중년여성에서 근막이완요법이 신경전도와 통증에 미치는 영향 (Effects of Myofascial Release on Nerve Conduction Studies and Pain Scale in Middle-Aged Women)

  • 본 연구는 근막이완요법의 효과를 보기 위하여 중년여성에서 근막요법 전후의 피로와 신경전도에 미치는 영향을 관찰하였다. 28명의 중년여성을 대상으로 오후 6시 이후에 2일 간격으로 총 3회(1, 3, 5 일) 근막이완요법을 실시하였으며, 근막이완요법 전 후의 통증 정도 변화를 설문 조사하였으며, 통각계를 이용하여 승모근에서 압력통각역치와 시각적 통증강도를 측정하였다. 정중신경에서 운동신경전도와 감각신경전도 검사를 실시하여 잠복기, 진폭, 신경전도속도를 측정하였다. 설문조사결과 피로를 많이 느끼는 시간은 18~21시였으며, 피로와 통증을 가장 많이 느끼는 부위는 어깨부위로 조사되었다. 근막이완요법 후에 통증의 정도, 압력통각역치, 시각적 통증강도는 근막이완요법전보다 유의적으로 감소하였다. 근막이완요법전보다 근막이완요법이후 운동신경의 잠복기는 유의적으로 감소되었고 진폭은 유의적으로 증가하였으며 감각신경의 잠복기는 유의적으로 감소되었다. 이와 같은 결과 근막이완요법은 중년여성에서 통증 개선을 위한 대체요법으로 사용될 수 있을 것으로 사료된다.

요추 신경근 평가를 위한 1.5T MR의 단일 방향 경사자장을 사용한 확산강조 자기공명신경조영 (Diffusion-Weighted MR Neurography with Unidirectional Motion-Probing Gradient to Evaluate Lumbar Nerve Roots at 1.5T MR)

  • 목적 최근 확산강조 자기공명신경조영(diffusion-weighted MR neurography; 이하 DW MRN)이 신경근 평가에 도움이 된다고 보고되었다. 본 연구는 1.5T MR에서 단일방향 경사자장을 사용한 DW MRN의 요추 신경근 평가의 유용성을 확인하고자 한다. 대상과 방법 앞뒤 방향 경사자장의 DW MRN을 포함한 64요추 MR을 후향적으로 분석했다. 제3 요추에서 제1 천추까지 총 512개 요추 신경근의 변화를 T2 강조영상, 조영증강 T1 강조영상, 그리고 DW MRN에서 평가하고 일치도와 상관관계 분석을 했다. 결과 T2 강조영상에서 78개의 신경근 압박이 있었고, 조영증강 T1 강조영상에서 52개 신경근이 조영증강되었다. DW MRN에서 67개 신경근의 부종과 고신호강도가 있었다. 조영증강 T1 강조영상과 DW MRN 모두 신경근의 변화가 나타난 경우는 42개였다. DW MRN과 조영증강 T1 강조영상, T2 강조영상 간에 중간 또는 상당한 일치도와 양의 상관관계를 보였다(κ = 0.59-0.65, ρ = 0.600-0.653). 결론 앞뒤 단일방향을 사용한 DW MRN은 척추 신경근의 변화 평가에 도움이 되며, 가돌리늄 조영증강을 대체 또는 보완하는 역할을 할 수 있을 것이다.

전이성 척추 악성 종양의 진단 : 골스캔과 자기공명영상의 비교 (Detection of Spinal Metastases: Comparison of Bone Scan and MR Imaging)

  • 악성 종양의 척추 전이를 진단하는 데 bone scan과 MRI의 소견을 비교하여 보았다. 20명의 악성 종양환자를 대상으로 하였으며 환자의 평균 연령은 60.1 세였고 두 검사의 시행 간격은 평균 16.6일 이었다. 원발암은 폐암이 8명으로 가장 많았고 대장암, 위암, 유방암이 각각 2명, 전립선암, 갑상선암, 악성 임파종이 각각 1명이었으며 3명은 원발 장소를 모르는 전이성 선암 환자였다. 방법으로 척추를 7개의 구역 즉 경추(Cl-C7), 상부 흉추(D1-D4), 중부 흉추(D5-D8), 하부 흉추(D9-D12), 상부 요추(L1, 2), 하부 요추(L3-5) 및 천골로 나누어 총 105개 구역을 서로 비교하였다. 총 105개 구역 중 46개 구역에서 bone scan이나 MRI에서 양성으로 나왔는데 bone scan에서는 30개 구역(65.2%), MRI에서는 44개 구역(95.7%)에서 양성으로 나와 MRI 민감도가 bone scan 보다 높았다. 비교한 105개 구역 중 87개 구역(82.9%)에서 서로 일치하는 결과를 보인 반면 18개 구역(17.1%)에서 두 검사 결과가 서로 일치하지 않았는데 2개 구역에서는 bone scan에서 양성이나 MRI 검사에는 정상인 경우이고 16개 구역에서는 MRI 검사 결과 양성치나 bone scan에서 음성인 경우였다. 일치하지 않는 구역은 경추 구역이 가장 많았으며 진단별로 보면 위암의 경우에 가장 많았다. 한편 bone scan에서 12명의 환자에서 척추 이외의 다른 골격계에 전이 소견을 보였으며 2명의 환자 3개 구역이 양성으로 나왔으나 이 부위가 MRI 검사에 포함되지 않아 비교에서 제외되었다. 악성 종양의 척추전이를 진단하는데 MRI가 bone scan보다 민감하였지만 bone scan은 전골격계에 대하여 손쉽게 전이 여부를 평가할 수 있으므로 일차적 선별 검사로 매우 유용하다. 그리고 bone scan의 결과와 임상 소견이 일치하지 않을 때 MRI를 선택적으로 시행하면 진단의 정확성을 더욱 높일 수 있다.

