A Study on Process Safety Incident Precursors to Prevent Major Process Safety Incidents in the Yeosu Chemical Complex

  • Process safety incidents and loss events can be prevented if we identify and adequately take measures on process safety incident precursors in a timely manner. If we look into and take action against the process safety hazard factors causing the incident in the refinery and petrochemical plant, major process safety incidents can be prevented through eliminating or decreasing hazardous factors. We conducted a survey for the major process safety incident precursor to look specifically into the potential process safety hazardous factors of refineries and petrochemical plants in the Yeosu chemical complex. A self-assessment checklist, which was published by Center for Chemical Process Safety "Recognizing catastrophic incident warning signs in the process industry" on major incidents warning sign, was used for the survey. Through this survey, the major process safety incident leading indicators in the process industry were found by process safety management elements, and each site and/or facility can use these leading indicators for activities for process safety incident prevention. In addition, we proposed action items required to eliminate the root cause of those process safety incident leading indicators.

  • In terms of deregulation requests from business, incidents at construction sites are rapidly increasing after reducing the regular safety training hours. Especially, to prevent incidents through safety trainings for construction site workers, who have lots of hazardous tasks than general manufacturing, is not only to protect property of both the companies and the country but also to increase their profits. That is why there are loud voices regarding that the safety training is a core mandatory to prevent industrial incidents. But, most companies, as soon as the deregulation for safety training is implemented, mitigated conducting safety trainings to prevent incidents. With this, theincidents are currently to be increased and caught in a vicious circle of many incidents at the smaller companies because of their absentmindedness regarding safety training due to their poorness as subcontractors of major companies, comparing to major companies. This study explains that incident factors are roughly divided into technical factor, managerial factor, and educational factor. And, we confirmed that the educational factor led by deficiency safety awareness and knowledge was the major factor.

  • Marine casualties and environmental pollution have increased recently. Especially, the rate of incident of Coastal tankers is higher, but the assessment tool for safety management system is lack in this fields. However Oil Major Vetting System being professional assessment tool for tanker is widely applied in ocean going tanker by the worldwide Oil Majors. According to the analysis of the marine tankers' incidents which applied Oil Major Vetting System, the incidents were reduced rapidly for recent about 5 years. Using Oil Major Vetting System is helped to improve safety management and to prevent marine incidents. Therefore if applying a parts of the Oil Major Vetting Systems to the coastal tankers' Safety Management System, the Coastal tankers incidents would be reduced and improved gradually.

문제고객에 대한 탐색적 연구 - 서비스 종업원의 관점에서 - (An Exploratory Study for Problem Customers - Service Employees' Perspectives-)

  • This study was interested in dissatisfaction in the service encounter from the service employee's viewpoint by exploring employee dissatisfaction and the problem customers as the causes. The study explored the incidents leading to dissatisfactory service encounters from the service employee's viewpoint, the causes of the dissatisfactory incidents, and the attitudinal, verbal and behavioral expressions of the customers involved in the incidents. Employees of the beauty service business were individually interviewed, and 204 incidents were analyzed. The incidents were categorized to identify the events and related behaviors of customers that caused employee dissatisfaction. Three major groups were classified: the way dissatisfied customers complain; customers' misbehaviors; and customers' no harmful behaviors. Further the major groups were classified into 8 categories. Based on the findings the study provided implications for employee and customer management.

주요 식품안전사건에서 정부와 언론이 사용한 보도용어의 차이가 리스크 커뮤니케이션에 미치는 영향 (The Effect of Difference between Reporting Terms of Government and Media on Risk Communication in Major Food Safety Incidents)

