Thromboembolic Events after Coil Embolization of Cerebral Aneurysms : Prospective Study with Diffusion-Weighted Magnetic Resonance Imaging Follow-up

  • Objective : In order to assess the incidence of thromboembolic events and their clinical presentations, the present study prospectively examined routine brain magnetic resonance images (MRI) taken within 48 hours after a coil embolization of cerebral aneurysms. Methods : From January 2006 to January 2008, 163 cases of coil embolization of cerebral aneurysm were performed along with routine brain MRI, including diffusion-weighted magnetic resonance (DW-MR) imaging, within 48 hours after the embolization of the aneurysm to detect the silent thromboembolic events regardless of any neurological changes. If any neurological changes were observed, an immediate brain MRI follow-up was performed. High-signal-intensity lesions in the DW-MR images were considered as acute thromboembolic events and the number and locations of the lesions were also recorded. Results : Among the 163 coil embolization cases, 98(60.1%) showed high-signal intensities in the DW-MR imaging follow-up, 66 cases (67.0%) involved the eloquent area and only 6cases (6.0%) showed focal neurological symptoms correlated to the DW-MR findings. The incidence of DW-MR lesions was higher in older patients (${\geq}60$ yrs) when compared to younger patients (<60 yrs) (p=0.002, odd's ratio=1.043). The older patients also showed a higher incidence of abnormal DW-MR signals in aneurysm-unrelated lesions (p=0.0003, odd's ratio=5.078). Conclusion : The incidence of symptomatic thromboembolic attacks after coil embolization of the cerebral aneurysm was found to be lower than that reported in previous studies. While DW-MR imaging revealed a higher number of thromboembolic events, most of these were clinically silent and transient and showed favorable clinical outcomes. However, the incidence of DW-MR abnormalities was higher in older patients, along with unpredictable thromboembolic events on DW-MR images. Thus, in order to provide adequate and timely treatment and to minimize neurological sequelae, a routine DW-MR follow-up after coil embolization of cerebral aneurysms might be helpful, especially in older patients.

Magnetic Resonance Imaging Evaluation of the Prostate in Normal Dogs

  • The aims of this study were to describe the appearance and size of the normal canine prostate using magnetic resonance (MR) imaging and to calculate the apparent diffusion coefficient (ADC) values. MR images were obtained from seven intact male beagle dogs using a 1.5 T MR unit. The sequences included pre- and post-contrast T1- and T2-weighted imaging with and without fat saturation. The signal intensity of the prostate was compared with the adjacent musculature, fat, and urine in the urinary bladder. We recorded the mean prostatic length, width, and height and the length of the sixth lumbar vertebral body (L6). In addition, the prostatic length (rL), width (rW), and height (rH) ratios to L6 were calculated. Diffusion-weighted images of the prostate were obtained and ADC values were calculated. The prostate was bilobed and oval-shaped, homogenous on T1-weighted images, and heterogeneous with radiating lines on T2-weighted images. Post-contrast T1-weighted sequences showed contrast enhancement of the central and radiating striations. The prostatic capsule was clearly identified on post-contrast T1-weighted images with fat saturation. The ADC values were 1.72-2.04 × 10-3mm2/sec (mean, 1.88 × 10-3mm2/sec). Knowledge of the normal appearance of the prostate on MR images is essential to assess prostatic diseases in dogs.

유방에 발생한 일차성 비호지킨림프종의 자기공명 분광법 및 확산강조 영상: 2예 (MR Spectroscopy and Diffusion Weighted Imaging Findings of Primary Non-Hodgkin Lymphoma of the Breast: Two Case Reports)

  • 유방의 일차성 비호지킨림프종은 드문 종양으로 유방촬영술과 초음파에서 다양한 영상소견을 보인다. 하지만 자기공명영상소견은 여러개의 증례로만 보고되어 있고, 자기공명분광법과 확산강조영상에 대한 보고는 거의 없다. 저자들은 유방의 일차성 비호지킨림프종을 진단받은 두명의 여자환자의 증례를 기본 자기공명영상에 더하여 자기공명 분광법 및 확산강조 영상소견과 함께 보고하고자 한다. 종양은 유방촬영술과 초음파에서 경계가 좋은 종괴로 나타났으며, 조영증강자기공명영상에서 강한 조영증강을 보였다. 또한 다른 악성종양에서 나타나는 것과 같이 확산강조영상에서는 확산제한을 보이고, 자기공명분광법에서는 콜린값이 증가하는 소견을 보였다.

