Efficient Coding Technique for 4X4 Intra Prediction Modes using the Statistical Distribution of Intra Modes of Adjacent Intra Blocks (주변 인트라 블록 예측 모드의 통계적 분포를 이용한 효율적인 인트라 4X4 예측 모드 부호화 방법)

  • In this paper, we propose a novel method which efficiently codes $4{\times}4$ intra prediction modes in H.264/ AVC video encoding. There are nine optional prediction modes for each $4{\times}4$luma block and 4 modes for each $16{\times}16$luma block. To code each $4{\times}4$ intra prediction mode, lots of bits are required. To efficiently compress the intra mode bits in H.264/AVC, the most probable mode(MPM) is estimated by using the intra modes of the adjacent blocks, since intra modes for neighboring $4{\times}4$luma blocks are correlated. In this paper, a novel method for estimating the MPM is proposed by using the statistical distribution of intra modes of adjacent intra blocks. Experimental results show that the proposed method can achieve a coding gain of about 0.1dB.

Comparing the Performance of Three Severity Scoring Systems for ICU Patients: APACHE III, SAPS II, MPM II (중환자 중증도 평가도구의 타당도 평가 - APACHE III, SAPS II, MPM II)

  • Objectives : To evaluate the predictive validity of three scoring systems; the acute physiology and chronic health evaluation(APACHE) III, simplified acute physiology score(SAPS) II, and mortality probability model(MPM) II systems in critically ill patients. Methods : A concurrent and retrospective study conducted by collecting data on consecutive patients admitted to the intensive care unit(ICU) including surgical, medical and coronary care unit between January 1, 2004, and March 31, 2004. Data were collected on 348 patients consecutively admitted to the ICU(aged 16 years or older, no transfer, ICU stay at least 8 hours). Three models were analyzed using logistic regression. Discrimination was assessed using receiver operating characteristic(ROC) curves, sensitivity, specificity, and correct classification rate. Calibration was assessed using the Lemeshow-Hosmer goodness of fit H-statistic. Results : For the APACHE III, SAPS II and MPM II systems, the area under the receiver operating characterist ic(ROC) curves were 0.981, 0.978, and 0.941 respectively. With a predicted risk of 0.5, the sensitivities for the APACHE III, SAPS II, and MPM II systems were 81.1, 79.2 and 71.7%, the specificities 98.3, 98.6, and 98.3%, and the correct classification rates 95.7, 95.7, and 94.3%, respectively. The SAPS II and APACHE III systems showed good calibrations(chi-squared H=2.5838 p=0.9577 for SAPS II, and chi-squared H=4.3761 p=0.8217 for APACHE III). Conclusions : The APACHE III and SAPS II systems have excellent powers of mortality prediction, and calibration, and can be useful tools for the quality assessment of intensive care units(ICUs).

  • In this paper, we investigated the maximum ankle joint plantarflexion moment(MPM) of young and elderly subjects during various modes of isokinetic contractions with special interest in the elderly females who were reported to have higher fall rate than the elderly males. Subjects include the young subjects(14 males: $22.7{\pm}2.5yrs$, 13 females: $20.5{\pm}1.3yrs$) and the elderly subjects(17 males: $65.8{\pm}10.5yrs$, 26 females: $71.4{\pm}5.2yrs$). The MPM was normalized by each subject's body weight. The MPM of elderly females was the weakest among the groups, particularly, at the eccentric contractions. The male-to-female ratio of MPM was greater in the elderly than in the young and the young-to-elderly ratio of MPM was greater in female than in male, both during the eccentric contractions. The result suggests that the decreased muscle force per body weight, especially at the eccentric contraction, may be one reason for the more frequent fall of the elderly female than the elderly male.

