carriers.

Incorporating ground motion effects into Sasaki and Tamura prediction equations of liquefaction-induced uplift of underground structures

  • In metropolitan areas, the quantity and density of the underground structure increase rapidly in recent years. Even though most damage incidents of the underground structure were minor, there were still few incidents causing a great loss in lives and economy. Therefore, the safety evaluation of the underground structure becomes an important issue in the disaster prevention plan. Liquefaction induced uplift is one important factor damaging the underground structure. In order to perform a preliminary evaluation on the safety of the underground structure, simplified prediction equations were introduced to provide a first order estimation of the liquefaction induced uplift. From previous studies, the input motion is a major factor affecting the magnitude of the uplift. However, effects of the input motion were not studied and included in these equations in an appropriate and rational manner. In this article, a numerical simulation approach (FLAC program with UBCSAND model) is adopted to study effects of the input motion on the uplift. Numerical results show that the uplift and the Arias Intensity (Ia) are closely related. A simple modification procedure to include the input motion effects in the Sasaki and Tamura prediction equation is proposed in this article for engineering practices.

국내 주요 환경보건 재난의 전개 과정과 그 교훈, 그리고 환경보건 전문가의 역할 (Lessons Learned from Major Environmental Health Disasters in South Korea and the Role of Environmental Health Experts)

  • In today's civilization, it can be impossible to prevent disasters that cause large-scale human and material harm, and the environmental industry is not excepted from this. Over the last 50 years, several large and small environmental health catastrophes have occurred in Korea. Notable instances include the phenol pollution accident in the Nakdong River, the Hebei Spirit oil spill in Taean, Chungcheongnam-do, and the humidifier disinfectant disaster. Looking at these instances, it is clear that the government failed to prevent similar incidents and accidents after the tragedies. The government created and executed different policies to prevent such incidents and accidents, but the majority of them were highly fragmented. It is understandable that depending on the political and social level of the society in which the environmental health hazard incident/accident happened, the investigation of the cause, countermeasures, and policy reaction may differ. To put it another way, the more authoritarian and non-democratic a political social system is, the more likely it is to cover up occurrences and accidents without a deep examination. This is in line with the members of society's level of political awareness and acknowledgment of the importance of life and safety. In 1985, when the Onsan pollution disease was discovered, and in 2011, when we recognized the realities of the humidifier disinfectant disaster, South Korea's political and social systems were entirely different.

국내 해양선박사고의 인적 오류의 요인 평가 (Evaluation of Human Factors in Ship Accidents in the Domestic Sea)

  • In this study, we investigated and identified criterial human factors(errors), most of which lead to terrible ship accidents such as collisions, sinking, fire and explosions resulting both in human lives and physical damages to ships as well as surrounding environments. To this end, we went through the accident reports of 413 cases over 2005~2009 period and classified the human factors into 6 major factors with 19 sub ones which were constructed in hierarchical order. The relative importance of major factors was calculated and among others the lack of awareness turned out to be the most important factor with the weight of 0.391. The contributions of the results in the research are two fold: it will help (i) identify the root causes of ship accidents and prevent further potential ship related incidents, (ii) analyze the degree of the risk associated with the ship accidents, when risk analysis is performed.

  • 본 논문에서는 대형 사고나 자연재해 등 외상적 사건 발생 후 초기의 심리적 중재방법 중의 하나로 제공되어온 심리적 경험보고(psychological debriefing)의 대표적 모델들에 대해 소개하고, 그 효과에 대한 논란들에 대해 살펴보았다. 심리적 경험보고의 효과에 대해서는 기존의 경험적 연구들을 바탕으로 일관된 결론을 이끌어 내기가 어렵다. 현재로서는 CISD 등의 기법들에 대해 지적되어온 관련 문제들을 고찰하여 위기 현장에서 심리적 경험보고를 올바르게 사용하기 위한 방법을 모색하는 것이 중요하며, 논의 부분에 이를 제시하였다.

환자안전사고 정보매체가 간호대학생의 환자안전에 대한 지식, 인식, 수행자신감에 미치는 영향 (Influences of Information Media of Patient Safety Incident on Nursing Students' Knowledge, Perception, and Confidence in Performance toward Patient Safety)

  • 본 연구는 간호대학생을 대상으로 정보매체를 통한 환자안전사고 노출 정도와 학생들의 환자안전관련 지식, 인식, 수행자신감의 관계를 알아보기 위해 시행되었다. 연구의 대상자는 일개 여대의 간호학과에 재학 중인 대학생으로 서면동의서를 작성한 학생 348명 중 불충분한 설문지를 제외하고 337부를 자료 분석에 활용하였다. 자료수집은 2018년 6월 4일에서 12일까지 이루어졌으며 수집된 자료는 SPSS 21.0프로그램을 활용하여 기술 통계, t-test, one-way ANOVA, Pearson's correlation coefficient 방법으로 분석되었다. 연구결과 대상자의 환자안전에 대한 지식은 평균 6.43점, 인식은 평균 41.02점, 수행자신감은 평균 39.61점이었으며 TV와 인터넷 매체를 통해 환자안전사고에 노출된 정도는 각 1.25점과 1.35점으로 나타났다. 환자안전 지식은 연령, 학년, 환자안전 교육 경험에 따라 통계적으로 유의한 차이를 보였으며, 환자안전 인식은 전공만족도에 따라, 환자안전 수행자신감은 학년, 환자안전 교육 경험, 전공만족도에 따라 통계적으로 유의한 차이를 보였다. TV를 통한 환자안전사고 정보매체의 노출 정도는 지식(r=.32, p<.000), 수행자신감(r=.21, p<.000)과, 인터넷을 통한 정보매체의 노출 정도는 지식(r=.34, p<.000), 인식(r=.12, p=.028), 수행자신감(r=.24, p<.000)과 유의한 순상관관계를 보였다. 본 연구 결과를 바탕으로 임상실습 전부터 학생들에게 환자안전에 대한 체계적인 교육을 제공할 뿐 아니라 정보매체를 통해 전달되는 환자안전사고와 같은 이슈에 비판적 사고를 가지고 접근할 수 있는 환자안전 교육프로그램을 개발, 적용하고 그 효과를 평가하는 추후 연구를 제언하는 바이다.

Investigating the Determinants of Major IT Incident Tickets: A Case Study of an IT Service Provider Firm for Logistics and Distribution Industry

  • Purpose - This study investigates the determinants that affect the number of IT Incident tickets of an IT Service Provider ("ITSP") to logistics industry in order to improve its management process by reducing the incident tickets. Research design, data, and Methodology - This study uses weekly data of IT incident tickets from September 2012 to June 2015. Correlation and regression analyses are conducted. Six identified determinants i.e., IT Change, User Errors, Shipment Volume, Network, Hardware and Software Issues are used as the explanatory variables. Results - Our findings show as following. First, our analysis indicates that IT Change is not a significant determinant as opposed to what commonly believed by many as the most important factor. Second, Software issue is the highest contributor to the Major IT incident tickets, followed by User Error, Network and Hardware issues. Third, it seems there is lead-lag relationship between IT Change and Major IT Incidents tickets as indicated by earlier studies. Fourth, the relationship between IT Change and Major IT tickets is also affected by shipment volume. Conclusions - As policy recommendation, all identified determinants should be treated according to priority. In addition, improving the way IT Changes are implemented will definitely reduce the IT incident tickets.
