Organ preservation with neoadjuvant chemoradiation in patients with orbit invasive sinonasal cancer otherwise requiring exenteration

  • Purpose: We sought to determine if organ preservation (OP) with neoadjuvant chemoradiation (CRT) was feasible in patients with sinonasal cancer determined to require exenteration. Materials and Methods: Twenty patients were determined to require exenteration for definitive treatment from 2005 to 2014. Fourteen patients underwent OP and 6 patients received exenteration with adjuvant CRT. Exenteration free survival (EFS), locoregional control (LRC), progression-free survival (PFS), and overall survival (OS) were estimated. Results: Five patients (36%) receiving OP had complete disease response at time of surgery. With a median follow-up of 18.8 months, EFS was 62% at 2 years for patients undergoing OP. At 2 years, there were no significant differences in LRC, PFS or OS (all all p > 0.050) between the groups. Less grade 3 or greater toxicity was seen in patients undergoing OP (p = 0.003). Visual function was preserved in all patients undergoing OP. Conclusion: For patients with sinonasal cancer, OP may avoid exenteration, offering similar disease control and improved toxicity.

  • The jet pump is being used in many fields for several purposes because of its simple construction and easy operation. The characteristics of the geometrical variables, pressure gradient and velocity distribution of the jet pump are studied using the CFD technique. The flow calculations through a bended nozzle. a mixing chamber and a venturi are presented and phenomenological aspects are discussed. This study solve 3-D steady incompressible Navier-Stokes equations using the Iterative time marching scheme. The governing equations are differenced with 1st-order accurate backward difference scheme for the time derivatives and 3rd-order accurate QUICK scheme for the convective terms. The Mark-and-cell concept was applied efficiently to solve continuity equation, which is differenced 2nd-order accurate central differenced scheme. The 4th-order artificial damping is added to the continuity equation for numerical stability. A O-type of grid system is generated inside a nozzle and venturi of the jet pump. It has concluded that the results of present study properly agree with physical flow phenomena.

  • In silico methods to predict toxicity include the use of (Quantitative) Structure-Activity Relationships ((Q)SARs) as well as grouping (category formation) allowing for read-across. A challenging area for in silico modelling is the prediction of chronic toxicity and the No Observed (Adverse) Effect Level (NO(A)EL) in particular. A proposed solution to the prediction of chronic toxicity is to consider organ level effects, as opposed to modelling the NO(A)EL itself. This review has focussed on the use of structural alerts to identify potential liver toxicants. In silico profilers, or groups of structural alerts, have been developed based on mechanisms of action and informed by current knowledge of Adverse Outcome Pathways. These profilers are robust and can be coded computationally to allow for prediction. However, they do not cover all mechanisms or modes of liver toxicity and recommendations for the improvement of these approaches are given.

Experimental Study of Adjustable Pulmonary Artery Banding Device (가변성 폐동맥 협착기구의 실험적 연구)

  • We developed a subcutaneously adjustable new pulmonary artery banding device which can be easily tightened or released in patients with unstable postoperative hemodynamics. The banding device consists of stainless spring which is enveloped with PTFE, nd polyvinyl catheter(5F). And the adjuster consists of another polyvinyl tube with forming cap. We mark transluscent polyvinyl tube 1mm each in length. 6 dogs weighing 15 to 25 kg underwent banding of descending thoracic aorta with the device. At postoperative 1 and 2 day, the device was effectively banded and released. Even 3 months later, we noted effective banding with some degree of vessel injury. The preliminary animal study suggests that this new device may be applicable in patient with excessive pulmonary blood flow.

  • Frequent outbreaks of foodborne illness have been increasing the awareness of food safety. Conventional methods for pathogen detection and identification are labor-intensive and take days to complete. Some immunological, rapid assays are developed, but these assays still require prolonged enrichment steps. Recently developed biosensors have shown potential for the rapid detection of foodborne pathogens. In this study, an impedimetric biosensor was developed for rapid detection of Salmonella entritidis in food sample. To develop the biosensor, an interdigitated microelectrode (IME) was fabricated by using a semiconductor fabrication process. Anti-Salmonella antibodies were immobilized based on neutravidin-biotin binding on the surface of the IME to form an active sensing layer. To evaluate the effect of electrode gap on sensitivity of the sensor, 3 types of sensors with different electrode gap sizes (2, 5, and $10{\mu}m$) were fabricated and tested. The impedimetric biosensor could detect $10^3\;CFU/mL$ of Salmonella in pork meat extract with an incubation time of 5 min. This method may provide a simple, rapid, and sensitive method to detect foodborne pathogens.

