폐 이식 수술의 10년 치험 (The 10 Years Experience of Lung Transplantation)

  • 배경: 폐 이식 수술은 호흡부전을 동반한 말기 폐질환 환자에서 유용한 치료방법 중의 하나로 본원에서는 1996년 국내에서 처음으로 일측 폐 이식 수술을 성공한 이래로 현재까지 재 이식 2예를 포함한 총 13예의 폐 이식 수술을 시행하였다. 저자 등은 그동안 환자들의 수술성적 및 합병증, 생존율 등을 분석하고자 하였다. 대상 및 방법: 1996년 7월부터 2005년 7월까지 영동세브란스병원 흉부외과에서 폐 이식을 시행받은 13예, 11명의 환자(2명은 재 이식환자)를 대상으로 후향적으로 임상기록지를 분석하였다. 결과: 남녀비는 9:4, 평균연령은 $45.2{\pm}10.7$세(범위 $25{\sim}59$세)였으며, 폐기종 및 만성폐쇄성 폐질환이 5예로 가장 많았고, 동맥관 개존증으로 인한 아이젠맹거 증후군 2예, 폐섬유증, 폐고혈압, 림프관 평활근종증, 기관지확장증이 각 1예였다. 조기 합병증으로는 출혈, 이식 폐 부전, 감염이며 후기 합병증으로는 감염 및 이식 후 림프증식증이었다. 조기사망 3예를 제외한 평균생존기간은 16.5개월($2{\sim}60$개월)이었다. 재 이식 2예는 이식 폐의 기능 부전에 의하여 각각 첫 이식 후에 2주, 13개월째 시행 받았다. 결론: 폐 이식 수술 후 장기성적을 향상시키기 위해서는 수술술기의 향상뿐만 아니라 수술 후 집중적인 환자관리를 통하여 합병증을 조기에 발견하고 필요한 내과적, 외과적 처치를 즉시 시행하는 것이 예후에 좋은 영향을 미칠 것으로 생각된다.

미만성 폐질환에 대한 폐생검의 의의 (Open Lung Biopsy for Diffuse Infiltrative Lung Disease)

  • Retrospective review of 26 patients undergoing open lung biopsy at the Yonsei University during 10 years period was conducted to evaluate open lung biopsy for DILD. From January 1980 to August 1990, open lung biopsy was performed in 26 patients through a limited thoracotomy incision[a limited anterior or a posterolateral thoracotomy]. Open lung biopsy was indicated for diffuse interstitial pulmonary diseases undiagnosed by indirect clinical and radiological diagnostic methods. The types of incision were limited anterior[11] and limited posterolateral[15]. Preoperative evaluation of the lung disease included sputum culture[26], sputum cytology [19], bronchoscopy[9] and TBLB[7]. In 23 patients the histologic appearances after open lung biopsy were sufficiently specific histologic pictures to confirm diagnosis. The results of the biopsies changed usual therapeutic plan in 17 patients among them. The complications were resp. insufficiency[3], pulmonary ed6ma[3], sepsis[2], and others[3] in 6 patients. Diagnosis from the open lung biopsy was included respiratory pneumonia[7], fibrosis[7], infection[5], malignancy[2], others[5]. 4 patients died of respiratory insufficiency. The causes of the other three death were not due to direct result of the biopsy itself. Open lung biopsy in the patient with a diffuse infiltrative lung disease is an one of the accurate diagnostic method and frequently leads to change of the therapeutic plans. So we conclude that open lung biopsy remains our diagnostic method of choice in diffuse infiltrative lung disease undetermined etiology.

Analysis of 1,067 Cases of Video-Assisted Thoracic Surgery Lobectomy

  • Background: Video-assisted thoracic surgery (VATS) lobectomy has been performed with increasing frequency over the last decade. However, there is still controversy as to its indications, safety, and feasibility. Especially regarding lung cancer surgery, it is not certain whether it can reduce local recurrences and improve overall survival. Materials and Methods: We retrospectively reviewed 1,067 cases of VATS lobectomy performed between 2003 and 2009, including the indications, postoperative morbidity, mortality, recurrence, and survival rate. Results: One thousand and sixty seven patients underwent VATS lobectomy for the following indications: non-small cell lung cancer (NSCLC) (n=832), carcinoid tumors (n=12), metastatic lung cancer (n=48), and benign or other diseases (n=175). There were 63 cases (5.9%) of conversion to open thoracotomy during VATS lobectomy. One hundred thirty one (15.7%) of the 832 NSCLC patients experienced pathologic upstaging postoperatively. The hospital mortality rate was 0.84% (9 patients), and all of them died of acute respiratory distress syndrome. One hundred forty-nine patients (14.0%) experienced postoperative complications. The median follow-up was 22.9 months for patients with NSCLC. During follow-up, 120 patients had a recurrence and 55 patients died. For patients with pathologic stage I, the overall survival rate and disease-free survival rate at 3 years was $92.2{\pm}1.5%$ and $86.2{\pm}1.9%$, respectively. For patients with pathologic stage II disease, the overall survival rate and disease-free survival rate at 3 years was $79.2{\pm}6.5%$ and $61.9{\pm}6.6%$, respectively. Conclusion: Our results suggest that VATS lobectomy is a technically feasible and safe operation, which can be applied to various lung diseases. In patients with early-stage lung cancer, excellent survival can be also achieved.