Outcomes of the Multimodal Treatment of Malignant Pleural Mesiothelioma: The Role of Surgery

  • Background: The treatment of malignant pleural mesothelioma (MPM) is challenging, and multimodal treatment including surgery is recommended; however, the role of surgery is debated. The treatment outcomes of MPM in Korea have not been reported. We analyzed the outcomes of MPM in the context of multimodal treatment, including surgery. Methods: The records of 29 patients with pathologically proven MPM from April 1998 to July 2015 were retrospectively reviewed. The treatment outcomes of the surgery and non-surgery groups were compared. Results: The overall median survival time was 10.6 months, and the overall 3-year survival rate was 25%. No postoperative 30-day or in-hospital mortality occurred in the surgery group. Postoperative complications included tachyarrhythmia (n=4), pulmonary thromboembolism (n=1), pneumonia (n=1), chylothorax (n=1), and wound complications (n=3). The treatment outcomes between the surgery and non-surgery groups were not significantly different (3-year survival rate: 31.3% vs. 16.7%, respectively; p=0.47). In a subgroup analysis, there was no significant difference in the treatment outcomes between the extrapleural pneumonectomy group and the non-surgery group (3-year survival rate: 45.5% vs. 16.7%, respectively; p=0.23). Conclusion: Multimodal treatment incorporating surgery did not show better outcomes than non-surgical treatment. A nationwide multicenter data registry and prospective randomized controlled studies are necessary to optimize the treatment of MPM.

Survival Effect of Complete Multimodal Therapy in Malignant Pleural Mesothelioma

  • Background: Malignant pleural mesothelioma (MPM) is an aggressive pleural malignancy, and despite all multimodal treatment modalities, the 5-year overall survival rate of patients with MPM is less than 20%. In the present study, we aimed to analyze the surgical and prognostic outcomes of patients with MPM who received multimodal treatment. Methods: In this retrospective, single-center study, the records of patients who underwent surgery for MPM between January 2010 and December 2020 at our department were reviewed retrospectively. Results: Sixty-four patients were included in the study, of whom 23 (35.9%) were women and 41 (64.1%) were men. Extrapleural pneumonectomy, pleurectomy/decortication, and extended pleurectomy/decortication procedures were performed in 34.4%, 45.3%, and 20.3% of patients, respectively. The median survival of patients was 21 months, and the 5-year survival rate was 20.2%. Advanced tumor stage (hazard ratio [HR], 1.8; p=0.04), right-sided extrapleural pneumonectomy (HR, 3.1; p=0.02), lymph node metastasis (HR, 1.8; p=0.04), and incomplete multimodal therapy (HR, 1.9; p=0.03) were poor prognostic factors. There was no significant survival difference according to surgical type or histopathological subtype. Conclusion: Multimodal therapy can offer an acceptable survival rate in patients with MPM. Despite its poor reputation in the literature, the survival rate after extrapleural pneumonectomy, especially left-sided, was not as poor as might be expected.

  • The condition of a high voltage motor was monitored with the motor performance monitor (MPM) at the motor control center. The MPM detected defects in the rotor bar and end ring and input power according to motor and load conditions. The assessment of the condition of a coal pulverizer motor indicated it was clearly in good condition in terms of the rotor bars, over voltage, and motor performance. However, the side bands at frequency, 56.48 Hz indicated existence of rotor end-ring fault. The large torque ripple indicated abnormal operating conditions. After visual inspection, it has been observed that an impeller blade of the circulating water pump was broken off causing the irregular torque pattern.

  • ITU-T와 ISO/IEC의 공동 작업으로 제정된 H.264/AVC는 기존 비디오 표준들에 비해 동일한 화질에서 약 30%~70%의 비트 량을 절감할 수 있으며, 동일한 비트 량으로 PSNR이 2~3dB 가량 우수한 영상을 제공할 수 있다. H.264/AVC는 인트라 부호화 효율을 높이기 위해 공간 영역에서 주변 화소를 이용하여 $4{\times}4$ luma 예측의 9가지 모드와 $16{\times}16$ luma 예측의 4가지 모드에 대한 비트율-왜곡 최적화 기법을 사용하여 최적의 인트라 예측 모드를 선택한다. 본 논문에서는 $4{\times}4$ luma 예측에서 발생하는 정보량을 줄이기 위해 사용하는 MPM을 기존의 방법과는 다르게 연산을 통해 상관도를 높임으로써 보다 정확한 MPM 결정으로 부호화 효율을 높이는 방법을 제안하였다. 실험결과, 제안한 방법은 기존의 방법과 비교하여 BD-RATE 평균 약 -1.4%의 성능 개선 효과를 보였다.

