유방절제술 후 발생하는 림프부종 환자에 대한 림프마사지의 효과 (The Effects of Lymph Massage on the Patients with Lymphedema after Mastectomy)

  • Purpose: to apply lymph massage to the patients that contracted lymphedema after mastectomy to resolve blood congestion, stimulate circulation, and control swelling long-term. Methods: The subjects include 20 female patients that were diagnosed with breast cancer histologically and contracted lymphedema within one to two years after mastectomy. They received lymph massage from March to August, 2008. Using a measuring tape(capable of measuring mm), the arm circumference was measured in both the sick and normal arm. Only the data from the sick arm were compared before and after the treatment. The measuring points were five; the back of the hand(a certain distance[about 10cm] was set between the end of the middle finger to the center of the palm), the wrist(the smallest section), the lower arm(at a certain distance[about 10cm] from the internal bending part of the elbow), the elbow(the bending part), and the upper arm(at a certain distance[about 10cm] from the internal bending part of the elbow). Collected data were analyzed using the SPSS/WINDOWS Version 12.0 program. Results: There was a reduction of lymphedema observed in all the points including 10cm in the upper arm, the elbow, 10cm in the lower arm, the wrist and the back of the hand after lymph massage, and the reduction had statistically significant differences(p<.05). Conclusion: Those results indicate that lymph massage has effects on the reduction of lymphedema in the early patients that contracted it after mastectomy.

림프마사지 교육프로그램이 유방절제술환자의 상지 기능상태와 삶의 질에 미치는 효과 (Effects of Educational Program of Manual Lymph Massage on the Arm Functioning and the Quality of Life in Breast Cancer Patients)

  • Purpose: The purpose of this study was to determine the effect of EPMLM(educational program of manual lymph massage) on the arm functioning and QOL(quality of life) in breast cancer patients with lymphedema. Method: Subjects in the experimental group(n=20) participated in EPMLM for 6 weeks from June to July, 2005. The EPMLM consisted of training of lymph massage for 2 weeks and encourage and support of self-care using lymph massage for 4 weeks. The arm functioning assessed at pre-treatment, 2weeks, and 6weeks using Arm functioning questionnaire. The QOL assessed at pre-treatment and 6 weeks using SF-36. The outcome data of experimental group was compared with control group(n=20). The collected data was analyzed by using SPSS 10.0 statistical program. Result: The arm functioning of experimental group was increased from 2 weeks after(W=.224, p=.011) and statistically differenced with control group at 2 weeks(Z=-2.241, p=.024) and 6 weeks(Z=-2.453, p=.013). Physical function of QOL domain increased in experimental group(Z=-1.162, p=.050), also statistically differenced with control group(Z=-2.182, p= .030) at 6weeks. Conclusion: The results suggest that the educational program of manual lymph massage can improve arm functioning and physical function of QOL domain in breast cancer patients with lymphedema.

유방암 절제술 후 발생한 림프부종 환자의 치험1례 (A Case Report of lymphedema due to axillary lymph node dissection)

  • Objectives : The purpose of this case is to report the improvement of a patient with lymphedema due to axillary lymph node dissection after acupuncture therapy and herbal medicine. Methods : We treated the patient with Dong-Qi acupuncture, herbal medication(木通大安湯), lymph absorption massage, compresses, abdominal breathing. We measured the circumference of the patient's elbow and wrist every day at 6pm. Results : Since being hospitalized, the circumference of the patient' elbow and wrist has been decreased and VAS score has gotten lower gradually. Conclusions : We considered Dong-Qi acupuncture, herbal medication had a useful effect on lymphedema due to axillary lymph node dissection with lymph absorption massage, compresses, abdominal breathing.

아로마 마사지가 림프부종에 미치는 영향 (Effects of Aromatherapy Massage on Edema Reduction in the Treatment of Lymphedema)

  • The purpose of this study was to determine whether aromatherapy massages with manual lymph drainage (MLD) are significantly more effective than sham massages with MLD in reducing edematous limb volume in lymphedema. This study was performed on 46 patients who had developed unilateral upper or lower lymphedema. Twenty-three patients served as the experimental group and were treated with complex decongestive physiotherapy (CDP) applications including aroma massages with MLD, multi layered compression bandages, remedial exercises and skin care. Twenty-three patients in the control group were treated with CDP applications including sham massages with MLD, bandages, exercises and skin care. Patients undergo a therapy program once a day; 5 days a week for 2 weeks. Percentage excess volume (PEV) and body mass index (BMI) were recorded before and after treatment. PEV and BMI were significantly decreased before and after treatment in the experimental as well as in the control groups (p<.05). However, the percentage excess volume and BMI were not significantly improved after treatment between the two groups (p>.05). This study there suggests that aroma massages is not effective in the edema of patients with lymphedema.