  • One-lung ventilation (OLV) is the isolation and selective ventilation of one lung field. OLV allows the collapse of lung lobes on the side of the thoracic surgical approach to facilitate observation of intrathoracic structures and to achieve lung immobility. OLV be achieved by endotracheal intubation with double lumen tubes or bronchial blockers. In this study, cardiopulmonary consequences of two-lung ventilation (TLV), OLV and Re-TLV (TLV after OLV) were evaluated in 5 dogs. The dogs were anesthetized with mask induction and maintained with isoflurane in oxygen. Tidal volume and respiratory rates were set to maintain end-tidal $CO_2$ at $40{\pm}2mmHg$ during instrumentation. Following instrumentation, the dogs were placed in right lateral recumbency and induced spontaneously respiration state. Effect of TLV on hemodynamic and pulmonary variables were recorded. Then, the left bronchus was obstructed by endotracheal intubation with double lumen endotracheal tube to achieve OLV state and recording was continued. After OLV, double lumen endotracheal tube was extubated, and standard endotracheal tubes was intubated again. In this study, spontaneous OLV caused significant decrease in $PaO_2$, arterial oxygen saturation, mixed-venous oxygen saturation, and increase in $PaCO_2$. Especially, a significant elevation in $PaCO_2$ and respiratory acidosis were remarkable findings. So spontaneous ventilation in OLV affected gas exchange and hemodynamic function.

폐장의 반흔에서 발생하는 반흔암종 1례 보고 (Peripheral Lung Cancer Arising in Lung Cancer -A case report-)

  • On the right lower lobe, scar carcinoma was presented in the Korean male, 56 years old, in February, 1982. His tumor is moderate differentiated adenocarcinoma. Scar carcinoma, a rare form of adenocarcinoma, is usually discovered on autopsy and rarely on surgical resected specimen and these tumors are found in areas of lung scarring. Most of the tumors are adenocarcinoma and found in the upper lobes and related to infarcts, tuberculous scars. No relationship between smoking and scar cancers were reported. The scar cancers are becoming more common. An apparent increase in scar cancer in the periphery of the lung was reported in America. Scar cancer does not present special clinical symptoms and signs, except manifestations of surrounded tissue compressed by large tumors in far advanced stage. These tumors progress slowly and metastasize late but characteristic signs on scar area of the lung can be discovered from the early stage on X-ray examination [solitary peripheral nodule and scar]. So careful study on chest x-ray film was acquired and if necessary, conservative resection of the lesion is indicated because the long term prognosis is not at all bad.

폐관류스캔에 의한 폐절제술후 폐기능 예측 (Prediction of Postoperative Pulmonary Function Following Thoracic Operations - Perfusion Lung Scanning Method -)

  • The purpose of this study is to predict postoperative lung function by perfusion lung scanning method. 40 patients who underwent lobectomy or pneumonectomy between 1983-1985 were analyzed. Mean preoperative FEV1 was 2.36 L in lobectomy cases and 1.73 L in pneumonectomy cases. Preoperative and postoperative lung function were measured by routine spirometry in sitting position. Perfusion lung scanning was performed by 99mTc-MAA radioisotope. Postoperative FEV1 and VC were predicted by the formula; Postoperative FEV1 [VC]=Preoperative FEV1 [VC] x percent function of regions of lung not to be resected. In this study, I concluded that perfusion lung scanning is a simple and useful method to predict postoperative ventilatory function after pneumonectomy of lobectomy.

전폐절제술시 폐관류스캔을 이용한 폐기능의 예측에 대한 평가 (Evaluation of the Predictive Pulmonary Function after Pneumonectomy Using Perfusion Lung Scan)

  • Surgical resection of lung cancer or other disease is recently required in patients with severely impaired lung function resulting from chronic obstructive pulmonary disease or disease extension. So prediction of pulmonary function after lung resection is very important in thoracic surgeon. We studied the accuracy of the prediction of postoperative pulmonary function using perfusion lung scan with 99m technetium macroaggregated albumin in 22 patients who received the pneumonectomy. The linear regression line derived from correlation between predicting[X and postoperative measured[Y values of FEV1 and FVC in patients are as follows: 1 Y[ml =0.713X + 381 in FEV1 [r=0.719 ,[P<0.01 2 Y[ml =0.645X + 556 in FVC [r=0.675 ,[P<0.01 In conclusion,the perfusion lung scan is noninvasive and very accurate for predicting postpneumonectomy pulmonary function.

