Comparison of Physical Activity and Sedentary Life on Health in Obesity Men and Women (신체활동과 좌식생활에 따른 비만남성과 여성의 건강상태의 차이 비교)

  • The purpose of this study was to investigate effects of physical activity and sedentary life in metabolically healthy obesity(MHO) and metabolically unhealthy obesity(MUO). A total of 1585 middle-aged obesity men and women participated in this study. The subjects were divided to MHO(n=840) and MUO(n=745) by metabolic syndrome markers(National Cholesterol Education Program-Adult Treatment Panel III). We analyzed measurement data using t-test. As results, physical activity and exercise participant between group had not significant difference in men subjects. But, in women subjects, total physical activity volume and muscle strength exercise participant between group had a significant difference. In conclusion, we suggest that physical activity and exercise participant are important factors affecting health status in the same level of obesity women.

Heart Rate Recovery in Metabolically Healthy Obesity and Metabolically Unhealthy Obesity Korean Adults

  • Heart rate recovery (HRR) is simply an indicator of autonomic balance and is a useful physiological indicator to predict cardiovascular morbidity and mortality. The purpose of this study was to compare the differences in HRR between metabolically healthy obesity group and metabolically unhealthy obesity and to ascertain whether heart rate recovery is a predictor of metabolic syndrome. Metabolic syndrome was defined according to the standards of the National Cholesterol Education Program Adult Care Panel III. Obesity was assessed according to WHO Asian criteria. It was classified into three groups of metabolically healthy non-obesity group (MHNO, n=113), metabolically healthy obesity group (MHO, n=66), metabolically unhealthy obesity (MUO, n=18). Exercise test was performed with Bruce protocol using a treadmill instrument. There was no difference in HRR between MHO and MUO ($32.71{\pm}12.25$ vs $25.53{\pm}8.13$), but there was late HRR in MUO than MHNO ($25.53{\pm}8.13$ vs $34.51{\pm}11.80$). HRR in obese was significantly correlated with BMI (r=-0.342, P=0.004), waist circumference (r=-0.246, P=0.043), triglyceride (r=-0.350, P=0.003), HbA1c (r=-0.315, P=0.009), insulin (r=-0.290, P=0.017) and uric acid (r=-0.303, P=0.012). HRR showed a lower prevalence of abdominal obesity, hypertriglyceridemia, and low HDL-cholesterol in the third tertile than in the first tertile. In conclusion, MHO had no difference in vagal activity compared with MHNO, but MUO had low vagal activity. HRR is associated with metabolic parameters and is a useful predictor of abdominal obesity, hypertriglyceridemia, and low HDL-cholesterolemia.

  • Ipsilateral tonsillectomy and panendoscopy-guided biopsy following imaging studies are considered a standard procedure in the search for a primary origin in patients with cervical metastatic carcinoma of unknown origin(MUO). However, many authors recommended bilateral tonsillectomy for the determination of the primary site of a MUO, because cervical metastasis may occur contralateral to tonsillar carcinoma. The authors attempted to address the clinical implications of using routine bilateral tonsillectomy to determine the primary site of MUOs based on a case report of cervical MUO that was finally diagnosed as a bilateral synchronous tonsillar carcinoma with cervical metastasis after a diagnostic work-up that included bilateral tonsillectomy.

  • Diagnosis of primary origin site in the management of malignancy of unknown origin (MUO) is the most important issue. According to the histopathologic subtype of primary lesion, specialized treatment can be given and survival gain is expected. F-18 FOG PET (PET/CT) has been estimated as useful in detection of primary lesion with high sensitivity and moderate specificity. F-18 FDG PET (PET/CT) study before conventional studies is also recommended because it has high diagnostic performance compared to conventional studies. Although there has few data, F-18 FDG PET (PET/CT) is expected to be useful in diagnosis of recurrence, restaging, evaluation of treatment effect, considering that PET (PET/CT) has been reported as useful in other malignancies.

Effects of Health Behavior Factors and Mental Health Factors in Korean Obese Adults on Their Metabolic State: Utilizing the Korea National Health and Nutrition Examination Survey Data

  • This is a descriptive research study that classified Korean adults with obesity into those with Metabolically Healthy Obesity and those with Metabolically Unhealthy Obesity based on the data from the fifth and sixth South Korea's National Health and Nutrition Examination Surveys, designed due to the development of information and communication technology, to examine the impacts of obese adults' health behavior factors and mental health factors on their metabolic state. With respect to data analysis, the collected data were analyzed by complex sample statistics. The results of this study can be summarized as follows: Men who were smoking at the time of the survey had a 1.29 times higher probability of inclusion in the MUO group than in the MHO group. Women who had a high stress cognition rate had a 1.02 times higher probability of inclusion in the MUO group than in the MHO group. This study is significant as it provides the basic data for establishing strategies of nursing intervention for the promotion of obese adults' health, and it suggests that it is necessary to develop a program for the promotion of obese adults' health based on these results.

