Muscle Strength and Biochemical Markers as Predictors of Depression in Hemodialysis Patients: A Cross-Sectional Study

  • Patients with chronic renal failure, many of which treated with hemodialysis, present a high prevalence of impaired muscle strength which suggest that muscle mass parameters may be used as markers for changes in muscle in these patients. Measurement of handgrip strength (HGS) is a common, simple, and quick measure of muscle function an indicator of overall muscle strength which has been associated with physical activity and several anthropometric traits. Intercellular adhesion molecule-1 (ICAM-1) and insulin-like growth factor-1 (IGF-1) are biochemical markers associated with inflammatory processes which are a common consequence of dialysis. Additionally, hemodialysis patients frequently present signs of malnutrition and depression. This cross-sectional study aimed to evaluate if muscle and biochemical markers could be used to predict the risk of depression in hemodialysis patients. Several anthropometric parameters, nutrient intake, depression state and the serum levels of ICAM-1 and IGF-1 were determined and Pearson's correlation coefficient and/or Spearman's correlation coefficient were used to test the correlation between them. Our results do not show a correlation between HGF, IGF-1 and ICAM-1 with the depression status of the patients, but mid-arm muscle circumference (MAMC) was statistically and positively correlated with depression. Additionally, ICAM-1 levels were negatively correlated with HGS, MAMC, and IGF-1. Overall, the results of the present study suggest that HGS may be used as an indicator of cardiovascular diseases and MAMC may be a good predictor of the level of depression in hemodialysis patients, although further studies are required.

Assessment of Appetite and Nutritional Status in Cancer Patients Undergoing Radiation Therapy : A Prospective Study (방사선치료를 받은 암환자의 식욕상태 및 영양상태 변화양상 평가)

  • This study was conducted to identify appetite and nutritional status of 48 cancer patients who have been irradiated over 150$\textrm{cm}^2$ on chest or pelvic area over the three-month period. The data were gathered 3 phases, Each from initiation to completion of radiotherapy through the questionnaires of anorexia, the anthropometric and biochemical measures were used such as weight, TSF, MAC, MAMC, serum albumin and hemoglobin, TLC. Using SAS program, data were analyzed by percentage, Mean$\pm$SD, and two-way repeated measures ANOVA. The results were summarized as follows : 1. Eighty five percent(85%) of the subjects were aged from fifties to sixties. Cancers in the chest area occurred in 100% of men, 56% of the all subjects. The other 44% were pelvic cancer and 71% of the pelvic cancer occurred in women. 2. There were no significant differences in the appetite scores by all groups(characteristics). Changes of the appetite score over time were statistically significant by age, sex, cancer areas staging, treatment modality, and radiation dosage (F=4.0, p=.022; t=6.09, p=.003; t=4.90, p=.009; F=3.28, p=.042; t=5.04, p=.0084; t=4.76, p=.011). The appetite score on the 2nd phase (4 weeks after initiating radiotherapy) decreased from the 1st phase (initiating irradiation), and then increased on the 3rd phase (completing irradiation). 3. There were no significant differences in the body weight and MAMC by all characteristics, and no changes in the body weight and MAMC over time. However there were significant differences of TSF, MAC, level of hemoglobin, level of albumin, and TLC by all characteristics during the three phases. TSF of the men and the chest cancer were lower than those of the women and the pelvic cancer (t=73.20, p=.0001; t=22.91, p=.0001). And there was significant difference by cancer staging(F=3.19, p=.050). But there was no change in TSF over time. MAC of the men and the chest cancer were lower than those of the women and the pelvic cancer each(t=9.23, p=.004; t=17.85, p=.0001). But no change in MAC over time. Levels of hemoglobin had significant differences by age, sex and cancer areas; levels of hemoglobin of older than the fifties, men, and chest area were higher than those on the others(F=3.82, p=.029; t=21.75, p=.0001; t=8.71, p=.005). Levels of albumin were significant differences by sex and cancer areas; levels of albumin on women, and pelvic area were higher than those on the others(t=6.34, p=.015; t=15.23, p=.0003). While the levels of hemoglobin were changed over time, levels of albumin were not changed and within normal limit. TLC of the men was higher than women(t=5.05, p=.029). Changes in the level of hemoglobin over time were statistically significant according to sex, cancer areas, and radiation dosage(t=3.49, p=.035; t=3.36, p=.039; t=4.04, p=.021).

