일부 초등학생의 대표적 신체활동의 에너지소비량 측정 및 에너지소비량과 체중과의 상관성 분석 (Energy expenditure measurement of various physical activity and correlation analysis of body weight and energy expenditure in elementary school children)

  • 본 연구는 만 9~12세의 초등학생 총 20명(남아 11명, 여아 9명)을 대상으로 18가지의 일상생활에서의 대표적인 신체활동에 따른 에너지소비량을 측정하여, 아동의 일상생활에서 수행하는 신체활동에 따른 에너지소비량을 평가하고, 체중이 METs 값에 미치는 영향을 분석해보았다. 1. 연구대상자의 평균연령은 $10.4{\pm}0.6$세였으며, 평균 신장 및 체중은 $145.1{\pm}8.5cm$$43.6{\pm}10.3kg$ 이었다. 대상자의 허리둘레는 남아와 여아 각각 $73.6{\pm}9.6cm$$63.5{\pm}7.1cm$로 남녀 간의 유의한 차이를 보였다 (p < 0.05). 2. 연구대상자의 휴식 시 산소소비량 ($VO_2$), 에너지소비량 및 METs 값은 각각 $5.41{\pm}0.89mL/kg/min$, $1.44{\pm}0.17kcal/kg/h$$1.5{\pm}0.4METs$로 나타났다. 3. 18가지의 신체활동에 따른 METs 값은 앉아서 숙제하기 및 책읽기 ($1.6{\pm}0.3METs$), 앉아서 비디오 혹은 핸드폰으로 게임하기 ($1.6{\pm}0.5METs$), 앉아서 텔레비전 시청하기 ($1.7{\pm}0.5METs$), 서서 비디오 혹은 핸드폰으로 게임하기 ($1.9{\pm}0.6METs$), 빗자루로 방 쓸기 ($2.7{\pm}1.1METs$) 및 보드게임 ($2.8{\pm}2.2METs$)이 가벼운 신체활동에 속하였으며, 진공청소기로 청소하기 ($3.0{\pm}0.2METs$), 피구 ($3.6{\pm}0.9METs$), 삽질하기 ($3.8{\pm}0.4METs$), 계단오르내리기 ($3.8{\pm}1.3METs$), 농구 슛팅 ($4.4{\pm}.1.5METs$), 줄넘기 ($4.6{\pm}2.6METs$), 음악에 맞추어 스트레칭 ($4.7{\pm}2.3METs$), 천천히 걷기 ($4.8{\pm}2.6METs$) 및 배드민턴 ($4.8{\pm}1.0METs$)의 활동이 중간강도의 신체활동에 속하였다. 빨리 걷기 및 달리기는 각각 $6.6{\pm}1.7$$6.7{\pm}1.5METs$로 6.0 METs 이상의 고강도 신체활동에 속하는 것으로 나타났다. 4. 본 연구결과를 성인 METs 값 및 기존에 보고된 아동 METs 값과 비교하기 위하여 Bland-Altman 분석을 실시한 결과 각각 mean difference $0.18{\pm}1.57$, upper and lower limit of agreement 0.99 and -0.62 및 mean difference $0.28{\pm}1.49$, upper and lower limit of agreement 1.05 and -0.48로 나타났으며, ICC (interclass correlation coefficient)를 통한 일치도 분석결과는 각각 0.815 및 0.837로 성인의 METs 값보다는 기존에 보고된 아동 METs 값과의 일치도가 더 높은 것으로 나타났다. 5. 체중이 신체활동에 따른 에너지소비량에 미치는 영향을 살펴본 결과, 앉아서 게임하기 (METs 1.6), 계단오르내리기 (METs 3.8), 천천히 걷기 (METs 4.8) 및 달리기 (METs 6.6)에서 $R^2$값이 각각 0.116, 0.176, 0.246 및 0.455로 증가하였으며, 활동 강도가 높을수록 체중이 METs 값에 대한 설명력이 증가함을 알 수 있었다. 신체활동은 건강증진 및 질병예방에 중요한 역할을 하며, 특히, 일상생활에서 신체활동을 증진시키는 것은 많은 질병에 있어 최선의 예방책 가운데 필수적인 요소이다. 이러한 면에서 영양 및 운동처방의 목표에 정확하게 도달하기 위해서는 에너지소비량, 특히, 일상생활에서의 신체활동이 정확하게 평가되어져야한다. 결론적으로, 신체활동에 따른 에너지소비량을 평가하기 위해 성인의 데이터가 존재할지라도 아동들의 일상적인 활동에 대한 에너지 cost 자료는 부족한 실정이다. 이에 본 연구는 초등학생의 신체활동에 따른 에너지소비량에 대한 데이터를 직접 측정하여 평가하였다는데 큰 의의가 있으며, 추후 연구에서는 더 넓은 범위의 그룹 및 다양한 연령대의 아동을 대상으로 한 연구가 이루어져야 할 것이다.

