Adverse Effects of Air Pollution on Pulmonary Diseases

  • Environmental exposure to air pollution is known to have adverse effects on various organs. Air pollution has greater effects on the pulmonary system as the lungs are directly exposed to contaminants in the air. Here, we review the associations of air pollution with the development, morbidity, and mortality of pulmonary diseases. Short-and long-term exposure to air pollution have been shown to increase mortality risk even at concentrations below the current national guidelines. Ambient air pollution has been shown to be associated with lung cancer. Particularly long-term exposure to particulate matter with a diameter <2.5 ㎛ (PM2.5) has been reported to be associated with lung cancer even at low concentrations. In addition, exposure to air pollution has been shown to increase the incidence risk of chronic obstructive pulmonary disease (COPD) and has been correlated with exacerbation and mortality of COPD. Air pollution has also been linked to exacerbation, mortality, and development of asthma. Exposure to nitrogen dioxide (NO2) has been demonstrated to be related to increased mortality in patients with idiopathic pulmonary fibrosis. Additionally, air pollution increases the incidence of infectious diseases, such as pneumonia, bronchitis, and tuberculosis. Furthermore, emerging evidence supports a link between air pollution and coronavirus disease 2019 transmission, susceptibility, severity and mortality. In conclusion, the stringency of air quality guidelines should be increased and further therapeutic trials are required in patients at high risk of adverse health effects of air pollution.

폐암에서 혈중 Cytokeratin 8, 18, 19의 진단적 가치 (Diagnostic Value of Serum Cytokeratin 8, 18 and 19 in Lung Cancer)

  • 배 경 : 지금까지 폐암의 종양표지자로 알려진 것들 중에 최근에는 CK 19뿐만 아니라 폐암에서 많이 발견되는 CK 8, 18에 대해 특이적인 세가지 단클론항체를 이용한 면역방사선계수법(MonoTotal$^{TM}$)이 개발되어 폐암의 진단에 이용할 수 있는 것으로 보고되었다. 따라서 저자들은 Cyfra 21-1에 비교하여 CK 8, 18, 19의 폐암표지자로서의 진단적 가치에 대해 연구하였다. 대상 및 방법 : 2002년 5월 l일부터 2002년 9월 30일까지 서울대학교병원 내과에 입원하여 기관지내시경 또는 객담도말세포검사, 경피적 조직생검으로 확진된 원발성 폐암환자 129명을 폐암군으로 하고 폐암이 배제된 비악성 호흡기 질환자 160명을 환자 대조군으로 연구에 동의한 건강한 성인 57명을 정상 대조군으로 하여 본 연구를 시행하였다. Cyfra 21-1의 측정은 Fujibebio Diagnostics사(USA)의 immnoradiometric assay kit인 Cyfra$^{TM}$ 21-1을 사용하였고 CK 8, 18, 19의 측정은 IDL biotech사의 2-hour immunoradiometric sandwich test kit인 Monototal$^{TM}$을 사용하였다. 결 과 : 폐암의 조직학적 분류에 따른 Monototal의 혈중평균치(mean${\pm}$SD)는 비소세포폐암이 $412.47{\pm}455.45$ U/L, 소세포폐암이 $237.08{\pm}145.15$ U/L로 환자 대조군 $126.54{\pm}95.72$ U/L, 정상 대조군 $63.68{\pm}31.66$ U/L에 비해 유의하게 증가되어 있었다(p<0.01). Monotoal의 정상범위를 정상 대조군의 가장 높은 수치인 188U/L 이내로 하였을때 비소세포폐암의 민감도는 66.4%, 특이도는 81.9%였다. 병기를 I,II군(32명)과 III,IV군(84명)의 두 군으로 나누어 Monototal의 평균을 비교해 보았을 때 각각 $227.6{\pm}185.2$ U/L, $482.9{\pm}506.4$ U/L로 두 군간에 통계적으로 유의한 차이가 있었다(p<0.001). 환자대조군 세부질환별로 Monototal의 평균치를 보면 미만성폐질환 환자군(11명)에서 $298.7{\pm}210.0$ U/L로 환자 대조군 평균치인 $126.54{\pm}95.72$ U/L에 비해 통계적으로 유의하게 높게 측정되었다(p<0.001)(Table 4). 결 론 : cytokeratin 8, 18, 19의 측정은 폐암의 대표적인 종양 표지자중의 하나인 Cyfra 21-1과 비슷한 민감도와 특이도를 보이고 있어 효과적인 종양표지자로 앞으로 사용될 수 있다.