상지의 림프종에 적용한 PNF 기법의 효과 (The Effects of PNF Techniques on Lymphoma in the Upper Limbs)

  • Purpose : The purpose of study was aimed to identify whether the painless dynamic PNF techniques makes any differences in reducing lymphedema by applying this technique to lymphedema patients, and to provide the basic reference data for the purpose of applying to lymphedema patients. Methods : A total of 40 women participated in this study, and they did PNF techniques before applying lymph compression bandages. Group 1 of 20 subjects performed PNF techniques three times a week with 30 minutes each time. Group 2 of 20 subjects only practiced edema reducing massaging for 30 minutes. Results : In addition, the interaction between treatment method and treatment time was significant (p<.0001), which indicates that the changes in edema rates by measurement times appear differently according to treatment methods. In this study, it means that the 'PNF techniques' group has a steeper slope of decline than the 'message' group. Conclusion : In conclusion, both massaging and PNF techniques helped to lower edema rates. Four weeks after the beginning of treatment, PNF techniques was exhibited as generating a large degree of decline in edema rates than massaging.

유방암환자 전초림프절의 유방 림프신티그라피에 관한 연구 (The Study for Breast Lymphoscintigraphy of Sentinal Lymph Node in breast cancer)

  • 과거 유방암 환자에서 대부분을 수술로 불필요한 액와림프절 절개술을 하였지만, 그 결과로 부작용이 발생하였다. 본 논문에서는 유방암 환자에게 $^{99m}Tc-Tincolloid$를 이용하여 수술 전 유방림프절 검사를 먼저 시행한 뒤 처음 나오는 전초림프절을 영상화한 뒤 전초림프절의 전이 여부에 따라 수술범위를 달리 결정함으로써 환자의 수술 시간 및 수술 후의 부작용 최소화가 목적이다. 논문의 실험 결과는 실험대상 20명의 환자 중 15명에서는 액와림프절 절개술을 시행하지 않아도 되기에 수술로 인한 부작용을 줄일 수 있다. 그러나 아직까지 표준방법, 즉 방사성의약품 주사부위, 방사선량, 주사량, 마사지여부, 림프신티그라피 시행여부 등이 결정되지 않았으므로, 환자에 대한 고통경감과 부작용을 최소화 하고 수술부위를 최소화하기 위해서는 유방암 림프절검사법이 유용할 것이라 생각된다. 실험의 대상이 되는 환자는 초음파, 유방촬영술, 생검에서 유방암 진단을 받고 수술할 예정인 환자 20명으로, 환자의 연령 범위는 $31{\sim}71$세로 평균 연령은 45.4세이며, 임상 병기 I이 9명, 임상병기 II가 11명이다. 실험 장비로는 전초림프절을 묘출시켜주는 추적자로 $^{99m}Tc-Tincolloid(Amesham)$과 전초림프절의 위치를 찾는 장비로 Micro Probe : Neoprobe 2000(이하 감마 프로브), 영상 묘출 장비로 MS-II Gamma Camera : SIEMENS (이하 MS-II Gamma Camera)가 사용되었다. 실험 방법은 3가지 방법을 사용하며, 그 중 하나를 선택해 전초림프절 검사 및 수술실에서 감마프로브를 이용하여 생검을 실시한 결과의 20명 모두에서 전초림프절을 영상화 했다. 그리고 감마프로브로 전초림프절을 찾아 조직 생검 결과 20명 중에서 전초림프절의 전이가 발견된 환자는 5명이다. 본 실험에서 전이가 발견된 환자 5명을 다시 액와림프절을 생검한 결과의 3명에서 액와림프절 전이가 발견되었다.

말기 암 환자에서의 하지 부종 (Lower Extremity Edema in Terminal Cancer Patients)