A Study on the Eating Behavior, Nutrient Intake and Health Condition of College Students Attempting Weight Control in the Daegu Area (체중조절 중인 대구지역 대학생의 식사행동, 영양소 섭취 및 건강상태에 관한 연구)

  • The purpose of this study was to investigate eating behavior, nutritional status and health conditions of college students attempting a weight control. The subjects are 88 students of the Daegu area. Their weight, height, triceps, and mid-arm circumference were measured and their dietary intake and eating behavior were obtained by using questionnaires. The 24-hour recall was obtained from the subjects. The results are summarized as follows: The average height, weight and BMI of the attempt and no-attempt male and female students were 171.2cm, 70.7kg and 24.1; 170.4cm, 79.9kg and 27.5; 159.3cm, 60.9kg and 24.0; 157.7cm 60.1kg and 24.2, respectively. Energy intake of the attempt and no-attempt male and female group was 63.9%, 61.8%, 76.2% and 83.9% of RDA respectively. Protein intake of each group was 97.5%, 83.9%, 60.1% and 67.3% of RDA respectively. The following items registered a negative correlation weight and carbohydrate, weight and Na intake, weight and vitamin C intake, PIBW and Na intake, TSF and fiber intake, TSF and Na intake, TSF and vitamin C intake, MAMC and Na intake, and MAMC and vitamin C intake. A relative magnitude of factors affecting weight control was analyzed by Stepwise multiple regression analysis. Overall results about relative influence of independent variables to the dependent variable(weight control) indicated that the BMI (p<0.01) was the most significantly correlated with weight control in all subjects. The results of this study suggest that the extensive nutrition education in the weight control program should be emphasized to prevent obesity early.

Effects of Swallowing Rehabilitation Program among Elderly In Geriatric Hospitals (요양병원 입원노인의 연하재활프로그램의 효과)

  • Purpose: The purpose of this study was to examine the effect of a swallowing rehabilitation program for elderly on the stage of dysphagia, nutritional status, and swallowing-quality of life. Methods: The research design was Pretest-Posttest Nonequivalent Groups Design. The subjects of this study consisted of 85 elderly patients hospitalized at geriatric hospitals between August 23rd and October 25th in 2012. Eighty five research subjects were assigned to experimental (42 patients) and control (43 patients) groups. The rehabilitation program was provided to the experimental group 5 days per week for 8 weeks. The effects were evaluated by measurement of Mid-upper Arm Muscle Circumference (MAMC), Total Lymphocyte Counts, albumin, Total Cholesterol, and Swallowing Quality of Life (SWAL-QOL). The collected data were analyzed with x2-test and independent t-test using PASW WIN 18.0. Results: Changes in the stage of dysphagia were not significant (p>.05). In nutrition status, the degree of MAMC points in the experimental group was significantly increased compared with that in the control group (t=2.36, p=.021). In swallowing quality of life, the score of SWAL-QOL in the experimental group was significantly increased, particularly burden (t=3.34, p=.002), frequency (t=2.68, p=.011), fear (t=2.44, p=.019), mental health (t=2.47, p=.018), and sleep (t=2.08, p=.044). Conclusion: Based on the results, this swallowing rehabilitation program for elderly would be helpful in promoting their nutritional status and swallowing-QOL in geriatric hospitals.

A Study on Nutritional Status, Biochemical Parameters, Lipid and Electrolytes Concentrations According to the Duration of Enteral Nutrition Tube-feeding (경장영양 기간에 따른 영양상태, 생화학적 지표, 지질 및 전해질 농도에 관한 연구)

  • The objective of this study was to investigate the nutritional status, biochemical parameters, lipid and electrolytes concentrations of the enteral nutrition patients according to the duration of enteral nutrition. Eighteen neurosurgery patients in the intensive care unit (ICU) at K University Hospital were subjected in this study. The duration of enteral nutrition was classified into under or over six month of period. Anthropometric, biochemical, clinical, and dietary assessments were performed. Patients' intakes of energy and protein were insufficient, from 82% to 95% of their requirements. Mid-arm muscle circumference (MAMC) and mid-am muscle area (MAMA) were significantly lower in patients over six months of enteral nutrition than those in patients under six months. The subjects were malnourished as indicated by nutrition-related parameters such as hemoglobin, albumin, total lymphocyte count (TLC), tricep skinfold thickness (TSF), mid-arm circumference (MAC), MAMC, and MAMA. Serum chloride level of the patients eve, six months of enteral nutrition was lower (94.7 $\pm$ 3.4 mmo1/1) significantly as compared to that of patients (99.3 $\pm$ 3.5 mmol/ 1) under six months. Urinary sodium and chloride levels were lower in the longer time of enteral nutrition patients than those of shorter period of enteral nutrition patients (p < .05). While serum phospholipid level was higher in the patients over six months of enteral nutrition, other blood biochemical parameters and electrolyte concentrations did not show any differences with the duration of enteral nutrition. Neurosurgery patients in the ICU undergoing long-term enteral nutrition tube-feeding were malnourished and had a variety of metabolic complications. The duration of enteral nutrition could affect the patients' nutritional status, biochemical parameters, and electrolytes balance. The patients who require nutritional support over an extended time need the continuous follow-up care and monitoring by the nutrition support team for laboratory, clinical, and nutritional assessments.