DDI 메타데이터를 활용한 METS 설계에 관한 연구 (A Study on METS Design Using DDI Metadata)

  • 이 연구는 데이터세트를 관리, 보존, 서비스하기 위해 DDI 메타데이터를 기반으로 METS를 활용하는 방안을 제시하였다. DDI는 통계 데이터 처리를 위한 표준으로 현재 DDI Codebook(DDI-C)과 DDI Lifecycle(DDI-L) 두 가지 버전이 존재한다. 본 연구에서는 DDI-C의 주요 요소를 주로 하였다. 이를 위해 우선 METS와 DDI-C의 구조와 요소를 분석하였다. 그리고 METS와 DDI-C의 주요 요소들에 대한 매핑작업을 수행하였다. 여기서 기준은 최종적으로 이를 표현할 형식인 METS로 삼았다. METS와 DDI-C가 완벽하게 1:1의 매핑을 보이지 않기 때문에 기준인 METS의 각 요소들에 가장 적합하게 부합하는 DDI-C 요소를 선택하였다. 그 결과 DDI-C 메타데이터요소를 활용한 새로운 데이터세트 관리전송 표준 METS를 설계하여 제시하였다.

운동부하가 움직임-호흡 결합에 미치는 영향 (Effect of the Exercise Load on the Locomotor-Respiratory Coupling)

  • The nature of entrainment between the locomotor and the respiratory rhythm was investigated while normal human subjects were walked or running on a treadmill. The purpose of this study was to analyze the incidence and type of coordination between the locomotor and the respiratory rhythm during running at different work load. The experiments were carried out on 12 untrained volunteers exercising at 3 work loads (2 METs, 3 METs, 4 METs in randomized order). The gait cycle was measured by electromyography (EMG) signal of gastrocnemius firing and the respiratory cycle was measured by a thermometer. We found that the ratio between the locomotor and the respiratory rhythm existed and 2:1 ratio between the locomotor-respiratory coupling was dominant at 2 METs and 3 METs.

Influence of Physical Activity on Metabolic Syndrome according to Smoking Intensity

  • Purpose: This study was conducted to determine if physical activity levels relieve the risk of development of metabolic syndrome (Mets) according to smoking intensity among smoking men. Methods: Secondary data from the 2010-2015 Korea National Health and Nutrition Examination Survey were used for this cross sectional study. A total of 3,027 men over the age of 19 were included for data analysis. Complex samples logistic regression was used to analyze the combined effects of smoking and physical activity on Mets. Smoking intensity was categorized as light (${\leq}20cigarettes/day$) or heavy (> 20 cigarettes/day), while physical activity was categorized as light (< 600 metabolic equivalent [MET]-min/week), moderate (600-3,000 MET-min/week), and vigorous (${\geq}3,000MET-min/week$). Results: Mets was less prevalent among light smokers with vigorous physical activity (Adjusted Odds Ratio [AOR]: 0.632, 95% Confidence Interval [CI]: 0.475-0.840) compared to those with light physical activity. Among heavy smokers, physical activity level was not significantly associated with Mets. Conclusion: Only vigorous physical activity exerted protective effects against development of Mets among low intensity smokers. For high intensity smokers, lowering smoking intensity and smoking cessation are also necessary to prevent development of Mets.

Analysis of the association between health-related and work-related factors among workers and metabolic syndrome using data from the Korean National Health and Nutrition Examination Survey (2016)

  • BACKGROUND/OBJECTIVE: This study analyzed health-related factors for metabolic syndrome (Mets) among workers in South Korea. SUBJECTS/METHODS: This analysis included 4,666 adults aged 19-64 years to analyzed health-related risk factors for Mets from the Korean National Health and Nutrition Examination Survey (2016). The sociodemographic, working, health-related, and biochemical characteristics were presented as percentages (%) by chi-square tests. Multiple logistic regression analysis was performed to analyze the 95% confidence intervals (CIs) and associations between health-related factors characteristic of workers and the odds ratios of Mets. RESULTS: An increased prevalence of Mets was associated with male day workers compared to male shift workers (1.726-fold increase, 95% CI: 1.077-2.765), and with ${\geq}8hrs/day$ sleep duration compared to < 6 hrs/day sleep duration in female workers (2.133-fold, 95% CI: 1.041-4.368). In addition, reduced odds of high Mets were associated with male workers consumed of breakfast 5-7 times/wk compared to those consumed < 1 time/wk (0.593-fold decrease, 95% CI: 0.372-0.944). CONCLUSIONS: Health-related factors (sleep duration, frequency of breakfast) and working type in Korean workers may affect the prevalence of Mets.

