MRI of Hydrosyringomyelia Combined to Hydrocephalus and Occipital Dysplasia in a Dog (개에서 뇌수두증과 후두골 이형성증을 동반한 척수공동증의 자기공명영상학적 평가 1례)

  • Hydrosyringomyelia is a dilation of the spinal cord central canal. In human it may be caused by congenital malformations such as Dandy-Walker syndrome and Chiari malformations or may be acquired as a result of infection, trauma or neoplasia. Hydrocephalus is an excessive accumulation of cerebrospinal fluid within the ventricles and occipital dysplasia is the dorsal extension of the foramen magnum. Hydrosyringomyelia and hydrocephalus can be confirmed by computed tomography or magnetic resonance imaging (MRI). A 3-year-old male maltese was presented with a history of long-term seizure. Blood examination was all unremarkable. On rostrodorsal-caudoventral oblique radiograph of the skull showed severe occipital dysplasia. On brain sonography through the persistent fontanelle, severe lateral ventriculomegaly was revealed. MRI examination revealed hydrocephalus and hydrosyringomyelia. Diuretic therapy didn't reduce clinical symptoms and surgical decompression was conducted. The dog responded well with ventriculo-peritoneal shunting. MRI is the most superior modality to diagnose hydrocephalus and hydrosyringomyelia, to plan therapy and to determine the prognosis.

Evaluation of Computed Tomography and Magnetic Resonance Imaging of Sinonasal Inverted Papilloma (비부비동 반전성 유두종의 전산화 단층촬영상과 자기공명영상의 분석)

  • Background: Computed tomography (CT) is commonly used to evaluate the degree of sinus involvement in cases of inverted papilloma (IP). However, CT cannot differentiate tumor from adjacent inflammatory mucosa or retained secretions. By contrast, magnetic resonance imaging (MRI) has been reported to be useful in distinguishing IP from paranasal sinusitis. This study investigated whether preoperative assessment with MRI and CT accurately predict the extent of IP.1) Materials and methods: CT and MRI were retrospectively reviewed in 9 cases of IP. Patients were categorized into stages based on CT and MRI findings, according to the staging system proposed by Krouse. The involvement of IP in each sinus was also assessed. Results: Differentiation of IP from inflammatory disease may be more successful in routine cases where the inflammatory mucosa has low signal intensity on T1-weighted images and very high signal intensity on T2-weighted images. CT imaging could not differentiate tumor from adjacent inflammatory mucosa or retained secretions. Conclusion: Preoperative MRI of IP can predict the location and extent of the tumor involvement in the paranasal sinuses and sometimes predicts malignant changes.

Standards for Applying Reasonable Receive Bandwidth to Suppress Metal Artifacts in MRI (MRI 검사 시 금속 인공물 억제를 위한 합리적인 수신대역폭 적용 기준)

  • This study aimed to present reasonable reception bandwidth application standards for the purpose of suppressing metal objects during MRI examinations. For this purpose, T2 contrast images were acquired using high-speed spin echo technology on a phantom made of screws for spinal surgery, and metal objects were detected. In addition, images were obtained by increasing the reception bandwidth from 100 Hz/PX to 800 Hz/PX by 100 Hz/PX. The metal artifacts were determined as the sum of the areas of the signal attenuation area and the signal accumulation area. In addition, Pearson correlation analysis was performed to analyze the pattern of metal artifacts according to imaging variables. As a result, the signal accumulation area did not change significantly as the reception bandwidth increased (p>0.05), but the signal loss area and the area of metal artifacts decreased as the reception bandwidth increased (p<0.05). Interestingly, the area of metal objects decreased to a maximum in the section where the reception bandwidth was increased from 100 Hz/PX to 200 Hz/PX, consistent with the section where the echo spacing was reduced to a maximum due to the increase in reception bandwidth. In addition, the correlation analysis results also showed that the eco spacing was more related to the signal attenuation area and the area of metal objects than to the reception bandwidth. Therefore, if the reception bandwidth is increased for the purpose of reducing metal objects, it is reasonable to set it based on a value that minimizes the echo spacing in consideration of image quality factors.