Spine Computed Tomography to Magnetic Resonance Image Synthesis Using Generative Adversarial Networks : A Preliminary Study

  • Objective : To generate synthetic spine magnetic resonance (MR) images from spine computed tomography (CT) using generative adversarial networks (GANs), as well as to determine the similarities between synthesized and real MR images. Methods : GANs were trained to transform spine CT image slices into spine magnetic resonance T2 weighted (MRT2) axial image slices by combining adversarial loss and voxel-wise loss. Experiments were performed using 280 pairs of lumbar spine CT scans and MRT2 images. The MRT2 images were then synthesized from 15 other spine CT scans. To evaluate whether the synthetic MR images were realistic, two radiologists, two spine surgeons, and two residents blindly classified the real and synthetic MRT2 images. Two experienced radiologists then evaluated the similarities between subdivisions of the real and synthetic MRT2 images. Quantitative analysis of the synthetic MRT2 images was performed using the mean absolute error (MAE) and peak signal-to-noise ratio (PSNR). Results : The mean overall similarity of the synthetic MRT2 images evaluated by radiologists was 80.2%. In the blind classification of the real MRT2 images, the failure rate ranged from 0% to 40%. The MAE value of each image ranged from 13.75 to 34.24 pixels (mean, 21.19 pixels), and the PSNR of each image ranged from 61.96 to 68.16 dB (mean, 64.92 dB). Conclusion : This was the first study to apply GANs to synthesize spine MR images from CT images. Despite the small dataset of 280 pairs, the synthetic MR images were relatively well implemented. Synthesis of medical images using GANs is a new paradigm of artificial intelligence application in medical imaging. We expect that synthesis of MR images from spine CT images using GANs will improve the diagnostic usefulness of CT. To better inform the clinical applications of this technique, further studies are needed involving a large dataset, a variety of pathologies, and other MR sequence of the lumbar spine.

족근 관절 연부조직 충돌 증후군에서 MRI의 진단적 의의 및 관절경적 치료 결과 (Diagnostic efficacy of specialized MRI & clinical results of arthroscopic treatment in ankle soft tissue impingement syndrome)

  • Introduction: Soft-tissue impingement syndrome is now increasingly recognized as a significant cause of the chronic ankle pain. As a method to detect soft-tissue ankle impingement, a characteristic history and physical examination, routine MR imaging, and direct MR arthrography were used. The efficacy of routine MR imaging has been controversial for usefulness because of low sensitivity and specificity. Direct MR artrhography was recommaned for diagnosis because of the highest sensitivity, specificity and accuracy, but it requires an invasive procedure. The purpose of this study is to investigate the diagnostic accuracy of Fat suppressed, contrast enhanced, three-dimensional fast gradient recalled acquisition in the steady state with rediofrequency spoiling magnetic resonance imaging(CE 3D-FSPGR MRI) and to evaluate the clinical outcome of the arthroscopic treatment in assessing soft-tissue impingement associated with trauma of the ankle. Materials and Methods: We reviewed 38 patients who had arthroscopic evaluations and preoperative magnetic resonance imaging studies(3D-FSPGR MRI) for post-traumatic chronic ankle pain between January 2000 and August 2002. Among them, 24 patients had osteochondral lesion, lateral instability, loose body, malunion of lateral malleoli, and peroneal tendon dislocation. The patient group consisted of 23 men and 15 women with the average age of 34 years(16-81 years). The mean time interval from the initial trauma to the operation was 15.5 months(3 to 40 months), The mean follow-up duration of the assessment was 15.6months(12-48 months). MRI was simultaneously reviewed by two radiologists blinded to the clinical diagnosis. The sensitivity, specificity and accuracy of MRI was obtained from radiologic and arthroscopic finding. Arthroscopic debridement and additional operation for associated disease were performed. We used a standard protocol to evaluate patients before the operation and at follow-up which includes American Orthopedic Foot and Ankle Society Ankle-Hindfoot Score. Results: For the assessment of the synovitis and soft tissue impingement, fat suppressed CE 3D-FSPGR MR imaging had the sensitivity of 91.9%, the specificity of 84.4 and the accuracy of 87.5%. AOFAS Ankle-Hindfoot Score of preoperative state was 69.2, and the mean score of the last follow-up was 89.1. These were assessed as having 50% excellent(90-100) and 50% good(75-89). The presence of other associated disease didn't show the statistically significant difference(>0.05). Conclusion: Fat suppressed CE 3D-FSPGR MR imaging is useful method comparable to MR arthrography for diagnosis of synovitis or soft-tissue impingement, and arthroscopic debridement results in good clinical outcome.