  • 목적: 하지 부종은 진행된 말기 암 환자에서 흔하게 발생하는 증상이며 치료하기 어렵다고 생각되고 있다. 우리는 말기 암 환자에서 하지 부종의 특성을 분석하고 원인과 치료에 관한 기본 정보를 얻기 위해 연구를 하였다. 방법: 하지 부종이 있는 환자에서 이학적 검사를 시행하고 부종의 범위, 정도, 혈액 검사(creatinine, albumin)을 시행하였다. 심부정맥 혈전증이 의심되는 환자는 Doppler 초음파를 시행하고 림프부종이 의심되는 환자는 복부 컴퓨터 단층 촬영을 시행하였다. 부종 정도는 NCI의 림프부종 평가를 이용하여 시행하였으며 증상의 호전은 부종의 정도가 1 이상 감소한 경우 호전이라 하였다. 결과: 2003년 3월부터 2004년 1월까지 총 154명의 호스피스 입원 환자 중 하지부종 환자는 39명이었으며 이 중 6명은 상지부종과 하지부종이 모두 있었다. 이 들의 원인 질환은 위암(7명), 폐암(6명), 간담도암(5명), 간암(5명), 대장암(5명), 췌장암(2명), 기타(9명)이었다. 원인으로써는 저알부민혈증(11명), 림프부종(10명), 심부정맥혈전증(7명), 하대정맥 또는 문정맥 폐쇄(6명), 의존성 부종(5명)이었다. 치료로는 하지 거상과 이뇨제를 사용하였으며 심부정맥 혈전증이 있는 경우는 탄력 스타킹을, 림프부종이 있는 경우에는 맛사지와 공기 압축을 이용한 재활 치료를 하였다. 전체 2/3환자에서 증상 호전이 관찰되었다. 결론: 말기 암 환자에서 하지 부종의 빈도는 25.3%로 높으며 원인 질환으로는 저알부민 혈증, 림프부종, 심부정맥혈전증, 의존성 부종 등 다양하다. 적극적인 비침습적 치료로 충분한 증상호전을 가져 올 수 있다.

유방암 환자의 항암화학요법 후 한방병원 입원치료에 대한 일개 한방병원 환자 특성 및 치료 고찰 (Analysis of the Patients and Treatment of Korean Medicine Hospital after Chemotherapy in Patients with Breast Cancer)

  • Objectives: The purpose of this study is to analyse current status and treatment of the Korean medicine hospital after chemotherapy in patients with breast cancer. Methods: We investigated the medical records of 21 patients who admitted to Korean medicine hospital after chemotherapy in patients with breast cacner from March 1, 2017 to December 31, 2019. We searched medical records retrospectively and analyzed current status and treatment of Korean medicine hospital. Results: The average age of 21 participants was 52.81±8.38 years and 40s and 50s accounted for 85.6% of the total. After receiving chemotherapy, the average time to hospitalization was 1.87±3.13days and average hospital stay was 9.78±4.14 days. The subjects were classified as 28.6% of stage I, 52.4% of stage II, 9.5% of stage III, and 9.5% of stage IV. The analysis according to the presence of metastasis was 57.1% without metastasis, 33.3% with axillary lymph node metastasis, and 9.5% with distant metastasis. The main symptoms complained when hospitalized by 21 subjects were nausea (54.2%), fatigue (54.2%), and anorexia (50.8%) in over 50%, pantalgia (47.5%), and insomnia (47.5%), dizziness (44.1%), cold sweating (42.4%), lower extremity pain (40.7%), 37.5~37.9℃ fever (39.0%), headache (37.3%), hot flush (37.3%), pruritus (30.5%) are 30% or more. Korean medicine treatment was performed in 87.4% of all hospitalizations and Gwakhyangjunggi-san-gami was the most administered prescription. Extracts of Korean medicine was performed in 100.0% of all patients and Eunkyo-san was most administered extracts medicine. Acupuncture, moxibustion, and cupping treatments were performed in all 21 study subjects. Other treatments was performed at a frequency of hyperthermia (90.5%), lymph massage (23.8%), air compression therapy (23.8%), and Interference current therapy (19.0%) Conclusion: Korean traditional medicine can be used as a countermeasure for side effects after chemotherapy in breast cancer patients.

림프부종 환자의 자가관리에 관한 실태 조사 (A Study on Self-care among the Lymphedema Patients)

  • Purpose: The purpose of this descriptive study was to gain basic data to develop a self-care protocol for the lymphedema patients. Method: The subjects of this study consisted of 115 patients with lymphedema from 8 hospitals and two community health and welfare centers in Busan and Seoul. The data was collected with questionnaire by self reporting of patients between March 2001 and December 2001. Data was analyzed by mean and percentage. Result: For self-care activities in daily life, compliance of 'use skin care preparations', 'use heat and cold', 'protect from local compression on affected limbs', 'protect from insect biting', 'use aids to protect affected limbs', 'take diuretics and take protein diet' did not reach to 50%. For self-care activities related to complex physical therapy, 28.7% of subjects complied with compression garment, 14.8% with manual lymph drainage, and 13.0% with exercise. 20.0% of subjects tried to treat with acupuncture and 13.9% with heat therapy. Conclusion: From this study, it is suggested that patients need to get a self-care education with correct information about self care activities and health care professionals need to develop more convenience self-care techniques of massage and exercise.
