Recovery of an Injured Corticospinal Tract in a Patient With Infarction on the Corona Radiata

  • Purpose: Many studies have attempted to elucidate the motor recovery mechanisms of stroke. In the current study, we report on a patient with infarction who showed recovery of an injured corticospinal tract (CST), using diffusion tensor imaging (DTI). Methods: A 20-year-old male presented with severe paralysis of the left extremities following an infarction in the right corona radiate and basal ganglia. The patient showed good motor recovery as much as being able to manipulate object using his left hand and could walk independently on stairs at 9 months after onset. Results: At 1 month after onset, the right CST showed decreased fiber volume due to the partial injury at the level of the corona radiate, and no (motor evoked potential) MEP was evoked from the right hemisphere. By contrast, the results at 9 months showed increased fiber volume of the right CST and the amplitude of MEP had improved to normal range on TMS. Conclusion: These results suggest that the severely injured right CST and MEP amplitude had been recovered during 9 months. Therefore, this result has important implications for brain plasticity and brain rehabilitation in patients with infarction.

Effects of Respiratory Muscle Training on the Respiratory Function, Balance, and Activities of Daily Living in Patients with Stroke

  • Purpose: This study evaluated the effects of respiratory muscle training on respiratory function, balance, and activities of daily living (ADL) in patients with stroke. Methods: The study included 21 patients with stroke, divided into the experimental group and control group. Both groups underwent traditional physical therapy once a day for 30 minutes, five times weekly for 6 weeks. The experimental group underwent additional respiratory muscle training once a day, five times a week during the study period. Maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP), Berg balance scale (BBS), and Korean-modified Barthel index (K-MBI) were measured before and after the study period. For statistical analysis, paired t-test was used to compare the difference between the pre and post values. Independent t-test was used compare the differences between groups. Results: Both groups had significantly improved MIP, MEP, BBS, and K-MBI scores after the study period. The experimental group had significantly improved MIP, MEP, BBS, and K-MBI scores. Conclusion: These results suggest that respiratory muscle training improves respiratory function, balance, and ADL in patients with stroke.

Effects of Five-month Training of Playing Harmonica on Pulmonary Function in Patients With Neuromuscular Disease: A Pilot Study

  • Background: Progressive muscle weakness is aggravated not only in the skeletal muscles but also in the respiratory muscles in many patients with neuromuscular diseases (NMD). Inspiratory muscle training (IMT) has been reported as therapy for pulmonary rehabilitation to improve respiratory strength, endurance, exercise capacity, and quality of life, and to reduce dyspnea. Objects: The purpose of this study was to determine the effect of playing harmonica for 5 months on pulmonary function by assessing the force vital capacity (FVC), peak cough flow (PCF), maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP), and maximal voluntary ventilation (MVV) in patients with NMD. Methods: Six subjects with NMD participated in this study. The subjects played harmonica once a week for 2 hours at a harmonica academy and twice a week for 1 hour at home. Thus, training was performed thrice a week for 23 weeks. The examiner assessed pulmonary function by measuring FVC in the sitting and supine positions and PCF, MIP, MEP, and MVV in the sitting position at the beginning of training and once a month for 5 months. Results: Both sitting and supine FVC significantly increased after playing harmonica (p=.042), as did MIP (p=.043) and MEP (p=.042). Conclusion: Playing harmonica can be used as an effective method to improve pulmonary function in patients with NMD.

뇌경색 환자의 운동장애에 대한 주파수별 전침치료효과의 비교 (A Comparative Study of Motor Recovery from Stroke between High and Low Frequency Electrical Acupoint Stimulation)

  • Objectives : Electrical acupoint stimulation (EAS) has been used to treat motor dysfunction of stroke patients with reportedly effective results. The purpose of this study was to evaluate the efficacy of EAS with different frequencies in treating motor dysfunction of ischemic stroke patients. Methods : The subjects of this study were sixty-two ischemic stroke patients with motor dysfunction at Kyunghee Oriental Medicine Hospital who were hospitalized for one week to one month from onset. They were treated with 2Hz or 120Hz EAS for two weeks, and motor evoked potentials (MEP) were measured before and after EAS treatment. To compare the effect of 2Hz EAS with 120Hz, we measured latency, central motor conduction time (CMCT) and amplitude of MEP before and after EAS treatment. Results : After two weeks of treatment, we compared MEP data of the affected side between the 2Hz group and the 120Hz group. The 2Hz group showed more significant improvement than the 120Hz group in latency, CMCT and amplitude (P=0.008, 0.002, 0.002). In case of the affected side MEP data divided by normal side MEP data, the 2Hz group also showed improvement on latency, CMCT and amplitude with significant differences (P=0.003, 0.000, 0.008). Conclusions : These results suggest that low frequency EAS activates the central motor conduction system better than high frequency EAS, and it means that EAS with low frequency is more helpful for motor recovery after ischemic stroke than that with high frequency.

