Lung cancer has a high mortality rate and is often diagnosed at the metastatic stage. Gefitinib is a targeted molecular therapeutic drug used to treat patients with non-small-cell lung cancer (NSCLC). Korean herbal medicines may also have therapeutic efficacy against lung cancer, reduce the side effects associated with chemotherapy, and improve patient quality of life (QOL). This case report describes the effects of a Korean herbal medicine regimen combined with gefitinib in a patient with NSCLC and bone metastasis. The Korean herbal medicine regimen included woohwanggeosa-dan, hwanggibujeong-dan and geonchilgyebok-jeong. The computed tomography (CT) findings showed that following combination treatment, the size of the tumor was markedly decreased without serious adverse events. Moreover, the Eastern Cooperative Oncology Group (ECOG) performance status was improved and cancer-related pain was decreased. These results suggest that a combination of Korean herbal medicines and gefitinib may be an effective therapeutic option for patients with advanced NSCLC and bone metastasis. Further studies are needed to examine the mechanism and the clinical efficacy of Korean herbal medicines against NSCLC.
Metastases to the liver can be found in various malignancies, most commonly originating from the colon, rectum, pancreas, stomach, esophagus, breast, lung, and melanoma. Surgical resection of liver metastasis is generally considered to be the definitive therapy fore cure. However, many patients are unable to undergo surgical resection due to medical comorbidities or multifocal extent of malignant disease affecting the liver. Among patients not eligible for surgery, other therapies exist for treatment in order to down stage the disease for surgical resection or for palliation. Radioembolization of hepatic metastases has shown to improve outcomes among patients with variety of malignancies including more common malignancies such as colorectal cancer. Yttrium-90 (Y-90) radioembolization has been successfully used in the management of hepatic metastases. A small series of metastatic sarcoma to the liver treated with radioembolization showed a promising response. We report a case of metastatic gastric leiomyosarcoma to the liver treated with Y-90 glass microspheres therapy using the radiation segmentectomy approach, previously described for hepatocellular carcinoma.
1. Objectives: We aimed to propose a sub-classification system for the Taeeumin symptomatology by examining the Taeeumin pathology and symptomatology descriptions appearing in "Donguisusebowon". 2. Methods: The Gabo Edition and the Sinchuk Edition (the upgraded and revised edition) of "Donguisusebowon" were reviewed and examined for relevant information on the Taeeum pathology and symptomatology. 3. Results and Conclusions: 1) In the Taeeumin symptomatology, the Exterior disease develops from the basic pathology of Esophagus-Cold and the Interior disease from that of Liver-Heat, eventually progressing to damage of the expirational and dispersive energy of the Lung Sector, the Prime Core Organ or the excessively small organ of the Taeeum constitutional type. The resulting pathology can be broadly defined as the "Lung-Dryness symptomatology". 2) The case reports introduced in the Exterior disease section, including the Zhang Zhongjing Mahuang-tang treatment, Prolonged-affliction disease treatment, and Exterior disease Pestilential disease treatment, share several points in common. They all arise from the pathology of "weakness in the Lung sector and deficiency in the Exterior sector", and they can all be assigned to the same symptomatological division that presents with systemic heat and cold intolerance; this symptomatology can be defined as the "Esophagus-Cold symptomatology", the milder subdivision of the exterior symptomatology. 3) The body of text appearing in the last part of the Interior disease section commonly referred to as the "Taeeumin Conspectus" is in fact not a conspectus when its contents are actually examined. Instead, it can be understood from its pathological and symptomatological descriptions that the passage is explaining the more severe subdivision of the exterior symptomatology that has progressed from Esophagus-Cold to a pathology characterized by damaged expirational and dipersive energy of the Lung Sector. 4) The relocation of the "dry-related pathology" indicates a change in perspective regarding the "Dry-related symptomatology", which caused the rearrangement of the Interior disease into divisions of Liver-Heat symptomatology that is characterized by fulminant heat pathology and Dry-Heat symptomatology that is also accompanied by Lung-Dryness. 5) The Interior disease Yin-Blood Consumptive symptomatology should be included in the Dry-Heat symptomatology in the pathological scheme. 6) Based on the above, the subdivisions of the Taeeumin symptomatology should be arranged as "Esophagus-Cold symptomatology" and "Lung-Dry-Cold symptomatology" in the Exterior disease and "Liver-Heat symptomatology" and "Dry-Heat symptomatology" in the Interior disease.