Risk Factors for Lung Cancer in the Pakistani Population

  • Background: Lung cancer is one of the most prevalent malignancies in the world and both incidence and mortality rates are continuing to rise in Pakistan. However, epidemiological studies to identify common lung cancer determinants in the Pakistani population have been limited. Materials and Methods: In this retrospective case-control study, 400 cases and 800 controls were enrolled from different hospitals of all provinces of Pakistan. Information about socio-demographic, occupational, lifestyle and dietary variables was extracted by questionnaire from all subjects. Odd ratios (ORs) and 95% confidence intervals (CIs) were calculated. and dose-response associations were also assessed for suitable factors. Results: Strong associations were observed for smoking (OR=9.4, 95%CI=6.9-12.8), pesticide exposure (OR=5.1, 95%CI=3.1-8.3), exposure to diesel exhaust (OR=3.1, 95%CI=2.1-4.5), red meat consumption (OR=2.9, 95%CI=1.8-4.7) and chicken consumption (OR=2.8, 95%CI=1.7-49). Other associated factors observed were welding fumes (OR=2.5, 95%CI=1.0-6.5), sedentary living (OR=2.0, 95%CI=1.6-2.6), family history (OR=2.0, 95%CI=0.8-4.9), wood dust (OR=1.9, 95%CI=1.2-3.1), tea consumption (OR=1.8, 95%CI=1.2-2.6), coffee consumption (OR=1.8, 95%CI=1.1-2.8), alcoholism (OR=1.7, 95%CI=1.1-2.5) and asbestos exposure(OR=1.5, 95%CI=0.5-4.4). Consumption of vegetables (OR=0.3, 95%CI=0.2-0.4), juices (OR=0.3, 95%CI=0.3-0.4), fruits (OR=0.7, 95%CI=0.5-0.9) and milk (OR=0.6, 95%CI=0.5-0.8) showed reduction in risk of lung cancer. Strongest dose-response relationships were observed for smoking ($X^2=333.8$, $p{\leq}0.0000001$), pesticide exposure ($X^2=50.9$, $p{\leq}0.0000001$) and exposure to diesel exhaust ($X^2=51.8$, $p{\leq}0.0000001$). Conclusions: Smoking, pesticide exposure, diesel exhaust and meat consumption are main lung cancer determinants in Pakistan. Consuming vegetables, fruits, milk and juices can reduce the risk of lung cancer risk, as in other countries.

An update on the role of bronchoscopy in the diagnosis of pulmonary disease

  • Bronchoscopy has evolved over the past few decades and has been used by respiratory physicians to diagnose various airway and lung diseases. With the popularization of medical check-ups and growing interest in health, early diagnosis of lung diseases is essential. With the development of endobronchial ultrasound, ultrathin bronchoscopy, and electromagnetic navigational bronchoscopy, bronchoscopy has been able to widen its scope in diagnosing pulmonary diseases. In this review, we have described the brief history, role, and complications of bronchoscopy used in diagnosing pulmonary lesions, from simple flexible bronchoscopy to bronchoscopy combined with several up-to-date technologies.

Butane gas가 흰쥐 혈청과 조직의 Lactatedehydrogenase 및 Cholinesterase에 미치는 영향 (The Effect of Butane gas on Rat Cholinesterase and Lactatedehydrogenase)

  • Acute poisoning with organic solvents and other volatile compounds now usually follows deliberate inhalation (volatile substance abuse) or ingestion of these compounds. The effect of butane gas inhalation was analyzed for serum, liver, brain, lung and muscle. And the observations are revealed on rat cholinesterase activity, lactatedehydrogenase activity and electrophoretic pattern of lactatedehydrogenase isozyme. The results are as follows: 1. The rat cholinesterase activity on serum, liver and muscle show the decreased by increasing of inhalation time of butane gas in particular the lung cholinesterase activity was greatly affected. 2. Butane gas inhalation brought out the lactatedehydrogenase activity increased of the serum and the tissues and had an important effect especially in both the liver and muscle 1actatedehydrogenase activities. 3. Each tissue was found to have a characteristic distribution of lactatedehy-drogenase isozymes on celluloseacetate electrophoresis and the development of inhalation time is shown the disappearance and diffusion of band. The toxicity of butane gas inhalation was most prominence in the liver and lung toxicity was occurred also.