A Study of the Nutritional Status and its Related Factors in the Elderly Hemodialysis Patients (노인 혈액투석 환자의 영양상태와 이에 관련된 요인에 관한 연구)

  • The purpose of this study was to evaluate the nutritional status and to identify related factors in elderly hemodialysis patients. Sixty-four patients who were registered in Asan Medical Center, Seoul, Korea, participated in the study. The data was collected between September and December, 2003. General characteristics were obtained with the use of an interviewer-administered questionnaire. Anthropometric and biochemical data, 3-day dietary records and dietary habits were also obtained. The results of this study were analyzed with t-test or $X^2$-test using SPSS package program. The percentage of elderly hemodialysis patients who were undergoing mild to severe malnutrition (MN group) and were normal nutrition (NN group) by subjective global assessment (SGA) criteria were 46.9% and 53.1 %, respectively. Appetite (p < 0.05) and dietary cholesterol intakes (p < 0.05) were significantly lower in MN group than the NN group. Body mass index (BMI, p < 0.001), lean body mass (LBM, p < 0.05), triceps skinfold thickness (TSF, p < 0.01), mid ann circumference (MAC, p < 0.01) and mid ann muscle circumference (MAMC, p < 0.05) were also lower in the MN group than the NN group. There were also significant differences in blood urea nitrogen (BUN, p < 0.05), creatinine (p < 0.05), prealbumin (p < 0.001) and C-reactive protein (CRP, p < 0.05). SGA was negatively correlated with serum BUN, creatinine and pre albumin, dry weight, BMI, LBM, total body water, TSF, MAC and MAMC, and positively correlated with age. In stepwise multiple regression analysis, SGA was related to BMI, CRP, age and BUN. In conclusion, almost half of the subjects were in malnourished status and had lower values in anthropometric and biochemical data. Our results suggest that SGA is a simple and adequate method for assessing the nutritional status in elderly hemodialysis patients and adequate dietary guidelines based on individual nutritional status are needed in the patients.

Effects of Nutrition Education on Nutritional Status of Hemodialysis Patients (영양교육이 혈액투석환자의 영양상태에 미치는 효과)

  • Malnutrition is a common problem in patients undergoing maintenance hemodialysis (HD) and compromised food intake is an important cause. Malnutrition is one of the important factors influencing mortality in these patients. This study was conducted to evaluate the effectiveness of nutrition education at improving nutritional status of 23 Korean HD patients (mean : 48.6 $\pm$ 10.4 years, men : 8, women : 15). Anthropometric indices, nutrient intakes, and biochemical blood indices were measured before and after a 6-month nutrition education intervention. Anthropometric indices such as percent ideal body weight [PIBW (%)], body fat, body mass index (BMI), mid-arm circumference (MAC), mid-arm muscle circumference (MAMC), and calculated arm muscle area (CAMA) of subjects were within the normal range and not changed by nutrition education. Subjective global assessment (SGA) was significantly increased (p < 0.05) after nutrition education. Intake of total energy, carbohydrate, lipid, Ca, and vitamin B1 was increased significantly (p < 0.05) but intake of phosphorus, potassium, and sodium was decreased (p < 0.05). The serum concentrations of albumin, total protein, and Ca were significantly increased (p < 0.05), but levels of P and K were decreased (p < 0.05) after the intervention. These findings suggest that nutrition education for HD patients can be effective for positively changing nutrient intakes, leading to improvements in blood indices and nutritional status.