Caprine arthritis encephalitis (CAE) virus is a causative agent of caprine arthritis-encephalitis. In our previous study we reported a prevalence of CAE. In this study, we described the further detailed pathological features of CAE and examined the detection of virus by in situ hybridization (ISH). Histopathologically, interstitial pneumonia and bronchopneumonia in lung, focal inflammation in mammary glands, perivascular cuffing in brain, arthritis, and focal necrosis, mild steatosis, inflammatory cell infiltration of liver were noted. CAEV proviral-DNA was identified by nested polymerase chain reaction (PCR) in blood cells, brain, synovial fluid, and lymph node. Confirmation by nested PCR involved amplification of a 296 bp ($1^{st}$ PCR) and 185 bp ($2^{nd}$ PCR) fragments corresponding to a conserved region on the gag gene of CAEV. Positive ISH signals were detected in the brain and liver. In conclusion, significant histopathological findings included parenchymal infection in various organs, including the lung, liver, brain, joint, and mammary gland were noted in the CAEV infected dairy goat. ISH can help confirm the diagnosis of CAE in formalin-fixed samples.
Tissue distribution of $SiO_2$ nanopaprticles was investigated in mice after oral administration or skin treatment. ICR Male mice were treated with $SiO_2$ nanoparticles 2.5 g/kg/day for five consecutive days and sacrificed at 24 hours after the last administration. As results, the orally administered $SiO_2$ nanoparticels were distributed in the testis and kidney but not in lung at 24 hours after the last treatment. In case of skin treatment, $SiO_2$ nanoparticles were distributed to lung as well as testis, brain, kidney and liver. The results suggested that $SiO_2$ nanoparticles (12 nm) are easily absorbed through entero-gastric system or skin.
The dose dependence of the severity of radiation-induced thymic lymphoma in C57BL/6J mice was studied. Mice were exposed to fractionated irradiation at the total doses of 4.0, 6.0 and 8.0 Gy (four irradiations at 8-day intervals) starting from 33 days after birth. Pathological and histological changes of each mouse were observed after periodical sacrifice at day 75, 100, 125, 150, 175, 200, 250, 300 after the first irradiation. The severity of cancers were classified into 4 stages by clinical signs with respect to the enlargement of the thymus, spleen, liver, the progression of the cancer in the thymus, and the metastasis to the spleen, liver, lung and the lymphatic nodes. Among the 490 mice observed, 146 mice had thymic lymphoma. A clear dose-effect relationship was observed as well as the dose-response relationship. Also, periodical observation showed that thymic lymphoma was first induced in mice sacrificed at day 100 (130days old), and metastasize in the order of spleen, lung, liver and then the lymphatic nodes. The results suggest that radiation may be involved not only as a tumor initiator but also as a tumor promoter, and a tumor progression-enhancing agent.
The author came to analize and get the following problems after researched radiographic quality in order to find out the necessary factors to improve in radiographs of frontal projection of the chest selected at random of the adult patients (1545 male, 1520 female) who had been examined in 4 departments of radiology of the general hospitals in Seoul and Kyungki area. 1. Problems of x-ray film or of radiographic cassette appeared in 2.97% radiographs on account of selection of the film size (except costophrenic sinus) (1.79%), poor screen-film contact (0.85%), light leakage of cassette (0,33%). 2. Problems of patients' positioning or breathing appeared in 16.57% radiographs of all because of overlapping of lung apex and clavicle (6.98%), overlapping of scapula and lung field (5.87%), asymmetrical projection of clavicles (1.76%), errors in positioning and breathing of the patient (1.96%). 3. Problems of x-ray exposure factors or film processing appeared in 22.25% radiographs because of over-density (2.64%), under-density (3.95%), fog (0.59%), demonstration density under clavicles or lung marking unsharply (3.82%), not clear of lung marking from breast region (0.94%), not clear the lung marking from the part overlapped of heart and lungs (3.92%), not clear the lung marking from the part overlapped of liver and lung (6.49%).