Benign versus Malignant Soft-Tissue Tumors: Differentiation with 3T Magnetic Resonance Image Textural Analysis Including Diffusion-Weighted Imaging

  • Purpose: To investigate the value of MR textural analysis, including use of diffusion-weighted imaging (DWI) to differentiate malignant from benign soft-tissue tumors on 3T MRI. Materials and Methods: We enrolled 69 patients (25 men, 44 women, ages 18 to 84 years) with pathologically confirmed soft-tissue tumors (29 benign, 40 malignant) who underwent pre-treatment 3T-MRI. We calculated MR texture, including mean, standard deviation (SD), skewness, kurtosis, mean of positive pixels (MPP), and entropy, according to different spatial-scale factors (SSF, 0, 2, 4, 6) on axial T1- and T2-weighted images (T1WI, T2WI), contrast-enhanced T1WI (CE-T1WI), high b-value DWI (800 sec/mm2), and apparent diffusion coefficient (ADC) map. We used the Mann-Whitney U test, logistic regression, and area under the receiver operating characteristic curve (AUC) for statistical analysis. Results: Malignant soft-tissue tumors had significantly lower mean values of DWI, ADC, T2WI and CE-T1WI, MPP of ADC, and CE-T1WI, but significantly higher kurtosis of DWI, T1WI, and CE-T1WI, and entropy of DWI, ADC, and T2WI than did benign tumors (P < 0.050). In multivariate logistic regression, the mean ADC value (SSF, 6) and kurtosis of CE-T1WI (SSF, 4) were independently associated with malignancy (P ≤ 0.009). A multivariate model of MR features worked well for diagnosis of malignant soft-tissue tumors (AUC, 0.909). Conclusion: Accurate diagnosis could be obtained using MR textural analysis with DWI and CE-T1WI in differentiating benign from malignant soft-tissue tumors.

Hypointensity on Susceptibility-Weighted Images Prior to Signal Change on Diffusion-Weighted Images in a Hyperacute Ischemic Infarction: a Case Study

  • Susceptibility-weighted imaging (SWI) is well known for detecting the presence of hemorrhagic transformation, microbleeds and the susceptibility of vessel signs in acute ischemic stroke. But in some cases, it can provide the tissue perfusion state as well. We describe a case of a patient with hyperacute ischemic infarction that had a slightly hypodense, patchy lesion at the left thalamus on the initial SWI, with a left proximal posterior cerebral artery occlusion on a magnetic resonance (MR) angiography and delayed time-to-peak on an MR perfusion performed two hours after symptom onset. No obvious abnormal signals at any intensity were found on the initial diffusion-weighted imaging (DWI). On a follow-up MR image (MRI), an acute ischemic infarction was seen on DWI, which is the same location as the lesion on SWI. The hypointensity on the initial SWI reflects the susceptibility artifact caused by an increased deoxyhemoglobin in the affected tissue and vessels, which reflects the hypoperfusion state due to decreasing arterial flow. It precedes the signal change on DWI that reflects a cytotoxic edema. This case highlights that, in some hyperacute stages of ischemic stroke, hypointensity on an SWI may be a finding before the hyperintensity is seen on a DWI.

MR Findings of Seizure-Related Cerebral Cortical Lesions during Periictal Period

  • Purpose: This study investigated the MRI, MR angiography (MRA) and MR perfusion findings of seizure-related cerebral cortical lesions during the periictal period. Materials and Methods: From a retrospective review of the institutional database between 2011 and 2014, a total of 21 patients were included in this study. Two radiologists assessed periictal MRI, including MRA and MR perfusion, in patients with seizure-related cortical lesions. The parameters examined include: location of cortical abnormality, multiplicity of the affected cortical region, cerebral vascular dilatation, perfusion abnormality and other parenchymal lesions. Results: All patients showed T2 hyperintense cerebral cortical lesions with accompanying diffusion restriction, either unilateral (18/21, 85.7%) or bilateral (3/21, 14.3%). Of the 21 patients enrolled, 10 (47.6%) had concurrent T2 hyperintense thalamic lesions, and 10 (47.6%) showed hippocampal involvement. Of the 17 patients (81%) who underwent MRA, 13 (76.5%) showed vascular dilatation with increased flow signal in the cerebral arteries of the affected cortical regions. On MR perfusion, all 5 patients showed cortical hyperperfusion, corresponding to the region of cortical abnormalities. Conclusion: Seizure-related cerebral cortical lesions are characterized by T2 and diffusion hyperintensities, with corresponding cerebral hyperperfusion and vascular dilatation. These findings can be helpful for making an accurate diagnosis in patients with seizure.

Fast MRI in Acute Ischemic Stroke: Applications of MRI Acceleration Techniques for MR-Based Comprehensive Stroke Imaging

  • The role of neuroimaging in patients with acute ischemic stroke has been gradually increasing. The ultimate goal of stroke imaging is to make a streamlined imaging workflow for safe and efficient treatment based on optimized patient selection. In the era of multimodal comprehensive imaging in strokes, imaging based on computed tomography (CT) has been preferred for use in acute ischemic stroke, because, despite the unique strengths of magnetic resonance imaging (MRI), MRI has a longer scan duration than does CT-based imaging. However, recent improvements, such as multicoil technology and novel MRI acceleration techniques, including parallel imaging, simultaneous multi-section imaging, and compressed sensing, highlight the potential of comprehensive MR-based imaging for strokes. In this review, we discuss the role of stroke imaging in acute ischemic stroke management, as well as the strengths and limitations of MR-based imaging. Given these concepts, we review the current MR acceleration techniques that could be applied to stroke imaging and provide an overview of the previous research on each essential sequence: diffusion-weighted imaging, gradient-echo, fluid-attenuated inversion recovery, contrast-enhanced MR angiography, and MR perfusion imaging.