Dramatic Tumor Response to 2nd-line Pemetrexed/Cisplatin Combination Chemotherapy in Patient with Malignant Pleural Mesothelioma (Pemetrexed/cisplatin 병합 2차 항암화학요법에 극적 반응을 보인 악성 흉막 중피종 1예)

  • Malignant pleural mesothelioma (MPM) is a rare tumor that is difficult to clearly distinguish from an adenocarcinoma but usually has a poor prognosis. Numerous cytotoxic agents have been used in the primary treatment of MPM with limited success. A complete response is unusual and a partial response occurs in less than one-third of patients. Recently, a phase III trial showed that a combination of pemetrexed with cisplatin resulted in a significantly higher response rate and median survival time than with cisplatin alone. We encountered a case of a dramatic tumor response to pemetrexed/cisplatin combination chemotherapy in patients with MPM, which was resistant to the 1st-line gemcitabine/cisplatin therapy. After six cycles of pemetrexed/cisplatin combination chemotherapy, the tumor volume had decreased dramatically with complete symptom relief. There was no chemotherapy-related toxicity or scheduled violation. The patient is under maintenance chemotherapy with the same regimen.

Antifungal activities of sulphamide and dicarboximide fungicides against Botrytis cinerea in several in vitro bioassays (여러 종류의 in vitro 생물검정에서 Botrytis cinerea에 대한 sulphamide계와 dicarboximide계 살균제의 활성 특성)

  • Two sulphamide (dichlofluanid and tolylfluanid) and three dicarboximide fungicides (iprodione, vinclozolin, procymidone) were used to investigate the correlation between in vitro antifungal activities and in vivo disease controlling activities against Botrytis cinerea, a causal agent of tomato gray mold and to develop efficient in vitro assays. They controlled effectively the development of tomato gray mold disease in vivo and their controlling activities were similar one another. However, several in vitro assays revealed that their in vitro antifungal activities were quite different between sulphamide and dicarboximide fungicides; the formers showed stronger inhibition activities for spore germination than the latters, whereas the formers inhibited mycelial growth less severely than the latters. The results indicate that the fungicides having different modes of action can show different in vitro antifungal activities according to in vitro assays, even if they have similar in vivo disease controlling activities. On the other hand, two rapid and efficient in vitro assays named Microtiter plate methods I (MPM I) and II (MPM II) were developed for the evaluation of fungicides for inhibitory activities against spore germination and mycelial growth of B. cinerea, respectively. The antifungal activities of five fungicides of two chemical groups in MPM I and II were correlated with the inhibitory activities against spore germination and mycelial growth using solid media, respectively.

Prediction and Historical Analysis of Long-term Bed Elevation Change in the Mankyung-gang River (만경강의 장기하상변동 예측 및 이력분석)

  • This study presents prediction and historical analysis of the long-term bed elevation change in the Mankyung-gang River. The study area is a 25 km long reach including middle and lower courses of the Mankyung-gang River. HEC-RAS program was used for numerical predictions, and values of roughness coefficients were calibrated. Then, predictions were made in the two periods, seven years from 1986 to 1993 and twelve years from 1993 to 2005. Simulation results were compared with two sets of measured data for bed elevations. Four sediment transport formulas, namely MPM's, Toffaleti's, MPM-Toffaleti's, and Yang's formula, were tested. Simulation results showed that none of the four sediment transport formulas predicted the bed elevation change in the period of 1986 - 1993. This is related to the fact that dredging work was performed in the upstream reach in the period of 1986 - 1993, and sediment was deposited in this part severely later. However, it was found that MPM-Toffaleti's formula predicted properly the bed elevation change for the period of 1993 - 2005.