Metastasis of lung cancer to the skin is uncommon, presenting in 0.22% to 12% of lung cancer patients, and it is extremely rare for skin metastasis to be the first clinical manifestation of lung cancer. In the few cases where skin metastasis has been reported as the first sign of lung cancer, the patients were typically heavy smokers or had preexisting respiratory diseases and symptoms. This prompted clinicians to consider skin metastasis of a pulmonary malignancy. Large cell neuroendocrine carcinoma (LCNEC) is a rare type of lung cancer that accounts for approximately 3% of lung cancers. LCNEC mainly metastasizes to visceral organs, such as the liver, bone, and brain, and it only shows metastasis to the skin in very rare cases. Herein, we report an unusual case of a metastatic skin lesion as the first sign of primary pulmonary LCNEC, in a 63-year-old woman with no pulmonary symptoms or personal history of smoking or pulmonary disease.
The hypoglycemic effects of butanol(BuOH) fraction of Polygonatum odoratum with vitamin E in streptozotocin(STZ)-induced diabetic rats were investigated. Sprague-Dawley rats weighing 200-230g were devided into five groups, and four groups induced diabetes mellitus by the STZ injection(45mg/kg b.w.) into the tail vein : Normal, diabetic-control, and three diabetic experimental groups(p. odoratum group, P. od-vit. E group and Vit. E group). All groups were fed on a AIN-76 diet, and the experimental groups were orally administered with the BuOH fraction of Polygonatum odoratum(500mg/kg b.w.) and vitamin E(10mg/kg b.w.) for 14 days. The body weight, diet intake and organ weights were monitored. The plasma levels of glucose, insulin, cholesterol, triglyceride, HDL-cholesterol and aspartate and alanine aminotransferase activities were analyzed. The levels of glycogen in liver and muscle, cholesterol in liver were determined. The malondialdehyde(MDA) levels in liver, kidney and lung were assayed. The body weight loss was seen in P. odoratum group, P. od-vit. E group, Vit. E group and diabetic control group, while the loss in P. odoratum group was much less than that in the diabetic control group. The plasma glucose levels were significantly lowered in P. odoratum group compared to diabetic control group. The plasma insulin levels were noticeably higher in P. odoratum and Vit. E groups. The rats in P. odoratum and P. od-vit. E group showed higher liver glycogen levels than in the diabetic control group. The MDA levels in liver, kidney and lung were also significantly reduced in P. od-vit. E and Vit. E groups compared to the diabetic control group. The results suggest that the administration of BuOH fraction of Polygonatum odoratum along with vitamin E reduced blood glucose levels and peroxidative tissue damage in STZ-induced diabetic rats, showing the possibility of preventive and therapeutic use of the wild edible plant to the diabetes mellitus. These experiments were designed to study the influence of protein undernutrition during lactation period(3 wks) or after-weaning period(8 wks) on growth of organs, and on brain and liver composition of the experimental rats. The following experimental groups were studied. Most of the organs except the spleen could not catch up with the normal group in their weights for the group of protein undernutrition during lactation(3 wks), even after 17 weeks of rehabilitation. For the group of protein undernutrition during after-weaning period(8 wks) brain, lung, heart, spleen and pancreas could catch up with the normal group after rehabilitation. According to this result it is assumed that the growth of brain, lung, heart and pancreas might be developed mostly during lactation and that the growth of liver and kidney might be developed after-weaning period continuously. 2. For the groups of protein underuntrition during lactation period or after-weaning period the amounts of total lipid, cholesterol and phospholipid of brain were lower than those of normal group. Especially, cholesterol level was significantly lower than normal group. And there was also a significant difference in the phospholipid level of the after-weaning(8 wks) deprivation group. 3. The groups of protein undernutrition during lactation or after-weaning period(8 wks) showed lower level of liver nitrogen and higher level of liver fat. Especially, protein undernutriton during lactation gave a greater influence on the lever of liver fat.