  • 식품안전사건의 보도에 사용되는 용어의 차이가 risk communication에 미치는 영향을 분석하기 위해 18가지 주요 식품안전사건에 대한 자료를 수집하고, 전문가 설문조사를 통해 각 사건의 보도에 사용된 용어의 차이를 비교 분석하였다. 주요 식품안전사건의 자료를 수집하여 분석한 결과, 정부, 지자체, 언론 및 소비자 단체가 사건의 보도에 사용한 용어의 불일치는 사건을 확대시키고, 사회경제적 손실에 큰 영향을 주는 것으로 판단되었다. 전문가 설문조사 결과, 보도 및 대응용어의 파급효과차이 정도와 소비자가 느끼는 두 용어의 차이 정도에서 과자 아토피 유발 사건, 포르말린 통조림 사건, 고름우유 사건, 불량만두 사건 등 높은 순위를 차지한 사건들이 유사하게 나타났으며, 이 둘의 상관관계를 분석한 결과 상관계수가 0.865로 높은 정의 상관관계를 보였다. 따라서 보도에 사용되는 용어에 따라 사건의 파급효과가 크게 영향을 받는 것으로 판단되고, 사건보도 시 용어의 표준화가 필요한 것으로 판단되었다. 본 조사 결과는 정부, 기업, 소비자간 성공적인 risk communication 방안 마련을 위한 기초자료로 사용될 수 있을 것이다.

철도 사고 및 장애의 인적오류 유형 분석 (An Analysis of Human Error Mode and Type in the Railway Accidents and Incidents)

  • Human error is one of the major contributors to the railway accidents or incidents. In order to develop an effective countermeasure to remove or reduce human errors, a systematic analysis should be preferentially performed to identify their causes, characteristics, and types of human error induced in accidents or incidents. This paper introduces a case study for human error analysis of the railway accidents and incidents. For the case study, more than 1,000 domestic railway accidents or incidents that happened during the year of 2004 have been investigated and a detailed error analysis was performed on the selected 90 cases, which were obviously caused by human error. This paper presents a classification structure for human error analysis, and summarizes the analysis results such as causes of the events, error modes and types, related worker, and task type.

독성물질 사용.저장시설에 대한 개인적 위험성 산정에 관한 연구 (A Study on the Individual and Societal Risk Estimation for the Use and Storage Facility with Toxic Materials)

  • These days leakage incidents of toxic materials cause serious effects on the nearby residents as well as the workers around the accidents accompanying massive material losses and human damages through widening influential areas. The risk measure through adequate quantitative analysis as well as the qualitative analysis of the leakage incidents of toxic materials becomes an urgent issue. The damage of the leakage incident on the surrounding area of the dangerous toxic material facilities was calculated quantitatively by adopting several models in this research. First, the calculations of the leakage velocity from the factories were performed by using source model for the assessment of the influential area, and the damages on the nearly residents were calculated by using the dispersion model and the effort model. The probability of the Incidents was computed based on "The manual for classification and priorization of major incidents" published by IAEA( International Atomic Energy Agency ). Above calculated damage area and incident probability were further adopted in this study to induce the individual and societal risk, quantitatively. The calculated data of the real Incident of the toxic material leakage showed reasonable agreements to the actual damage of the incidents, which showed a validity of this study. The result of this study might be a helpful measure for predicting damages and preparing safety systems for similar kinds of incidents.incidents.

  • Maritime transport is dominant in the overall volume of all international transportation. Existence and overcoming of problems, which cause pressure on shipping safety, remain actual and fully concern both maritime and inland transport. Increasing speed and cargo capacity of the ships along with the reduction of crew members lead to the automation of a growing number of work processes, which indicates the need to actively introduce appropriate measures in the security system of sea-going ships and commercial ports and to develop modern approaches to minimize negative events and incidents in the process of ship operation. Advantages in use of modern methods of monitoring the safety of ship operations, management of possible events and incidents, including investigation of accidents, first, aimed at prevention of negative occurrences and ways of prevention on this basis. Considering statistics on incidents increase, this work presents analysis of general ship accident rate, study of major accidental events growth annually, and investigation of causes of incidents, which most frequently occur in port waters and at open sea. A survey of current approaches to ensuring the safety of shipping by implementing effective tools, such as event and incident management, has been conducted.