대전지역 한방병원 중풍환자의 대사증후군 분포에 대한 연구 (Study on Pattern of Metabolic Syndrome about Stroke Patients in Oriental Hospital)

  • The purpose of this study is to investigate the pattern of metabolic syndrome(Mets) in stroke patients who were hospitalized in Daejeon oriental hospital. The present study was done over 445 hospitalized patient with stroke in the Daejeon University Oriental Medical Hospital in the period of November 2006 to December 2008. Stroke patients had been interviewed by residents and specialists who studied standard operation procedures in Fundamental study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. We analyzed all registered data and evaluated the prevalence of Mets and specific components of Mets. The distribution of Mets in stroke patients according to American Heart Association/National Heart Lung and Blood Institute(AHA/NHLBI) criteria is 65.63%. The distribution of Mets in stroke patients according to International Diabetes Federation(IDF) criteria is 48.05%. The distribution of female patients is higher than that of male patients. The distribution of central obesity, low HDL cholesterol and hypertension is higher in female patients. There is no significant relationship between Mets and stroke type in AHA/NHLBI criteria.. But, there is significant relationship between Mets and stroke type in IDF criteria.

A Comparison of Exercise Intensity between Two Horticultural and Four Common Physical Activities among Male Adults in Their 20s

  • This study aimed to identify the exercise intensity associated with four common physical activities for adults (running, skipping rope, walking, and muscle strength exercises) and two horticultural activities (creating a vegetable bed and garden maintenance). For this experiment, 19 males (mean age: $25.8{\pm}2.3$ years) randomly participated in the activities at a glasshouse at Konkuk University. Each of the six activities lasted for 5 minutes; the subjects rested for 5 minutes during intervals between the activities. A portable metabolic analyzer was used to store the oxygen and energy consumption values obtained upon measurement of each activity. In addition, a wireless heart rate monitor was used to measure the subjects' heart rate. According to the results, the four types of physical activities and two horticultural activities carried out by the subjects are moderate- to high-intensity activities [i.e., $3.8{\pm}0.9$ to $9.9{\pm}2.1$ metabolic equivalents (METs)]. Running ($9.9{\pm}2.1$ METs) and skipping rope ($8.8{\pm}2.2$ METs) were categorized as high-intensity physical activities, whereas creating a vegetable bed ($5.0{\pm}1.2$ METs), walking ($4.9{\pm}0.8$ METs), muscle strength exercises ($4.5{\pm}1.3$ METs), and garden maintenance ($3.8{\pm}0.9$ METs) were classified as moderate-intensity physical activities. The exercise intensities of horticultural activities in this study were similar to those of walking and muscle strength exercises. Therefore, participating in these horticultural activities is expected to garner health benefits similar to those reaped from the physical activities described in the current study.

한국 성인의 채소와 김치 섭취량에 따른 영양상태와 대사증후군 위험도에 관한 연구: 2010-2011년 국민건강영양조사 자료를 이용하여 (Risk of Metabolic Syndrome according to Intakes of Vegetables and Kimchi in Korean Adults: Using the 5th Korea National Health and Nutrition Examination Survey, 2010-2011)

  • Objectives: The objective of this study was to examine the relations between total vegetable and Kimchi intakes and the risk of metabolic syndrome (Mets) in Korean adults. Methods: This study used dietary intake and health data of 6668 subjects aged 20 years and over from the 2010-2011 Korea National Health and Nutrition Examination Survey (KNHANES). Daily intakes of total vegetables and Kimchi were assessed by 24-hour recall data. The odds ratio of Mets risk according to daily intake of vegetables and Kimchi was analyzed, respectively. Results: The highest consumption of total vegetables was associated with a lower risk of abdominal obesity (multivariable adjusted OR=0.56, 95% CI: 0.33, 0.93) in men and lower risk of Mets (multivariable adjusted OR=0.67, 95% CI: 0.47, 0.94) in women. Kimchi consumption was not related to the risk of Mets in both men and in women. However, a higher intake of Kimchi was associated with an increased risk of elevated blood pressure (Q1 vs Q5, multivariable adjusted OR=1.34, 95% CI: 0.95, 1.90, P for trend= 0.0261) in women. Conclusions: A higher intake of vegetables was associated with decreased risk of abdominal obesity and Mets in both men and women, respectively. A higher consumption of Kimchi was not related to the risk of Mets in both in men and in women. However, a higher intake of Kimchi was associated with an increased risk of elevated blood pressure in women.