Recurrent dipolarizations of near-Earth magnetotail during high-speed solar wind streamers

  • Recurrent substorms occur when high-speed solar wind streamers pass by Earth's magnetosphere. Most of the previous researches have been done using the observations obtained at the geosynchronous orbit focusing on the relationship between the solar wind disturbances and the occurrence of substorms. However, it is important to investigate the dynamics of the magnetotail because the magnetotail is the place where substorms develop. In this study we investigated the observations of recurrent dipolarizations in the near-Earth magnetotail that occurred during high-speed solar wind streamers. The dipolarizations and subsequent stretchings have occurred for more than three days with the average period of ~2 - 3 hours. The average period of ~2 - 3 hours is consistent with the average occurrence period of recurrent substorms. Also, the observed signatures on the geosynchronous orbit and the ground show recurrent substorms have occurred during the event. These suggest that the recurrent dipolarizations in the near-Earth magnetotail should be closely related to the recurrent substorms. On the other hand, there was no clear flow activities directly associated with the dipolarizations, except for some intermittent bursty flow activities. We will discuss the detailed characteristics of the dipolarizations and the relationship with recurrent substorms.

Structural Analysis on the Arm and Floater Structure of a Wave Energy Converter

  • Ocean waves have huge amounts of energy, even larger than wind or solar, which can be extracted by some mechanical device. This can be done by creating a system of reacting forces, in which two or more bodies move relative to each other, while at least one body interacts with the waves. This moves the floater up and down. The floaters are connected to an arm structure, which are mounted on a fixed hull structure. Hence, the structure of the floater is very important. A static structural analysis with FSI (Fluid-Structure Interaction) analysis is conducted. To achieve the pressure load for the FSI analysis, the floater is simulated on a wave generator using rigid body motion. The structural analysis is done to examine the stresses on the whole system, and four types of flange and floater are optimized. The result shows that the structure of floater with wood support is the safest.

  • The objective of this paper is to investigate the dynamic strength characteristics of LNG carriers cargo containment system under impact loads experimentally. The material properties were experimentally obtained for individual components of MARK III insulation system. A series of impact tests was performed using a custom-built drop experiment facility as varying heights and weights of the drop object. Crack initiation and propagation were measured during the cyclic dry drop experiment. The quantitative relationship between impact load and crack initiation as well as the cycle number and crack propagation were reported.

  • The purpose of this study was to present the basic data for the education of nutrition to female teachers by surveying the degree of nutritional knowledge, the attitute toward nutrition and eating habits of the female teachers, and analyzing the present dietary food. 1. The average score of their nutritional knowledge was 17.53 (the highest mark was 25) and the percentage of the right answers was 73.6. 2. The nutritional knowledge was related to their ages, and whether or not they majored in domestic science. Younger and domestic science majors scores of nutitional knowledge were shown to be higher. 3. They had dinner more regularly than breakfast or lunch and the score of nutritional knowledge was higher in proportion to the regularity of their breakfasts. As nutritional knowledge was reflected in their daily lives and bread was used as a substitute of their meals, the degree of their nutritional knowledge was higher. 4. For self evaluation of nutritional knowledge, the higher the score of their nutritional knowledge was, the more they thought they knew and those with high scores seemed to get information about nutrition through specialized books and cookbooks.

Evolution of Automatic Ordering System in Retail Market : Analyzing Inventory Data

  • The purpose of this paper is to reveal two problems in the existing inventory systems in retail market, and to suggest a Two-Bin System under Automatic Ordering System considering only base-stock. Large retailers already have a sophisticated inventory system based on an automatic ordering principle. However, why does the out-of-stock (OOS) happen in large discount stores in spite of having a good inventory system? This paper suggests two systems after finding the root causes concerning the previous question. For evaluating the performance of each system, the random 200 data set in each sample group was generated from MINITAB 16 and obeyed the Poisson distribution. The existing inventory system in retail market cannot help generating OOS due to indwelling contradiction in itself. The reasons are the ordering deadline and the relationship between ordering quantity and base stock. This paper also demonstrates that these previous studies on inventory fall into the closed loop. Also the paper shows that the performance of the replenishment policy was better than traditional methods dealing with ordering quantity and base stock.