Enhancement of Return Routability Mechanism for Optimized-NEMO Using Correspondent Firewall

  • Network Mobility (NEMO) handles mobility of multiple nodes in an aggregate manner as a mobile network. The standard NEMO suffers from a number of limitations, such as inefficient routing and increased handoff latency. Most previous studies attempting to solve such problems have imposed an extra signaling load and/or modified the functionalities of the main entities. In this paper, we propose a more secure and lightweight route optimization (RO) mechanism based on exploiting the firewall in performing the RO services on behalf of the correspondent nodes (CNs). The proposed mechanism provides secure communications by making an authorized decision about the mobile router (MR) home of address, MR care of address, and the complete mobile network prefixes underneath the MR. In addition, it reduces the total signaling required for NEMO handoffs, especially when the number of mobile network nodes and/or CNs is increased. Moreover, our proposed mechanism can be easily deployed without modifying the mobility protocol stack of CNs. A thorough analytical model and network simulator (Ns-2) are used for evaluating the performance of the proposed mechanism compared with NEMO basic support protocol and state-of-the-art RO schemes. Numerical and simulation results demonstrate that our proposed mechanism outperforms other RO schemes in terms of handoff latency and total signaling load on wired and wireless links.

Operation of battery-less and wireless sensor using magnetic resonance based wireless power transfer through concrete

  • Although the deployment of wireless sensors for structural sensing and monitoring is becoming popular, supplying power to these sensors remains as a daunting task. To address this issue, there have been large volume of ongoing energy harvesting studies that aimed to find a way to scavenge energy from surrounding ambient energy sources such as vibration, light and heat. In this study, a magnetic resonance based wireless power transfer (MR-WPT) system is proposed so that sensors inside a concrete structure can be wirelessly powered by an external power source. MR-WPT system offers need-based active power transfer using an external power source, and allows wireless power transfer through 300-mm thick reinforced concrete with 21.34% and 17.29% transfer efficiency at distances of 450 mm and 500 mm, respectively. Because enough power to operate a typical wireless sensor can be instantaneously transferred using the proposed MR-WPT system, no additional energy storage devices such as rechargeable batteries or supercapacitors are required inside the wireless sensor, extending the expected life-span of the sensor.

어지와 지능지수에 대한 한약치료의 전망 (Prospect of Treatment with Herb Medicine for Developmental Delay of Language and Intelligence Quotient)

  • It is widely assumed that Intelligence Quotient (IQ) is determined by inherent disposition and environmental factor. IQ is estimated by age-conversion score and stabilized around age 4 and IQ of adult age can be predicted after age 10. Though children with Mental Retardation (MR) are delayed in language development since early infant period, they receive only special education including speech and language therapy, but no special medication. In traditional Korean medicine, the etiology and treatment for developmental delay of language have been handed down for a long time. Some studies on herbs and prescriptions for improving language development have been undertaken recently. We have found several cases of significant elevation of IQ in the children treated with long term medications of Korean herbal medicine for improvement of language. Analyzing these cases, especially performance IQ showed significant change. Therefore we suggest that Korean herbal medicine might improve cognition development in children with MR.

  • $M\ddot{o}bius$ syndrome is a rare congenital condition, characterized by abducens and facial nerve palsy, resulting in limitation of lateral gaze movement and facial diplegia. However, to our knowledge, there have been few studies on evaluation of cranial nerves, on MR imaging in $M\ddot{o}bius$ syndrome. Herein, we describe a rare case of $M\ddot{o}bius$ syndrome representing limitation of lateral gaze, and weakness of facial expression, since the neonatal period. In this case, high-resolution MR imaging played a key role in diagnosing $M\ddot{o}bius$ syndrome, by direct visualization of corresponding cranial nerves abnormalities.

Comparison of Pre-processed Brain Tumor MR Images Using Deep Learning Detection Algorithms

  • Detecting brain tumors of different sizes is a challenging task. This study aimed to identify brain tumors using detection algorithms. Most studies in this area use segmentation; however, we utilized detection owing to its advantages. Data were obtained from 64 patients and 11,200 MR images. The deep learning model used was RetinaNet, which is based on ResNet152. The model learned three different types of pre-processing images: normal, general histogram equalization, and contrast-limited adaptive histogram equalization (CLAHE). The three types of images were compared to determine the pre-processing technique that exhibits the best performance in the deep learning algorithms. During pre-processing, we converted the MR images from DICOM to JPG format. Additionally, we regulated the window level and width. The model compared the pre-processed images to determine which images showed adequate performance; CLAHE showed the best performance, with a sensitivity of 81.79%. The RetinaNet model for detecting brain tumors through deep learning algorithms demonstrated satisfactory performance in finding lesions. In future, we plan to develop a new model for improving the detection performance using well-processed data. This study lays the groundwork for future detection technologies that can help doctors find lesions more easily in clinical tasks.