Changing Pattern and Comparison of Nutritional States before and after Nasogastric Tube Feeding for the Severe Brain Injury Patients in Critical Period (초급성기 중증 뇌 손상 환자의 영양 상태 변화양상과 인공영양 전·후의 영양 상태에 대한 비교)

  • Purpose: The purpose of the study were to examine the nutritional status of severe brain injury adult patients in critical period, and to compare the nutritional states before and after tube feeding. Methods: Data from 19 patients admitted to the SICU in a university hospital due to severe brain injury were analyzed. Nutritional states were measured by anthropometric and blood biochemical indicators. Results: MAC and MAMC were significantly decreased only at 7 days after admission compared with those on the day of admission. TSF was significantly decreased from 7 days to 14 days after admission. Fat rate was significantly decreased from 3 days to 14 days after admission. Hb was significantly decreased only at 3 days after admission. Albumin was significantly decreased from 3 days to 14 days after admission. However, lymphocyte was significantly increased at 14 days after admission. TSF and Albumin became significantly worse even after initiating tube feeding. Conclusions: Nutritional status of severe brain injury patients in SICU became worse after admission whichever indicators were adopted to evaluate nutritional status, anthropometric or blood biochemical indicators, and became worse even after initiating tube feeding.

Clinical and radiographical evaluation of mineral trioxide aggregate, biodentine and propolis as pulpotomy medicaments in primary teeth

  • Objectives: The purpose of this study was to evaluate the efficacy of mineral trioxide aggregate (MTA), Biodentine and Propolis as pulpotomy medicaments in primary dentition, both clinically and radiographically. Materials and Methods: A total of 75 healthy 3 to 10 yr old children each having at least one carious primary molar tooth were selected. Random assignment of the pulpotomy medicaments was done as follows: Group I, MTA; Group II, Biodentine; Group III, Propolis. All the pulpotomized teeth were evaluated at 3, 6, and 9 mon clinically and radiographically, based on the scoring criteria system. Results: The clinical success rates were found to be similar among the three groups at 3 and 6 mon where as a significant decrease in success rate was observed in Group III (84%) compared to both Group I (100%) and Group II (100%) at 9 mon. Radiographic success rates over a period of 9 mon in Groups I, II, and III were 92, 80, and 72%, respectively. Conclusions: Teeth treated with MTA and Biodentine showed more favorable clinical and radiographic success as compared to Propolis at 9 mon follow-up.

Characteristics in Nutritional Status of Patients on Hemodialysis and Continuous Ambulatory Peritoneal Patients in Chonbuk Area (전북지역 혈액투석 환자와 지속성 외래 복막 투석 환자의 영양상태 비교)

  • The present study was designed to compare the nutritional status of hemodialysis (HD) and continuous ambulatory peritoneal dialysis (CAPD) patients. Subjects were 58 HD patients (male/female = 29/29) and 33 CAPD patients (male/female = 23/10) undergoing dialysis treatment in Artificial Kidney Unit of Chonbuk National University Hospital. For nutritional assessment, %IBW (ideal body weight), %TSF (tricep skin fold), %MAC (mid arm circumference), %MAMC (mid arm muscle circumference), serum albumin, serum transferrin, TLC (total lymphocyte count), SGA (subjective global assessment) and estimated energy and protein intakes by 1-month food frequency method were used. Between HD and CAPD group, mean age (50 $\pm$ 12 vs. 52 $\pm$ 12 yr), dialysis durations (37 $\pm$ 36 vs. 30 $\pm$ 26 mon), dietary energy intakes (28.3 $\pm$ 9.0 vs. 28.8 $\pm$ 8.6 kcal/kg/day), dietary protein intakes (1.1 $\pm$ 0.4 vs. 1.2 $\pm$ 0.3 g/kg/day) and incidence of co-morbid conditions (69.0% vs. 69.7%) were not significantly different. Data by using SGA showed a higher incidence of malnutrition in CAPD patients (45.6%) than in HD patients (36.2%). %IBW (p < 0.001), %TSF (p < 0.001) and %MAC (p < 0.001) were higher in CAPD patients than were in HD patients. But serum albumin (p < 0.001) and transferrin (p < 0.001) were significantly lower in CAPD patients than were in HD patients. A higher incidence of malnutrition was shown in CAPD Patients than in HD Patients due to different dialysis type. A significant finding was that CAPD showed protein deficient malnutrition and HD did calorie deficient malnutrition. It suggests that an adequate dietary intake considering dialysis type prevents a prevalence of malnutrition.