전폐절제술에 관한 임상적 연구 (Clinical Evaluation of Pneumonectomy)

  • For study the influencing factors to the complication after pneumonectomy, authors performed retrospective analysis in 33 patients managed surgically from February 1985 to February 1994 in the Department of Thoracic and Cardiovascular Surgery, Pusan National University Hospital.Among 33 patients, the underlying diseases were distributed 15 patients[45.5% pulmonary tuberculosis, 14[42.% lung cancer and 4[12.1% bronchiectasis. Numbers of complication according to the underlying disease after pneumonectomy were 8 in pulmonary tuberculosis, 2 in lung cancer and 1 in brochiectasis. Study was analyzed on age, sex distribution, etiology of underlying diseases and operated sides contributing to the complication. The results were characterized that the rate of occurrence of complication after pneumonectomy was not affected by age, sex and operated side differences but affected by the underlying disease.The development of complication after pneumonectomy in patients with pulmonary tuberculosis revealed statistically borderline significance comparing to the others[p=0.07 .

청색증을 동반한 다발성 폐동정맥루 치험 1례 (Multiple Pulmonary Arteriovenous Fistula combined with Cyanosis Report of one Case)

  • Pulmonary arteriovenous fistula is a rare congenital vascular malformation in the lung, resulting from erroneous capillary development, with incomplete formation or disintegration of the vascular septa that would normally divide the primitive connection between the venous and arterial plexus. The pathogenesis of its symptom is that unoxygenated and desaturated arterial blood enter into the pulmonary venous system directly. Recently we have experienced a case of multiple pulmonary arteriovenous fistula in a 15 year old male patient, who presented the symptom of cyanosis and dyspnea on exertion. The operation revealed well circumscribed and multilobulated aneurysmal lesion in left lower lobe with its subpleural and posterolateral basal location, and another aneurysmal lesion in inferior lingular segment of left lung. There was no abnormal connection between the fistula and systemic circulation. The left lower lobectomy was performed along with local extirpation of the inferior lingular segment of left lung. Both lesions showed angiomatous dilatation of the various sized vessels embedded in the parenchyma microscopically. Postoperative clinical course disclosed much improvement in symptoms and in the value of blood gas analysis. The patient was discharged without any complication.

Technical Aspects of Combined Heart-Lung Transplantation

  • Although organ transplants have become quite common, combined heart-lung transplantation (CHLTx) is unfamiliar at most institutions. While the remarkable rate of development in treatment options, such as drugs and mechanical circulatory support, have reduced the need for CHLTx, it remains the sole treatment option for a subset of patients with end-stage cardiopulmonary failure. For many cardiothoracic surgeons, CHLTx is not technically new or difficult, but it does pose challenges due to its low frequency and relative complexity. Thus, this review aims to describe the CHLTx technique in technical detail using the existing literature.

Recent Advances in Adjuvant Therapy for Non-Small-Cell Lung Cancer

  • After the successful development of targeted therapy and immunotherapy for the treatment of advanced-stage non-small cell lung cancer (NSCLC), these innovative treatment options are rapidly being applied in the adjuvant setting for early-stage NSCLC. Some adjuvants that have recently been approved include osimertinib for epidermal growth factor receptor-mutated tumors and atezolizumab and pembrolizumab for selected patients with resectable NSCLC. Numerous studies on various targeted therapies and immunotherapy with or without chemotherapy are currently ongoing in the adjuvant setting. However, several questions regarding optimal strategies for adjuvant treatment remain unanswered. The present review summarizes the available literature, focusing on recent advances and ongoing trials with targeted therapy and immunotherapy in the adjuvant treatment of early-stage NSCLC.

Association of the G134A and G184C Polymorphisms in the CYP1A1 Gene with Lung Cancer Incidence

  • The G184C and G134A single nucleotide polymorphisms(SNPs) of the CYP1A1 gene result in Ala62Pro and Gly45Asp substitutions, respectively. Here, we tested whether these SNPs are associated with an alteration in lung cancer incidence. We examined 80 Korean subjects with lung cancer and 240 age- and sex-matched controls. For each subject, the CYP1A1 gene was PCR amplified and sequenced. We observed that the odds ratio(OR) for lung cancer was 3.37 higher in subjects with the G184C polymorphism than in controls(95% confidence interval(CI), $0.89{\sim}12.73$, P=0.07). In contrast, the OR for lung cancer was 1.23 in subjects with the G134A polymorphism compared to controls(95% CI, $0.68{\sim}2.20$, P=0.49). The G184C polymorphism exacerbated the effects of smoking on lung cancer development. Gene-smoking interaction analyses revealed that past or present smokers with the G184C polymorphism had a higher incidence of lung cancer(OR, 24.72; 95% CI, $4.48{\sim}136.31$; P<0.01) than control smokers(OR, 6.65; 95% CI, $2.72{\sim}16.28$; P<0.01). However, there was only a slight difference in the ORs for lung cancer between control smokers and smokers with the G134A polymorphism. These findings suggest that the G184C polymorphism, but not the G134A polymorphism, is associated with an increased risk of lung cancer.