공복혈당수치와 운동유발전위의 상관관계에 대한 후향적 분석 : 중추운동신경과 말초운동신경의 비교 (A Retrospective Study on the Correlation between Fasting Blood Sugar and Motor Evoked Potentials : Comparison between Central and Peripheral Motor Nerve)

  • Objectives : Peripheral neurodegeneration occurs in diabetes mellitus (DM), both sensory and motor nerve. but we don't know exactly if DM affects central nerve pathway for all studies. Electrophysiologic study is one of the most important diagnostic tools for diabetic neuropathy. Electroneurography and electromyography are usually used. but evoked potentials (EP) is more sensitive to small nerve fiber damages and useful for central nerve evaluation in addition to peripheral nerves. Most diabetic neuropathy studies by EP have been performed with somatosensory evoked potentials (SSEP). In contrast, the objective of this study is to investigate if DM targets central motor neurons by assessing the relation between fasting blood sugar (FBS) and motor evoked potentials (MEP) latency. Methods : We inspected the medical records of 34 patients who had MEP tests during admitting days. The latency from cervical portion to abductor pollicis brevis was used as peripheral motor conduction time (PMCT). and the latency from vertex to cervical portion was used as central motor conduction time (CMCT). Then, they were correlated to FBS using correlation analysis. Results : There was a significant linear relation between FBS and PMCT (Pearson's correlation coefficient r=0.487, p<0.01), but a poor linear relation between FBS and CMCT (Pearson's correlation coefficient r=-0.l97. p>0.05). Conclusions : This study suggests that prolonged latencies of MEP in DM may be due to peripheral neuropathy rather than dysfunction of central motor pathway. therefore the clinical use of MEP to diabetic neuropathy has to be divided segmentally.

수종(數種)의 약제(藥劑)가 배나무의 낙과(落果) 및 에틸렌 발생(發生)에 미치는 영향(影響) (Effects of Some Chemials on Ethylene Evolution and Abscission of Fruits and Leaves in Oriental Pera)

  • 낙과(落果)를 유기할 수 있는 약제(藥劑)인 NAA, MEP제(劑), carbaryl 및 ethephon을 배 장십랑(長十郞)과 금촌추(今村秋)에 처리(處理)하여 낙과(落果)와 ethylene과의 관련성을 조사(調査)하는 한편 낙과량(落果量)을 조절하기 위한 방법으로 calcium acetate를 가용(加用)한 ethephon과 carbaryl를 처리(處理)하였을 때의 낙과(落果)에 미치는 영향(影響)을 조사(調査)하기 위(爲)하여 본(本) 실험(實驗)을 실시(實施)하였다. 1. 장십랑(長十郞)의 경우는 MEP제(劑), NAA, carbaryl, ethephon 모두 적과효과(摘果效果)가 있었으나 금촌추(今村秋)의 경우는 MEP와 carbaryl만 적과효과(摘果效果)가 인정(認定)되었다. 2. ethylene 발생량(發生量)은 ethephon 처리구(處理區)만이 촉진(促進)되었고 MEP제(劑), NAA, carbaryl는 영향(影響)을 미치지 않았다. 3. 과실내(果實內)의 당함량(糖含量)에는 MEP제(劑), NAA, carbaryl, ethephon 모두 영향(影響)을 미치지 않았다. 4. calcium acetate를 가용(加用)하므로서 ethephon이나 carbaryl에 의(依)한 적과효과(摘果效果)가 소실(消失)되었다. 5. ethephon에 가용(加用)한 calcium acetate의 농도(濃度)에 따라 낙엽율(落葉率)을 조절(調節)할 수 있었다. 이상(以上)의 결과(結果)로 보아 공시(供試)한 약제(藥劑)들이 낙과(落果)를 일으키는 것은 ethephon을 제외하고는 ethylene이 직접적(直接的)인 낙과원인(落果原因)이 아니라는 것을 알 수 있었고, 한편 calcium acetate를 적량가용(適量加用)하므로서 적과정도(摘果程度)의 조절(調節)이 가능(可能)하다는 것이 밝혀졌다.