Platysma Infiltration on CT or MRI in Parotid Pathology (이하선 병변에서 활경근 침윤의 빈도와 조건)

  • Purpose To evaluate the incidence and condition of platysma infiltration in benign tumors, malignant tumors, and inflammatory disease in the parotid gland using CT or MRI. Materials and Methods Patients with benign tumors (n = 314), malignant tumors (n = 52), and inflammatory disease (n = 22) in the parotid gland were included. The incidence of platysma infiltration and the relationship between platysma infiltration and the location, capsular involvement, and focality of the mass were retrospectively evaluated using CT or MRI. Results The incidence of platysma infiltration was 0% in benign tumors,19.2% in malignant tumors (10/52), and 50.0% in inflammation (11/22). Platysma infiltration was positive in 10 of 13 patients with inflammatory lesion with capsular involvement. Platysmal infiltrations in inflammatory lesion showed diffuse lesion in 10 patients and focal lesion in one patient. Malignant tumor with platysmal infiltration showed all capsular involvement, and diffuse lesion in seven and focal lesion in three. Conclusion Platysma infiltration was more common in patients with inflammatory disease than those with malignant tumors or benign tumors. In inflammatory disease, platysma infiltration was more common in patients with capsular involvement or diffuse lesion.

  • 목적: MR 영상에 나타나는 bias field, 즉 영상의 특정 부분이 주위보다 어둡거나 밝게 나타나는 현상을 보다 균일하게 보정시키는 방법으로 제시된 N3 방법에서 Gaussian kernel을 사용한 smoothing 방법 대신에 Wavelet(Daubechies, D4)함수를 smoothing기법으로 사용했을 때 어느 정도 균일함에 향상이 일어나는지를 알아보는 것이다.

  • MRI에서 펄스 시퀀스(pulse sequence)란 고주파 RF(radiofrequency) 펄스 및 경사자장(gradient) 펄스를 가하고 MR 신호를 획득하는 순서를 시간대별로 도식화 한 pulse diagram을 이야기한다. 이러한 pulse sequence는 실제로 영상을 획득하기 위한 RF amplifier, gradient amplifier 등의 하드웨어를 순차적(sequential)으로 구동하는 역할을 한다. 따라서 이러한 pulse sequence는 현재 임상적으로 사용되는 다양한 영상기법들을 이해하는데 필수적이다.

Usefulness of Reverse Flow Prevention Valves for Back-flow Prevention of Gadolinium Based Contrast Agent in Dynamic Examination of Magnetic Resonance Imaging (자기공명영상 동적검사 시 조영제 역류방지용 밸브의 유용성)

  • The purpose of this study is to seek an alternative method to prevent the contamination of normal saline caused by the back-flow of Gadolinium Based Contrast Agent(GBCA) by the use of auto-injector for dynamic MRI. The research method is to manufacture the non-return valve as an alternative to the existing, and to examine the usefulness of the normal saline by dividing the state of normal saline into three groups. The signal intensity were compared. As a result, there was no statistically significant difference between normal saline before injection of group 1 and group 3 non-return valve (p> 0.05). It is analyzed that the self-produced non-return valve completely blocked the GBCA back to normal saline when the GBCA was injected. In conclusion, the application of the non-return valve presented in this study for dynamic MRI imaging using the auto injector can prevent normal saline contamination due to GBCA back-flow.

Neural correlates of the aesthetic experience using the fractal images : an fMRI study (프랙탈 이미지를 이용하여 본 미적 경험의 뇌 활성화: 기능적 자기공명영상 연구)

  • The current study examined brain regions associated with aesthetic experience to fractal images using functional MRI. The aesthetic estimations of the images showed that there is a general consensus regarding the perception of beautiful images. Out of 270 fractal images, fifty images rated highest(beautiful images) and fifty images rated lowest(non-beautiful images) were selected and presented to the participants. The two conditions were presented using the block design. Frontal lobes, cingulate gyri, and insula, the areas related to the cognitive and emotional processing in aesthetic experience, were activated when beautiful images were presented. In contrast, the middle occipital gyri and precuneus, the areas associated with experience of negative emotions, were activated when non-beautiful images were presented. The conjunction analysis showed activations in temporal areas in response to beautiful images and activations in parietal areas in response to non-beautiful images. These results indicate that beautiful images elicit semantic interpretations whereas non-beautiful images facilitate abstract processes.