초급성 뇌경색을 일으킨 개에서 Gd-조영제의 주입이 뇌의 확산에 미치는 영향 (Effect of Gd-DTPA on Diffusion in Canine Brain with Hyperacute Stroke)

  • 목적: 체내에 주입된 Gd-DTPA가 뇌의 확산강조 자기공명영상 신호강도 및 현성확산계수에 미치는 영향을 알아보았다. 대상 및 방법 : 성숙한 잡견 5마리에 대하여 동맥내 도관삽입에 의한 좌측 내경동맥 색전방법을 이용하여 초급성 뇌경색 동물모델을 만들었다. 색전 후 1시간째 확산강조영상을 시행하고, Gd-DTPA를 주입한 다음 다시 90분까지 11회의 추가 확산강조영상을 얻었다. 관심영역을 설정하여 측정한 초급성 뇌경색부위와 반대측 정상부위의 확산강조영상 신호강도 및 현성확산계수를 분석하였다. 결과: 뇌경색은 색전 후 1시간에 시행한 확산강조 자기공명영상에서 잡견 5마리 모두에서 발견되었다. 확산강조영상에서 초급성 뇌경색부위의 신호강도는 Gd-DTPA 주입 여부와 관계없이 시간이 경과함에 따라 증가하였으나, 관류가 유지된 정상부위의 신호강 도는 Gd-DTPA 주입 후 2분에 시행한 첫 검사에서 오히려 저하된 후, 시간경과에 따라 다시 증가하였다. 현성확산계수는 초급성 뇌경색부위에서 Gd-DTPA주입여부에 관계없이 시간 이 경과함에 따라 지속적으로 감소되었으나, 관류가 유지된 반대측 정상부위에서는 변화하지 않았다. 결론: 체내에 주입된 Gd-DTPA는 초급성 뇌경색부위 및 정상부위의 현성확산계수에 영향을 미치지 않으나, 정상부위에서는 조영제 주입 직후 초기의 자화율효과에 의해 확산강조영상의 신호 강도를 저하시켰다. 조영제 주입 후 시행한 확산강조영상 신호 강도의 정량적인 측정이 필요한 연구 혹은 임상 증례에 대하여는 현성확산계수를 측정함으로써 Gd-DTPA의 자화율효과에 의한 영향을 배제하여야 할 것이다.

  • 지난 10년간 신경계의 MR영상은 컴퓨터를 비롯한 하드웨어적인 발달에 힘입어 functional imaging, perfusion imaging, diffusion imaging, MR spectroscopy, MR angiography 등의 분야에서 괄목한 만한 성장이 있었다. 지난 2001년 4월 국제자기공명의과학회 (ISMRM) 학술대회에 약 2300개의 논문이 발표되었으며 이 중 신경계와 관련된 논문의 수는 저자가 파악하기에 약 500개 정도이었다. 매년 수많은 논문이 발표되고 있으나 이 중 기초 또는 임상의학적으로 유용한 논문만이 후속 연구로 연결되어 궁극적으로 많은 사람들이 활용할 수 있게 되고 있다. 따라서 지난 수 년 동안의 연구 동향을 모두 follow-up하는 것은 무의미하며, 이 중 functional, perfusion, diffusion imaging을 중심으로 이들 영상 기법의 기술적 측면과 적용 분야를 알아보고자 한다.

The Emerging Role of Fast MR Techniques in Traumatic Brain Injury

  • Post-concussion syndrome (PCS) following mild traumatic brain injury (mTBI) is a major factor that contributes to the increased socioeconomic burden caused by TBI. Myelin loss has been implicated in the development of PCS following mTBI. Diffusion tensor imaging (DTI), a traditional imaging modality for the evaluation of axonal and myelin integrity in mTBI, has intrinsic limitations, including its lack of specificity and its time-consuming and labor-intensive post-processing analysis. More recently, various fast MR techniques based on multicomponent relaxometry (MCR), including QRAPMASTER, mcDESPOT, and MDME sequences, have been developed. These MCR-based sequences can provide myelin water fraction/myelin volume fraction, a quantitative parameter more specific to myelin, which might serve as a surrogate marker of myelin volume, in a clinically feasible time. In this review, we summarize the clinical application of the MCR-based fast MR techniques in mTBI patients.