척수 종양 제거 술 중 운동유발전위의 호전과 근력 호전의 관계 (Increased Intraoperative Motor Evoked Potentials and Motor Recovery after Spinal Cord Tumor Removal)

  • Objective: To evaluate whether the increase of the amplitude of motor evoked potentials (MEPs) during surgery can imply favorable prognosis postoperatively in spinal cord tumor surgery. Method: MEPs were monitored in patients who underwent spinal cord tumor surgery between March 2016 and March 2018. Amplitude changes at the end of monitoring compared to the baselines in limb muscle were analyzed. Minimum and maximum changes were set to $MEP_{min}$ (%) and $MEP_{max}$ (%). Strengths of bilateral 10 key muscles which were documented a day before ($Motor_{pre}$), 48 h ($Motor_{48h}$) and 4 weeks ($Motor_{4wk}$) after the surgery were reviewed. Results: Difference of $Motor_{48h}$ from $Motor_{pre}$ ($Motor_{48h-pre}$) and $Motor_{4wk}$ from $Motor_{pre}$ ($Motor_{4wk-pre}$) positively correlated with $MEP_{min}$, suggesting that smaller the difference of MEPs amplitude, less recovery of muscle strength. There was a negative correlation between the amount of bleeding and $MEP_{min}$, indicating that the greater the amount of bleeding, the smaller the $MEP_{min}$, implying that MEPs amplitude is less likely to improve when the amount of bleeding is large. It also showed significant difference between patients with improved or no change of motor status and patients with motor deterioration after surgery according to anatomical tumor types. Conclusion: Improve of muscle strength was less when the increase of MEPs amplitude was small, and improvement of MEPs amplitude was less when the amount of bleeding was large. Correlation between changes of status of muscle strength after surgery and tumor types was observed. With amplitude increase in MEPs monitoring, restoration of muscle strength can be expected.

  • Purpose : The purpose of this study was to determine the effect of pelvic floor muscle exercise (PFME) on an unstable support surface on maximal inspiratory pressure (MIP), maximum expiratory pressure (MEP), and abdominal muscle thickness as a method for effective PFME. Methods : This study was performed on 22 subjects. They were matched and divided into two groups based on body mass index; the experimental group (EG) performed PFME on a foam roller (n= 11), the control group (CG) performed PFME on a stable support surface (n= 11). Kegel exercise was performed with 10 seconds of contraction, 10 seconds of relaxation, and 4 sets of 10 reps per set. Both of group executed the exercise 3 times a week for 2 weeks. MIP and MEP was measured using a spirometer. Abdominal muscle thickness was measured using ultrasound. The paired t-test was used to compare difference on each group and the comparison between groups was analyzed using the independent t-test. A significance level of α= .05 was used to verify statistical significance. Results : The EG showed a significant increase in the MEP (p<.05). The CG showed a significant increase in the MEP (p<.05). There was no significant difference in the two groups (p>.05). The EG showed a significant increase in the external oblique, internal oblique and transverse abdominis (p<.05). The CG showed a significant increase in the internal oblique (p<.05). There was no significant difference in the two groups (p>.05). Conclusion : Based on the results of this study, additional research should be conducted to correct the limitations of this study to confirm that PFME performed on a foam roller has a positive effect on respiratory muscle strength and abdominal muscle thickness.

말초신경-피질 연계자극에 의해 유도되는 운동피질 가소성의 비대칭성 (Hemispheric Asymmetry of Plasticity in the Human Motor Cortex Induced by Paired Associative Stimulation)

  • Background: In the brain, the dominant primary motor cortex (M1) has a greater hand representation area, shows more profuse horizontal connections, and shows a greater reduction in intracortical inhibition after hand exercise than does the non-dominant M1, suggesting a hemispheric asymmetry in M1 plasticity. Methods: We performed a transcranial magnetic stimulation (TMS) study to investigate the hemispheric asymmetry of paired associative stimulation (PAS)-induced M1 plasticity in 9 right-handed volunteers. Motor evoked potentials (MEPs) were measured in the abductor pollicis brevis (APB) muscles of both hands, and MEP recruitment curves were measured at different stimulation intensities, before and after PAS. Results: MEP recruitment curves were significantly enhanced in the dominant, but not the non-dominant M1. Conclusions: These results demonstrate that the dominant M1 has greater PAS-induced plasticity than does the non-dominant M1. This provides neurophysiological evidence for the asymmetrical performance of motor tasks related to handedness.

Doppler effect on Matched Field Processing in Ocean Acoustics

  • Matched field localization schemes often show a high sensitivity to acoustic variabilities due to mismatch between assumed and actual environments. In this paper, we focus on the effect of source motion or Doppler on matched field processing (MEP). to accomplish this, MFP is extended to treat a moving source problem with normal mode description of the sound field. the extension involves both the temporally nonstationary and spatially inhomogeneous nature of the sound field generated by a time-harmonic point source moving uniformly in a stratified oceanic waveguide. It is demonstrated that the impact of source motion can be significant to MEP although the velocity of a moving source is much smaller than the sound velocity of the oceanic waveguide. In addition, a criteria for minimizing the effect of Doppler on MFP is discussed.