A Study on the Planting Records of Needle Fir in Gwangneung (광릉 전나무 식재기록에 관한 고찰)

  • The study was done to analyze the records of planting time of Gwangneung needle firs referring to the historical literatures, the Annals of Joseon Dynasty(朝鮮王朝實錄), Seungjeongwonilgi(承政院日記: Journal of the Royal Secretariat). Bibyeonsadeungnok(備邊司謄錄: Records of the Bibyeonsa Defense Council, a government agency during the Joseon Dynasty), Ilseongnok(日省錄: Journal on Dynasty affairs of the kings in the 36th year of Youngjo to 1910) to lay the historical ground for needle firs in Gwangneung. The following results were derived from previous research, Report on Gwangneung Forest Ecosystem, and overlaid fir tree rings. The research findings are as follow. First, since the system of making the grave is the most conservative system, the planting during the Gwangneung construction would have been preceded by precedent. Second, the problem of cutting the tree in the late Joseon Dynasty became serious. It is published the Law of Muo In the 22nd year of Jeongjo, and then planted trees regularly in spring and autumn. Third, the Law of Muo was preserved for 31 years during the reign of King Gojong, and the type of specific tree, the number of trees, from the 22nd year until the 25th year of King Gojong, four years of fir trees were planted in Gwangneung, and Gwangneung was the only place where korean pine trees were planted. Fourth, it was possible to identify the age of the tree through fallen fir tree rings, and the period of planting fir trees growing in the existing mausoleum is judged to be the equivalent year of King Gojong's reign from the 10th year of King Cheoljong's reign.

  • The purpose of this study was to investigate difference of Physical Activity and Sedentary Life according to Health and Obesity status. A total of 4,783 adults participated in this study. The subjects were divided to four group(metabolically healthy normal weight; MHNW, metabolically unhealthy normal weight; MUNW, metabolically healthy obesity; MHO, metabolically unhealthy obesity; MUO) and analyzed clinical character, physical activity, and sedentary life time. As results, Compared with MHNW, MHO was higher physical activity and sedentary time but there was no notable differences. Compared with MHNW, MUNW was higher physical activity and lower sedentary time but there was no notable differences. Compared with MUNW, MHO was lower physical activity and sedentary time but there was no notable differences. In conclusion, we suggest that MHO has high physical activity comparing nomal weight group relatively.

Mechanochemistry on Self-Assembled Monolayer(SAM) /Electrodes after Contacting with Polymeric Stamp (고분자물질과 접촉한 자기조립단분자막 전극 물질의 기계화학 현상 분광학적 연구)

  • We investigated mechanochemical radical, which is concomitant with chemical lift-off lithography(CLL), on the self-assembled monolayer(SAM)/electrodes and a polydimethylsiloxane(PDMS) using a colorimetric and a spectroscopic method. The 11-mercaptoundecanol(MUO)/Au or the 11-hydroxyundecylphosphonic acid (HUPA)/ITO were contacted with bare or activated PDMS. After contact, the each of SAM/substrates and the PDMS were immersed in a 2,2 Diphenyl-1-picrylhydrazyl(DPPH) radical scavenger. The color of the DPPH exposed to the PDMS was changed from purple to yellow and the absorbance decreased definitely at 515 nm wavelength. The SAM/substrates, however, have caused small changes in spectroscopic property, indicating no existence of radical species. We concluded that mechanochemical radicals were formed by homolytic cleavage of PDMS molecules upon external force and hardly transferred on the SAM/substrates.

Fenestrated popliteal vein pierced by a branch of the tibial nerve

  • Knowledge of anatomical variations is important so as to avoid potential iatrogenic injury or misdiagnosis on imaging. Here we report an unusual finding and relationship between the tibial nerve and popliteal vein. During the routine dissection of an adult cadaver, it was noted that a branch of the tibial nerve in the popliteal fossa pierced the most distal part of the popliteal vein. This unusual finding is described and relevant reports in the literature discussed. Our hopes are that such a report might help surgeons avoid injury to such a fenestrated popliteal vein and the tibial nerve branch traveling through it therefore decreasing patient morbidity.

Treatment Outcome of Metastatic Carcinoma of Cervical Lymph Node from an Unknown Primary (원발병소 블명의 경부림프절 전이의 치료결과)

  • Purpose: To analyze the outcome of radiation therapy for patients with a metastatic carcinoma of cervical lymph nodes from an unknown primary (MUO), and identify the prognostic factors for these patients. Materials and Methods: Between July 1981 and June 1999, 39 patients with MUO underwent radiation therapy with curative intent. Twelve patients were treated with radiation therapy alone (Group 1), 8 with neoadjuvant chemotherapy followed by radiation therapy (Group 2), and 19 with either an excision or neck dissection and postoperative radiation therapy (Group 3). There were 31 males and 8 females, with a median age of 55 years, ranging from 25 to 77 ears. The median duration of follow-up was 38 months, ranging from 3 to 249 months. Results: The 5-year overall survival rate was $55\%$. According to the treatment modality, the 5-year disease-free survival rates of Groups 1, 2 and 3 were 48, 19 and $75\%$, respectively (p=0.0324). In addition to the treatment modality, the appearance of the primary site was a significant prognostic factor for disease-free survival (p=0.0085). Conclusion: Surgical resection and radiation therapy achieves a superior disease-free survival compared to radiation therapy alone, either with or without chemotherapy Further investigation Is needed to evaluate the role of chemotherapy in the treatment of MUO.