Utilization of Supplemental Regional Anesthesia in Lobectomy for Lung Cancer in the United States: A Retrospective Study

  • Background: Pulmonary lobectomy is the standard of care for the treatment of early-stage non-small cell lung cancer. This study investigated the rate of utilization of supplemental anesthesia in patients undergoing video-assisted thoracoscopic surgery (VATS) or open lobectomy using a national database and assessed the effect of regional block (RB) on postoperative outcomes. Methods: Patients who underwent lobectomy for lung cancer between 2014-2019 were identified in the American College of Surgeons National Surgical Quality Improvement Program. The patients' primary mode of anesthesia and supplemental anesthesia were recorded. Preoperative characteristics and postoperative outcomes were compared between 2 surgical groups: those who underwent general anesthesia (GA) alone versus GA with RB. Multivariable regression analyses were performed on the outcomes of interest. Results: In total, 13,578 patients met the study criteria, with 87% undergoing GA and the remaining 13% receiving GA and RB. The use of neuraxial anesthesia decreased over the years, while RB use increased up to 20% in 2019. Age, body mass index, and preoperative comorbidities were comparable between groups. Patients who underwent VATS were more likely to receive RB than those who underwent thoracotomy. RB was most often utilized by thoracic surgeons. An adjusted analysis showed that RB use was associated with shorter hospital stays and a reduced likelihood of prolonged length of stay, but a higher rate of surgical site infections (SSIs). Conclusion: In a large surgical database, there was underutilization of supplemental anesthesia in patients undergoing lobectomy for lung cancer. RB utilization was associated with a shorter length of hospital stay and an increase in SSI incidence.

Contralateral Pulmonary Resection after Pneumonectomy

  • Background: Contralateral pulmonary resection after pneumonectomy presents considerable challenges, and few reports in the literature have described this procedure. Methods: We retrospectively reviewed the medical records of all patients who underwent contralateral lung resection following pneumonectomy for any reason at our institution between November 1994 and December 2020. Results: Thirteen patients (9 men and 4 women) were included in this study. The median age was 57 years (range, 35-77 years), and the median preoperative forced expiratory volume in 1 second was 1.64 L (range, 1.17-2.12 L). Contralateral pulmonary resection was performed at a median interval of 44 months after pneumonectomy (range, 6-564 months). Surgical procedures varied among the patients: 10 underwent single wedge resection, 2 were treated with double wedge resection, and 1 underwent lobectomy. Diagnoses at the time of contralateral lung resection included lung cancer in 7 patients, lung metastasis from other cancers in 3 patients, and tuberculosis in 3 patients. Complications were observed in 4 patients (36%), including acute kidney injury, pneumothorax following chest tube removal, pneumonia, and prolonged air leak. No cases of operative mortality were noted. Conclusion: In carefully selected patients, contralateral pulmonary resection after pneumonectomy can be accomplished with acceptable operative morbidity and mortality.

폐좌상의 정량분석 (Quantitative Analysis of Lung Contusion)

  • Lung contusion due to blunt chest trauma is the most common lung injury and correlated with the clinical course and prognosis. Its diagnosis by CT[Computerized Tomogram] gives a more clear and understandable three dimensional view, by which we are able to measure the volume of the contused and entire lung. Other variables are arterial blood gas, number of rib fracture, presence of hemopneumothorax, sternal fracture and clavicle fracture, number of associated non-thoracic injuries, ventilator time and presence of pulmonary complication. Percentage[%] of lung contusion are expressed as mean $\pm$ standard deviation and data analysis was performed by means of multivariate repeated measures analysis of variance to detect significant differences in variables between positive thoracic injury group and negative group. The paired t-test was used. Differences of percentage of lung contusion between groups were assessed by one-way analysis of variance. Simple linear regression was used to perform correlation analysis in the number of rib fracture and ventilator time. A p value less than 0.05 was considered statistically significant. Pneumothorax and the number of associated other injuries affect the amount of lung contusion and pulmonary complication group has more contused lung volume. Arterial blood gas study shows no correlation with the amount of lung contusion statistically. The number of rib fracture correlated with the amount of lung contusion, which also correlated with ventilator time[r=0.56, p<0.05]. In conclusion, quantitative anlysis of lung contusion by CT predicts the clinical course and treatment such as ventilator care.