고등학생의 비만 여부에 따른 8가지 걷기 활동의 에너지 소비량 비교 - 간접열량계 및 허리와 발목에 착용한 가속도계를 이용하여 - (Energy Expenditure of Eight Walking Activities in Normal Weight and Obese High School Students - Using an Indirect Calorimeter and Accelerometers Worn on Ankle and Waist -)

  • The purposes of this study were to assess energy expenditure of eight walking activities in normal weight and overweight or obese high school students and to evaluate the accuracy of two accelerometers worn on the ankle and waist. Thirty-five (male 17, female 18) healthy high school students participated in this study. They were classified into normal weight (n=21) and overweight or obese (n=14) groups. The subjects completed five treadmill walking activities (TW2.4, TW3.2, TW4.0, TW4.8, TW5.6), followed by three self-selected hallway walking activities (walk as if walking and talking with a friend: HWL, walk as if hurrying across the street at a cross-walk: HWB, walk as fast as you can but do not run: HWF). Energy expenditure and metabolic equivalents (METs) were measured using a portable indirect calorimeter, and predicted energy expenditures and METs were derived from two accelerometers placed on the ankle and waist. Measured energy expenditures per body weight (kg) of eight walking activities were significantly higher in the normal weight group than in the obese group and significantly higher in female than male. The ankle accelerometer overestimated energy expenditures and METs (bias 49.4~105.5%), whereas the waist accelerometer underestimated energy expenditures and METs (bias -30.3~-85.8). Except for HWF (fast) activity, METs of seven activities were moderate intensity based on Compendium METs intensity categories. HWF (fast) activity was vigorous intensity. METs from the ankle accelerometer were vigorous intensity except TW2.4 activity (moderate intensity). METs from the waist accelerometer were low intensity (TW2.4, TW3.2, TW4.0, TW4.8, HWL) and moderate intensity (TW5.6, HWB, HWF). Physical activity guidelines were developed based on measured physical activity level of high school students. Further studies should investigate the effects of body composition in larger subjects.

Comparison of the Metabolic Costs of Gardening and Common Physical Activities in Children

  • This study investigated the exercise intensity and energy expenditure involved in two gardening activities (planting transplants and sowing seeds in a garden plot) and four common physical activities (running, skipping rope, walking, and throwing a ball) in children. Eighteen children aged 11 to 13 years (mean age, $12.3{\pm}0.7$ years) participated in this study. The children made two visits to a high tunnel in Cheongju, Chungbuk, South Korea and performed randomly selected activities. Each activity was performed for 10 min, with a 5-min rest period between activities. The children wore a Cosmed $K4b^2$ (Cosmed $K4b^2$; Cosmed, Rome, Italy), which is a portable calorimetric monitoring system, to measure indicators of metabolic cost such as oxygen uptake and energy expenditure. The children's heart rates during the activities were measured by radiotelemetry (Polar T 31; FitMed, Kempele, Finland). We found that the two gardening and four physical activities performed by the 11-13 years old children in this study were moderate-to high-intensity physical activities [i.e.,$5.4{\pm}0.7$ to $9.1{\pm}1.4$ metabolic equivalents (METs)]. Running ($9.1{\pm}1.4$ METs) and skipping rope ($8.8{\pm}1.1$ METs) were high-intensity physical activities, whereas walking ($6.1{\pm}0.9$ METs), planting transplants ($5.8{\pm}1.1$ METs), throwing a ball ($5.6{\pm}1.1$ METs), and sowing seeds ($5.4{\pm}0.7$ METs) were moderate intensity physical activities. Running and skipping rope were significantly more intense than the other activities (P < 0.0001). The gardening tasks such as planting transplants and sowing seeds in a garden plot showed similar exercise intensities and energy costs as walking and throwing a ball. This study indicates that gardening can be used as a physical activity intervention to provide health benefits similar to more common physical activities such as walking and running.
